Description
技术领域
本发明涉及医疗设备技术领域,更具体地说,它涉及一种肠黏膜组织氧饱和度监测监护仪。
背景技术
肠道组织极易在各类应激条件下,如手术、创伤烧伤、感染等情况下,出现因微循环紊乱和组织氧供需失衡导致的肠损伤,因漏诊率较高,常常导致肠功能恢复延迟以及因肠道缺血缺氧进一步引起肠道屏障损害。而肠屏障的损害又会加重肠腔内菌群易位,导致内毒素入血以及全身炎性反应,形成恶性循环出现各类并发症,使病程延长,并增加医疗负担。但肠损伤又是临床上较易忽略的重要临床病症,其主要原因即对肠屏障损伤的判断缺乏客观便捷的评价方法。不同于肝肾功能的生化检查,呼吸功能的血气分析与肺部CT检查。肠损伤的评估目前仍局限在临床症状的评价上,如:是否存在胃潴留、肠鸣音减弱、恶心呕吐、腹胀腹泻等。但当病人出现临床症状时,往往损伤即已发生。而一旦出现损伤则进一步会影响患者的营养治疗等系列救治策略,形成恶性循环。
另一方面,腹部手术围术期肠道组织微循环障碍和肠道组织氧供需失衡也是影响患者术后预后的关键因素。肠道组织氧供需失衡易导致吻合口愈合不良,吻合口漏的发生。因此,加强动态实时监测和管理对于降低此类外科并发症至关重要。目前已有胃肠黏膜内pH值的监测,但方法为间接测定方法,存在监测延迟和受到多种外界因素影响的缺陷,导致其难以满足临床实际需要。
近红外光谱技术(Near Infrared Spectroscopy,NIRS)是一种无创检测组织血氧饱和度的方法,机体组织对于波长在700nm~900nm之间光具有低吸收、高散射的特性。该范围内光子从人体表面的皮肤射入,可达到2厘米左右深度组织,从而可以检测组织的氧饱和度。近红外光谱技术在组织氧、血氧饱和度监测等方面已有四十多年的发展历史。传统的血氧检测设备是利用光电容积脉搏波来测量动脉中的血氧饱和度,多用来初步评估患者呼吸系统的氧合功能。但光电容积脉搏波测量技术无法测量静脉的血氧饱和度,也不适合在没有脉搏波的部位使用。
因此亟需一种可以连续、直观、便捷快速的评价肠道潜在损伤的指标,做到早发现早治疗的目的。鉴于肠道有效的微循环是维持肠屏障稳态的根本,检测肠组织的氧饱和度则可更为早期的评价肠道微循环异常,预测肠屏障损伤的发生。并可用于指导治疗措施(如营养治疗)的实施及治疗疗效的评价。小肠的血供来自肠系膜上动脉,且大部分营养物质的吸收在小肠,因此对小肠的肠黏膜组织氧饱和度监测则可做到更加准确评估肠道损伤的情况。
发明内容
为了克服现有技术中所存在的上述缺陷,本发明提供了一种肠黏膜组织氧饱和度监测监护仪,可用于直接、客观连续评估小肠组织黏膜微循环与组织氧耗变化情况,可用于多种临床环境;且基于空肠导管放置技术,该技术为成熟的临床操作技术,技术难度低,可床旁操作,适用于基层医院以及普通病房开展。
本发明的上述技术目的是通过以下技术方案得以实现的:一种肠黏膜组织氧饱和度监测监护仪,包括监护仪本体和导管,导管的一端的外壁套设有气囊,气囊的外壁设有近红外光谱发射及探测装置,近红外光谱发射及探测装置与监护仪本体之间通过导线连接;气囊连接有充气管,充气管的另一端设有充气装置;导管内设有隔板,隔板将导管内部分隔为管路一和管路二,管路一包括分别供导线和充气管通过的两个子管路,管路一靠近气囊的一端封堵处理,管路二为营养液输送管路。
进一步的,近红外光谱发射及探测装置包括单束近红外谱发射端和接受端感应电极;单束近红外谱发射端和接受端感应电极整合于电极片上。
