Description
Technical Field
The utility model relates to the technical field of medical instruments, in particular to an acetabulum posterior wall defect reconstruction system.
Background
The posterior wall fracture of the acetabulum is the most common type of acetabulum fracture, and according to the principle that in addition to the simple posterior wall fracture in the lotournel type in 1964, other types of fractures are combined with the posterior wall fracture, such as posterior column combined posterior wall fracture and transverse row combined posterior wall fracture, and the incidence rate of the fractures accounts for 35% -47% of the total acetabulum fracture. The degree of acetabular posterior wall fracture damage is primarily related to the degree and angle of femoral head impingement, with different impingement classifying the posterior wall fracture types into 3 categories: simple posterior fractures, comminuted posterior fractures, and posterior fractures with compression of the articular surfaces. Regardless of the type of posterior fracture, surgical treatment is often used, because most non-surgically treated patients do not have the expected prognosis and the posterior bone mass accumulates to more than 25%, which severely degrades the stability of the hip joint and must be treated surgically.
At present, clinical orthopedic wounds are mostly fixed by means of rebuilding steel plates, but different fixing methods are adopted for different fracture types. The simple rear wall fracture can play a strong and stable fixing effect on the rear wall only by using single or double steel plates or lag screws; for the crushed rear wall fracture, a spring steel plate or a small screw is additionally added on the basis of the reconstruction steel plate to fix a small bone block; for posterior fractures with compression of the articular surfaces, autologous or allogeneic bone is also used to reconstruct the site of compression. The prognosis of simple posterior wall fracture can basically achieve expected treatment effect, and the latter two types of posterior wall fracture often cause unevenness of joint surfaces after treatment and absorption of bone blocks after a period of operation because of various bone blocks or joint surface compression, so that the wearing of femoral heads can cause arthritis to cause pain and movement disorder, and finally the joint replacement is rapidly ended. The effect of the surgery is quite different from the clinical expected effect.
The most clinically effective treatment for severe posterior acetabular wall defects is hip replacement, but this procedure often requires revision in younger patients, and therefore care must be taken to consider if it is worth changing the hip in younger patients with posterior defects. The Chinese patent application with the application number of 201610539646.6 discloses an acetabular defect reconstruction prosthesis, which comprises an iliac support, wherein the iliac support comprises a bracket and a connecting wing plate arranged on the bracket, and the bracket is in contact fit with the iliac; the acetabular cup is rotationally connected with the bracket, and the technical scheme of the utility model can effectively solve the problems of reconstruction and long-term stability of the defects around the acetabular cup in the prior art. However, the utility model is mainly used for reconstructing huge pelvis bone defects generated after surgical resection of the tumor around the acetabulum, but the reconstructed prosthesis has too large volume and is only suitable for reconstructing huge defects participated after the resection of the tumor, and has no effect on reconstructing the small range of the defect of the back wall. The Chinese patent application with the application number of 201510266407.3 discloses an acetabulum reconstruction prosthesis system suitable for different types of acetabulum defects, which comprises an acetabulum reconstruction cushion block, an acetabulum reconstruction titanium net and an acetabulum reconstruction bracket, wherein screw holes are uniformly distributed on the surface of the acetabulum reconstruction prosthesis system, and the acetabulum reconstruction prosthesis system can be fixed on a position with better acetabulum sclerotin through acetabulum screws, so that the filling and supporting effects of the defect position are realized, and the effect of immediately stabilizing after operation is achieved; the acetabulum reconstruction cushion block, the acetabulum reconstruction titanium net and the acetabulum reconstruction bracket can meet the requirements of acetabulums with different sizes. However, this patent is directed to the use of a total acetabular defect (i.e., a total acetabular replacement technique that requires the total acetabulum to be destroyed and then reconstructed), and the use of this technique is not suitable for localized posterior wall defects. The Chinese patent application with the application number of 202011184480.3 discloses a construction method of an individualized 3D printed acetabular wall defect repair and reconstruction system, the Chinese patent application with the application number of 201921884497.2 discloses an embedded type acetabular posterior wall reconstruction prosthesis based on 3D printing, both are methods and devices aiming at the replacement of the whole acetabulum, and partial acetabular defects cannot be reconstructed only by reconstructing defects, and the reconstruction of the whole acetabulum after being destroyed is often needed.
In the prior art, the best treatment strategy for the defect of the posterior wall of the acetabulum is hip replacement, but when the defect of the posterior wall of a young patient or the simple posterior wall defect of an old person has no complications such as joint wear, the hip replacement is not the best choice.
In view of the above, there is a need to design a prosthetic technique for reconstructing a defect of posterior acetabular wall that can solve the above problems.
