Description
Technical Field
The present disclosure relates to ultrasonic electrosurgical systems for performing surgical procedures, and more particularly, to an apparatus for outputting drive signals to a surgical instrument.
Background
Ultrasonic surgical instruments (referred to as ultrasonic knives for short) and high-frequency electrosurgical instruments (referred to as electric knives for short) can be used for surgical operations. Ultrasonic cutters have better cutting performance, but have poorer coagulation performance in surgical operations. The electric knives are classified into monopolar electric knives and bipolar electric knives according to their operation modes, and the bipolar electric knives have excellent coagulation performance but poor cutting performance in surgical operations. The multifunctional ultrasonic surgical instrument is called an "ultrasonic electric knife" because the ultrasonic knife and the bipolar electric knife can achieve the effects after the electrode is arranged on the end effector of the ultrasonic knife.
Ultrasonic, monopolar, or bipolar electrosurgical blades are used as needed in surgery, several of which are often used in a single procedure. At present, the output devices for driving these surgical instruments are independent devices, which not only occupy the limited space of the operating room, but also have much inconvenience in controlling these output devices during use, and even affect the operation effect. For example, when the ultrasonic scalpel is driven, an ultrasonic output device is used for driving the ultrasonic scalpel, and a high-frequency output device is used for driving the monopolar electric scalpel or the bipolar electric scalpel, the ultrasonic output device and the high-frequency output device are connected by a cable, and the ultrasonic scalpel is driven by a device with a dual energy source, but an ultrasonic driving signal and a high-frequency electric driving signal output by the device are superposed single-path signals, a special ultrasonic transducer is required to separate the signals to drive the ultrasonic scalpel, and the ultrasonic transducer is a vulnerable component, so that the application cost is undoubtedly increased.

According to the embodiment of the present disclosure, the main board 123 is configured to control the output parameter based on the ultrasonic feedback signal and the high-frequency electric feedback signal, so as to better match the surgical procedure, and facilitate the achievement of the surgical effect with good coagulation effect and cutting performance; meanwhile, the ultrasonic feedback signal and the high-frequency electric feedback signal are acquired through the independent acquisition and processing circuits so that the main board 123 can adaptively control the signals, and the structure can reduce the interference between the ultrasonic signal and the high-frequency signal.
According to the embodiment of the present disclosure, the main board 123 is further configured to obtain an acoustic impedance based on the ultrasound feedback signal, obtain an electrical impedance based on the high-frequency electrical feedback signal, match the acoustic impedance and/or the electrical impedance with impedance data to determine a tissue type, and determine the power of the ultrasound driving signal and the power of the high-frequency electrical driving signal based on the tissue type.
As known to those skilled in the art, from the voltage signal v (t) and the current signal i (t), the impedance in the load circuit can be obtained, i.e., r (t) ═ v (t)/i (t). In the embodiment of the disclosure, the impedance R in the ultrasonic circuit can be obtained through the voltage signal output by the ultrasonic energy circuit board and the current signal in the load circuitUS(t), simply acoustic impedance, impedance R in the high frequency circuit is obtained from the voltage and current signals output from the high frequency electric energy circuit boardES(t), referred to as electrical impedance.
According to embodiments of the present disclosure, the acoustic impedance, electrical impedance are related to the load circuit, i.e., to the impedance characteristics of the tissue being cut. As mentioned above, the type of the cut tissue can be identified by detecting the acoustic impedance and/or the electrical impedance or the change rule thereof in the surgical cutting process, and the type of the cut tissue is used as a basis for adjusting the power of the driving signal, so that the power self-adaptive adjustment of the driving signal is realized, and a better surgical effect is obtained.
For example, a time window for identifying the tissue type may be set in the initial stage of cutting, and the detected acoustic impedance and/or electrical impedance may be compared with impedance characteristic values of different types of tissues, where the highest matching degree is the identified tissue type. Due to the differences in the physical properties of different tissues, which also have impedance differences during cutting, the processing and analysis of experimental and empirical data allows the differential acoustic and/or electrical impedance of different types of tissues at the initial stage of cutting to be obtained, and these results can be stored and used to match the tissue type.
Illustratively, during the initial stage of cutting, the identified tissue type is type 1, which corresponds to the power P1(n +1) of the ultrasonic drive signal at the next instant and the power P2(n +1) of the high frequency electric drive signal and the acoustic impedance R at the previous instantUS(n) and electrical impedance RES(n) are respectively as follows:
P1(n+1)=f1(RUS(n),RES(n)),
P2(n+1)=f2(RUS(n),RES(n)),
if the identified tissue type is type 2, the power P1(n +1) of the ultrasonic driving signal and the power P2(n +1) of the high-frequency electric driving signal at the next moment correspond to the acoustic impedance R at the previous momentUS(n) and electrical impedance RES(n) are respectively as follows:
P1(n+1)=f3(RUS(n),RES(n)),
P2(n+1)=f4(RUS(n),RES(n))。
wherein f1, f2, f3 and f4 are functions, and can be empirical functions or trained neural network models, for example; rUS(n)、RES(n) the acoustic and electrical impedance at time n, respectively. By adopting the embodiment of the disclosure, when different tissues are encountered in surgical cutting, the cutting power more suitable for the tissues can be automatically adjusted, and the cutting performance is improved.
