The utility model provides a guide piece for circular tube type anastomat and circular tube type anastomat comprising the guide piece. The circular tube anastomat comprises an anastomat body, an operating handle movably connected with the anastomat body and a nail anvil assembly matched with the anastomat body. The anastomat body comprises a firing assembly and a nail bin assembly arranged on the far end side. During operation, the nail bin assembly is required to enter the tissue and move forwards to reach the operation position. The guide is used to better guide the cartridge assembly into a surgical site. Specifically, the guide includes a guide body and a pull-out portion. The guide piece body comprises a first end and a second end, the second end of the guide piece body is sleeved on the outer side of the far end side of the nail barrel in an initial state, the guide piece body can well guide the nail bin assembly to enter tissue, and damage to the tissue in the moving process of the nail bin assembly is reduced. The distal end side of the pull-out portion is connected with the first end of the guide body, and in the initial state, the pull-out portion is at least partially located in the internal cavity of the cartridge and the proximal end side of the pull-out portion extends out of the cartridge.
In the initial state, when the pulling-out part receives an external force towards the proximal end side, the pulling-out part can pull the guide body to move towards the proximal end side of the nail barrel so as to enable the guide body to enter the inside of the nail barrel from the outer side of the distal end side of the nail barrel and move towards the proximal end side of the nail barrel, the guide body can be separated from the outer side of the nail barrel and at least partially enter the inner cavity of the nail barrel, the separation state is entered, in the separation state, the guide body does not cause obstruction to cutting and suturing of tissues by the nail cartridge assembly, and the anastomat can be normally fired.
The structure of the guide according to various embodiments of the present invention will be described in detail with reference to the accompanying drawings, and it should be understood that the embodiments are not intended to limit the scope of the present invention.
Fig. 1 is a schematic structural view of a staple cartridge assembly, which includes a staple cartridge 8. As shown in fig. 2 to 4, the structure of the guide 1 and the fitting of the guide 1 to the cartridge assembly according to the first embodiment of the present invention are shown. The guide 1 includes a guide body 11 and a pull-out portion. The guide body 11 includes a first end 111 and a second end 112, and in the initial state, the second end 112 of the guide body 11 is sleeved outside the distal end side 81 of the cartridge 8. The guide body 11 has a smooth outer surface, is a main part for guiding the cartridge assembly, and can reduce friction between the guide body 11 and tissue when the cartridge assembly moves in the tissue, so that damage to the tissue is reduced. In this embodiment, the outer surfaces of the guide body 11 and the pull-out portion may be coated with a layer of lubricant to improve the guiding effect of the cartridge assembly and further reduce the friction between the guide body 11 and the tissue, for example, the outer surfaces of the guide body 11 and the pull-out portion are coated with a hydrophilic coating, and the surfaces of the guide body 11 and the pull-out portion are dipped in water before use and become very lubricated, which is widely used in the surgical field.
As shown in fig. 2, in this embodiment, the pull-out portion includes a pull cord 12, and a distal end side 121 of the pull cord 12 is connected to the first end 111 of the guide body 11. In the initial state, the pull cord 12 is at least partially within the internal cavity of the cartridge 8, and the proximal side 122 of the pull cord 12 protrudes outside of the cartridge 8.
In this embodiment, the state of the guide body 11 is divided into an initial state and a disengaged state. In the initial state, the guide body 11 is connected to the cartridge 8 through the second end 112, and since the outer surface of the guide is very lubricated, the friction between the stapler and the tissue can be reduced, and the stapler can be more easily entered. In the initial state, when the surgeon pulls the pull rope 12 towards the proximal side, that is, when the pull rope 12 receives an external force towards the proximal side of the stapler, the pull rope 12 may pull the guide body 11 to move towards the proximal side 82 of the cartridge 8, so that the guide body 11 enters the inside of the cartridge 8 from the outside of the distal side 81 of the cartridge 8 and moves towards the proximal side 82 of the cartridge 8, and the guide body 11 may be detached from the outside of the cartridge 8 and at least partially enter the internal cavity of the cartridge 8, that is, enter a detached state, at this time, since the guide body 11 is no longer sleeved on the distal side 81 of the cartridge 8, no obstacle is formed to the cutting and stapling actions performed on the cartridge assembly, and the firing of the stapler may be normally completed.
Further, the cartridge 8 may include at least one window 83, and the proximal side 122 of the pull cord 12 may extend from the window 83 outside of the cartridge 8. When the guide body 11 is in the disengaged state, the physician may further pull the pull rope 12 towards the proximal side, and further pull the pull rope 12 and the guide body 11 towards the proximal side of the stapler, so that the pull rope 12 may bring the guide body 11 together and completely pull out from the viewing window 83, which is the removed state. In another alternative embodiment, the pull cord 12 and the guide body 11 may not be fully pulled out together, but may be left in the internal cavity of the cartridge 8, and the guide 1 may be removed from the tissue together with the cartridge assembly after the procedure is completed.
