Abstract
本发明提供了肝癌衍生生长因子HDGF作为酪氨酸激酶抑制剂耐药靶点及相关应用。本发明提供了肝癌衍生生长因子HDGF作为靶点在筛选和/或制备具有改善患者对酪氨酸激酶抑制剂耐药作用的药物中的应用。本发明发现HDGF高表达是分子靶向药吉非替尼等酪氨酸激酶抑制剂耐药的机制之一。以HDGF为靶点可有效改善NSCLC对吉非替尼的耐药,提高疗效。

技术领域
本发明是关于与酪氨酸激酶抑制剂(TKI)耐药相关的靶点,特别是关于肝癌衍生生长因子(HDGF)作为非小细胞肺癌吉非替尼等TKI耐药靶点及相关应用。
背景技术
酪氨酸激酶(tyrosine kinase,TKs)是细胞信号转导途径中的重要因子,参与调节细胞生长、分化和凋亡等一系列生理生化过程。研究表明,在肿瘤组织中,酪氨酸激酶常被激活,继而激活下游的信号传导通路,促进细胞增殖,抑制细胞凋亡,促使肿瘤发展,由于酪氨酸激酶在肿瘤发生中的关键作用,因此酪氨酸激酶抑制剂(TKI)通过特异性阻断细胞增殖信号,成为了肿瘤治疗的重要靶向药物。
肺癌是目前世界上发病率和死亡率最高的恶性肿瘤,全球肺癌的发病率和死亡率均呈上升趋势。肺癌主要分为非小细胞肺癌(NSCLC)和小细胞肺癌(SCLC),其中非小细胞肺癌约占所有肺癌病例的80%。目前,肺癌的治疗仍是世界上最具挑战的难题之一。针对肺腺癌驱动基因的靶向药物已成为晚期非小细胞肺癌治疗的一线药物。表皮生长因子受体-酪氨酸激酶抑制剂(EGFR-TKI)能显著延长患者中位生存期,吉非替尼(gefitinib)是我国临床广泛应用的第一代TKI。虽然TKI对具有EGFR敏感突变的晚期非小细胞肺癌疗效较好,但终会发生耐药出现疾病进展。被寄予厚望的第二代TKI阿法替尼和第三代TKI AZD9291也同样存在耐药的问题。因此,耐药仍然是限制TKI临床疗效的重要原因,如何克服TKI耐药使非小细胞肺癌患者获益是关键科学问题。
目前已经明确的TKI耐药机制包括:EGFR的二次突变(T790M)(50-60%);旁路的异常活化(c-MET,HGF,AXL)(1–25%);下游信号通路异常(BRAF、PIK3CA或K-RAS突变、PTEN缺失等),非小细胞转变为小细胞或出现上皮-间质转化(EMT)组织病理转化(5–10%)、肿瘤的异质性等。此外,尚有20-30%的耐药原因尚未阐明。目前耐药后的主要挽救措施包括:化疗、第二代或第三代TKI、旁路抑制剂联合TKI,PD-1/PD-L1免疫疗法联合TKI。第二代或第三代TKI虽然对T790M突变的耐药具有一定的疗效,但会发生再次耐药,后面两种方案的疗效正在临床试验中。总体来讲,大部分临床研究用的单药或药物联合对第一代TKI耐药患者来说效果并不尽如人意。因此,耐药仍然是困扰TKI临床疗效的问题。如何克服TKI耐药仍需进行新的探索。
发明内容
本发明的一个目的在于寻找酪氨酸激酶抑制剂(TKI)耐药机制,以筛选能够减轻患者特别是非小细胞肺癌患者对于TKI耐药性的相关药物或方法,克服或延缓耐药,提高TKI对患者的疗效。
本案发明人在研究中发现,肝癌衍生生长因子(HDGF)的表达水平与分子靶向药吉非替尼(gefitinib)等酪氨酸激酶抑制剂耐药性相关。以HDGF为靶点可有效改善NSCLC对吉非替尼等酪氨酸激酶抑制剂的耐药,提高疗效。
在本发明的一些具体实施方案中,本发明发现,HDGF高表达是分子靶向药吉非替尼等酪氨酸激酶抑制剂耐药的机制之一。吉非替尼耐药非小细胞肺癌细胞中HDGF表达水平显著高于敏感细胞。高水平的HDGF不仅与NSCLC的恶性表型相关,而且可以诱导吉非替尼耐药。与TKI耐药相关的信号通路,如PI3K/Akt和MEK/ERK通路,可以被HDGF以非EGFR依赖的方式旁路激活。抑制HDGF可增强吉非替尼对耐药NSCLC细胞的作用。敲低HDGF在耐药细胞的表达能显著降低吉非替尼的半数有效浓度(IC50)、减弱细胞克隆形成能力,促进肿瘤细胞凋亡,抑制耐药相关蛋白p-Akt和p-ERK的表达。将吉非替尼敏感细胞过表达HDGF则能显著提高吉非替尼的IC50,促进细胞克隆形成能力和划痕愈合能力,诱导耐药相关蛋白p-Akt和p-ERK的表达。体内研究则表明:过表达HDGF能促进NSCLC肿瘤的生长,促进耐药相关蛋白的表达。体内外研究结果表明:HDGF高表达是吉非替尼耐药机制之一,敲低HDGF表达可提高耐药NSCLC对吉非替尼的响应。HDGF是克服吉非替尼耐药的潜在靶点。
