Abstract

技术领域
本发明涉及医学影像与人工智能技术领域,具体为一种基于双期增强计算机断层扫描图像的Delta深度学习影像组学方法。
背景技术
非小细胞肺癌是最常见的恶性肿瘤,晚期患者5年生存率较低,随着新的化疗及免疫疗法的出现,患者预后得到明显改善。Ki-67反映了肿瘤细胞的增殖指数,与肿瘤化疗敏感性和缓解率密切相关。目前Ki-67指数需穿刺活检后行免疫组织化学技术获取,在晚期肺癌治疗过程中无法动态无创地获取肿瘤的增殖指数。影像组学能通过提取医学图像中的大量高维数据,预测肿瘤的生物学和病理学特征。有研究证实单期增强CT图像的组学研究能区分肺内肿瘤性病变类型,亦能预测早期肺癌的Ki-67指数。目前,大量研究集中在常规的影像组学方法之上,基于双期增强CT的影像组学预测晚期非小细胞癌Ki-67指数表达的研究有限。
本发明针对上述存在的问题提出一种基于双期增强计算机断层扫描图像的Delta深度学习影像组学方法预测晚期非小细胞肺癌Ki-67指数。
发明内容
本发明的目的在于提供一种基于双期增强计算机断层扫描图像的Delta深度学习影像组学方法预测晚期非小细胞肺癌Ki-67指数的方法,目标是为晚期非小细胞肺癌患者的个体化治疗提供无创的病理学评价指标。该方法首先分别提取晚期非小细胞癌患者静脉增强CT的影像组学特征和动脉增强CT的影像组学特征,其次使用静脉增强CT影像组学特征减去动脉增强CT影像组学特征计算得到Delta影像组学特征,然后采用极端随机树算法获取重要性相对较高的排名前100位的特征,最后使用深度学习神经网络模型预测Ki-67指数,以解决上述背景技术中提出的问题。
为实现上述目的,本发明提供如下技术方案:
一种基于双期增强计算机断层扫描图像的Delta深度学习影像组学方法,其步骤具体如下:
步骤1,病人数据采集:据纳入、排除标准采集符合标准的晚期非小细胞肺癌患者;
步骤2,CT扫描参数;
步骤3,图像分割:对动脉增强CT的肺癌肿瘤区域进行ROI标注图像分割并在静脉增强CT影像进行配准;
步骤4,影像组学特征提取:对标注的ROI区域提取一阶统计、形状、纹理、滤波影像组学特征;
步骤5,Delta影像组学特征筛选:剔除形状、及ICC小于0.75的特征后计算Delta影像组学特征并使用极端随机树筛选特征;
步骤6,通过构建深度学习影像组学模型预测Ki-67指数。
作为本发明优选的方案,所述步骤1中病人数据采集的具体步骤如下:
步骤11,研究对象:回顾性分析2019年1月至2022年12月期间多中心病理确诊的IIIb-IV期非小细胞肺癌患者。
步骤12,纳入标准:(1)组织病理学检测Ki-67指数;(2)临床治疗及穿刺活检前行胸部增强CT检查。
步骤13,排除标准:(1)CT检查前接受过放疗、化疗、活检的患者;(2)大空洞型肺癌、小细胞肺癌、肺炎型腺癌;(3)病灶边界无法与实变肺组织区分的患者。
作为本发明优选的方案,所述步骤2中CT扫描参数的具体步骤如下:
所有受试者的胸部成像均使用螺旋CT扫描仪进行,其中扫描过程中管电压为120kv,管电流为自动电流,重建层厚为1-1.5mm,矩阵为512×512,视野为350mm×350mm;
对比剂通过高压注射器以2.5-3.0ml/s的速度注射入受试者体内,快速扫描获得动脉、静脉相CT图像,延迟时间分别为30s、60s;
其中CT扫描仪的型号为Somatom Force,Somatom Flash双源CT,西门子,德国,对比剂注射剂量采用60-85ml碘造影剂。
作为本发明优选的方案,所述步骤3中图像分割的具体步骤如下:
步骤31,窗口设置:在肿瘤分割和影像特征分析过程中,利用纵隔窗和肺窗水平勾画出动脉和静脉期CT图像中的病变ROI;两个放射科医生进行半自动病灶在动脉期CT图像分割,确保较粗大血管不包含在感兴趣轮廓内,其中纵隔窗为400HU和40HU,肺窗水平为1600HU和-600HU;
步骤32,数据配准:然后将动脉相CT图像上的轮廓转移到静脉相CT图像上,如果存在呼吸幅度影响的轮廓失配,需要手动校准轮廓以匹配在静脉相位上观察到的轮廓;
步骤33,ICC:一个月后,随机选择30例,两名放射科医生重复分割以评估观察者内和观察者间的可重复性。

