The first embodiment of the present disclosure provides a cartridge assembly for a stapler, where the cartridge assembly can be used in various surgical staplers, and is particularly suitable for being installed on a linear cutting stapler, where the cartridge assembly can form a stapler with an anvil assembly, a firing assembly, and the like, where the cartridge assembly can be based on any structure in the prior art, and the cartridge assembly contains staples inside the cartridge assembly, and specifically, when the cartridge assembly is pressed on the anvil assembly, the staples in the cartridge assembly are pressed on the anvil assembly through the firing of the firing assembly so as to perform a stapling operation on human tissue in a surgical operation.
The cartridge assembly of the present disclosure is structured as shown in fig. 1 and 2, and comprises a body 100, the body 100 having a tissue contacting surface 110, the tissue contacting surface 110 being used for placing human tissue during a cutting or stapling operation, the body comprising a proximal end portion 110a, a middle portion 110b and a distal end portion 110c distributed along the length thereof, the end portion of the body 100 being connected to a firing device in the stapler, wherein the proximal end portion 110a is located at a position close to the firing device, and the distal end portion 110c is located at a position far from the firing device. Here, the proportions of the three portions of the proximal portion 110a, the middle portion 110b, and the distal portion 110c in the tissue contact surface 110 may be adjusted, for example, the three portions occupy one third of the length or area respectively, of course, the length or area of the middle portion 110b may also occupy a larger proportion, for example, 60%, the lengths or areas of the proximal portion 110a and the distal portion 110c each occupy 20%, and it should be noted that the proportions of the proximal portion 110a, the middle portion 110b, and the distal portion 110c in the tissue contact surface 110 are all within the scope of the embodiments of the present disclosure. Further, when a cutting or stapling operation is performed on a body tissue, the body tissue is placed on the tissue contact surface 110 of the cartridge assembly and the cartridge assembly is pressed on the anvil assembly, the portion of the body tissue located at the distal end portion 110c of the tissue contact surface 110 is relatively small due to the pressing force, the gap between the cartridge assembly and the anvil assembly may be increased due to the thickness of the body tissue, the body tissue is easily slipped outwards, at least one boss 1 is provided at the middle portion 110b and/or the proximal end portion 110a of the tissue contact surface 110, the boss 1 fixes the body tissue by forming a friction force between the end surface thereof and the body tissue for preventing the body tissue from slipping at the position of the distal end portion 110c when the cutting or stapling operation is performed, thereby offsetting the problem of the increased clearance of the distal end portion 110 c.
Specifically, the shape of the boss 1 in the present embodiment may be, for example, a trapezoidal shape as shown in fig. 1, a frustum shape as shown in fig. 2, a cylinder shape, or the like, and of course, any other shape of the boss 1 capable of forming a friction force with a human tissue to prevent the human tissue from slipping is within the scope of the embodiments of the present disclosure.
In one embodiment, in order to increase the friction force between the human tissue and the bosses 1, a plurality of the bosses 1 may be provided at the middle portion 110b and/or the proximal end portion 110a of the tissue contact surface 110, and a plurality of the bosses 1 may be provided at the distal end portion 110c of the tissue contact surface 110, as shown in fig. 3 and 4, the concentration of the bosses 1 of the middle portion 110b and/or the proximal end portion 110a is less than the concentration of the bosses 1 of the distal end portion 110 c. The concentration here refers to the number of the lands 1 per unit area on the tissue contact surface 110.
Further, the proximal portion 110a is provided with a plurality of the bosses 1 having a first concentration, the middle portion 110b is provided with a plurality of the bosses 1 having a second concentration, the distal portion 110c is provided with a plurality of the bosses 1 having a third concentration, and the first concentration is less than the second concentration, which is less than the third concentration. In this way, the concentration of the bosses 1 per unit area on the tissue contact surface 110 from the distal end portion 110c to the middle portion 110b to the proximal end portion 110a is gradually decreased, and thus, the number of the bosses 1 per unit area on the distal end portion 110c is maximized so that the frictional force of the human tissue mass from the bosses 1 on the distal end portion 110c is maximized, so that the human tissue mass does not slip out from the position of the distal end portion 110 c.
In another embodiment, when said middle portion 110b and/or said proximal portion 110a of said tissue contacting surface 110 is provided with a plurality of said bosses 1, in the case where the distal end portion 110c of the tissue contact surface 110 is also provided with a plurality of the bosses 1, the disclosed embodiments may be further defined with respect to the height of the boss 1, for example, the bosses 1 may have a predetermined height, for example may be all set to 0.1mm, and can, of course, have different heights, where the height generally should not exceed the distance between the surface of the cartridge assembly and the anvil assembly when the cartridge assembly is closed, of course, it is also possible to define the height of the boss 1 at different positions, for example the height of the boss 1 of the middle part 110b and/or the proximal part 110a is lower than the height of the boss 1 of the distal part 110 c.
Further, as shown in fig. 5, the proximal portion 110a is provided with a plurality of the bosses 1 having a first height, the middle portion 110b is provided with a plurality of the bosses 1 having a second height, and the distal portion 110c is provided with a plurality of the bosses 1 having a third height, and the first height is lower than the second height which is lower than the third height; specifically, in order to prevent the human tissue from slipping toward the distal end portion 110c, the height of the bosses 1 is gradually decreased in a direction from the distal end portion 110c to the middle portion 110b to the proximal end portion 110a, so that the height of the bosses 1 at the distal end portion 110c is highest, so that the frictional force applied to the human tissue at the position of the distal end portion 110c is maximized, so that the human tissue does not slip outward from the position of the distal end portion 110 c.
