Description

Auxiliary fixing instrument and auxiliary fixing method
Technical Field
The invention belongs to the technical field of medical instruments, and particularly relates to an auxiliary fixing instrument and an auxiliary fixing method.
Background
Pelvic Organ Prolapse (POP) is a type of organ position and dysfunction caused by pelvic floor support tissue weakness, which is caused by various reasons. The vulvar mass is taken as the main symptom, and urination and abnormal defecation, vulvar bleeding, inflammation and the like are accompanied or not, so that the life quality of patients is affected to different degrees. The patients with mild symptoms generally have no discomfort, the patients with severe symptoms can voluntarily have vaginal mass to deviate from, have lumbosacral part ache or falling sense with different degrees, have obvious symptoms after standing for a long time or tiredness, and can be accompanied with difficult urination after bedridden rest. The exposed cervical or vaginal wall rubs with clothes for a long time, which can cause ulcer, bleeding and the like of local cervical or vaginal wall, and purulent secretion after secondary infection. Uterine prolapse rarely affects menstruation and even does not affect conception, pregnancy and delivery. The person with the bulging vaginal front wall may have urination disorders such as feeling of urinary insufficiency, urinary retention, urinary incontinence, etc., and sometimes the person lifts the vaginal front wall upward to urinate. The swelling of the vaginal back wall may be accompanied by difficult defecation, and sometimes the swelled vaginal back wall may be pushed with a finger to discharge feces.
Pelvic organ prolapse is often the simultaneous existence of multiple sites, such as uterine prolapse is often accompanied by vaginal anterior and posterior wall distension, thickening and keratinization of vaginal mucosa, cervical hypertrophy and extension. The anterior vaginal wall is spherically distended, the distended bladder is soft, and the folds of vaginal mucosa disappear. When the back wall of the vagina is distended, the old perineum lacerations are often accompanied, and the rectum protruding into the vagina can be touched during anus diagnosis.
Pelvic organ prolapse suspension ligament surgery is a safe and effective surgical mode for treating pelvic organ prolapse. Depending on the patient's condition, the patient's viscera are suspended from different ligaments (e.g., ilio-pubic ligament, sacral ligament, sacrospinous ligament) using thicker non-absorbable sutures. The suspension method is divided into a suspension method by using a mesh and a suspension method by using an autologous tissue, wherein the mesh has the defect of erosion and exposure, and the mesh is forbidden to be used for pelvic organ prolapse suspension in some countries. However, the autologous tissue is suspended on the ligament, and a needle with a thick non-absorbable thread which can be directly used does not exist, before an operation, the thick non-absorbable thread needs to be reversely folded and knotted after passing through a suture needle hole of a ligament at a suture part to fix the suture on the suture needle, but the suture is thicker, so that the knot is larger. While the ligament is dense, larger knots are difficult to pass through, and large blood vessels are arranged around the ligament, and the larger knots have the risk of damaging the blood vessels and bleeding.
The present invention is directed to the above-mentioned problems, and provides an auxiliary fixing apparatus and an auxiliary fixing method.

Furthermore, the terms "first," "second," and the like, are used for descriptive purposes only and are not to be construed as indicating or implying a relative importance or implicitly indicating the number of technical features indicated. Thus, a feature defining "a first" or "a second" may explicitly or implicitly include one or more such feature. In the description of the present invention, the meaning of "a plurality" is two or more, unless explicitly defined otherwise.
Example 1
An auxiliary fixation apparatus as shown in fig. 1-11, comprising:
a placement section 1 including a first placement area 11 for placing the suture needle 4 and a first placement groove 12 for placing the suture thread 5;
a restriction portion 2 including a first restriction structure 21 and a third restriction structure 23; the first limiting structure 21 is matched with the first placement area 11 to limit the posture of the suture needle 4; the third limiting structure 23 is matched with the first placing groove 12 to limit the posture of the suture thread 5;
A puncture part 3 including an auxiliary needle 31, an auxiliary wire 32 and a pushing assembly; the auxiliary needle 31 is detachably connected with the auxiliary wire 32; the pushing component is in sliding connection with the placement part 1; the pushing assembly pushes the auxiliary needle 31 to move along the working path; the axis of the auxiliary needle 31 coincides with the working path; the working path intersects the first placement groove 12; the working path is clung to the tail of the suture needle 4;
the first limiting structure 21 fixes the suture needle 4 in the first placement area 11; the third limiting structure 23 fixes the suture 5 in the first placement groove 12; the pushing assembly pushes the auxiliary needle 31 to move the auxiliary wire 32 along the working path, and when the auxiliary needle 31 completely passes through the suture 5, the auxiliary wire 32 also passes through the suture 5.