使用PyRadiomics从医学影像中提取影像组学特征,在特征提取之前,对CT图像进行重采样和灰度离散化,以实现图像的标准化;
对于每个三维ROI,提取一阶统计、形状、纹理、滤波1688个放射学特征,每个患者共提取包括动脉期(FA)和静脉期(FV)的肿瘤3376个特征,其中所有的放射特征都符合图像生物标志物标准化倡议IBSI制定的标准。
作为本发明优选的方案,所述步骤5中Delta影像组学特征筛选的具体步骤如下:
由于CT扫描期间肿瘤形状一致,因此不计算基于形状的Delta特征,即n=14;ICC大于0.75的稳定特征被保留并对其进行归一化处理,计算1674个静脉和动脉期特征之间的算术差值作为Delta放射组学特征,其中Delta放射组学特征的公式为ΔF=FA-FV;从Delta放射组学特征中选取非零的有用特征,利用极端随机树算法获取放射特征的重要性,将重要性相对较高的排名前100位的特征输入深度Delta影像组学模型。
作为本发明优选的方案,所述步骤6中通过构建深度学习影像组学模型预测Ki-67指数的具体步骤如下:
通过深度神经网络构建,即深度神经网络包含一个输入层、两个全连接层和一个输出层,使用Softmax作为激活函数,将交叉熵损失函数用于分类任务;在训练阶段使用Dropout和权重衰减策略,以增加网络的泛化能力。
与现有技术相比,本发明的有益效果是:
本发明区别于传统影像组学特征的双期增强计算机断层扫描图像的Delta深度学习影像组学预测Ki-67指数,该系列特征是通过从静脉增强CT影像组学特征减去动脉增强CT影像组学特征计算得到,从而选择相关性最高的Delta影像组学特征使用DDLR模型预测Ki-67指数,即通过收集、分割、提取晚期非小细胞癌患者静脉增强CT的影像组学特征和动脉增强CT的影像组学特征相减计算得到Delta影像组学特征,然后采用利用极端随机树算法获取重要性相对较高的排名前100位的特征,最后使用Delta深度学习影像组学模型预测Ki-67指数,为晚期非小细胞肺癌患者的个体化治疗提供无创的病理学评价指标。

具体的一种实施方式如下:
A.病人数据采集
A1.研究对象:回顾性分析2019年1月至2022年12月期间多中心病理确诊的IIIb-IV期非小细胞肺癌患者。
A2.纳入标准:(1)组织病理学检测Ki-67指数;(2)临床治疗及穿刺活检前行胸部增强CT检查。A3.排除标准:(1)CT检查前接受过放疗、化疗、活检的患者;(2)大空洞型肺癌、小细胞肺癌、肺炎型腺癌;(3)病灶边界无法与实变肺组织区分的患者。
B.CT扫描参数
所有受试者的胸部成像均使用螺旋CT扫描仪(Somatom Force,Somatom Flash双源CT,西门子,德国)进行。扫描过程中管电压为120kv,管电流为自动电流,重建层厚为1-1.5mm,矩阵为512×512,视野为350mm×350mm。对比剂(60-85ml碘造影剂)通过高压注射器以2.5-3.0ml/s的速度注射入受试者体内,快速扫描获得动脉、静脉相CT图像,延迟时间分别为30s、60s。
C.图像分割
C1.窗口设置:在肿瘤分割和影像特征分析过程中,利用纵隔窗(400HU和40HU)和肺窗水平(1600HU和-600HU)勾画出动脉和静脉期CT图像中的病变ROI。两个放射科医生进行半自动病灶在动脉期CT图像分割,确保较粗大血管不包含在感兴趣轮廓内。
C2.数据配准:然后将动脉相CT图像上的轮廓转移到静脉相CT图像上。如果存在呼吸幅度影响的轮廓失配,需要手动校准轮廓以匹配在静脉相位上观察到的轮廓。

