Abstract
The invention provides a five-cereal insect debridement bag, a preparation method, a system and application, and relates to the field of biomedical basic research. The specific experimental procedures or conditions are not noted in the examples and may be followed by the operations or conditions of conventional experimental procedures described in the literature in this field. The reagents used, as well as other instruments, are commercially available conventional reagent products, not identified to the manufacturer.
The five cereal insects are named as cereal insects, fagopyrum tataricum, fly maggots and maggots. The five-cereal insect is larvae of lucilia sericata, chrysomya megacephala or other closely related insects of the family of the blowflies. In ancient China, the forty volumes of the worm mesh of Ben Cao gang mu indicate that: "maggots and flies" are also used. For furuncle and ecthyma, it is ground into powder and mixed with sesame oil for external application. The fresh people can take salty-cold and detoxify peristaltic attack and dispersion more well. According to the description of Chinese medicine "Ben Cao Ji Yuan" and "Ben Cao Bian Ji Shen", the insect is dried and ground and then applied externally to treat ecthyma, lip furuncle, carbuncle and other infectious surgical diseases. All of the Chinese medicinal materials Zhi and northeast animal medicine are received by the names of five cereals.
For centuries, fresh five-cereal worms have been used to clean wounds by degrading, liquefying and only taking up the deactivated/necrotic tissue, and promote wound healing. This therapy is internationally known as biological debridement therapy (MDT). The fresh five-cereal worms are very effective in clinical treatment and are approved by the U.S. Food and Drug Administration (FDA) for medical use, and the use of the fresh five-cereal worms is also brought into the Liaoning province Chinese medicine processing Specification and the treatment standard of diabetic foot in China.
The existing fresh five-cereal worm processing standard is relatively simple and convenient, and the processing method comprises the following steps: the artificially cultivated larva of the five-cereal worm is washed. The invention provides a five-cereal insect debridement bag suitable for clinical application and a preparation method thereof without specific processing methods and processes.
Specifically, as shown in fig. 1, a flowchart of a method for preparing a five-cereal insect debridement bag in an embodiment of the present invention is shown, and the method for preparing a five-cereal insect debridement bag specifically includes:
s1, under the aseptic condition, placing fresh five-cereal worms in 0.3% -2% iodophor solution, standing for 5-40 minutes, and then rinsing by using sterile physiological saline containing 0.9% sodium chloride.
In the embodiment of the invention, the selected fresh five-cereal insects are larvae of lucilia sericata, chrysomya megacephala and adult flies of the family Leucophaceae. Under the aseptic condition, selecting artificially cultivated five-cereal insects or 1-3-year-old five-cereal insects after incubation of aseptic eggs, and taking the five-cereal insects with the length of 1mm-1.5cm as fresh five-cereal insects for subsequent treatment.
In the embodiment of the invention, the step S1 is repeated for 3 to 5 times, so that the preliminary disinfection of the fresh five-cereal insects can be finished.
S2, taking out the rinsed fresh five-cereal insects under the aseptic condition, placing the fresh five-cereal insects in aseptic animal milk or fetal bovine serum for standing for 5-40 minutes, and rinsing the fresh five-cereal insects with aseptic physiological saline containing 0.9% sodium chloride to empty intestinal contents of the fresh five-cereal insects.
In the embodiment of the invention, the fresh five-cereal insects after preliminary disinfection are treated according to the step S2 and repeated for 3-5 times, so that the intestinal contents of the fresh five-cereal insects can be emptied, the fresh five-cereal insects tend to be aseptic, and the method is suitable for clinical application.
S3, taking out the fresh five-cereal insects under the aseptic condition, placing the fresh five-cereal insects in aseptic normal saline containing a bacteriostatic agent, standing for 2-40 minutes, and then rinsing the fresh five-cereal insects with the aseptic normal saline containing 0.9% sodium chloride to obtain the aseptic fresh five-cereal insects.
In the embodiment of the present invention, in practical application, the step S3 may be repeated 3-5 times.
Alternatively, in an embodiment of the present invention, the bacteriostatic agent is a nanoparticle of silver, or the bacteriostatic agent is a mixture of nanoparticles of gold, silver and platinum, or the bacteriostatic agent is a solution containing 5-10% lactic acid, 5-10% sorbic acid, 6-10% citric acid anhydrous, 1-3% copper sulfate, 25-30% fatty acid glyceride, and 25-40% propylene glycol.
In the embodiment of the invention, during actual operation, any broad-spectrum bacteriostat can be adopted to perform bacteriostasis treatment on fresh five-cereal insects which empty intestinal contents.
S4, transferring the sterile fresh five-cereal insects into a dressing bag under a sterile condition to obtain the five-cereal insect debridement bag.