In this embodiment, the distal side and the proximal side are relative to the operator, the end closer to the operator is the proximal side, the end farther from the operator, i.e. the end closer to the surgical site, is the distal side, and the direction along the axial center of the stapler is the axial direction, i.e. the direction from the distal side to the proximal side of the stapler, or the direction from the proximal side to the distal side of the stapler. For example, in the perspective of FIGS. 1 and 2, the distal side 81 is the upper side and the proximal side 82 is the lower side of the cartridge 8. In the present invention, for the nail barrel 8, the inside and the outside are with respect to the shaft center S1 of the nail barrel 8, the side close to the shaft center S1 is the inside, and the side far from the shaft center S1 is the outside. The guide body 11 has an outer side and an inner side with respect to the axial center S1 of the cartridge 8, and the side facing the axial center S1 of the cartridge 8 is an inner side, and the side facing away from the axial center S1 of the cartridge 8 is an outer side, that is, the side of the guide body 11 directly contacting the tissue after the cartridge assembly enters the tissue is the outer side.
As shown in fig. 3 and 4, in this embodiment, the guide body 11 is integrally formed with the pull rope 12. In other alternative embodiments, the guide body 11 and the pull cord 12 may be separate two pieces, and the distal side 121 of the pull cord 12 is connected to the second end 112 of the guide body 11.
In this embodiment, the guide body 11 is at least partially flexibly deformable, and in the initial state, when the pull cord 12 is pulled proximally, the pull cord 12 pulls the guide body 11 from outside the distal side 81 of the cartridge 8 into the interior of the cartridge 8 and moves in the direction of the proximal side 82 of the cartridge 8, the guide body 11 deforming somewhat to more easily disengage from the outside of the cartridge 8 and more easily enter the internal cavity of the cartridge 8. Further, the second end 112 of the guide body 11 is an elastic portion, and in the initial state, the elastic portion of the guide body 11 is in an expanded state to clamp the guide body 11 outside the cartridge 8, so that the connection strength between the guide body 11 and the cartridge 8 in the initial state is improved, and the guide body 11 is prevented from being separated from the cartridge 8 when the cartridge assembly does not reach the surgical site. After the guide body 11 is pulled by the pull cord 12 to be disengaged from the outside of the cartridge 8, the elastic portion of the second end 112 of the guide body 11 may return to the contracted state.
In this embodiment, the guide 1 may be integrally made of a thin silicone layer having a certain elasticity and flexibility, forming an integral silicone sleeve. In other alternative embodiments, the guide 1 may also consist only partially of a thin layer of silicone, or the guide 1 may also be made entirely or partially of a plastic material, a rubber material, or the like. In other alternative embodiments, the guide member 1 may be made of a flexible fabric, such as a woven fabric, a knitted fabric, etc., and the fabric may be made of a rayon material or a spun yarn obtained by a polymer extrusion method, etc. And the guide 1 is preferably made of an elastic fabric. In order to reduce the influence of the guide 1 on the outer diameter of the cartridge 8 and avoid the difficulty of entering the tissue due to the excessive diameter of the cartridge 8, the thickness of the portion of the guide body 11 that is sleeved outside the cartridge 8 in the initial state is less than 0.1 mm, for example, the thickness of the portion may be between 0.01 mm and 0.1 mm. In other alternative embodiments, the thickness of the guide body 11 may have other values, and is not limited to this range.
In this embodiment, in order to further improve the guiding effect of the cartridge module and avoid scraping of the side walls of the viewing window 83 of the cartridge 8 against the tissue, the guide body 11 at least partially covers the viewing window 83. I.e., the distance from the upper side of the viewing window 83 to the distal side 81 of the cartridge 8 is less than the distance from the second end 112 of the guide body 11 to the distal side 81 of the cartridge 8 for better guidance and protection.
Fig. 5 and 6 are schematic views showing the structure of the guide 2 and the cartridge assembly according to the second embodiment of the present invention. In this embodiment, the guide 2 also includes a guide body 21 and a pull-out, the second end 212 of the guide body 21 being disposed outside of the cartridge 8 and preferably on the distal side 81 of the cartridge 8.
This embodiment differs from the first embodiment in that: the pull-out portion includes a pull cord 22 and a support member 23, and the pull cord 22 is connected to the guide body 21 via the support member 23. Specifically, the support member 22 is at least partially disposed within the internal cavity 84 of the cartridge 8, the distal side 231 of the support member 23 is connected to the first end 211 of the guide body 21, the proximal side 232 of the support member 23 is connected to the distal side 221 of the pull cord 22, and the proximal side 222 of the pull cord 22 extends out of the cartridge 8 through the window 83. In other alternative embodiments, the proximal side 232 of the support member 23 may extend outside of the cartridge 8 through the viewing window 83, and the pull cord 22 may be located entirely outside of the cartridge 8, i.e., both the distal side 221 and the proximal side 222 of the pull cord 22 may be located outside of the cartridge 8.