从而,一方面,本发明提供了肝癌衍生生长因子HDGF作为靶点在筛选和/或制备具有改善患者对酪氨酸激酶抑制剂耐药作用的药物中的应用。
另一方面,本发明还提供了检测肝癌衍生生长因子HDGF的表达水平的试剂在制备评估患者对酪氨酸激酶抑制剂耐药性的检测系统中的应用。
另一方面,本发明还提供了针对肝癌衍生生长因子HDGF的拮抗剂在制备用于治疗肿瘤和/或降低患者酪氨酸激酶抑制剂耐药性的药物中的应用。
本发明中,所述检测包括辅助检测,所述治疗包括辅助治疗。
本发明中所述针对肝癌衍生生长因子HDGF的拮抗剂为降低HDGF的表达水平和/或拮抗HDGF的作用的试剂。具有这样功能的试剂例如可包括但不限于sgRNA、小分子抑制剂、天然产物、抗体或其组合等。这样的试剂对于本领域技术人员而言可根据现有技术获得,可以是现有技术中已知的任何可降低HDGF的表达水平和/或拮抗HDGF的作用的拮抗剂本身,也可是基于该分子式进行修饰、改构后仍具有降低HDGF的表达水平和/或拮抗HDGF作用的功能的试剂。在本发明的一些具体实施方案中,利用基因技术敲低了HDGF的表达,可以有效地抑制肿瘤细胞的增殖、迁移扩散和/或侵袭能力,同时,也能够恢复对吉非替尼等酪氨酸激酶抑制剂的敏感性。
在本发明的一些具体实施方案中,降低酪氨酸激酶抑制剂耐药性包括降低酪氨酸激酶抑制剂的半数有效浓度(IC50)。
在本发明的一些具体实施方案中,降低酪氨酸激酶抑制剂耐药性包括减弱肿瘤细胞克隆形成能力,和/或抑制肿瘤细胞的增殖、迁移扩散和/或侵袭能力。
在本发明的一些具体实施方案中,降低酪氨酸激酶抑制剂耐药性包括促进肿瘤细胞凋亡,和/或抑制体内肿瘤生长。
在本发明的一些具体实施方案中,降低酪氨酸激酶抑制剂耐药性包括抑制耐药相关蛋白p-Akt和p-ERK的表达。
根据本发明的另一方面,本发明提供了使酪氨酸激酶抑制剂敏感细胞过表达HDGF的试剂在制备具有以下至少一种功效的研究制剂中的应用:提高酪氨酸激酶抑制剂的IC50;促进细胞克隆形成能力和/或划痕愈合能力;增强细胞的增殖、克隆形成以及迁移和侵袭能力;和/或诱导耐药相关蛋白p-Akt和p-ERK的表达。所述的研究制剂对于研究HDGF驱动吉非替尼等酪氨酸激酶抑制剂耐药机制具有重要意义。
根据本发明的具体实施方案,所述酪氨酸激酶抑制剂包括但不限于吉非替尼、埃克替尼、阿法替尼、AZD9291等中的一种或多种。
根据本发明的具体实施方案,所述患者为肿瘤患者。
根据本发明的具体实施方案,所述肿瘤为肺癌,例如非小细胞肺癌。
在本发明的一些具体实施方案中,本发明证明了HDGF促进NSCLC的发生和转移。在HDGF表达水平相对较低的H292和PC-9细胞中,强制表达HDGF可增强细胞的增殖、克隆形成以及迁移和侵袭能力。同时,外源性rhHDGF增强了H292和PC-9细胞的增殖,证实了HDGF作为一种生长刺激因子的作用。在HDGF过表达的PC-9异种移植小鼠中验证了HDGF促进肿瘤生长的功能。相反,通过CRISPR沉默HDGF可以抑制H1975细胞的恶性表型,并延缓体内肿瘤的发展。
在本发明的一些具体实施方案中,本发明的研究表明,敲低HDGF可以增加NSCLC对吉非替尼的敏感性,而HDGF过表达在一定程度上降低了吉非替尼的疗效。在体内外实验中,敲低HDGF抑制了H1975细胞的肿瘤生长,而在PC-9细胞中过表达HDGF则减弱了吉非替尼的作用。小鼠肿瘤组织切片中Ki-67和HDGF的免疫组化染色结果与上述结果趋势一致。这些数据表明HDGF的表达水平与吉非替尼在NSCLC中的疗效相关。