In order to facilitate clinical application, the embodiment of the invention provides that the sterile fresh five-cereal insects are transferred into the dressing bag under the sterile condition to obtain the five-cereal insect debridement bag.
In the embodiment of the invention, the material of the dressing bag can be selected from any one or more of the following, and in particular, different dressing bags can be designed according to the type and the requirement of the wound:
superabsorbent fibre dressing: has strong water absorption performance, can quickly absorb wound exudates, keep the wound dry and promote healing.
Collagen dressing: is rich in collagen, can provide good wound support and protection, and promotes tissue regeneration and healing.
Castor oil dressing: is rich in castor oil, has antibacterial, antiinflammatory, and moisturizing effects, and can promote wound healing and prevent infection.
Film dressing: the film dressing with the transparent texture can play a role of a physical barrier, prevent external pollution and bacterial infection, and has air permeability, thereby being beneficial to wound healing.
Gauze dressing: can absorb exudates, provide a protective covering, and at the same time be breathable, facilitating wound healing.
In the embodiment of the invention, the five-cereal insect debridement can be placed in a test tube containing sterile culture solution for storage and transportation.
In the embodiment of the invention, before the step S4, fresh five-cereal worms can be placed on the sterile absorbent paper to absorb surface moisture, and the vitality of the five-cereal worms is observed. Selecting the five-cereal worms with normal vitality to prepare the five-cereal worm debridement bag.
In practical application, after the steps S1 to S3 are completed, the obtained fresh five-cereal worms may be further inspected, which specifically includes: randomly selecting 100 fresh five-cereal worms, crushing to prepare homogenate, and performing bacterial inspection and virology inspection. Making qualified five-cereal insects in the same batch into a five-cereal insect debridement bag, or directly transferring into a sterile unidirectional ventilation culture bottle, and preserving at 4-30deg.C and relative humidity of 27% -85% for use.

the first sterile treatment device is used for placing fresh five-cereal worms in 0.3% -2% iodophor solution for standing for 5-40 minutes under a sterile condition, and then rinsing the fresh five-cereal worms with sterile physiological saline containing 0.9% sodium chloride;
the emptying treatment device is used for taking out the rinsed fresh five-cereal worms under the aseptic condition, placing the fresh five-cereal worms in aseptic animal milk or fetal bovine serum for standing for 5-40 minutes, and then rinsing the fresh five-cereal worms with aseptic physiological saline containing 0.9% sodium chloride to empty intestinal contents of the fresh five-cereal worms;
the second aseptic processing device is used for taking out the fresh five-cereal insects under aseptic conditions, placing the fresh five-cereal insects in aseptic normal saline containing a bacteriostatic agent, standing for 2-40 minutes, and then rinsing the fresh five-cereal insects with the aseptic normal saline containing 0.9% sodium chloride to obtain the aseptic fresh five-cereal insects;
the preparation device is used for transferring the sterile fresh five-cereal insects into the dressing belt under the sterile condition to obtain the five-cereal insect debridement bag.
In the embodiment of the present invention, the preparation system corresponds to the preparation method provided in the steps S1 to S4, and the specific apparatus used to complete the preparation method may be a conventional culture apparatus, for example: sterile glass petri dishes.
A third aspect of the embodiments of the present invention provides a five-cereal insect debridement pack, the five-cereal insect debridement pack comprising: the dressing bag with micropores and fresh five-cereal insects are cultivated in a sterile environment and subjected to aseptic treatment to obtain the fresh five-cereal insects.
In the embodiment of the invention, the pore diameter of the micropores on the dressing bag can be determined according to the size of the fresh five-cereal insects, and the diameter of the micropores on the dressing bag is smaller than that of the fresh five-cereal insects.
Specifically, in the embodiment of the present invention, the fresh five-cereal worms may be obtained by the treatment in the steps S1 to S3.
Optionally, the five-cereal insect debridement bag further comprises: and (5) filling.
In the embodiment of the invention, the five-cereal insect debridement bag can also comprise a filler so as to maintain the temperature and the humidity in the debridement bag and further maintain the activity of the five-cereal insects.
Specifically, the filler may include any one or more of the following:
gauze (2): different thicknesses and sizes can be set according to different requirements so as to adapt to wounds of different sizes and types;
high absorbency sponge: such a filler has excellent water absorption properties, can rapidly absorb wound secretions, reduces the risk of infection, and a highly absorbent sponge is generally sterile to ensure a clean environment around the wound.
Collagen filler: collagen is a natural protein, which plays an important role in wound repair; collagen fillers can provide structural support and promote the healing process.
Transparent negative pressure sponge: the transparent negative pressure sponge is a filler with negative pressure function, can be used for wound surface closure and the formation of moist environment, can help to reduce infection rate and promote wound healing.
