Abstract
本发明提供了加巴喷丁在制备用于治疗心肌梗死后心力衰竭的心房肥大或房颤的药物中的用途。在心房肌细胞肥大模型中,α2δ1抑制剂加巴喷丁能显著改善心肌细胞面积、ANP蛋白表达水平、肥大相关基因ANP、BNP以及β‑MHC表达水平等病理性肥大表型。加巴喷丁改善病理性肥大的机制为通过抑制α2δ1与GluN1的相互作用,抑制了病理性肥大时GluN1/GluN2B型NMDA受体向心肌细胞膜的运输,从而抑制了病理性肥大时p‑CAMKII/p‑HDAC4通路的激活。在心肌梗死后心力衰竭的大鼠模型中,给予α2δ1抑制剂加巴喷丁,能显著改善左心房内径、重量、间质纤维化以及心肌细胞肥大等左心房重构表型,降低心房颤动的发生率。
Description
技术领域
本发明属于生物医药领域,涉及加巴喷丁,具体来说是加巴喷丁在制备用于治疗心肌梗死后心力衰竭的心房肥大或房颤的药物中的用途。
背景技术
随着人口老龄化的加剧,心力衰竭(Heart failure,HF)的发病率和患病率逐年增长,给社会经济带来巨大负担。HF的病因包括冠状动脉疾病、高血压、扩张性心肌病以及心律失常等。其中,心肌梗死(Myocardial infarction,MI)是HF最常见的病因。MI导致的瘢痕形成、炎症反应,以及交感神经系统、肾素-血管紧张素-醛固酮系统的异常激活,促进了广泛的心脏重构,最终导致慢性HF不良预后的产生。
近年来,HF患者的左心房重构日益受到关注。左心房重构可打破HF患者脆弱的液体平衡,导致HF失代偿反复发生,增加HF的住院率和全因死亡风险。左心房重构是评估HF患者预后和不良事件发生的有效指标之一。因此,探究HF后左心房重构的分子机制并对其进行早期干预,对于改善HF预后具有重要的临床意义。
α2δ1是电压依赖性钙通道的辅助亚基,在神经系统中广泛表达,能促进钙内流。近年发明显示,α2δ1也能与血小板反应蛋白1、GluN1、GluA1等蛋白相互作用,提示α2δ1蛋白具有广泛的生物学功能。在心脏中,α2δ1在心房的表达水平是心室的两倍,但关于其心血管系统的病理发明不充分。转录组测序结果显示,房颤患者左房比右房的α2δ1表达水平较窦性心律患者升高,提示α2δ1可能参与左心房生物学功能的调控。
加巴喷丁(Gabapentin、GP),化学式为C9H17NO2,是美国Warner-Lanbert公司首先开发的抗癫痫药,于1993年首次在英国上市,为白色至灰白色结晶性粉末。目前的发明显示,加巴喷丁能与α2δ1、α2δ2亚基vWA结构域前三个精氨酸序列结合。加巴喷丁对α2δ1及其互作蛋白的干扰,是其发挥药理作用的关键,而与α2δ2的结合,主要介导了药物的副作用。
目前,α2δ1与心力衰竭中左心房重构的关系尚不清楚。本发明通过体内和体外实验,首次证实了α2δ1参与心肌梗死后心力衰竭中左心房肥大的发生,并证实了加巴喷丁通过抑制α2δ1的生物学功能,在治疗心肌梗死后心力衰竭的心房肥大或房颤中具有积极效果。
发明内容
针对现有技术中的上述技术问题,本发明提供了加巴喷丁在制备用于治疗心肌梗死后心力衰竭的心房肥大或房颤的药物中的用途,所述的这种加巴喷丁在制备用于治疗心肌梗死后心力衰竭的心房肥大或房颤的药物中的用途要解决现有技术中的药物对于治疗心肌梗死后心力衰竭的心房肥大或房颤的效果不佳的技术问题。
本发明提供了加巴喷丁在制备用于治疗心肌梗死后心力衰竭的心房肥大或房颤的药物中的用途。
本发明在大鼠中,通过前降支结扎建立心肌梗死心衰大鼠模型,用心脏彩超评价心脏结构和功能,HE、Masson和WGA染色评价左心房重构,用免疫印迹、免疫荧光染色分析α2δ1在左心房中的表达和分布。在HL-1细胞中,用血管紧张素II刺激建立心肌细胞肥大模型,用α2δ1抑制剂加巴喷丁进行干预,通过免疫荧光、qPCR、免疫印迹等方法发现α2δ1对心肌细胞病理性肥大的调控。通过免疫共沉淀和免疫印迹分析,探讨α2δ1参与心肌细胞病理性肥大的分子机制。在心衰大鼠模型中,给予不同剂量的加巴喷丁治疗,通过组织病理学、ELISA、qPCR以及电生理检查等方法,评估加巴喷丁对左心房肥大和房颤的疗效。
