Abstract

本发明提供了一种人脑血管血液动力学参数的正常值及检测方法。本发明采用脑血管血液动力学分析模型及脉膊波在脑动脉系统中的传播和反射规律分析而提出利用在颈总动脉处用超声多普勒探头测量流速及其压力脉膊波信息计算脑血管血液动力学指标的方法。它通过检测脑血管功能的改变程度来反映脑血管的受损情况,所得参数有较好的临床意义。

Description

人脑血管血液动力学参数的正常值及检测方法
本发明涉及医学检测方法,具体涉及一种人脑血管血液动力学参数的正常值及检测方法。
中风具有高死亡率、高复发率和高致残率的特点,它严重危害着人类的健康和生命。据报告(1986年),我国有中风患者约300万,每年新发生中风132万,每年因中风死亡97万。上海是中风高发区,预计1998年新发中风患者达3万人以上,根据长海医院的统计,一个中风病人急诊期的月住院费用约一万元人民币,据此计算,上海每年因中风造成的直接损失超过3亿元人民币。可想而知,全国的损失有多大。中风的威胁如此之大,关键是及早预防。为此,我们进行了中风监测和预防的研究。我们的研究证实中风实际上同其他疾病一样,也是由平日各种病理损害不断累积形成,它的发生最终是通过各种中风危险因素对脑血管的损害所导致的。如果能尽早发现这些危险因素并对其危险性进行评估,及时采取相应有效的措施,切断疾病发生、发展的病理基础,那么就有可能防止中风的发生。
我们的研究表明,脑血管病血液动力学的某些指标是脑血管功能的直接反映,而中风又是脑血管功能低下或损伤的结果,中风高危个体的筛选可依据脑血管血液动力学检测结果。依据中风危险因素(老年、高血压、心脏病、糖尿病、中风家族史、高血压家族史)在40岁以上中选取中风易患人群,再通过测量其个体的脑血管血液动力学状态并根据其指标积分值确定中风干预对象。
本发明的目的在于通过检测脑血管功能的改变程度来反映脑血管的受捐情况,检测人脑血管血液动力学参数的正常值。
本发明提供了一种人脑血管血液动力学参数的正常值及检测方法。
本发明采用脑血管血液动力学分析模型及脉搏波在脑动脉系统中的传播和反射规律分析而提出利用颈总动脉处用超声多普勒探头测量流速及其压力脉博信息计算血管血液动力学指标的方法和研制的脑血管功能检测仪。
脑血管血液流动力学参数分运动学参数(流速、流量等)和动力学参数(压力、阻抗等)两种,因较多的检测集中在后者,常通称为脑血管血液动力学参数。现有的脑血管血液动力学分析仪大多可检测和分析计算出下列脑血管血液动力学指标:
①颈总动脉血流速度的最大值Vmax、最小值Vmin、平均值Vmean
②颈总动脉的平均血流量Qmean
③脑血管外周阻力Rv
④脑血管特性阻抗Zcv
⑤脑血管脉搏波波速WV
⑥脑血管扩张度指标DI
⑦临界压CP
⑧舒张压与临界压差DP
⑨动态阻力DR以上所检测指标按血液动力学和生理学含义可分为以下几类:①反映脑血管床供血状态的指标:平均血流量Qmean
Qmean是表示单位时间内流过颈动脉检测点横截面的血液体积。它主要反映该侧颈动脉系统血管床的供血状况。如果检测出血流量偏低,表示该侧脑供血不足;如果检测出血流量偏高,表示该侧脑供血过度,或者是由于其它血管床供血不足,靠这一侧颈动脉来补偿。
影响脑血流量的因素较多,不仅脑血管疾病对它有影响,而且全身系统的其他一些疾病如心脏病、高血压等对它也会有一定的影响。②反映脑血管血流状况的指标:颈总动脉血流速度最大值Vmax、最小值Vmin、和平均值Vmean
血流速度是反映血液在血管中流动的运动学指标,和血流量一样,不仅脑血管疾病会导致血流速度的异常改变,而且心血管系统的一些疾病也会对它产生影响。一般说,平均血流速度Vmean偏低,往往提示该侧脑血管床供血不足,如果病人左右两侧流速同时减小,而脑血管的其它动力学指标都正常,那么,影响血流速度降低的因素可能主要是其它系统的病变,如冠心病或其它心血管疾病,此类病人需要作进一步检查明确诊断;如果病人血流速度的改变是左右两侧严重不对称,那么往往提示患者脑血管动力学状态有异常改变,具体病因需进一步根据动力学指标的变异情况来判断。
平均血流速度偏高,往往提示该侧脑血管床供血过度,或者该侧颈动脉严重狭窄。对血流速度偏高的病人,在排除颈动脉严重狭窄后,应考虑颅内血液循环异常,如Willis环的一些交通支异常开放等。临床实践发现,许多椎—基底动脉供血不足的病人往往具有这种特征。
血流速度最大值Vmax和年龄关系非常大。对于正常人,随着年龄的增加,Vmax将显著降低。这一生理性的改变,反映了人体循环系统随年龄的增加逐渐衰老。因此,它也可作为判断人体心脑血管循环系统衰老程度的一个定量指标。
血流速度最小值Vmin的显著减小,往往提示该侧脑血管床有痉挛或梗塞等微循环障碍,尤其是对于Vmin小于4cm/s的病人,需要特别注意,对这类病人,即使CT检查结果呈阴性,有条件时仍应进行MRI检查,进一步查明病因,无条件时可作为中风高危病人,或血管严重痉挛病人进行预防性治疗,在治疗过程中应进行随访跟踪,观察血液动力学状态的变化情况,不断调整治疗方案,直至动力学参数基本恢复正常。③反映脑动脉弹性状况的指标:脑动脉脉搏波波速WV、脑血管特性阻抗Zcv、和扩张度指标DI