Hydrogel filler: the hydrogel filler has effects of keeping moisture and promoting healing, and can absorb wound secretion and maintain moist environment of wound, thereby promoting wound healing.
In the embodiment of the invention, the density of the fresh five-cereal insects in the five-cereal insect debridement bag is 5-30 pieces/square centimeter, and the fresh five-cereal insects are 1-3 years old fresh five-cereal insects. When the method is specifically applied, the wound surface of a patient can be covered by the five-cereal insect debridement, so that 5-30 fresh five-cereal insects with 1-3 ages are placed on the wound surface of the patient every square centimeter. In specific application, the wound can be disinfected by using the iodophor before using the five-cereal insect debridement bag, and is flushed by using normal saline, so that exudation of the chronic wound is eliminated, and meanwhile, the wound is kept moist.
In the embodiment of the invention, after the wound surface of the patient is covered by the wound debridement of the five-cereal insects, bandaging and fixing can be carried out by using a bandage.
When the method is specifically applied, the fresh five-cereal worms are used for 2-3 days each time, the wound surfaces of the patients are covered with the fresh five-cereal worms in a debridement mode all the time, then the wound surfaces are washed by using iodophor, the fresh five-cereal worms are taken out, and according to clinical requirements, the wound surfaces of the patients can be covered with the fresh five-cereal worms in a debridement mode.
The fourth aspect of the embodiment of the invention provides an application of fresh five-cereal insects in preparing a chronic wound debridement bag, wherein the fresh five-cereal insects are cultivated in a sterile environment and subjected to aseptic treatment, and the fresh five-cereal insects are used for cleaning wounds through degradation, liquefaction and ingestion of deactivated/necrotic tissues.
Optionally, the processing step of the fresh five-cereal worms comprises the following steps:
placing fresh five-cereal worms in 0.3% -2% iodophor solution under aseptic condition, standing for 5-40 min, and rinsing with sterile physiological saline containing 0.9% sodium chloride;
taking out the rinsed fresh five-cereal insects under aseptic conditions, placing the fresh five-cereal insects in aseptic animal milk or fetal bovine serum, standing for 5-40 minutes, and rinsing the fresh five-cereal insects with aseptic physiological saline containing 0.9% sodium chloride to empty intestinal contents of the fresh five-cereal insects;
taking out the fresh five-cereal insects under the aseptic condition, placing the fresh five-cereal insects in aseptic normal saline containing a bacteriostatic agent, standing for 2-40 minutes, and then rinsing the fresh five-cereal insects with the aseptic normal saline containing 0.9% sodium chloride to obtain the aseptic fresh five-cereal insects.
Optionally, the density of the fresh five-cereal insects is 5-30 strips/square centimeter.
In the embodiment of the invention, when the method is specifically applied, the fresh five-cereal worms can be clinically applied to preparing the five-cereal worm debridement bag, or the fresh five-cereal worms can be directly placed on the wound surface of a patient and then covered by dressing or gauze.
When the method is specifically applied, the wound surface of a patient can be observed every day, and a small amount of physiological saline is dripped into the dry part of the wound surface. If a large amount of liquid seeps out from the wound surface, the bandage wrapped by the outer layer is replaced.
When the five-cereal insect debridement bag is replaced in specific application, the debridement bag can be taken down by forceps and put into a test tube filled with 75% alcohol or chlorine-containing disinfectant for sealing.
In the embodiment of the invention, specific clinical application cases are also provided:
case 1: after 85 years old men have 30 years history of diabetes mellitus, the patients have complicated MRSA drug-resistant bacteria infection due to non-healing incision migration after toe interception of diabetic foot blood disorder, and the patients are rolled to 30 domestic hospitals for more than 10 years old, and the patients take more than 30 ten thousands of yuan, so that the treatment methods have no obvious effect. The external application treatment of the five cereal insect debridement bag is applied, the wound can be replaced every 3 days, after the wound is used for 2 weeks (about 2500 yuan), the infection is effectively controlled, the granulation of the wound surface is regenerated, the area of the wound surface is gradually reduced, and the wound surface is finally and completely healed.
Case 2: in 91-year-old men, the heel is pressed for a long time to cause huge defect wound surfaces, and the treatment method is combined with pseudomonas aeruginosa infection, takes hundreds of thousands of yuan, and has no obvious effect on various treatment methods. 1. The five-cereal insect debridement bag is characterized by comprising: the dressing bag with micropores and fresh five-cereal insects are cultivated in a sterile environment and subjected to aseptic treatment to obtain the fresh five-cereal insects.