本发明成功构建了大鼠心肌梗死后心力衰竭的左心房重构模型。心脏超声提示,左心房扩张,组织病理学显示心肌细胞增粗、排列紊乱,心肌间质纤维化。免疫荧光显示,α2δ1在心房肌细胞中表达。免疫印迹显示,α2δ1在左房的全细胞和细胞膜上表达上调,细胞浆中表达下调。在HL-1肥大细胞模型中观察到,α2δ1在全细胞和细胞膜上表达上调,在细胞浆中表达下调。应用加巴喷丁能显著降低病理性肥大心肌细胞的面积,下调肥大相关基因ANP、BNP以及β-MHC的表达水平。免疫共沉淀显示,α2δ1和GluN1存在相互作用,且在心肌细胞发生病理性肥大时,α2δ1和GluN1相互作用增强。应用加巴喷丁,能减少α2δ1和GluN1的相互作用,降低GluN1/GluN2B型N-甲基-D-天冬氨酸(N-methyl-D-aspartic acid,NMDA)受体的细胞膜表达水平。加巴喷丁以及NMDA受体抑制剂能抑制心肌细胞肥大相关通路p-CAMKII/p-HDAC4的激活。给予心衰大鼠50mg/kg、100mg/kg或200mg/kg的加巴喷丁治疗28天,能显著改善大鼠的左心房心肌细胞大小、纤维化程度,降低房颤诱发率以及房颤持续时间,抑制肥大相关基因ANP、BNP以及β-MHC的表达。在不同浓度的加巴喷丁治疗组中均观察到α2δ1和GluN1共定位的减少。在200mg/kg治疗剂量的心衰大鼠中观察到细胞膜的GluN1/GluN2B型NMDA受体表达下调,p-CAMKII/p-HDAC4通路抑制。
本发明和已有技术相比,其技术进步是显著的。在心房肌细胞肥大模型中,给予α2δ1抑制剂加巴喷丁,能显著降低病理性肥大心肌细胞的面积。在心肌梗死后心衰大鼠模型中,给予α2δ1抑制剂加巴喷丁,能显著改善心衰的左心房重构,降低房颤的发生率。

具体实施方式
实施例1:大鼠心衰模型的建立
动物实验根据实验动物护理和使用指南进行,并经哈尔滨医科大学动物伦理和使用委员会批准。术前需对雄性SD大鼠进行称重,选择体重250-300g的大鼠进行后续实验。抓取大鼠后予戊巴比妥钠45mg/kg腹腔注射,待大鼠麻醉成功后对其进行气管插管和心电监测。暴露大鼠心脏,用弯头镊轻柔撕开心包,通过寻找左心耳和肺动脉圆锥确定冠状动脉左前降支位置。在左心耳下缘2mm处进针并向肺动脉圆锥方向出针,进出针范围需横跨左心耳-肺动脉圆锥连线中点与心尖连线,进针深度约1.5mm,结扎左前降支后可见结扎点以下心肌变白,心电图出现ST段抬高。逐层缝合大鼠胸腔,待大鼠生命体征平稳后脱机拔管。术中仅穿针不结扎左前降支的为假手术组(Sham),术中穿针结扎左前降支的为HF组。
实施例2:大鼠心脏彩超检查和模型筛选
HF模型制作完成后对大鼠进行二维心脏超声检查,将结扎24小时内EF值小于60%的大鼠鉴定为HF模型成功大鼠。将HF组大鼠随机分笼饲养,并于术后第7天和第28天对大鼠进行心脏二维超声检查。
实施例3:大鼠心脏电生理检测
在HF 28天,对SD大鼠进行心脏电生理检测。首先对大鼠进行称重和麻醉。将麻醉成功的大鼠固定于手术台上,钝性分离大鼠肋间肌后用开胸器撑开,充分暴露左心耳。将定制的标测电极头端固定在大鼠的左心耳处,尾端连接至电生理记录仪,同时监测大鼠的肢体导联心电图。将电生理仪的基础周期长度设置为250ms,设置每8个基础起搏刺激S1后发放一个期前刺激S2,并按2ms步长持续递减发放S1和S2。将S2不能夺获左心房时的最长的S1S2间期记录为心房有效不应期(Atrial effective refractory period,AERP),需要连续测量3次AERP并求平均值作为最终的AERP值。