Specifically, a groove adapted to the needle tail 42 of the suture needle is provided in the first placement area 11. In use, the needle tail 42 is placed in the recess to define the pose of the needle 4. Defining the pose of the needle 4 has the following advantages:
1. preventing the suture needle 4 from crossing the working path, so that the auxiliary needle 31 cannot pass through the suture thread 5, and the suture thread 5 cannot be fixed on the suture needle 4;
2. Bringing the working path of the auxiliary needle 31 into close contact with the needle tail 42 of the suture needle to further limit the positional relationship between the suture thread 5 and the suture needle 4; if the distance between the working path of the auxiliary needle 31 and the needle tail 42 of the suture needle is larger, the auxiliary thread 32 will make the suture thread 5 form a larger loop, and the suture needle 4 will shake inside the loop, resulting in lack of tight connection between the suture thread 5 and the suture needle 4, which promotes the suture difficulty and increases the risk of damaging healthy tissue. It is known that the blood vessels around the ligaments, especially the pelvic ligaments, bear the task of supplying blood to the lower limbs, and the blood vessels are relatively thick, so that the blood vessels can be damaged by a little carelessness to cause massive hemorrhage, thereby endangering the life of patients.
As shown in fig. 12-13, the suture needle 4 includes a needle tip 41 and a needle tail 42. The blunt end of the needle tip 41 is fixedly connected with the needle tail 42. The maximum outer diameter of the needle tip 41 is the same as the outer diameter of the needle tail 42. The inside of the needle tail 42 is provided with a V-shaped groove 421. When the suture 5 is threaded, the suture 5 is inserted from the thick end of the V-shaped groove 421, and then the suture 5 is pulled toward the thin end of the V-shaped groove 421, so that the suture 5 is inserted into the thin end of the V-shaped groove 421. At this time, the suture thread 5 is pressed and deformed by the thin end of the V-shaped groove 421, and is caught inside the thin end of the V-shaped groove 421. Preliminary fixing of the suture 5 on the needle 4 is achieved.
Specifically, the thick end of the V-shaped groove 421 faces the direction of the needle tip 41, and the thin end of the V-shaped groove 421 faces the direction away from the needle tip 41. During suturing, the ligament applies a frictional force to suture 5 in a direction away from needle point 41, pulling suture 5 firmly inside the thin end of V-groove 421.
Further, a receiving groove is formed in the direction that the thin end of the V-shaped groove 421 is far away from the needle tip 41, and when in use, the suture thread 5 is placed in the receiving groove, so that the maximum outer diameters of the suture thread 5 and the suture needle 4 are further reduced, and the suture thread 5 and the suture needle 4 can conveniently penetrate through tissues such as compact ligaments.
Specifically, the needle tip 41 is an arc needle.
As shown in fig. 7, a puncture track 15 is provided inside the placement section 1; the puncture track 15 coincides with the working path; the puncture track 15 is close to the tail of the suture needle 4, so that the working path of the auxiliary needle 31 is tightly attached to the needle tail 42 of the suture needle, the position relationship between the suture thread 5 and the suture needle 4 is effectively limited, and the shaking between the suture thread 5 and the suture needle 4 is prevented.
Specifically, the placement section 1 is internally provided with a placement groove 17 for facilitating placement of the auxiliary needle 31 with the auxiliary wire 32 inside the puncture track 15.
As shown in fig. 11, an opening is provided in the side wall of the auxiliary needle 31, and when in use, the auxiliary wire 32 slides on the side wall of the auxiliary needle 31, so that the auxiliary wire 32 can be threaded into the auxiliary needle 31, the time required for threading the auxiliary wire 32 into the auxiliary needle 31 is shortened, and the working efficiency is improved.