2. The five-cereal insect debridement bag of claim 1, wherein the fresh five-cereal insect treatment process comprises:
placing fresh five-cereal worms in 0.3% -2% iodophor solution under aseptic condition, standing for 5-40 min, and rinsing with sterile physiological saline containing 0.9% sodium chloride;
taking out the rinsed fresh five-cereal insects under aseptic conditions, placing the fresh five-cereal insects in aseptic animal milk or fetal bovine serum, standing for 5-40 minutes, and rinsing the fresh five-cereal insects with aseptic physiological saline containing 0.9% sodium chloride to empty intestinal contents of the fresh five-cereal insects;
taking out the fresh five-cereal insects under the aseptic condition, placing the fresh five-cereal insects in aseptic normal saline containing a bacteriostatic agent and/or a bactericide, standing for 2-40 minutes, and then rinsing the fresh five-cereal insects with the aseptic normal saline containing 0.9% sodium chloride to obtain the aseptic fresh five-cereal insects.
3. The five-cereal insect debridement bag of claim 1, further comprising: and (5) filling.
4. The five-cereal insect debridement bag of claim 1, wherein the density of fresh five-cereal insects in the five-cereal insect debridement bag is 5-30 bars/square centimeter.
5. The preparation method of the five-cereal insect debridement bag is characterized by comprising the following steps:
placing fresh five-cereal worms in 0.3% -2% iodophor solution under aseptic condition, standing for 5-40 min, and rinsing with sterile physiological saline containing 0.9% sodium chloride;
taking out the rinsed fresh five-cereal insects under aseptic conditions, placing the fresh five-cereal insects in aseptic animal milk or fetal bovine serum, standing for 5-40 minutes, and rinsing the fresh five-cereal insects with aseptic physiological saline containing 0.9% sodium chloride to empty intestinal contents of the fresh five-cereal insects;
taking out the fresh five-cereal insects under the aseptic condition, placing the fresh five-cereal insects in aseptic normal saline containing a bacteriostatic agent, standing for 2-40 minutes, and then rinsing the fresh five-cereal insects with the aseptic normal saline containing 0.9% sodium chloride to obtain the aseptic fresh five-cereal insects;
transferring the sterile fresh five-cereal insects into a dressing bag under a sterile condition to obtain the five-cereal insect debridement bag.
6. The method according to claim 5, wherein the bacteriostatic agent is silver nanoparticles, or the bacteriostatic agent is a mixture of gold, silver and platinum nanoparticles, or the bacteriostatic agent is a solution containing 5-10% lactic acid, 5-10% sorbic acid, 6-10% citric acid anhydrous, 1-3% copper sulfate, 25-30% fatty acid glyceride and 25-40% propylene glycol.
7. A system for preparing a five cereal insect debridement kit, the system comprising:
the first sterile treatment device is used for placing fresh five-cereal worms in 0.3% -2% iodophor solution for standing for 5-40 minutes under a sterile condition, and then rinsing the fresh five-cereal worms with sterile physiological saline containing 0.9% sodium chloride;
the emptying treatment device is used for taking out the rinsed fresh five-cereal worms under the aseptic condition, placing the fresh five-cereal worms in aseptic animal milk or fetal bovine serum for standing for 5-40 minutes, and then rinsing the fresh five-cereal worms with aseptic physiological saline containing 0.9% sodium chloride to empty intestinal contents of the fresh five-cereal worms;
the second aseptic processing device is used for taking out the fresh five-cereal insects under aseptic conditions, placing the fresh five-cereal insects in aseptic normal saline containing a bacteriostatic agent, standing for 2-40 minutes, and then rinsing the fresh five-cereal insects with the aseptic normal saline containing 0.9% sodium chloride to obtain the aseptic fresh five-cereal insects;
the preparation device is used for transferring the sterile fresh five-cereal insects into the dressing belt under the sterile condition to obtain the five-cereal insect debridement bag.

The application of the fresh five-cereal insects in preparing the chronic wound debridement bag is characterized in that the fresh five-cereal insects are cultivated in a sterile environment and subjected to aseptic treatment, and the fresh five-cereal insects are used for cleaning wounds through degradation, liquefaction and ingestion of deactivated/necrotic tissues.

taking out the rinsed fresh five-cereal insects under aseptic conditions, placing the fresh five-cereal insects in aseptic animal milk or fetal bovine serum, standing for 5-40 minutes, and rinsing the fresh five-cereal insects with aseptic physiological saline containing 0.9% sodium chloride to empty intestinal contents of the fresh five-cereal insects;
taking out the fresh five-cereal insects under the aseptic condition, placing the fresh five-cereal insects in aseptic normal saline containing a bacteriostatic agent, standing for 2-40 minutes, and then rinsing the fresh five-cereal insects with the aseptic normal saline containing 0.9% sodium chloride to obtain the aseptic fresh five-cereal insects.
