Abstract

Description
技术领域
本发明涉及监测孕妇分娩过程的方法。
发明背景
当前分娩方法的一个问题是妇女在产程中遭受难产的痛苦。这能导致分娩不像预期的那样进行,而且产程也被延长,不能成功地自然分娩。孕妇可能变得失望并可能需要使用某些方法例如真空吸引、产钳或剖腹产来分娩婴儿。孕妇的难产也可能使胎儿损伤和疲劳。
在过去就已经对胎儿血液中的乳酸盐浓度进行测定来控制胎儿不遭受缺氧的痛苦。然而,胎儿中的乳酸盐浓度不能指示孕妇的状况。因此需要在早期更有效地确定和控制患有难产的妇女的状况,以避免在使用手术和其它分娩方法前的不必要产程。
发明概述
本发明的方法提供了对上述问题的解决方案。更特别的是,该方法用于监测孕妇的分娩过程。在第一个测定步骤中,测定阴道液的第一乳酸盐浓度。在比较步骤中,确定所测定的乳酸盐浓度是否大于能表明羊水已经从孕妇的羊膜中流出且羊膜已经破裂的预定乳酸盐浓度值。在第二个测定步骤中,测定第二个乳酸盐浓度。在第二个比较步骤中确定所测定的第二个乳酸盐浓度是否大于乳酸盐的阈值区间。当第二个乳酸盐浓度小于乳酸盐的阈值区间时,就在刺激步骤中刺激孕妇分娩。当第二个乳酸盐浓度大于乳酸盐的阈值区间且产程没有正常进行时,则可以对妇女使用其它的分娩选择例如手术分娩,以防止不必要的痛苦和延长分娩的耗力。

发明详述
参考图1,本发明的方法10包括测定液体例如阴道液中乳酸盐浓度15a的测定步骤12,并结合怀孕期来确定羊水是否已经流出或正在从羊膜流出的过程中。通常,孕妇的子宫肌肉能产生乳酸盐,所以可以测定阴道液的乳酸盐浓度来测定由子宫肌肉产生的乳酸盐量。非妊娠妇女阴道液中经常没有或有很少的乳酸盐。
如果乳酸盐浓度15a高于预定的乳酸盐浓度13,例如为4-5mmol/l,更优选高于4.5mmol/l,如比较步骤14所示的那样,那末就可以得出结论,膜已经破裂且羊水很可能已经流出,分娩产程很可能在等候期16后就开始了。可以理解,4-5mmol/l是用于多数妇女的例证性实例,本发明不限于用在实施例中的值。
如果乳酸盐浓度低于4.5mmol/l,那末高度可能的是羊水仍然保留在羊膜腔中。乳酸盐的浓度可以在等候期18后在测定步骤20中再次测定。在比较步骤14中再次确定乳酸盐浓度是否大于或小于4.5mmol/l。如果乳酸盐浓度还低于4.5mmol/l,那么可以稍后进行第二次测定,该测定可以在适宜的时间间隔内重复直至乳酸盐浓度超过4.5mmol/l或很明显羊水已经流出。
如以上指出的那样,如果在测定步骤12中测定的乳酸盐浓度在4.5mmol/l以上,那么接下来的步骤就是等候约两天左右以观察妇女是否自己开始分娩了。在确定步骤22中,确定产程是否已经开始。如果产程已经开始并正常进行,那么就可以开始分娩过程24。如果在确定步骤22中确定产程没有开始或产程没有正常进行,那么就在测定步骤26中测定乳酸盐的浓度15b。
然后在比较步骤28中确定在测定步骤26中测定的乳酸盐浓度15b是否在乳酸盐阈值区间29内,其可以是约8-10mmol/l。如果在步骤26中测定的乳酸盐浓度不在阈值区间29内,那么就在比较步骤30中确定乳酸盐浓度是否小于阈值区间29或约8mmol/l。如果在步骤26中测定的乳酸盐浓度大于阈值区间29那么可以开始等待步骤32,例如几个小时,以观察产程过程是否正常。如果产程没有正常进行,那么可以考虑其它的分娩选择例如剖腹产、产钳或使用连接真空的吸盘来拉出婴儿。本发明的一个重要特征是可以使用对乳酸盐浓度的监测来预测妇女是否可能进行正常分娩或不能,而不用强迫孕妇经受长时间且痛苦难忍的努力去进行分娩。因此可能的是在相对早的阶段内就使用其它分娩选择。应当理解,8-10mmol/l是用于多数妇女的例证性实例,本发明并不限于用在实施例中的值。
