本发明设及一种人造贲门,由管腔(1)、固定圈(2)、膜瓣(3)及膜裙(4)构成;固定圈(2)包覆在管腔(1)外表面上;膜瓣(3)呈弧面形,位于管腔(1)内,与管腔(1)内表面相连,其凸面对着管腔(1)的进口,凹面对着管腔(1)的出口;膜裙(4)呈管状,其一端与膜瓣(3)连为一体,另一端伸出管腔(1);在正常情况下,膜瓣(3)及膜裙(4)可通过食物,同时可防止返流。在患者进形食管各段肿瘤切除,贲门胃底切除和胃肠代替食管手术后将本发明植入吻合口,可有效防止吻合口瘘、吻合口狭窄及返流性食管炎等三大术后并发症的发生。

人造贲门
                              技术领域
本发明涉及一种人体器官代用品,具体地说是一种人造贲门。
                              背景技术
目前,无论是国内还是国外,胸外科所进行的食管肿瘤切除、贲门胃底肿瘤切除、胃肠代替食管等手术一直存在三大无法解决的难题:即患者术后易发生吻合口痿、吻合口狭窄及返流性食管炎。尽管医学界不断改进吻合方法,发明了各种类型的吻合器,使上述三大术后并发症的发病率有所下降。目前,国内使用吻合器进行吻合已使痿的发生率从5-7%降至2-4%,但吻合口狭窄和返流性食管炎仍是常见且相当严重的并发症。这些并发症的存在或者直接威胁受术者的生命,或者严重影响其术后的生活质量。
                              发明内容
本发明要解决的技术问题就是患者在进行食管各段肿瘤切除、贲门胃底切除和胃肠代替食管手术后易发生吻合口痿、吻合口狭窄及返流性食管炎等术后并发症的问题。
为解决上述技术问题,本发明提供了一种人造贲门,它由管腔、固定圈、膜瓣及膜裙构成,固定圈包覆在管腔外表面上,膜瓣呈弧面形,位于管腔内,与管腔内表面相连,其凸面对着管腔的进口,凹面对着管腔的出口;膜裙呈管状,其一端与膜瓣连为一体,另一端伸出管腔;在正常情况下,膜瓣3及膜裙4可通过食物,同时可防止返流。
本发明结构简单,制作方便,安全可靠,在患者进行食管各段肿瘤切除,贲门胃底切除和胃肠代替食管手术后植入吻合口,可有效防止吻合口瘘、吻合口狭窄及返流性食管炎等三大术后并发症的发生。

                        具体实施方式
参见各附图,本发明之管腔1由管状骨架11和管状被膜12构成,骨架11被封在被膜12内。所述骨架11由两段展开后均为连续性的“Z”宇型管状网架构成,该“Z”字型网架由医用不锈钢丝制成,其两端各个端点的连线形成一个正多边形,一般为正六边形—正十边形,本实施例为正八边形。两段网架相邻两端的各个端点均位于同一圆周上且相互交错排列,两者之间通过尼龙丝穿接成一体。整个骨架11被由医用硅橡胶薄膜制成的被膜12内外封膜后即形成一有固定形状和恒定弹力的管腔1。管腔1的直径是中间小两头大,两端端部形成喇叭口,中部直径为14-17MM,上下口即两端直径为22-25MM,长度为30-50MM。
本发明之固定圈2由医用涤纶材料制成,包覆在管腔1被膜的外表面上,其宽度为15-25MM。为了增大固定圈2与人体组织的接触面,可将固定圈2的表面做成螺旋状。
本发明之膜瓣3由医用弹性材料制成,呈弧面状,覆合在管腔1被膜的内表面上,其凸面对着管腔1的进口,凹面对着管腔1的出口。膜瓣3厚度为0.2-0.5MM,数量一般为2--6个,本实施例为3个。
本发明之膜裙4亦由医用弹性材料制成,其一端与膜瓣3覆合为一体,另一端伸出管腔1。其长度为35-55MM,厚度为0.08-0.15MM.直径为14-17MM。为了使膜瓣3的回弹力很好的作用到膜裙4上,覆合时应将膜瓣3拉平后完整的覆合在膜裙4上。这样,当本发明处于自然状态时,在膜瓣3回弹力的作用下,膜裙4伸出管腔1的一端是闭合的。
由于固定圈2是用医用涤纶材料制成,与人体组织有较好的亲合性,且管腔1的两端均呈喇叭口,因此本发明植入吻合口后能够很好的定位。
本发明的工作过程如下:在一般情况下,本发明之膜裙4呈闭合状。当人体进食时,食物下行到本发明的管腔1内时,食物的重力作用到膜瓣3上,使其向下打开,食物即可通过膜瓣3和膜裙4进入胃内。当返流时,由于返流物对膜裙4的压力和膜瓣3自身的回弹力的方向一至,这样膜裙4就在返流物压力和膜瓣3回弹力的双重作用下封闭并回缩至管腔1内,严密的封堵住返流物质,隔绝返流物质与食管的接触。当呕吐时,由子压力猛然加大,推动膜裙4迅速向上滑行翻起,并带动膜瓣3一起向上打开,此时膜瓣3完全失效,胃与食管之间畅通无阻。呕吐后,通过次水或进食,即可使膜裙4在水或食物重力的作用下下滑,并带动膜瓣3复位。

Claims (7)

1、人造贲门,由管腔、固定圈、膜瓣及膜裙构成,固定圈包覆在管腔外表面上,其特征在于膜瓣(3)呈弧面形,位于管腔(1)内,与管腔(1)内表面相连,其凸面对着管腔(1)的进口,凹面对着管腔(1)的出口;膜裙(4)呈管状,其一端与膜瓣(3)连为一体,另一端伸出管腔(1);在正常情况下,膜瓣(3)及膜裙(4)可通过食物,同时可防止返流。
2,如权利要求1所述的人造贲门,其特征在于管腔(1)由管状骨架(11)及管状被膜(12)构成,骨架(11)被封在被膜(12)内。
3、如权利要求2所述的人造贲门,其特征在于骨架(11)由两段展开来为连续性的“z”字型管状网架构成,该网架的两端各个端点的连线均形成一个正多边形,两段网架相邻两端的各个端点均位于同一圆周上且相互交错排列,两者之间通过尼龙丝穿接成一体。
4、如权利要求3所述的人造贲门,其特征在于“z”宇型管状网架由医用不锈钢丝制成,其两端各个端点连线所形成的正多边形为正六边形一正十边形。
5、如权利要求1所述的人造贲门,其特征在于膜瓣(3)由医用弹性材料制成,其数量为2-6个,厚度为0.2-0.5MM。
6、如权利要求1所述的人造贲门,其特征在于固定圈(2)表面呈螺旋状,由医用涤纶制成。
7、如权利要求1所述的人造贲门,其特征在于膜裙(4)由医用弹性材料制成,厚度为0.08-0.15MM。