在本发明的一些具体实施方案中,本发明的研究表明,敲低HDGF可抑制Akt和ERK的磷酸化,而HDGF过表达具有相反的作用,表明它调节p-Akt和p-ERK的激活。在吉非替尼敏感的NSCLC细胞中,过表达HDGF可削弱吉非替尼抑制p-Akt和p-ERK的能力;然而,在耐药的H1975细胞中,HDGF敲除可显著抑制Akt和ERK的激活。采用Akt或ERK抑制剂验证HDGF是否通过这两条途径诱导细胞生长和吉非替尼耐药。上述结果表明HDGF可激活EGFR下游p-Akt和p-ERK信号通路,与吉非替尼耐药有关。
在本发明的一些具体实施方案中,本发明的研究结果表明,HDGF和p-EGFR的表达水平在HDGF敲低和过表达的NSCLC细胞中是互补的。吉非替尼可降低EGFR磷酸化水平,增加HDGF表达,并异常激活PI3K/Akt和MEK/ERK通路。然而,在H1975细胞中敲低HDGF后,p-EGFR表达增加,吉非替尼可使HDGF重新升高,p-EGFR则被降低,提示在吉非替尼作用下,NSCLC细胞中HDGF和p-EGFR之间存在微妙的平衡和互补关系。综合所有相关信息,HDGF水平的升高可能作为旁路生存信号触发吉非替尼耐药,通过沉默HDGF可以克服耐药。
综合而言,本发明发现HDGF不仅促进了NSCLC细胞的恶性表型,而且促进了酪氨酸激酶抑制剂的耐药。HDGF作为旁路和代偿性信号通路激活EGFR下游PI3K/Akt和MEK/ERK通路,与吉非替尼等酪氨酸激酶抑制剂耐药相关。靶向HDGF可以通过维持HDGF表达和EGFR激活之间的微妙平衡,以及它们共同的下游分子,恢复吉非替尼等酪氨酸激酶抑制剂的敏感性。因此,本发明证实了HDGF是治疗和恢复肿瘤患者对吉非替尼敏感性、提高TKI疗效的新靶点,利用任何技术干扰HDGF基因表达或干预HDGF的作用,可以有效地缓解肿瘤患者例如非小细胞肺癌对于吉非替尼等TKI的耐药性问题,可以有效抑制癌细胞的增殖扩散、迁徙和侵袭能力。

具体实施方式
为了对本发明的技术特征、目的和有益效果有更加清楚的理解,现对本发明的技术方案进行以下详细说明,但不能理解为对本发明的可实施范围的限定。
实验材料与方法
1.1细胞系与培养基
人非小细胞肺癌细胞株H1975,H520,H157,H460,H292和PC-9购自美国模式培养物集存库(美国弗吉尼亚州马纳萨斯)。A549和H1650细胞购自国家实验细胞资源共享服务平台(中国北京)。细胞在含10%FBS的培养基中培养,培养环境为温度为37℃、CO2浓度为5%。
1.2试剂
吉非替尼(Iressa)购自阿斯利康,将其溶解于二甲基亚砜(DMSO)中,浓度为20mM,储存在-20℃。MTE(Marsdenia tenacissima提取物,商品名:消癌平注射液)购自南京圣和药业有限公司。抗Akt(9272)、ERK1/2(9102)和p-ERK1/2(T202/Y204)(9101)的抗体购自Cell Signaling Technology(Beverly,MA)。HDGF和p-Akt(Ser473)获自Abcam(英国Cambridge)。EGFR和p-EGFR购自ABclonal公司(中国武汉)。GAPDH、β-actin和Ki-67抗体购自TDYbio(中国北京)。重组人HDGF(rhHDGF)购自ProSpec-Tany TechnoGene Ltd(cyt-681-a,以色列)。
1.3差异蛋白的鉴定
吉非替尼耐药细胞系H1975用吉非替尼和MTE中的一种或两种结合处理24小时。二维凝胶的差异斑点经胰蛋白酶消化后,通过LC-MS/MS系统分析多肽混合物,使用DataAnalysis 4.0处理MS/MS数据,并通过MASCOT V.2.4(Matrix Science Ltd)在Swissprot蛋白质序列数据库中进行搜索。使用10mM NaTFA来校准TOF质量分析仪。用于MS/MS离子检索的肽电荷状态为+2,+3和+4,MS/MS容差为±0.1Da。通过将搜索结果与估计的随机匹配总体进行比较来估计基于概率的MASCOT得分,并将其报告为-10×log(P),其中P为绝对概率。显著性阈值设置为P<0.05。
