Abstract
本实用新型涉及可视通气喉镜。本实用新型采取如下技术方案:包含面罩,面罩上有与可视通气手柄配合的软质垫圈,可视通气手柄内含有通气管腔、可视结构,两者相邻、并行,成为整体。所述可视结构上端可与显示屏相连接或分离;所述通气管上、下端各有一通气口,可通气。采取如上技术方案的本实用新型,具有如下有益效果:本实用新型利用可视技术,使通气过程在可视条件下进行,便于明确梗阻部位,解除气道梗阻,且只是将现有可视喉镜手柄部分及传统麻醉紧闭面罩稍加改造,联合使用,结构简单、操作方便。

可视通气喉镜
技术领域:
[0001] 本实用新型涉及ー种麻醉用可视喉镜,尤其是困难气道时的可视通气喉镜。
背景技术:
[0002] 对困难气道的处理是否妥当,是麻醉安全和质量的关键。中华医学会麻酔分会专家组提出的“困难气道管理专家意见”(以下称“专家意见”)中指出:50%以上的严重麻酔相关并发症是由气道管理不当引起的,而其中面罩通气困难更是临床麻酔工作中遇到的最为紧急和严重的问题之一。如果发生这种情况,就无法实施有效人工通气,患者在短时间内就可因缺氧而导致心跳骤停,大脑损害甚至死亡。而“专家意见”中推荐了四种解决急症气道(困难面罩通气兼有困难气管插管)的工具,但这些工具在使用过程中均存在一定的不足之处,现分述如下:
[0003] I、面罩正压通气:即置入口咽或鼻咽通气道后双人面罩加压通气,而在日常的エ作中,如遇到严重的面罩通气困难时,这种方法通气效果并不十分理想,可能的原因是:ロ咽或鼻咽通气道放入后的位置相对固定,而在扣上面罩后又不能对它们的位置进行调整,对声门上呼吸道梗阻的解除作用有限。
[0004] 2、喉罩:是目前被广泛接受的最主要的声门上气道工具。它操作简便,不需要喉镜辅助,训练有素的医师可以在几秒内置入喉罩建立通气,但正如“专家意见”中指出的喉罩的缺点是:病人的张ロ度须大于3cm,并且咽喉结构要正常。
[0005] 3、食管-气管联合导管:此联合导管临床使用很少,大多基层的麻酔人员甚至未见过此联合导管的实物。一旦遇到困难气道时,会因使用的熟练程度不够而延误时机,导致患者的抢救失败。
[0006] 4.环甲膜穿刺置管和通气装置:这种方法为一有创方法,容易损伤气道,仅为临时措施。它只能用于供氧或接高频通气机,很容易导致ニ氧化碳潴留,需要快速建立后续方法,以解决通气。

[0008] 为了克服现有可视喉镜在处理困难气道过程中只能可视插管,而不能用于前期诱导通气这ー不足,本实用新型提供一种可视通气喉镜,该喉镜能够在遇到困难气道时,既能通气,又能利用可视技木,明确梗阻部位,解除气道梗阻,达到可视喉镜兼有通气功能这ー目的。
[0009] 为了达到如上目的,本发明采取如下技术方案:包含面罩、面罩上有与可视通气手柄配合的软质垫圈,可视通气手柄内含有通气管腔和可视结构,通气管腔与可视结构相邻、并行,成为整体。

[0026] 首先将传统的麻醉紧闭面罩进行改造,去除上端与麻醉螺纹管连接部分的硬质塑料接ロ,以软质垫圈代替,再将现有的可视喉镜的手柄部分进行改造,成为可视通气手柄2,其手握部分为圆柱状,外径与改造后的面罩上端软质垫圈的内口径相吻合,以达到套入时可滑动且密闭的效果。另外,在手柄内部増加一个通气管腔7,与原有可视结构6相邻、并行,上、下两端各开一通气ロ,可进行通气,这样,改造后的手柄,上端除可与显示屏I相连接或分离外,増加的通气管腔可通过上通气开ロ 4与麻醉螺纹管相连接,前端入口腔部分除増加的下通气开ロ 5タト,型似现有可视喉镜片样。在临床麻醉过程中,如遇困难气道,尤其是急症气道吋,应立即将可视通气手柄置于患者ロ中,动作与使用现有可视喉镜相似,然后从可视通气手柄上端套入备好的面罩,井分别连接可视显示屏及麻醉螺纹管,下压通气面罩,使面罩与患者面部紧密结合,然后轻轻上提可视通气喉镜,助手辅助通气,井根据可视通气喉镜看到声门上呼吸道是否通畅及梗阻部位,调整可视通气喉镜的前后角度及置入口中的深浅度,以达到良好的通气效果为止。