进一步的,单束近红外谱发射端的发射波长为760nm或840nm。
进一步的,近红外光谱发射及探测装置设有两个,两个近红外光谱发射及探测装置对称分布于气囊两侧。
进一步的,管路二内设有导丝。
综上所述,本发明具有以下有益效果:本发明可用于直接、客观连续评估小肠组织黏膜微循环与组织氧耗变化情况,可用于下述多种临床环境:
1.据已有文献报道,重症监护病房中有60%甚至更高的患者在住院期间出现肠损伤。但目前临床中评估方法仍局限在临床症状的评估。比如,以是否腹泻、腹胀、便秘、肠鸣音减弱等进行评估。本发明通过连续评估小肠组织黏膜微循环与组织氧供需变化情况,起到了早期诊断、早期预警的作用。
2.用于需行肠内营养治疗的各类患者。因各种原因所致的禁食患者,在启动营养治疗后会出现营养不耐受、再灌食综合征等并发症,漏诊误诊会导致病情的反复或恶化,延长住院时间并增加医疗负担。本发明可用于营养期间肠黏膜微循环血运和氧耗评价,反映肠内营养对当前肠道功能的匹配,是否存在肠道超负荷运作的问题,进行及时调整减量,降低并发症的发生。
3.用于各类胃肠道手术围术期高危人群。因肠梗阻、肠动脉栓塞、肿瘤等原因需行胃肠道手术的患者,手术区域的肠道往往存在肠壁水肿等问题,但切除过多肠组织不利于患者远期预后,切除过少则术后易发生术后并发症。本发明术中可视下放置该肠管致拟切除部位,可以直观评价肠黏膜的血运及微循环情况,间接评价肠道活力,帮助外科医生决定手术切除范围。切除后可固定至切口附近,用于术后评价切口吻合处肠道的恢复情况,利于早期发现肠漏等并发症。
本发明提供的一种肠黏膜组织氧饱和度监测监护仪基于空肠导管放置技术,即成熟的临床操作技术,技术难度低,可床旁操作,适用于基层医院以及普通病房开展。

具体实施方式
以下结合附图1-4对本发明作进一步详细说明。
实施例:一种肠黏膜组织氧饱和度监测监护仪,如图1至图3所示,包括监护仪本体1和导管2,导管2长度为2.5m,导管2的肠腔端的外壁套安装有气囊3,气囊3的外壁安装有近红外光谱发射及探测装置4,近红外光谱发射及探测装置4有两个,两个近红外光谱发射及探测装置4对称分布于气囊3两侧,近红外光谱发射及探测装置4与监护仪本体1之间通过导线5连接,可将监测数据显示于监护仪本体1的显示屏上;气囊3连接有充气管6,充气管6的另一端连接有充气装置;充气装置为注射器,可向气囊3内注入定量气体将气囊3撑起,导管2内固定安装有隔板7,隔板7将导管2内部分隔为管路一8和管路二9,管路一8包括供导线5和充气管6通过的两个子管路,管路一8靠近气囊3的一端封堵处理,管路二9用于营养液的输入,管路二9内安装有导丝,便于放置肠管,当肠管到位后将导丝退出,用于提供力度支撑,提高导管2放置的成功率,导丝最长可达到空肠部位。
近红外光谱发射及探测装置4包括单束近红外谱发射端和接受端感应电极;单束近红外谱发射端和接受端感应电极整合于电极片上;电极片安装于气囊3内壁,可防止消化液的腐蚀,单束近红外谱发射端的发射波长为760nm或840nm。
如图4所示,为术中情况示意图,将本装置伸入肠管内,两虚线之间的部分为拟切除手术区域,两斜线区域为待定手术边界,通过使用本装置检测该区域的组织氧饱和度,选定可接受的氧饱和度处为离断处,切除病灶肠组织。
本发明可应用于下述几种情况:
