一种腕掌关节重建假体
技术领域
本发明涉及重建假体技术领域,尤其是涉及一种诊断腕掌关节骨关节炎或其他疾病造成该关节毁损拟行关节置换术的腕掌关节重建假体。
背景技术
腕掌关节在手的复杂运动中起到了关键的作用,尤其是拇指的腕掌关节,该关节是由第一掌骨与大多角骨构成,彼此构成一个扭曲的鞍状关节,在运动过程中,掌骨相对于大多角骨进行屈伸、内收外展、内旋外旋等复杂的运动,在关节背侧韧带的作用下,在抓握时常会承受较大的压力,是手部活动量最大的关节。正是因为其解剖结构和运动都很复杂,导致拇指的腕掌关节骨关节炎的发病率非常高。
需要拇指腕掌关节假体植入的疾病有:1、腕掌关节的非炎症性退行性拇指腕掌关节疾病,包括骨关节炎、创伤后关节炎等。2、类风湿性关节炎。3、其他手段治疗失败的患者。其特征为腕掌关节的不同程度的疼痛、肿胀、僵硬和运动障碍。该疾病可以影响任何年龄段的人,但最常见于中年和老年人,女性发病率比男性高。腕掌关节骨关节炎主要是由于长时间的关节疲劳、过度使用或由于其他原因导致的磨损和损耗引起。
腕掌关节骨关节炎的治疗方法包括药物治疗、物理治疗、手术治疗等。当保守治疗无法有效缓解疼痛,亦无法进一步控制腕掌退化性关节炎的状况时,通常需要进行手术治疗。目前治疗腕掌关节关节炎主要是通过关节融合术或关节软骨切除肌腱填塞成形术。关节融合术会导致关节功能丧失,附近的关节可能会因为需要承受更多的压力和负荷,而出现过度磨损和生长异常。关节软骨切除肌腱填塞成形术术后能存在关节不稳定、疼痛复发等问题。
随着人工关节的设计及制作材料的逐渐成熟,人工关节置换已成为目前既能减轻关节炎的疼痛,还能保留腕掌关节的活动及手部高度的功能。
然而,现有的腕掌关节重建假体虽然能够使患者恢复关节的正常功能,但是也存在一些缺陷。首先最常见的并发症是出现术后脱位,一般是由于手术过程中骨赘切除不全和假体位置角度不良,使用更大头部尺寸的假体可显著地减少脱位概率。其次是在假体稳定性方面,若患者骨质量较差或者置入过程中假体位置不良,容易出现早期松动。并且由于假体的材料和结构与自然关节有很大差异,因此在日常生活中可能会感觉到不适或疼痛,远期易出现假体松动。此外由于腕关节承受着大量的活动和力量,因此重建腕关节需要经常检查并确保假体在长期使用中不会出现松动或磨损。
发明内容
有鉴于此,本发明的目的是设计一种腕掌关节重建假体,既能够恢复关节的功能,又能够确保假体与骨骼融为一体,提供长期稳定的作用。
为此,本发明提供了以下技术方案:
本发明公开了一种腕掌关节重建假体,包括:球窝式关节联接结构;球窝式关节联接结构包括:固定于大多角骨的窝状假体和用于髓内接合的球柄状假体;
窝状假体的外表面为第一多孔结构或仿生骨小梁结构;窝状假体的内表面光滑;
球柄状假体为球头部、颈部和柄部依次联接形成的一体结构,球头部与窝状结构相匹配,球头部与窝状假体内表面相接触;柄部与掌骨髓腔相匹配;柄部的外表面为第二多孔结构或仿生骨小梁结构。
进一步地,窝状假体的内表面为高抛光镜面或金属镜面镀层。
进一步地,窝状假体的外表面边缘为带有螺纹孔的翼状结构;窝状假体通过螺钉穿过翼状结构的螺纹孔固定于大多角骨。
进一步地,窝状假体的外表面分布有若干个刺样结构;窝状假体通过刺样结构插入到大多角骨。

本发明的优点和积极效果:本发明中的腕掌关节重建假体,其具体包括近端的窝状假体和远端的球柄状假体,所述的窝状假体通过表面的刺状结构和边缘的翼状钉孔固定于大多角骨上,实现初期稳定性。除此之外,窝状假体的外表面采用多孔结构,其能够促进骨长入,保证假体的远期稳定性。远端球柄状假体的柄部分与掌骨髓腔相匹配,彼此之间紧密压配,球状结构与上述的窝状结构内部相匹配。球状结构可在窝状结构内自由旋转运动,实现恢复关节的运动功能。这种腕掌关节重建假体设计更加贴近生理结构,可以有效地恢复患者的关节功能,进而改善生活质量。

