Abstract

本实用新型属于脊柱侧弯矫形用的矫形棒,它用恢复温度在30-45℃的钛镍形状记忆合金制成。设计的矫形棒由带防矫形棒转动的防转装置及柱体构成。临床试验证明,使用这类矫形棒,操作方便,可有效矫正病人的侧弯度,术后不需石膏固定,无神经损伤,骨折等并发症,在术后随诊中无明显角度丢失。

Description

本实用新型属于外科矫形用器械。尤其与治疗脊柱侧弯的矫形器械有关。
特发性脊柱侧弯是一种严重的难以治疗的疾病,不仅体形难看,而且严重影响心脏等脏器的正常发育。脊柱侧弯实质为三相的畸变,除冠状面的侧弯外,还有在矢状面前压弯曲及横端面的扭转畸形。脊柱侧弯矫形的主要目的应按人体的三相,即冠状面(Z轴)、横断面(X轴)及矢状面(Y轴)纠正曲度畸形和稳定脊柱,达到骨性愈合以免畸形进一步发展。治疗脊柱侧弯的方法主要是利用矫形器械进行手术治疗。各种器械矫正脊柱畸形的作用力均是对椎间盘产生的弯矩力,弯矩越大,作用力越强,则纠正曲度越大,目前最常采用的是用由不锈钢制造的哈林顿棒和Lugue棒矫正手术。哈林顿棒由柱体和固定件组成。使用哈林顿棒的主要缺点是矫形力过大时易发生脱钩,支撑点骨折、术前需牵引、术后需打石膏外固定,而且对脊柱的扭转畸形几乎无纠正作用。Lugue棒有一些纠正扭转畸形的作用,但对Z轴的力则相对减少。这两种矫形棒的共同缺点是〔1〕矫正当时的矫形力最大,但以后矫形力有所减退,即术后角度丢失严重,如哈林顿棒的矫正力随生物组织伸展下降很快,术后20分钟下降20%,15天后降到初始矫正力的30%。〔2〕矫形棒会在矫正过程中发生旋转。
本实用新型的目的是要设计一种脊柱侧弯矫形棒,这种矫形棒可以克服上述已有技术的缺点。
本实用新型设计的矫形棒由用恢复温度在30-45℃之间的TiNi合金制造的两根柱体和防转卡组成或者两根带防转弯头的柱体组成。
下面结合附图对本实用新型作进一步描述
图1为本实用新型设计的一种弯曲矫形棒。

图1中的矫形棒是一种带防转卡的弯曲型矫形棒,它是用镍钛合金制成的,包括柱体和防转卡。柱体由直线部分和弯曲部分两部分组成,弯曲部分的弧度符合人体生理弧度要求,一般为20度左右,柱体一端均有可设置防转卡的长方槽。防转卡的结构见图4。用两根这样的矫形棒置于脊柱两侧,在一端的长方槽内装一个防转卡后用医用合金丝通过椎板或棘突根部捆扎,另一端圆槽用医用合金丝捆扎防止棒材向上串动然后加热,矫形棒恢复原状,推动脊柱变形矫正脊柱弯曲。
图2是一种带防转卡的S型矫形棒,这种矫形棒有两个弯曲部分组成,因而更符合人体生理弧度。棒的两端也有可放置防转卡的长方槽。因此在矫形过程中不会发生矫形棒旋转,另一端圆槽防止棒串动。
这两种型式的矫形棒由于在两端设置了防转卡,因此在矫形过程中防止矫形棒旋转效果极好,而且防转卡的结构简单,使用方便。矫形棒的弯曲程度视病人侧弯程度而定,柱体的直径(一般为φ6-7.8mm),长度(一般为250-350mm)也要视病人的年龄,身高等具体情况选定。
图3是一种框架型矫形棒,也称为L型矫形棒,这种矫型棒的防转动部件是和柱体成一体的。矫形时需两根棒配合使用。这种结构的矫形棒由符合人体生理弧度要求的弯曲部分,直线部分和防转动部分组成,其中一根棒的直线部分的一端有供搭接的半柱面,弯曲部分一端有可防转动的L型弯头,弯头的端部也有供搭接的半柱面。另一根棒的弯曲部分端有供搭接的半柱面,直线部分端有可防转动的L型弯头,弯头的端部也有供搭接的半柱面。将两根矫形棒置于脊柱两侧,其弯曲部分正好符合人体生理弧度,将两端的供搭接的半柱面搭接,即将一根矫形棒的L型弯头的搭接面搭在另一根的直线部分的搭接半柱面,其直线部分的搭接面搭在另一根的L型弯头的搭接面上,然后按上述方法用医用合金丝捆扎紧。这种结构的矫形棒也有防转动效果,而且又可防止矫形棒上下串动。
矫形棒的具体使用方法是:手术前,先按照患者脊柱X线侧位片测得脊柱曲度,以确定矫形棒的弯度,将加工好的矫形棒清洗放入金属盒内高压灭菌后置于低温冰箱内冷冻备用。让患者俯卧于脊柱矫形托架上,全麻后,取脊中线切口,将骶棘肌向两侧推开,广泛显露畸形段的椎板及小关节突,以利矫形。将矫形棒在低温状态下按照脊柱的侧凸度弯成相应曲度,将捆扎的医用合金丝由椎板中央深面穿过,将两根矫形棒穿套在合金丝袢内,置于两侧椎板上,拧紧合金丝。再用45-50℃盐水纱布热敷于矫形棒上,同时手法推拉及牵引躯干以帮助矫正脊柱畸形,因矫形棒受热达到相变温度,很快便恢复到原有形状,故对所固定的脊柱产生水平矫形力,且牢固固定。术后不需要用石膏固定。32例临床脊柱侧弯手术中,病人年龄为11-26岁,侧弯比较重,伴有心脏等功能不全等症状,经手术矫正后,脊柱侧弯得到部分纠正。术前原发侧弯角度平均53.8度,术后角度平均25.8度,纠正角度平均25.9度,纠正角度百分比平均51.8%,术后平均增高3.9cm,病人经手术后心肺功能明显改善,不仅纠正胸廓畸形,而且劳累后的心慌气短现象也有所改善。术后无神经损伤等并发症,外形基本得到纠正,在随诊中无明显矫正角度变小情况,即术后角度无明显丢失,其中有三例病人术后一个月的X线片所测角度与术后脊柱矫形角度相比还有8-10度的改善,在37℃恒温环境中进行模拟人体体温条件下测定矫形棒的恢复力试验也证明,矫形棒在加温到相变温度后,在37℃情况下仍保持有一定的水平作用力,因此术后的矫形棒对脊柱仍然持续产生矫形力。

Claims (2)

1、一种由柱体和固定件组成的矫形棒,其特征是:
[1]所说的固定件是一个防转卡,
[2]所说的柱体有两根,每根都带有至少一个符合人体生理弧度要求的弯曲部分,所说的两根柱体两端均有设置防转卡的长方槽,另一端有防止向上串动的圆槽,
[3]所说的柱体用恢复温度在30-45℃间的钛镍记忆合金制成,防转卡采用同种材料制成。
2、一种由柱体组成的矫形棒,其特征是:
〔1〕所说的柱体有两根,每根都有直线部分、弯曲部分、防转弯头部分组成,所说的柱体中的一根柱体的防转弯头与弯曲部分相邻,另一根柱体的防转弯头与直线部分相邻,
所说的柱体的两端均有供两根柱体搭接用的半柱体,
〔2〕所说的柱体用恢复温度在30-45℃间的钛镍记忆合金制成。