WV是压力脉搏波沿血管壁传播的平均速度;Zcv是脉博波在血管床中沿一个方向传播时所遇到的阻抗;扩张度指标DI是量度脑血管可扩张度的参量。这三个指标和动脉管壁的整体弹性状况相关,而和血流速度几乎无关。动脉总体弹性越好,脉搏波波速越低,特性阻抗也越小,扩张度指标则越大;脉搏波波速越高,或特性阻抗越大,或扩张度指标越小,则说明被检测动脉管壁的整体弹性越差。因此,WV、Zcv和DI可以作为反映被检测血管弹性特性的定量指标,它们的变化和血管硬化或血管痉挛程度有密切的关系。④反映脑血管微循环状态的指标:血管外周阻力Rv
外周阻力是反映小血管和毛细血管通畅程度的定量指标。小血管梗塞、管腔变窄或血管痉李都将导致血管外周阻力Rv异常升高。
对大多数脑血管病人来说,在发病前期和发病的急性期,其动力学状态都有显著的异常改变,而病人的代偿功能往往还没有建立,因此这时动力学参数的异常变化对脑血管疾病的诊断(特别是早期诊断)非常有用。对脑梗塞病人,由于小血管管腔变窄、阻塞,外周阻力将异常升高;对脑出血病人,特别是蛛网膜下腔出血病人,由于小血管痉挛,外周阻力也往往会异常升高。但是值得注意的是,对单侧脑出血病人,病侧外周阻力往往会显著高于健侧;而对脑梗塞病人,往往双侧指标都会有异常升高,特别是在发病前期。⑤反映调节脑血流难易程度的指标:动态阻力DR
由于脑血管的自身调节功能,在正常情况下,随着平均血压的波动,平均血流量将维持在一个基本恒定的范围之内。但在一个心动周期之内,由于心脏的间歇性射血,血压在不断地变化着,随着血压的升高或降低,血流速度也将随之变化,动态阻力DR即为描写血流速度随血压变化的难易程度的指标。动态阻力DR越大,表示在同样的压力变化时血流速度变化得越小;动态阻力DR越小,则表示在同样的压力变化时血流速度变化得越大。⑥反映颅内血管闭锁状态的指标:临界压CP及舒张压与临界压之差DP
临界压CP是维持血液在血管中运动所需的最小压力,DP是舒张压与临界压之差。导致临界压CP异常升高的原因很多,如:动脉过度灌注,静脉回流受阻,颅内压升高等等。在临床中,DP的异常降低往往提示病人在每个心动周期的末期,脑组织可能会有短暂性的供血不足。长此以往,有可能导致脑细胞的营养不足、脑组织的损伤和早衰等。临床实践表明,对高血压病人,特别是晚期动脉硬化症患者,在采取降压措施时极易产生脑供血不足。为了防止这种情况发生,可将DP作为合理降压的监测指标,如果降压过程中DP显著降低时,应考虑停止降压,或采取其他措施以保证脑血管有足够的供血量。
本发明是按下列方法具体实施的:
1正常人标准及样本人群分布
我们采用同一质控标准,在国内东北、华北、华东、华南、西南、西北等六个地区筛选了4435例正常对照者,测定了其脑血管血液动力学参数(Cerebrovascular Hemodynamic Indices,CVDI)。本节叙述我国正常个体的CVDI的变化规律,并给出了其正常参照范围,为CVDI的应用推广提供参考依据。
1.1正常人标准与检测方法
正常观察对象的筛选标准:血压、血糖、血脂、心电图等指标均正常,无任何心脑血管疾病、肺部疾病及脑部其他疾病。
所有正常观察对象均由同一个神经科主任医师确定,检测自始至终由同一熟练的技术人员进行操作,以尽可能减小系统误差,保障数据的准确可靠。
检测设备为CBA-300型脑血管血液动力学分析仪(上海麦登电子设备有限公司生产)。受试对象充分休息放松后取平卧位,用多普勒超声探头(2.5MHz)和应变式压力探头,检测双侧颈动脉血流速度及脉搏压力变化情况,根据仪器内置的脑血管血液动力学分析模型(复旦大学生物力学研究室研制),对检测信号进行分析,得到脑血管各种血液动力学参数,包括脑血流平均流量(Qmean,单位:cm3/s)、脑血流平均流速(Vmean,单位:cm/s)、最大流速(Vmax,单位:cm/s)及最小流速(Vmin,单位:cm/s)、脉搏波波速(WV,单位:m/s)、外周阻力(Rv,单位:kPa*s/m)、特性阻抗(Zcv,单位:kPa*s/m)、动态阻力(DR,单位:kPa*s/m)、临界压(CP,单位:kPa)、临界压与舒张压之差(DP,单位:kPa)等。
1.2样本人群分布
该研究共选取正常检测对象共14654例(男7233例,女7421例),其中东北地区696例(男426例,女270例),华北地区1222例(男859例,女363例),华东地区8062例(男3494例,女4568例),华南地区1212例(男672例,女540例),西南地区543例(男357例,女186例),西北地区2919例(男1425例,女1494例)。年龄范围3~87岁,平均47.65岁。研究对象的性别及年龄分布见表1。