将电极固定于左心耳处,电刺激的参数设置为频率50Hz,输出振幅3.5V,脉宽2ms。连续发放电刺激3秒后终止刺激,观察并记录体表心电图II导联的房颤持续时间(Atrialfibrillation duration,AFD)。重复给予10次电刺激,观察10次中AF发生的次数即为房颤诱发率(Inducibility of atrial fibrillation,AFI)。AF诱发成功的标志为,在体表心电图II导联记录到持续的且大于1秒的快速心律失常,特征为p波消失且RR间期绝对不等。每只大鼠最终的AFD值为10次电刺激中所有AFD值的总和的平均值。
实施例4:形态学和组织病理学
术后4周,对Sham组和HF组的大鼠进行了安乐死。在安乐死前,记录大鼠体重(Bodyweight,BW)。随后,用生理盐水充分灌流后取出心脏。大鼠全心重量除以大鼠BW为全心肥大指数(Heart weight index,HWI),大鼠左心房重量除以大鼠体重为左心房肥大指数(Leftatrium weight index,LAWI)。大鼠心脏使用4%多聚甲醛固定后,依次进行脱水、包埋以及切片处理。对切片进行HE和Masson染色。
实施例5:细胞培养
HL-1细胞需培养在预先用5μg/mL纤维连接蛋白和0.02%明胶处理过的培养皿中。完全培养基由Claycomb培养基中加入2mM L-谷氨酰胺、100U/mL青霉素、100μg/mL链霉素、0.1mM肾上腺素和10%胎牛血清制备而成。在培养HL-1细胞时,完全培养基应每天更换。
实施例6:药物处理
在动物模型中,EF小于60%的大鼠被随机分配到HF组或加巴喷丁治疗组。将加巴喷丁溶解于质量百分比浓度为0.5%的羧甲基纤维素钠溶液。从术后第一天开始,每天予治疗组大鼠50mg/kg、100mg/kg或200mg/kg加巴喷丁灌胃,持续28天。
在细胞给药前,HL-1细胞需在补充2mM L-谷氨酰胺、100U/mL青霉素和100μg/mL链霉素的Claycomb培养基中至少培养24小时进行饥饿。使用相同的培养基进行药物稀释和后续培养。通过向培养基中添加1μmol/L血管紧张素II(AngII)处理24小时,建立病理性心肌肥大模型。对于GP干预,需先用100μM加巴喷丁预处理细胞2小时,随后将细胞孵育于含有100μM加巴喷丁和1μM AngII的培养基中。在发明NMDA受体功能时,将100μM NMDA受体抑制剂MK801与1μM AngII一同添加到培养基中。
实施例7:免疫荧光染色
为进行WGA染色,将石蜡切片进行脱蜡、水化和抗原修复。随后,切片在37℃与WGA孵育1小时。用PBS洗涤后,切片在室温下用DAPI处理10分钟,最后用抗荧光猝灭剂封片。为进行荧光双染,抗原修复的切片经通透、封闭后,依次与一抗、二抗以及DAPI孵育,最后封片。当HL-1细胞密度达60%时,进行鬼笔环肽染色。将细胞固定、通透及封闭后,依次与100nM鬼笔环肽以及DAPI孵育。
实施例8:蛋白提取和免疫印迹实验
用含有蛋白酶和磷酸酶抑制剂的RIPA缓冲液处理左心房组织和细胞,制备全细胞蛋白裂解液。按照膜蛋白提取试剂盒说明书提取膜蛋白。对提取的蛋白进行电泳、电转。对电转完成的PVDF膜进行封闭,依次孵育一抗以及二抗。最后,用ECL试剂检测蛋白条带。
实施例9:免疫共沉淀
将培养在10厘米培养皿中的细胞用冷PBS洗涤,然后用含有蛋白酶和磷酸酶抑制剂的NP40缓冲液裂解。收集蛋白上清液,用Protein A/G琼脂糖珠4℃孵育4小时。去除珠子后,用BCA法测上清液蛋白浓度。将1000μg的总蛋白均分成两组:α2δ1组和IgG组。α2δ1组加入10μLα2δ1抗体,而IgG组加入10μL IgG抗体,两组均在4℃孵育过夜。将琼脂糖珠加入每组中,4℃孵育过夜。离心收集琼脂糖珠,并将5×SDS上样缓冲液用RIPA稀释成2×SDS上样缓冲液,洗涤珠子。洗涤液和珠子用100℃煮10分钟,离心收集上清液进行Western blot分析。