According to the embodiment of the present disclosure, the main board 123 is further configured to obtain a first phase difference based on the ultrasonic voltage feedback signal and the ultrasonic current feedback signal; obtaining a second phase difference based on the high-frequency voltage feedback signal and the high-frequency current feedback signal; determining a power of the ultrasound drive signal and a power of the high frequency electrical drive signal based on the tissue type, the first phase difference, and the second phase difference.
The signal power applied to the load circuit can be accurately adjusted according to the phase difference between the voltage signal and the current signal in the load circuit. For example, when the actual signal power applied to the surgical instrument is P — UIcos θ, the actual signal power decreases when the phase difference θ between the voltage signal and the current signal is not zero. According to the embodiment of the present disclosure, after the tissue type is matched according to the impedance, if the phase difference in the current circuit is not zero, the signal power output by the energy source needs to be increased, so as to ensure that the signal power actually loaded on the surgical instrument meets the requirement. For example, the currently matched tissue type is type 1, the power recommended value of the corresponding ultrasonic signal is P01, the power recommended value of the high-frequency electric signal is P02, and the first phase difference θ calculated at this time is1I.e. the phase difference of the ultrasonic feedback signal is not zero, and the second phase difference theta2If the signal power is zero, the main board 123 controls the signal power output by the ultrasonic energy circuit board to be PUS=P01/cosθ1The output signal power of the high-frequency electric energy circuit board is PES=P02。
According to the embodiment of the present disclosure, the motherboard 123 may further control the time length of the output signals of the ultrasonic energy circuit board and the high-frequency electric energy circuit board. For example, when the ultrasonic scalpel is used for surgical cutting, the ultrasonic energy circuit board is controlled to output a driving signal and the high-frequency electric energy circuit board is not output in a certain period, at the moment, the ultrasonic scalpel only executes the cutting function of the ultrasonic scalpel, and the high-frequency electric energy circuit board is controlled to output a driving signal and the ultrasonic energy circuit board is not output in another period, at the moment, the ultrasonic scalpel only executes the blood coagulation function of the ultrasonic scalpel, so that the better surgical effect is also obtained.
According to an embodiment of the present disclosure, the main board 123 is configured to determine a cutting phase based on the ultrasonic feedback signal and/or the high-frequency electric feedback signal, and determine a power of the ultrasonic drive signal and a power of the high-frequency electric drive signal based on the cutting phase.
According to an embodiment of the present disclosure, the progress of the cutting may be divided into a cutting initial stage, a cutting proceeding stage, and a cutting final stage. Adjusting the drive signal power at each stage to adapt the cutting progress may provide benefits to the cutting zone. For example, in the initial stage of cutting, the ultrasonic blade needs to start working quickly, at this time, the power for driving the ultrasonic blade can be output at a higher value, and the power for the electric blade only needs to be at a lower value; at the cutting stage, the cutter head penetrates into the tissue, the tissue needs to be cut and coagulated synchronously, and the advantages of the ultrasonic knife and the electric knife are utilized, so that the power of the driving signal is output according to 50% of the total power of the ultrasonic knife and the electric knife; and at the final stage of cutting, cutting is to be finished, the fast cutting advantage of the ultrasonic knife is fully utilized, at the moment, the signal power for driving the ultrasonic knife is also output at a higher value, and the signal power for driving the electric knife only needs a lower value.
According to an embodiment of the present disclosure, the cutting phase may be determined by acoustic and or electrical impedance, such that the adapted ultrasound drive signal and the high frequency electric drive signal are output at different cutting phases. It has been found that the cutting phase is also represented by the impedance characteristic of the tissue, for example, the impedance increases and then decreases at the beginning of the cut, which is the initial phase of the cut, and then a longer and constant phase occurs, which is the ongoing phase of the cut, and the impedance increases rapidly through the final end of the cut. Therefore, when the acoustic impedance or the electrical impedance is detected to be firstly reduced and then stabilized within a set time window, the cutting progress stage can be identified, and when the acoustic impedance or the electrical impedance is detected to be rapidly increased and changed within the set time window, the cutting end stage can be identified.
According to the embodiment of the disclosure, the fixed power can be adopted in the initial stage and the final stage of the cutting, and the dynamically adjusted power can be adopted in the cutting process stage. Optionally, in the initial stage of cutting, the signal power for driving the ultrasonic knife and the electric knife can be proportionally output according to the ultrasonic power set value and the high-frequency electric power set value; in the cutting stage, the signal power for driving the ultrasonic knife and the electric knife can be adjusted in a self-adaptive manner according to the acoustic impedance and/or the electrical impedance; in the final stage of cutting, the power of the driving signal is still proportionally output according to the ultrasonic power set value and the high-frequency electric power set value. Therefore, the power adaptation efficiency can be improved, and a better operation effect can be obtained.