如果在步骤26中测定的乳酸盐浓度小于阈值区间29,那么就可以在刺激步骤34中用药物或通过其它帮助刺激妇女分娩。在确定步骤36中,可以确定产程是否正常进行。如果产程正常进行,那么妇女就可以进行分娩38。如果产程没有正常进行,那么可以在测定步骤26中再次测定乳酸盐浓度,并在比较步骤28中继续该过程,如以上所述的那样。
如果在比较步骤28中确定,在步骤26中测定的乳酸盐浓度在阈值区间29内,例如在8-10mmol/l之间,那么就在确定步骤40中确定产程是否正常进行。如果产程正常进行,那么妇女就可以进行分娩42。如果产程没有正常进行,那么就可以在刺激步骤34中刺激妇女分娩,并继续进行确定步骤36,如以上所述的那样。
可以继续各种操作回路直至妇女自己分娩或接受其它分娩操作。如以上所述的那样,本发明的一个重要特征是在使用其它分娩操作前可以使妇女免于痛苦难忍且漫长的分娩努力。当乳酸盐浓度指示子宫肌肉在不产生自然分娩的乳酸盐阈值之上工作时,也可以在较早的阶段使用其它的分娩操作。
虽然已经根据优选的组合物和实施方案对本发明经行了描述,但是我们可以理解,可以对其进行某些替换和改变而并不背离以下权利要求的精神和范围。
Claims (10)
1.监测孕妇分娩过程的方法,其包括:
在测定步骤(26)中,测定乳酸盐的浓度(15b);
在比较步骤(28)中,确定所测定的乳酸盐浓度(15b)是否大于乳酸盐的阈值区间(29);和
当乳酸盐浓度(15b)大于乳酸盐阈值区间(29)时,对孕妇进行其它分娩操作(32)。
2.根据权利要求1的方法,其中该方法还包括当乳酸盐浓度(15a)大于乳酸盐浓度(13)时,等待一个等候期(16)。
3.根据权利要求1的方法,其中该方法还包括当乳酸盐浓度(15a)小于乳酸盐浓度(13)时,等待一个等候期(18),并再次在测定步骤(20)中测定乳酸盐的浓度。
4.根据权利要求2的方法,其中该方法还包括在等候期(16)后的确定步骤(22)中确定产程是否正常进行。
5.根据权利要求4的方法,其中该方法还包括当在确定步骤(22)中确定产程没有开始或没有正常进行时,测定乳酸盐浓度(15b)。
6.根据权利要求5的方法,其中该方法还包括当乳酸盐浓度(15b)小于乳酸盐阈值区间(29)时,在刺激步骤(34)中刺激孕妇分娩。
7.根据权利要求1的方法,其中该方法还包括在确定步骤(36)中确定产程是否正常进行。
8.根据权利要求7的方法,其中该方法还包括当在确定步骤(36)中确定产程没有正常进行时,测定乳酸盐的浓度(15b)。
9.根据权利要求1的方法,其中该方法还包括当乳酸盐浓度(15b)在乳酸盐的阈值区间(29)内时,在确定步骤40中确定产程是否正常进行,以及当在确定步骤中确定产程不正常时则对孕妇提供刺激。
10.根据权利要求1的方法,其中该方法还包括在测定步骤(12)中测定阴道液中乳酸盐的浓度(15a);在比较步骤(14)中,确定所测定的乳酸盐浓度是否大于指示膜已经破裂且羊水已经从孕妇的羊膜腔中流出的预定乳酸盐浓度(13)。
