Abstract

本实用新型涉及一种槽式椎弓根钢板及固定螺 钉,属于脊柱外科医疗器械。其钢板采用含有连续排 列螺钉穴的中心槽式结构;固定螺钉采用松质骨加压 螺钉结构,较普通螺钉具有直径大、螺距宽、螺纹深等 特点。在手术中,这种钢板及固定螺钉能够适合于不 同椎弓根间距的患者个体,且符合椎体以松质骨为主 的解剖特点,具有良好的整复骨折脱位及脊椎滑脱的 功效,从而保证了手术操作的可靠性和安全性。

Description

本实用新型涉及一种槽式椎弓根钢板及固定螺钉,属于脊柱外科医疗器械。
椎弓根钢板及固定螺钉系统,最早由Roy-Camille首创(见:Clin    Orthop,1986;203:7),将其上沿纵向具有多个螺孔的两块钢板置于脊椎两侧的关节突上,然后用固定螺钉固定在椎弓根及椎体上,目前,国内外应用较多。但在临床应用过程中发现,该钢板及固定螺钉存在明显的缺陷,主要表现在:(1)Roy-Camille钢板每个螺孔之间的距离是固定的,而人体由于存在个体间的解剖差异,其椎弓根间距却是不恒定的,因此常发生手术中钢板螺孔位置难以与椎弓根间距相吻合的矛盾,以至给手术造成困难甚至失败;(2)椎体结构以松质骨为主,而Roy-Camille钢板采用的是普通螺钉固定,其直径细(3.5mm),螺纹浅,固定效果较差。
本实用新型的目的是提供一种新型槽式椎弓根钢板及固定螺钉,能够在手术中适合于不同椎弓根间距的患者个体,且符合椎体以松质骨为主的解剖特点,从而保证手术操作的可靠性和安全性。
本实用新型的目的是这样实现的:钢板采用中心槽式结构,即沿钢板纵向中心设置2~3个中心槽,每个中心槽中含有3~6个连续排列的螺钉穴;固定螺钉采用松质骨加压螺钉结构,入骨直径4~6mm(胸椎采用4mm),长度40~60mm,螺距为2~3mm,螺钉头部为六角型,其螺钉头基部呈圆锥形与螺钉穴相配套嵌接。
采用这种结构,固定螺钉可在钢板的每个中心槽内植入任一个螺钉穴,从而解决了Roy-Camille钢板的螺孔位置与椎弓根间距有时不相匹配的矛盾;每个螺钉穴可与螺钉头基部为圆锥形的螺钉相配套嵌接,增加了稳定性且便于手术中植骨;这种固定螺钉较普通螺钉的入骨直径粗、螺距宽、螺纹深,更适合椎弓根以松质骨为主的解剖特点,固定后握持有力,提高了固定的有效性,达到Denis所谓的“三柱”固定效果。

临床实践证明:本实用新型具有良好的整复骨折脱位及脊椎滑脱的功效。临床应用34例,没有1例发生钢板或螺钉折断,对脊锥滑脱病例术后可提高滑脱等级1-2级,平均整复29.2%;对脊柱骨折脱位患者,术后骨折脱位全部得到良好整复,伴有脊髓及马尾神经功能损害者,全瘫者无变化,不全瘫者术后全部恢复到正常水平,3~5周内可下床行走,半年内恢复原工作,无明显腰痛及下肢痛之后遗症。

Claims (2)

1、一种槽式椎弓根钢板及固定螺钉,其特征在于:
a、钢板采用中心槽式结构,即:沿钢板纵向中心设置2~3个中心槽,每个中心槽中含有3~6个连续排列的螺钉穴;
b、固定螺钉采用松质骨加压螺钉结构,入骨直径4~6mm,长度40~60mm,螺距为2~3mm,其螺钉头基部呈圆锥形,与螺钉穴相配套嵌接,螺钉头部为六角型。
2、根据权利要求1所述的槽式椎弓根钢板及固定螺钉,胸椎采用固定螺钉的直径为4mm。
