Description
Technical Field
The invention relates to suture reduction of an endoscope-assisted temporomandibular joint disc, in particular to a suture trocar. In addition, the invention also relates to a method for using the suture trocar comprising the suture trocar.
Background
Anterior temporomandibular joint discal displacement is a common disorder in temporomandibular joint disorders, and usually results in symptoms such as joint pain, abnormal sound, and limited mandibular movement. Temporomandibular joint disc replacement therapy may restore normal disc-condyle relationship, relieve pain and open mouth limitation. In 1989, medical researchers have sequentially proposed endoscopic assisted reduction and suturing of fixed articular discs to treat articular disc displacement or recurrent dislocation of joints, and then another group of medical researchers reported improved endoscopic suturing techniques in 1992, 1994 and 2012, respectively. An endoscopic assisted temporomandibular joint disc suture reduction is one of the common joint disc reduction treatment methods, and is an operation type in which a surgeon uses an endoscopic instrument to operate in the upper cavity of the temporomandibular joint, to reduce the joint disc and suture and fix it under the cartilage of the anterior wall of the external auditory canal.
Currently, the general use of Shanghai university of traffic Yang Chi teaches the invention of surgical procedures including releasing the anterior attachment of the disc, repositioning the disc, and suture fixation. The suture fixation needs to use a special joint disc reset suture line, and the two ends of the suture line are hardened by special treatment of medical biological glue with the length of about 1/6, so that the suture needle can be conveniently penetrated in the operation. When the joint disc is sutured and fixed, a 12-gauge arthroscope suture needle is needed to penetrate into the joint upper cavity, and on the junction line of the joint disc rear belt and the double-plate area, a middle-outer 1/3 junction point which is trisected from outside to inside is penetrated into the joint disc, and the joint disc is penetrated out from the middle-outer 1/3 junction point. Then, the operator inserts the suture needle and the loop from the anterior wall of the external auditory canal, adjusts the positions of the suture needle tip and the loop, penetrates the suture into the 12-gauge suture needle, passes the tip through the loop, rotates the loop by 90 degrees, tightens the suture and pulls out the suture from the external auditory canal. In operation, the suture is easy to buckle at one end of the hard treatment in the process of penetrating the suture needle, the pointed end of the suture is difficult to accurately penetrate through the ferrule, so that the suture is time-consuming and labor-consuming, in addition, the operation requirement on operators is extremely high in the operation process, the two operation processes of penetrating the suture needle and sleeving the puncture needle with the ferrule and pulling out the suture are the speed limiting steps of the operation, and the operation failure is easy to be caused by improper threading operation or improper double-needle matching, so that the suture is wasted, the operation time is prolonged, and the operation wound is extra to a patient.
In view of this, there is a need for a new type of suture trocar.