As shown in fig. 1 to 9, one end of the first restriction structure 21 is rotatably connected to the placement portion 1, and the other end is detachably connected to the placement portion 1. After the suture needle 4 is placed in the first placement area 11, the posture of the suture needle 4 is adjusted, then the suture needle 4 is pressed inside the first placement area 11 by rotating the first restriction structure 21, and the position of the suture needle 4 is defined by applying friction force.
Specifically, the first restriction structure 21 is a plate-like structure. One end of the first limiting structure 21 is provided with a hinge, and the other end is provided with a first buckle. The side wall of the placement part 1 is provided with a first clamping groove matched with the first buckle. In use, the first limiting structure 21 is rotated and the first buckle is connected with the first clamping groove, so that the suture needle 4 is limited in the first placing area 11.
In some embodiments of the present application, the first limiting structure 21 is a clamp-shaped clamping structure, and after the suture needle 4 is placed in the first placement area 11 and the posture is adjusted, the position and posture of the suture needle 4 can be fixed by using the first limiting structure 21.
Although only two schemes for limiting the suture needle 4 are shown in the present embodiment, other schemes for fixing the suture needle 4 at a certain position can be derived by those skilled in the art based on the present embodiment without departing from the spirit and scope of the present invention.
As shown in fig. 1 to 9, one end of the third restriction structure 23 is rotatably connected to the placement unit 1, and the other end is detachably connected to the placement unit 1. After the suture thread 5 is placed in the first placement groove 12, the posture of the suture thread 5 is adjusted to be in a tight state, then the third limiting structure 23 is rotated to press the suture thread 5 inside the first placement groove 12, and friction force is applied to limit the position of the suture thread 5.
Specifically, the third restriction structure 23 is a plate-like structure. One end of the third limiting structure 23 is provided with a hinge, and the other end is provided with a third buckle. A third clamping groove matched with the third buckle is formed in the side wall of the placement part 1. In use, the third limiting structure 23 is rotated and the third buckle is connected with the third clamping groove, so that the suture needle 4 is limited in the third placement area 14.
Further, a pressing structure 231 adapted to the first placing groove 12 is provided on a sidewall of the third limiting structure 23. When the third clip is connected with the third clip groove, the pressing structure 231 is inserted into the first placing groove 12, presses the suture 5, and defines the position of the suture 5 by applying friction.
In some embodiments of the present application, the third limiting structure 23 is a spool with a locking structure, the suture 5 is placed inside the spool, and the spool is pulled to place the suture 5 in tension, and then the locking structure is applied to prevent the spool from reversing. Specifically, the spool is provided on the back surface of the placement portion 1.
Although only two schemes for restricting the suture 5 are shown in this embodiment, other schemes for fixing the suture 5 at a certain place and in a tight state can be derived by those skilled in the art based on this embodiment without departing from the spirit and scope of the present invention.
Further, the restriction portion 2 further includes a second restriction structure 22; the second limiting structure 22 is disposed between the first limiting structure 21 and the third limiting structure 23. One end of the second limiting structure 22 is rotatably connected with the placement portion 1, and the other end is detachably connected with the placement portion 1. The second limiting structure 22 seals the placement groove 17 and the puncture track 15, preventing the auxiliary needle 31 from accidentally slipping off.
Specifically, the second restriction structure 22 is a plate-like structure. One end of the second limiting structure 22 is provided with a hinge, and the other end is provided with a second buckle. The side wall of the placement part 1 is provided with a second clamping groove matched with the second buckle.
As shown in fig. 1-9, the push assembly includes a push rod 33 and a push key 34; the push rod 33 is fixedly connected with the push key 34. The push rod 33 has a cylindrical structure, and the outer diameter of the push rod 33 is smaller than or equal to the maximum outer diameter of the sub-needle 31. The plunger 33 is adapted to the puncture track 15. The push key 34 is slidably connected to the placement portion 1, and a side of the push key 34 away from the push rod 33 protrudes through the placement portion 1 away from the suture needle 4.
Specifically, a slide rail adapted to the push key 34 is provided inside the placement portion 1. The slide rail coincides with the storage rail.