Continuing with fig. 1 and 2, the body 100 further includes a cutter path 2 disposed along the length of the tissue contacting surface 110, where the cutter path 2 may be disposed on the surface of the body 100, the cutter path 2 being adapted to receive a cutting blade (not shown) for cutting the body tissue mass, the cutting blade being reciprocally movable in the cutter path 2 to effect cutting of the body tissue mass.
In one embodiment, when the human tissue is placed on the tissue contact surface 110 of the cartridge assembly and the cartridge assembly is pressed on the anvil assembly, the closer to the knife path 2 the position of the human tissue needs to be closely arranged to the tissue contact surface 110 without slipping in order to ensure the accuracy of the cutting operation when the human tissue is cut along the knife path 2 by the cutting knife, and for this purpose, the number and height of the bosses 1 can be adjusted based on the position of the knife path 2, specifically, as shown in fig. 6, the number of the bosses 1 close to the knife path 2 is larger than the number of the bosses 1 far from the knife path 2 on one side of the knife path 2; in another embodiment, in case the body 100 further comprises a knife path 2 arranged along the length of the tissue contacting surface 110, as shown in fig. 7, the height of the boss 1 close to the knife path 2 is higher than the height of the boss 1 far from the knife path 2 at one side of the knife path 2.
In one embodiment, the area on one side of the tool path 2 between a position close to the tool path 2 and a position far from the tool path 2 may be divided into 3 sections, each section having a different number of bosses 1 or height of the bosses 1, such that 2 times the reduction of the number or height of the bosses 1 is presented from a position close to the tool path 2 to a position far from the tool path 2, and of course more sections may be provided to present more times the reduction of the number or height, which is within the scope of the present disclosure.
In this way, the boss 1 can prevent the human tissue arranged on the body 100 from slipping not only in the direction toward the distal end portion 110c of the tissue contact surface 110 but also in both side directions away from the cutter path 2, thereby enabling the human tissue to be stably fixed to the body 100 for the anastomosis operation.
In another embodiment, continuing to fig. 1 and 2, the body 100 further comprises a plurality of rows of staple pockets 3 extending along the length of the tissue contacting surface 110. of course, the staple pockets 3 may be disposed on either side of the knife path 2 in a predetermined arrangement, and the staple pockets 3 extend in the same direction as the knife path 2; preferably, the knife path 2 may be located at a middle position of the tissue contact surface 110 of the body 100, and the staple pockets 3 are symmetrically or asymmetrically disposed at both sides of the knife path 2. The staple channel 3 is here intended to receive staples and to enable firing from the staple channel 3, wherein the staple channel 3 has an elongated configuration in order to match the shape of the staples.
It should be noted that, for the nail grooves 3 arranged on the two sides of the cutter path 2 according to the predetermined arrangement, the predetermined arrangement may be the same or different arrangement on the two sides of the cutter path 2 according to actual requirements. Further, at least one row of the nail grooves 3 is provided on the surface of the body 100 on either side of the cutter path 2 and along the extending direction of the cutter path 2, and a plurality of the nail grooves 3 are arranged at a predetermined interval distance in each row; further, on either side of the cutter path 2 and along the extension direction of the cutter path 2, a plurality of rows of the staple grooves 3 may be provided on the surface of the body 100, and in the case of providing a plurality of rows of the staple grooves 3 on either side of the cutter path 2, the staple grooves 3 between adjacent rows may be arranged in a crossing manner, which facilitates the fixation and anastomosis of the human tissue.
Further, the boss 1 may be further disposed based on the position of the staple groove 3 to better fix the human tissue object when performing an anastomosis operation by the staple, specifically, the boss 1 may be disposed between at least partially adjacent staple grooves 3 of at least one row of the staple grooves 3, specifically, the boss 1 may be disposed at least one end portion of any one of the staple grooves 3, the boss 1 may be spaced from the end portion of the staple groove 3 by a certain distance, and the fixation of the human tissue object to prevent slippage may be achieved by the boss 1. In one embodiment, the boss 1 is provided at least on the distal side of the most distal staple magazine 3 or on the proximal side of the most proximal staple magazine 3.
Of course, a plurality of bosses 1 may be disposed between the adjacent nail grooves 3 in any one row, and a plurality of bosses 1 may be sequentially disposed along the length direction of the body 100, so that the bosses 1 have a higher distribution density on the body 100, and the friction force of the human tissue on the surface of the body 100 may be increased by increasing the number of the bosses 1 between the adjacent nail grooves 3. Since the concentration of the bosses 1 at the position from the distal end portion 110c to the middle portion 110b to the proximal end portion 110a in the tissue contact surface 110 is gradually decreased as described above, it may be possible to arrange one boss 1 every other one of the staple grooves 3 from the position of the distal end portion 110c until one boss 1 every third one of the staple grooves 3 at the position of the proximal end portion 110a is lowered, thereby achieving a change in concentration to further improve the fixing effect on the human tissue.
A second embodiment of the present disclosure provides a stapler including an anvil assembly and a firing assembly, further including a staple cartridge assembly as in any of the embodiments described above. Further, the stapler preferably employs a straight cutting stapler.
This above-mentioned embodiment of disclosure sets up the boss through on the tissue contact surface at nail storehouse subassembly to carry out different settings to the position, quantity and the height of boss, make when carrying out the anastomosis operation human tissue thing with form great frictional force between the nail storehouse subassembly, make human tissue thing realize well fixed and can not appear producing to the distal end portion of nail storehouse subassembly slides or slides to both sides, promotes the accuracy of anastomosis operation and improves the shaping effect that promotes the anastomosis nail simultaneously.