实施例10:实时荧光定量PCR
在细胞和组织中使用TRIzol提取总RNA。提取的RNA使用逆转录试剂盒逆转录成cDNA。将cDNA定量稀释至200ng/μL。稀释的cDNA与引物、SYBR Green PCR Master Mix以及DEPC水构成反应体系,进行实时荧光定量PCR。本实验所涉及的引物序列详见下表:
实施例11:ELISA检测
按照大鼠心房利尿肽(ANP)ELISA试剂盒说明书进行操作。在96孔板中,加入50μL不同浓度的标准品或大鼠血浆样本的二倍稀释液。随后,依次加入生物素标记的检测抗体、洗涤缓冲液、HRP结合物、底物试剂和终止溶液。测量每个孔450nm处的吸光度值,并根据标准品的吸光度值绘制标准曲线。使用标准曲线确定样本的浓度。
实施例12:统计分析
本发明采用SPSS19进行统计学分析。首先使用Shapiro-Wilk检验对数据正态性进行分析。符合正态分布的数据的呈现形式为平均数±标准差。不符合正态分布的数据用中位数(四分位数范围)进行描述。当两组数据符合正态分布时,用t检验进行统计分析。当两组数据不符合正态分布时,用Wilcoxon检验进行统计分析。多组数据间比较采用单因素方差分析。定义p<0.05时具有统计学意义。
实验结果:
2.1心衰模型的建立
我们通过结扎SD大鼠的冠状动脉左前降支,造成大鼠MI,建立HF模型。在术后24小时内,将心脏彩超显示左心室EF值小于60%的大鼠视为HF造模成功,并纳入HF组进行后续发明。为评估HF大鼠的心脏结构、功能改变情况,我们继续完善入组大鼠术后7天和28天的心脏彩超检查,并将第28天作为发明终点进行取材。心脏彩超结果显示,随着HF病情的进展,左心室收缩、舒张功能进行性损害,心输出量显著降低。心脏取材后发现,HF 4周的大鼠出现全心增大。对HF 4周的大鼠的左心室横截面进行Masson染色可见,梗死区域鲜有心肌细胞,心室壁明显变薄,间质纤维化严重,较Sham组具有显著统计学差异。如图1所示。
2.2心衰模型中左心房重构的评估
HF 4周大鼠的体重较Sham组无明显差异,但HWI、LAWI增加,这提示HF后存在心脏病理性重塑。心脏M型超声显示,HF 4周的左心房较Sham组出现明显扩张。对HF 4周大鼠的左心房进行HE、Masson及WGA染色可发现,与Sham组相比,HF 4周的左心房心肌细胞直径显著增加、心肌细胞排列紊乱、间质胶原明显沉积。如图2所示。
2.3α2δ1在心衰4周大鼠的左心房中表达上调
对左心房组织匀浆进行免疫印迹发明,结果显示,α2δ1在HF 4周的左心房中表达显著上调。既往发明报道α2δ1在肌肉组织中主要分布于细胞膜,故使用细胞膜提取试剂盒,对左心房组织的细胞膜进行分离并进行免疫印迹分析。结果显示,在HF 4周的左心房组织中,α2δ1的细胞膜表达水平显著上调、细胞浆表达水平显著下调,这提示在疾病状态下,α2δ1的亚细胞定位发生改变。我们认为,α2δ1在HF的左心房重构中发挥一定功能。如图3所示。
2.4α2δ1在心衰4周大鼠的左心房中定位于心肌细胞
免疫荧光双染显示,α2δ1和心肌细胞特异性标志物α-actinin具有良好的共定位,这提示α2δ1在心肌细胞中分布。在HF 4周的左心房中,α2δ1在心肌细胞中具有表达上调趋势。因此,我们将重点研究左心房心肌细胞中的α2δ1。如图4所示。
2.5心房肌细胞肥大模型的建立