D.影像组学特征提取
使用PyRadiomics从医学影像中提取影像组学特征。在特征提取之前,对CT图像进行重采样和灰度离散化,以实现图像的标准化。对于每个三维ROI,分别提取一阶统计、形状、纹理、滤波等1688个放射学特征,每个患者共提取包括动脉期(FA)和静脉期(FV)的肿瘤3376个特征。所有的放射特征都符合图像生物标志物标准化倡议(IBSI)[1]制定的标准;
E.Delta影像组学特征筛选,由于CT扫描期间肿瘤形状一致,因此本发明没有计算基于形状的Delta特征(n=14)。ICC大于0.75的稳定特征被保留并对其进行归一化处理。计算1674个静脉和动脉期特征之间的算术差值作为Delta放射组学特征(ΔF=FA-FV)。从Delta放射组学特征中选取非零的有用特征,利用极端随机树算法获取放射特征的重要性,将重要性相对较高的排名前100位的特征输入深度Delta影像组学模型。
F.深度学习影像组学模型构建
传统的影像组学使用如逻辑回归、支持向量机、随机森林等传统的机器学习方法建模,这些方法相比深度学习模型简单、学习能力差、泛化能力差等。本发明提出的深度神经网络,如附图3所示,包含一个输入层、两个全连接层和一个输出层,使用Softmax作为激活函数,将交叉熵损失函数用于分类任务。在训练阶段使用Dropout和权重衰减策略,以增加网络的泛化能力。

Claims (7)
1.一种基于双期增强计算机断层扫描图像的Delta深度学习影像组学方法,其步骤具体如下:
步骤1,病人数据采集:据纳入、排除标准采集符合标准的晚期非小细胞肺癌患者;
步骤2,CT扫描参数;
步骤3,图像分割:对动脉增强CT的肺癌肿瘤区域进行感兴趣区域(region ofinterest,ROI)标注图像分割并在静脉增强CT影像进行配准;
步骤4,影像组学特征提取:对标注的ROI区域提取一阶统计、形状、纹理、滤波影像组学特征;
步骤5,Delta影像组学特征筛选:剔除形状、及组内相关系数(Intraclasscorrelation coefcient,ICC)小于0.75的特征后计算Delta影像组学特征并使用极端随机树筛选特征;
步骤6,通过构建深度学习影像组学模型预测Ki-67指数。
2.根据权利要求1所述的一种基于双期增强计算机断层扫描图像的Delta深度学习影像组学方法,其特征在于:所述步骤1中病人数据采集的具体步骤如下:
步骤11,研究对象:回顾性分析2019年1月至2022年12月期间多中心病理确诊的IIIb-IV期非小细胞肺癌患者;
步骤12,纳入标准:(1)组织病理学检测Ki-67指数;(2)临床治疗及穿刺活检前行胸部增强CT检查;
步骤13,排除标准:(1)CT检查前接受过放疗、化疗、活检的患者;(2)大空洞型肺癌、小细胞肺癌、肺炎型腺癌;(3)病灶边界无法与实变肺组织区分的患者。
3.根据权利要求1所述的一种基于双期增强计算机断层扫描图像的Delta深度学习影像组学方法,其特征在于:所述步骤2中CT扫描参数的具体步骤如下:
所有受试者的胸部成像均使用螺旋CT扫描仪进行,其中扫描过程中管电压为120kv,管电流为自动电流,重建层厚为1-1.5mm,矩阵为512×512,视野为350mm×350mm;
对比剂通过高压注射器以2.5-3.0ml/s的速度注射入受试者体内,快速扫描获得动脉、静脉相CT图像,延迟时间分别为30s、60s;
其中CT扫描仪的型号为Somatom Force,Somatom Flash双源CT,西门子,德国,对比剂注射剂量采用60-85ml碘造影剂。
4.根据权利要求1所述的一种基于双期增强计算机断层扫描图像的Delta深度学习影像组学方法,其特征在于:所述步骤3中图像分割的具体步骤如下:
步骤31,窗口设置:在肿瘤分割和影像特征分析过程中,利用纵隔窗肺窗水平勾画出动脉和静脉期CT图像中的病变;两个放射科医生进行半自动病灶在动脉期CT图像分割,确保较粗大血管不包含在感兴趣轮廓内,其中纵隔窗为400HU和40HU,肺窗水平为1600HU和-600HU;
步骤32,数据配准:然后将动脉相CT图像上的轮廓转移到静脉相CT图像上,如果存在呼吸幅度影响的轮廓失配,需要手动校准轮廓以匹配在静脉相位上观察到的轮廓;
步骤33,ICC:一个月后,随机选择30例,两名放射科医生重复分割以评估观察者内和观察者间的可重复性。