Preferably, the second placement area 13 is provided inside the placement part 1, and a magnet is provided inside the second placement area 13. The auxiliary needle 31 is made of paramagnetic metal material such as titanium and iron. The second placement area 13 is arranged at the end of the puncture track 15 remote from the pushing assembly. After pushing the auxiliary needle 31 to drive the auxiliary wire 32 to pass through the suture 5, the auxiliary needle 31 enters the second placement area 13 along the puncture track 15, and the auxiliary needle 31 is attracted by the magnet to prevent the auxiliary needle 31 from falling off from the inside of the second placement area 13.
Further, a plurality of auxiliary needles 31 are stored in the second placement area 13 for easy use.
In some embodiments of the present application, the puncture track 15 is made of a magnetic material, such as: and (3) a magnet. The subsidiary needle 31 made of paramagnetic metal material moves inside the magnetic attraction download puncture track 15, preventing the subsidiary needle 31 from slipping off.
In some embodiments of the application, a third placement area 14 is provided inside the placement section 1, the third placement area 14 being used for storing spare auxiliary wires 32.
The working principle of the embodiment is as follows:
1. The suture needle 4 with the suture thread 5 is put into the first placement area 11, so that the needle tail 42 of the suture needle 4 is ensured to be positioned in the groove;
2. Securing the needle 4 inside the first placement area 11 using a first restriction 21;
3. Stretching the suture 5 so that the suture 5 is positioned inside the first placing groove 12 and is in a tight state;
4. Securing the suture 5 within the first placement slot 12 using a third limiting structure 23;
5. placing the auxiliary needle 31 with the auxiliary wire 32 into the puncture track 15, keeping the auxiliary needle 31 between the suture 5 and the push rod 33, and making the needle tip 41 of the auxiliary needle 31 face the suture 5;
6. the pushing assembly drives the auxiliary needle 31 with the auxiliary wire 32 to move along the puncture track 15, and the auxiliary needle 31 passes through the suture 5 during the movement until the auxiliary needle 31 enters the second placement area 13;
7. The restriction part 2 is opened, the finished product is taken out, the auxiliary thread 32 is used for making knots, and the connection relation between the suture thread 5 and the suture needle 4 is locked.

5. placing the auxiliary needle 31 with the auxiliary wire 32 into the puncture track 15, keeping the auxiliary needle 31 between the suture 5 and the push rod 33, and making the needle tip 41 of the auxiliary needle 31 face the suture 5;
6. the pushing assembly drives the auxiliary needle 31 with the auxiliary wire 32 to move along the puncture track 15, and the auxiliary needle 31 passes through the suture 5 during the movement until the auxiliary needle 31 enters the second placement area 13;
7. Pressing the cutting key 61 causes the cutter 63 to contact and sever a strand of suture 5.
8. The restriction part 2 is opened, the finished product is taken out, the auxiliary thread 32 is used for making knots, and the connection relation between the suture thread 5 and the suture needle 4 is locked.
Example 3
An auxiliary fixing method, comprising:
step one, the first limiting structure 21 fixes the suture needle 4 in the first placement area 11; the third limiting structure 23 fixes the suture 5 in the first placement groove 12;
the suture needle 4 with the suture thread 5 is fixed first, and then the suture thread 5 is fixed and the suture thread 5 is placed in a tight state so as to facilitate the penetration of the auxiliary needle 31.
Step two, the pushing component pushes the auxiliary needle 31 to drive the auxiliary wire 32 to move along the working path, and the auxiliary needle 31 passes through the suture 5.
The working path is closely attached to the trailing end of the needle 4 and the auxiliary wire 32 also effects the passage of the suture 5 after the auxiliary needle 31 has passed through the suture 5. At this time, the auxiliary needle 31 is removed, and the auxiliary thread 32 can be applied to define the connection relationship of the suture thread 5 and the suture needle 4. The volume of the knot is reduced while preventing the suture 5 from being separated from the suture needle 4, so that the suture needle 4 and the suture 5 can pass through the compact ligament more flexibly.
In some embodiments of the application, the auxiliary fixing method further comprises:
Step three, the cutting part 6 cuts one end of the suture 5.
The suture 5 is properly cut, so that the twin wires are changed into single wires, the number of the suture 5 which passes through the ligament once is reduced, the suture 5 can conveniently pass through the ligament, and the operation efficiency is improved.
The proposal of the invention can effectively reduce the knot volume, not only can be used for pelvic organ prolapse suspension ligament operation, but also can be used in the process of self-made suture line 5 with needle in other operations.