动物实验证实,α2δ1在HF 4周大鼠的左心房中表达上调并定位于心房肌细胞,且组织病理学结果显示,HF 4周大鼠的左心房心肌细胞存在病理性肥大。因此,我们认为α2δ1可能参与心房肌细胞的病理性肥大过程。既往发明显示,肾素-血管紧张素-醛固酮系统的过度激活在HF的左心房重构病程中发挥关键作用。因此,我们选择AngII作为心房肌细胞肥大模型的刺激药物。根据文献报道,心肌细胞肥大模型的AngII刺激浓度在0.1μmol/L-10μmol/L,其中1μmol/L为最常用的刺激浓度。我们选择1μmol/L作为模型的刺激浓度并进行模型验证。免疫荧光结果显示,Sigma公司所购得的HL-1细胞表达心肌细胞标志蛋白α-actinin,并且给予AngII 1μmol/L刺激24小时后,心肌细胞面积明显增大,较对照组具有统计学差异。免疫印迹结果显示,AngII 1μmol/L刺激24小时后,心肌细胞的ANP表达水平显著提高,肥大模型造模成功。如图5所示。
2.6α2δ1在心房肌细胞肥大模型中表达上调
免疫印迹结果显示,1μmol/L的AngII刺激HL-1细胞24小时后,全细胞以及细胞膜的α2δ1表达上调,细胞浆中的α2δ1表达下调,这与HF大鼠模型中左心房的α2δ1的变化趋势一致。这提示α2δ1可能参与心房肌细胞病理性肥大的发生。如图6所示。
2.7α2δ1抑制剂GP能改善心肌细胞肥大表型
为明确α2δ1是否参与调控心房肌细胞病理性肥大的发生和发展,我们使用针对α2δ1的药物-GP(加巴喷丁)进行干预,并观察药物干预后肥大表型的变化。通过CCK8实验筛选HL-1细胞的适合的GP药物浓度。结果显示,当GP浓度大于800μmol/L时出现明显的细胞活力降低。因此,选取200μmol/L作为GP(加巴喷丁)的给药浓度。如图7所示。
对心肌细胞骨架鬼笔环肽(Phalloidin)进行免疫荧光染色显示,AngII刺激时,心肌细胞表面积较对照组增大,而给予GP(加巴喷丁)治疗后,心肌细胞面积缩小,与对照组相当。在非病理状态下给予GP,不能缩小心肌细胞表面积。如图8所示。
qPCR结果显示,AngII刺激时给予GP(加巴喷丁)干预,能降低肥大相关基因ANP、BNP以及β-MHC的表达水平,而在生理情况下给予GP并不影响肥大相关基因的表达水平。免疫印迹结果显示,GP降低AngII刺激导致的全细胞α2δ1表达水平,并缓解AngII刺激造成的ANP蛋白表达上调。在生理情况下,给予GP并不影响α2δ1的蛋白表达水平。如图9所示。
综上所述,我们证实了α2δ1抑制剂GP的应用能够抑制病理性心肌肥大的发生。
2.8α2δ1抑制剂GP调控AngII刺激后NMDA受体的细胞膜分布
GP(加巴喷丁)可能导致α2δ1与其互作蛋白分离。免疫共沉淀结果显示,AngII刺激造成的α2δ1/GluN1复合体的增加,在使用200μmol/L的GP干预后下调。同时,应用GP造成AngII刺激时细胞膜的GluN1、GluN2B表达下调,细胞浆的GluN1上调,提示GP调控肥大的HL-1细胞的NMDA受体的细胞膜分布。如图10所示。
2.9GP抑制病理性肥大时p-CAMKII/p-HDAC4通路
免疫印迹结果显示,AngII刺激后p-CAMKII/p-HDAC4通路激活。而给予GP(加巴喷丁)干预能抑制AngII刺激导致的p-CAMKII/p-HDAC4通路的激活。我们认为,GP(加巴喷丁)干预可能是通过影响NMDA受体的细胞膜表达,从而影响p-CAMKII/p-HDAC4通路。在AngII刺激时给予NMDA受体抑制剂MK801能够显著抑制异常激活的p-CAMKII/p-HDAC4通路,这提示NMDA受体在AngII刺激时是具有活跃功能的。因此,GP(加巴喷丁)降低NMDA受体的细胞膜表达能够抑制p-CAMKII/p-HDAC4的激活。如图11所示。
2.10在心衰动物模型中用α2δ1抑制剂GP(加巴喷丁)干预
2.10.1GP给药方案的确定以及心脏彩超的评估