1.3正常值分组依据
脑血管各种血液动力学参数随年龄变化,有左右侧和性别的差异已为临床证实,而无地区等差异也是医学界的共识,因此对正常观察对象,分性别和左右侧以5岁为一年龄组,分别进行计算。给出正常参考值的均值及其方差。

脑血管血液动力学检测方法是一种脑血管功能的无创伤检测方法,它通过检测脑血管功能的改变程度来反映脑血管的受损情况,所得到的参数有较好的临床意义,且方法简便、无创无痛,因此近年来在脑血管疾病的防治方面得到愈来愈广泛的应用。
我们用复旦大学生物力学研究室建立的脑血管血液动力学分析模型(弹性腔模型)及脉搏波在脑动脉系统中的传播和反射规律分析而提出利用在颈总动脉处用超声多普勒探头测量流速及其压力脉搏波信息计算脑血管血液动力学指标的方法和研制的脑血管功能检测仪。脑血管血液动力学参数是直接反映脑血管功能的定量指标,检测脑血管血液动力学参数无论是对脑血管疾病的早期诊断、脑血管疾病治疗措施和药物疗效的客观评价及合理筛选,或是对于脑循环的生理、病理学研究都有十分重要的意义,已为上海、北京、西安、浙江及河南等数十家医院对数万名神经内科和脑外科患者的临床应用所证实。

1、一种人脑血管血液动力学参数的正常值及检测方法,其特征在于该正常值及检测方法是采用脑血管血液动力学分析模型及脉膊波在脑动脉系统中的传播和反射规律分析而提出利用颈总动脉处用超声多普勒探头测量流速及其压力脉博信息计算血管血液动力学指标的方法和研制的脑血管功能检测仪。
2、根据权利要求1所述的一种人脑血管血液动力学参数的正常值及检测方法,其特征在于其中所述的人脑血管血液动力学参数的正常值是:

我们采用同一质控标准,在国内东北、华北、华东、华南、西南、西北等六个地区筛选了4435例正常对照者,测定了其脑血管血液动力学参数(Cerebrovascular Hemodynamic Indices,CVDI),本节叙述我国正常个体的CVDI的变化规律,并给出了其正常参照范围,为CVDI的应用推广提供参考依据,
(1)正常人标准与检测方法
正常观察对象的筛选标准:血压、血糖、血脂、心电图等指标均正常,无任何心脑血管疾病、肺部疾病及脑部其他疾病,
所有正常观察对象均由同一个神经科主任医师确定,检测自始至终由同一熟练的技术人员进行操作,以尽可能减小系统误差,保障数据的准确可靠,
检测设备为CBA-300型脑血管血液动力学分析仪(上海麦登电子设备有限公司生产),受试对象充分休息放松后取平卧位,用多普勒超声探头(2.5MHz)和应变式压力探头,检测双侧颈动脉血流速度及脉搏压力变化情况,根据仪器内置的脑血管血液动力学分析模型(复旦大学生物力学研究室研制),对检测信号进行分析,得到脑血管各种血液动力学参数,包括脑血流平均流量(Qmean,单位:cm3/s)、脑血流平均流速(Vmean,单位:cm/s)、最大流速(Vmax,单位:cm/s)及最小流速(Vmin,单位:cm/s)、脉搏波波速(WV,单位:m/s)、外周阻力(Rv,单位:kPa*s/m)、特性阻抗(Zcv,单位:kPa*s/m)、动态阻力(DR,单位:kPa*s/m)、临界压(CP,单位:kPa)、临界压与舒张压之差(DP,单位:kPa)等,
(2)样本人群分布
该研究共选取正常检测对象共14654例(男7233例,女7421例),其中东北地区696例(男426例,女270例),华北地区1222例(男859例,女363例),华东地区8062例(男3494例,女4568例),华南地区1212例(男672例,女540例),西南地区543例(男357例,女186例),西北地区2919例(男1425例,女1494例),年龄范围3~87岁,平均47.65岁,研究对象的性别及年龄分布见表1,

(3)正常值分组依据
脑血管各种血液动力学参数随年龄变化,有左右侧和性别的差异已为临床证实,而无地区等差异也是医学界的共识,因此对正常观察对象,分性别和左右侧以5岁为一年龄组,分别进行计算,给出正常参考值的均值及其方差。