如图1~2所示,本发明实施例中一种腕掌关节重建假体为球窝式关节联接结构,包括:掌骨侧的球柄状假体20和大多角骨30侧的窝状假体10,远端球柄状假体20的柄部22与掌骨髓腔40相匹配,彼此之间紧密压配,球状结构与窝状结构内部相匹配,球状结构可在窝状结构内自由旋转运动,实现重建腕掌关节的功能。假体的材料包括钛、钽、陶瓷或聚乙烯。假体的内结构为经拓扑优化出的支撑骨架25。
如图8所示,球窝式联接结构假体采用球状结构与窝状结构相结合的设计,其旋转中心更接近生理旋转中心,球状结构可以在窝状结构内自由旋转运动,可以更好地模拟人体的运动状态,具有更灵活的运动能力。且凸面位于大多角骨30侧,更容易调整关节角度,可以提供更高的固定稳定性。
大多角骨30处将损坏的骨质切除,并使用特定大小的磨锉进行修整,使窝状假体10能够实现紧密压配与固定,同时窝状假体10在球面上分布有3个刺样结构12,边缘设计带有钉孔的翼状结构13,窝状假体10通过螺钉穿过翼状结构13的螺纹孔14固定于大多角骨30。翼状结构13可为1个或多个,其作用是便于螺钉将窝状假体10与大多角骨30进行固定,以增强其初始稳定性。
如图3~5所示,窝状假体10的外表面为第一多孔结构11或3D打印仿生骨小梁结构,有助于骨组织长入和周围软组织生长,与人体组织的相容性更好,实现长期稳定性。窝状假体10的内表面15为高抛光镜面或金属镜面镀层,以减小其摩擦系数,可以减少磨损和摩擦,延长假体的使用寿命。
如图6、图7所示,球柄状假体20分为球头部21、颈部23与柄部22,球头部21与窝状结构相匹配,柄部22与掌骨髓腔40相匹配,并且轮廓表面径向的向远端渐缩,并依据患者的掌骨髓腔40设计假体角度及大小,保证其稳定压配。球柄状假体20的球头部21为高抛光镜面或金属镜面镀层,以减小其摩擦系数。球柄状假体20的柄部22的外形与掌骨髓腔40相同,彼此可以紧密压配,获得良好的初始稳定性,其颈部23轮廓表面径向的向近端渐缩,并和球头部21及柄部22是一体成型。球柄状假体20柄部22的结构为第二多孔结构24或3D打印仿骨小梁结构,以利于骨长入,提供长期稳定性。
上述实施例中的腕掌关节重建假体,具体包括近端的窝状假体和远端的球柄状假体,窝状假体通过表面的刺状结构和边缘的翼状钉孔固定于大多角骨上,实现初期稳定性。除此之外,窝状假体的外表面采用多孔结构或仿生骨小梁结构,其能够促进骨长入,保证假体的远期稳定性。远端球柄状假体的柄部与掌骨髓腔相匹配,彼此之间紧密压配,球状结构与上述的窝状结构内部相匹配。球状结构可在窝状结构内自由旋转运动。这种腕掌关节重建假体设计更加贴近生理结构,可以有效地恢复患者的关节功能,进而改善生活质量,达到较好的治疗效果。

Claims (10)

1.一种腕掌关节重建假体,其特征在于,包括:球窝式关节联接结构;所述球窝式关节联接结构包括:固定于大多角骨(30)的窝状假体(10)和用于髓内接合的球柄状假体(20);
所述窝状假体(10)的外表面为第一多孔结构(11)或仿生骨小梁结构;所述窝状假体(10)的内表面(15)光滑;
所述球柄状假体(20)为球头部(21)、颈部(23)和柄部(22)依次联接形成的一体结构,所述球头部(21)与所述窝状结构相匹配,所述球头部(21)与所述窝状假体(10)的内表面(15)相接触,所述球头部(21)能够在所述窝状假体(10)内自由旋转运动;所述柄部(22)与掌骨髓腔(40)相匹配,所述柄部(22)与掌骨髓腔(40)之间紧密压配;所述柄部(22)的外表面为第二多孔结构(24)或仿生骨小梁结构。
2.根据权利要求1所述的腕掌关节重建假体,其特征在于,所述窝状假体(10)的内表面(15)为高抛光镜面或金属镜面镀层。
3.根据权利要求1所述的腕掌关节重建假体,其特征在于,所述窝状假体(10)的外表面边缘为带有螺纹孔(14)的翼状结构(13);所述窝状假体(10)通过螺钉穿过翼状结构(13)的螺纹孔(14)固定于所述大多角骨(30)。
4.根据权利要求1所述的腕掌关节重建假体,其特征在于,所述窝状假体(10)的外表面分布有若干个刺样结构(12);所述窝状假体(10)通过所述刺样结构(12)插入到所述大多角骨(30)。
5.根据权利要求1所述的腕掌关节重建假体,其特征在于,所述假体的材料包括钛、钽、陶瓷或聚乙烯。
6.根据权利要求1-5任一项所述的腕掌关节重建假体,其特征在于,所述假体的内结构为经拓扑优化出的支撑骨架(25)。
7.根据权利要求1所述的腕掌关节重建假体,其特征在于,所述柄部(22)的轮廓表面径向的向远端渐缩。
8.根据权利要求1所述的腕掌关节重建假体,其特征在于,所述颈部(23)的轮廓表面径向的向近端渐缩。
9.根据权利要求7或8所述的腕掌关节重建假体,其特征在于,所述球柄状假体(20)的角度和大小与患者的掌骨髓腔(40)相适应。
10.根据权利要求1所述的腕掌关节重建假体,其特征在于,所述仿生骨小梁结构由3D打印。