为探究α2δ1是否是慢性心力衰竭左心房重构的治疗靶点,我们在HF大鼠中,用GP进行治疗干预。将EF值低于60%的大鼠随机分入HF组或HF加不同浓度梯度的GP治疗组。在左前降支结扎术后完成心脏彩超检查并确定分组后,给予HF大鼠50mg/kg、100mg/kg或200mg/kg的GP每日灌胃治疗,持续28天后处死大鼠。心脏彩超结果显示,给予GP治疗28天后,GP100以及GP200组出现EF值显著改善。GP(加巴喷丁)治疗一周后,左心房前后径在三个治疗剂量中均无明显变化。在治疗28天后,三个剂量的左心房前后径均较HF组显著改善。但是,给予50mg/kg的GP治疗虽不能改善左心室EF值,但能显著缩小左心房前后径。因此,我们认为GP(加巴喷丁)能直接干预左心房的病理性重构过程,发挥左心房保护作用,降低心房颤动的发生率。如图12所示。
2.10.2在心衰动物模型中用GP干预后的HWI及LAWI评估
对不同浓度GP(加巴喷丁)治疗后的心脏以及左心房进行分离并称重,结果显示,不同浓度的GP均能有效减轻HF 4周大鼠的HWI以及LAWI。如图13所示。
2.10.3在心衰动物模型中用GP干预后的心房肌细胞横截面积、间质纤维化程度评估
对Sham组、HF组以及各种浓度梯度的GP治疗组进行HE、WGA以及Masson染色。HE及WGA染色结果显示,HF组的心房肌细胞截面积较对照组显著增大,给予GP(加巴喷丁)治疗后,截面积显著恢复至正常水平。Masson染色结果显示,不同浓度的GP(加巴喷丁)治疗均能显著缓解左心房纤维化。如图14所示。
2.10.4在心衰动物模型中用GP干预后的左心房α2δ1/GluN1共定位减少
对α2δ1和GluN1进行免疫荧光双染后发现,Sham组的左心房α2δ1和GluN1存在少量共定位。在HF 4周,左心房中的α2δ1和GluN1共定位增多,而给予GP(加巴喷丁)治疗后,共定位减少。这提示在体水平应用GP(加巴喷丁)能够干扰α2δ1与GluN1的相互作用,这与HL-1细胞中观察到的现象一致。如图15所示。
2.10.5在心衰动物模型中用GP干预后的左心房电生理学评估
对HF大鼠进行左心房电生理学检查后发现,与Sham组相比,HF组大鼠的左心房AERP显著缩短。而给予GP(加巴喷丁)治疗后,左心房AERP延长,同时AF诱发率和AF持续时间较HF组显著改善。因此,我们认为,在心肌梗死后心衰大鼠模型中,给予α2δ1抑制剂加巴喷丁能显著改善心衰的左心房重构,降低心房颤动的发生率。如图16所示。
2.10.6在心衰动物模型中用GP干预后的体表心电图评估
α2δ1和谷氨酸受体在心脏传导系统,包括窦房结和房室结等处表达。因此,我们通过体表心电图评估GP(加巴喷丁)对传导系统的不良影响。不同剂量的GP不影响心率、PR间期、QRS波宽度以及QT间期等心脏电学参数,对心脏传导系统无明显不良影响。如图17所示。
2.10.7在心衰动物模型中用GP干预后左心房的肥大相关基因的表达水平和血浆ANP水平评估
HF组的肥大相关基因ANP、BNP以及β-MHC表达水平较Sham组显著上调。予不同剂量的GP(加巴喷丁)治疗后,肥大相关基因表达下调。HF 4周大鼠的血浆ANP表达水平显著升高,而给予不同浓度的GP治疗后,血浆ANP可降至正常水平。上述实验证明,GP(加巴喷丁)能改善心肌梗死后心力衰竭的心房肌细胞病理性肥大。如图18所示。
2.10.8在心衰动物模型中用GP干预后的α2δ1、GluN1和GluN2B细胞膜表达以及p-CAMKII/p-HDAC4通路的变化
我们选择综合治疗效果最好的GP200组进行α2δ1、GluN1和GluN2B表达分析。免疫印迹结果显示,GP降低了HF 4周大鼠左心房全细胞的α2δ1表达水平。GP治疗后,细胞膜的α2δ1、GluN1和GluN2B恢复至正常水平,这提示GP降低了HF左心房中细胞膜分布的NMDA受体的数量。HF 4周大鼠左心房的p-CAMKII/p-HDAC4通路激活,而给予GP(加巴喷丁)后,CAMKII磷酸化水平和HDAC4磷酸化水平均降低。GP可在在体水平抑制p-CAMKII/P-HDAC4通路的激活,这与前述细胞模型的结果一致。我们认为,干预α2δ1,导致NMDA受体的心房肌细胞膜表达下调,应该是HF后左心房肥大的治疗靶点。如图19所示。