1.连续评估小肠组织黏膜微循环与组织氧供需变化情况。将本发明导管放置于空肠处,同时监护仪连接指夹式氧饱和监测设备用于与组织氧饱和数据相对照。在指夹式氧饱和度数据在正常范围内时,即说明此时循环中经肺氧合的血液中氧饱和度已达正常,即氧输送为正常范围。此时,如肠黏膜组织中氧饱和度降低或低于组织氧饱和度正常下限值,则说明胃肠道的氧气输送和消耗不匹配,即存在供需失衡,可能为胃肠道有效循环血量降低,或胃肠道消耗增加。此时提示临床医生需要结合病人临床情况进一步进行排除。
2.营养期间肠黏膜微循环血运和氧耗评价,反映肠内营养对当前肠道功能的匹配,是否存在肠道超负荷运作的问题。基于上述情况,当患者的血流动力学已达稳定状态,即各类原因所致的休克已纠正,非疾病的急性期,机体状态相对稳定。则此时临床往往考虑予以患者肠内营养,因此,此时认为胃肠的血供,即有效循环血量未出现异常,但由于胃肠道长期未接受营养治疗功能减退,同时疾病或应激状态的打击会导致肠道的损伤。基于此,如给予肠内营养前后,监测到的肠黏膜组织氧饱和度呈持续降低趋势,提示此时肠内营养量与肠道功能不匹配,需调整肠内营养速度和量。
3.用于各类胃肠道手术围术期高危人群。基于上述情况,此类患者如肠扭转、肠梗阻、肠系膜动脉栓塞等需行手术治疗的肠道疾病,多以胃肠道血运异常,即胃肠道的有效循环血量降低居多,因此,病变部位的组织黏膜往往存在氧供不足,导致无法满足组织的基本氧需求,组织为缺氧状态,即组织氧饱和度减低。则如在术中手术视野内放置导管至拟切除部位附近,检测附近的组织氧饱和度,即可反应此处血供是否存在,以及血供是否良好,用来辅助指导术者确定切除病灶肠管的长度。同时如行肠吻合术后,对于担心术后吻合口漏,肠瘘等高危人群,可将导管放置于吻合口附近。如术后恢复不佳,患者往往肠壁水肿较重,缝合处愈合不良,病变附近肠组织氧饱和度也因水肿及血运建立不良等原因而降低,因此,术后持续监测吻合口附近肠管氧饱和度可起到警示作用,有助于帮助外科医生区分术后疼痛性质为手术区域疼痛还是存在吻合口漏吻合口愈合不良等高危因素,及时予以进一步诊断和治疗,避免病情延误。另一方面,该导管于术中麻醉下完成放置后待患者术后恢复排气,还可通过本发明进一步予以肠内营养,减少术后放置营养导管的不适,对于术后仍较难恢复经口正常饮食的高危患者尤为适用。

1.一种肠黏膜组织氧饱和度监测监护仪,其特征在于,包括监护仪本体(1)和导管(2),所述导管(2)的一端的外壁套设有气囊(3),所述气囊(3)的外壁设有近红外光谱发射及探测装置(4),所述近红外光谱发射及探测装置(4)与监护仪本体(1)之间通过导线(5)连接;所述气囊(3)连接有充气管(6),所述充气管(6)的另一端设有充气装置;所述导管(2)内设有隔板(7),所述隔板(7)将导管(2)内部分隔为管路一(8)和管路二(9),所述管路一(8)包括分别供导线(5)和充气管(6)通过的两个子管路,所述管路一(8)靠近气囊(3)的一端封堵处理,所述管路二(9)为营养液输送管路。
2.根据权利要求1所述的一种肠黏膜组织氧饱和度监测监护仪,其特征在于,所述近红外光谱发射及探测装置(4)包括单束近红外谱发射端和接受端感应电极;所述单束近红外谱发射端和接受端感应电极整合于电极片上。
3.根据权利要求2所述的一种肠黏膜组织氧饱和度监测监护仪,其特征在于,所述单束近红外谱发射端的发射波长为760nm或840nm。
4.根据权利要求1所述的一种肠黏膜组织氧饱和度监测监护仪,其特征在于,所述近红外光谱发射及探测装置(4)设有两个,两个所述近红外光谱发射及探测装置(4)对称分布于气囊(3)两侧。