In this embodiment, under the action of the support 23, in the initial state, the first end 211 of the lead body 21 is located on the distal end side of the second end 212 of the lead body 21. Further, the diameter of the second end 212 of the guide body 21 is larger than the diameter of the first end 211 of the guide body 21, and the surface of the guide body 21 is a smooth surface, i.e. the guide body 21 is partially formed into a conical structure with a smooth outer surface, so as to better guide the movement of the cartridge assembly in the tissue. In other alternative embodiments, the guide body 21 may also be a cylindrical structure with the same diameter of the first end 211 and the second end 212, and the like, all falling within the scope of the present invention.
In this embodiment, the support member 23 is entirely smooth and streamlined. Further, the supporting member 23 is detachably disposed on the cartridge 8, and the supporting member 23 does not protrude from the outer side surface of the cartridge 8, so as to prevent the supporting member 23 from directly contacting with the tissue on the side surface to damage the tissue, when the anvil pull rod 9 moves towards the distal end side, the anvil pull rod 9 is engaged with the supporting member 23 to drive the supporting member 23 to separate from the cartridge 8, so that the supporting member 23 can be pulled towards the proximal end side under the action of the pull rope 22.
In the initial state, the doctor pulls the pulling rope 22 to the proximal side, the pulling rope 22 drives the first end 211 of the guide body 21 to move through the supporting member 23, the first end 211 of the guide body 21 moves to the proximal side first, and then drives the second end 212 of the guide body 21 to disengage from the outside of the cartridge 8 and enter the inner cavity 84 of the cartridge 8 along with the first end, and move to the direction of the proximal side 82 of the cartridge 8, i.e., enter the disengaged state. In the disengaged state, the pull cord 22 may also be pulled further so that the pull cord 22, the support member 23 and the guide body 21 are all pulled out of the cartridge 8 through the window 83 into the removed state. However, the removal state is not essential. The guide body 21 may optionally be made of a flexible deformable material and its portion that in the initial state is sleeved outside the cartridge 8 may optionally be elastic to grip the cartridge 8 in the initial state to avoid disengagement of the guide body 21 from the cartridge 8 when the cartridge assembly has not reached the surgical position.
Fig. 7 and 8 are schematic views showing the structure of the guide 3 and the cartridge assembly according to the third embodiment of the present invention. In this embodiment, the guide 3 also includes a guide body 31 and a pull-out portion. The second end 312 of the guide body 31 is sleeved outside the nail barrel 8. The pull-out portion includes the pull cord 32 and the support member 33, a distal end side 331 of the support member 33 is connected to the first end 311 of the guide body 31, a proximal end side 332 of the support member 33 is connected to the distal end side 321 of the pull cord 32, and the proximal end side 322 of the pull cord 32 protrudes outside the cartridge 8.
In this embodiment, the anvil pull rod 9 is disposed inside the cartridge 8, when the guide member 3 is in the initial state, the puncturing part 91 of the anvil pull rod 9 abuts against the proximal end side 332 of the supporting member 33 to jack the supporting member 33 to the distal side, and the first end 311 of the guide member body 31 is located on the distal side of the second end 312 under the supporting action of the supporting member 33, so as to better guide the movement of the cartridge assembly in the tissue. Further, the first end 311 of the guide body 31 has a smaller diameter than the second end 312 to form a tapered structure having a better guiding function.
When the cartridge assembly reaches the surgical position, as shown in fig. 8, the knob at the proximal side of the stapler is rotated such that the anvil pull rod 9 is moved a short distance towards the proximal side such that the distal side 91 of the anvil pull rod 9 is not in contact with the proximal side 332 of the support 33. At this time, there is a certain space between the distal end 91 of the anvil pull rod 9 and the proximal end 332 of the supporting member 33, so that when the doctor pulls the pulling rope 32 to the proximal end, the pulling rope 32 drives the guide body 31 to enter the cartridge 8 through the supporting member 33 and move toward the proximal end 82 of the cartridge 8, in the process, the first end 311 of the guide body 31 first moves to the proximal end, the second end 312 of the guide body 31 is driven to separate from the outer sidewall of the cartridge 8, and the guide body 31 at least partially enters the inner cavity of the cartridge 8, i.e., enters the separated state. In the disengaged state, the pull cord 32 may also be pulled further so that the pull cord 32, the support member 33 and the guide body 31 are all pulled out of the cartridge 8 through the window 83 into the removed state. However, the removal state is not essential. After the guide is removed, the knob is rotated again so that the anvil pull rod 9 is moved distally to protrude from the distal side of the cartridge 8 to pierce the tissue and complete the abutment with the anvil assembly.
The guide piece provided by the utility model can be independently placed and not mounted on the nail barrel part of the anastomat before being used, and can be selectively mounted by a user according to the needs, the operation is simpler, only the guide piece body needs to be mounted on the far end side 81 of the nail barrel 8, and the pull-out part passes through the inside of the nail barrel 8 and then extends out from the window 83; can also be directly arranged on the nail barrel part of the anastomat without being arranged by the user.