1.4稳定转染NSCLS细胞的建立
通过分子克隆获得重组HDGF质粒和pcDNA3.1-EGFR T790M突变质粒。将HDGF的单向导RNA(sgRNA)序列连接到LentiCRISPRv2质粒(sgRNA1:5′-GGAGTACAAATGCGGGGACC-3′(SEQ ID NO:1)和sgRNA2:5′-ACGTCCACACTTAACTGCGC-3′(SEQ ID NO:2))中。对照sgRNA(非靶向sgRNA-Con)的序列为5'-TTCTCCGAACGTGTCACGTT-3'(SEQ ID NO:3)。将质粒转染NSCLC细胞并筛选稳定细胞株用于后续研究。
1.5定量RT-PCR
使用Trizol试剂(Invitrogen,美国)提取总RNA。qRT-PCR具体操作按照文献:LiXH,He XR,Zhou YY,Zhao HY,Zheng WX,Jiang ST,et al.Taraxacum mongolicum extractinduced endoplasmic reticulum stress associated-apoptosis in triple-negativebreast cancer cells.J Ethnopharmacol.2017;206:55-64进行。
1.6细胞增殖实验
通过细胞计数来确定HDGF敲低或过表达以及rhHDGF诱导对NSCLC细胞生长的影响。MTT试验用于评估吉非替尼处理后的细胞生长抑制。用吉非替尼(0.001-50μM)处理细胞72h,测量570nm处的光密度,并使用GraphPad Prism 6.0软件计算IC50值。
1.7克隆形成实验
将H292,PC-9,H1975细胞及HDGF过表达/敲低的细胞接种在6cm培养皿中,孵育14天后,用4%多聚甲醛固定细胞,并用结晶紫染色,拍照后进行计数和比较。
1.8Transwell迁移和侵袭实验
在有或没有Matrigel胶的情况下,使用3×104密度的细胞在Transwell小室中进行迁移和侵袭分析。随机选取4个视野拍照,显微镜下计数细胞数。
1.9免疫印迹实验
免疫印迹按照文献:Han SY,Zhao MB,Zhuang GB,Li PP.Marsdenia tenacissimaextract restored gefitinib sensitivity in resistant non-small cell lungcancer cells.Lung Cancer.2012;75:30-7进行。使用增强型化学发光试剂盒对蛋白条带进行可视化,并通过ImageJ软件测定灰度。
1.10动物实验
所有动物均购自北京华阜康生物科技有限公司。将细胞接种于6~8周龄雄性BALB/c裸鼠皮下,建立NSCLC移植瘤模型。当肿瘤体积达到约50-100mm3时,使用Excel生成的随机数字将小鼠随机分组并按如下方法处理。实验结束后处死小鼠,收集血浆和肿瘤进行进一步分析。
将H1975细胞分为sgRNA对照组、sgRNA1组和sgRNA2组。其余小鼠接种过表达HDGF或其载体对照的PC-9细胞。HDGF敲低对吉非替尼作用的影响研究分为四组:sgRNA-control组、sgRNA2组、sgRNA2+吉非替尼(50mg/kg)组和sgRNA2+溶剂对照组。在PC-9细胞中,HDGF过表达对吉非替尼疗效的影响研究分为4组:载体对照组、HDGF过表达+吉非替尼(10mg/kg)组和HDGF过表达+溶剂对照组。采用盲法采集肿瘤体积数据。
1.11ELISA实验
采用酶联生物技术有限公司(上海)生产的试剂盒检测小鼠血浆中HDGF浓度。采用Anrc(天津)ELISA试剂盒检测NSCLC患者血浆中HDGF水平。
1.12免疫组织化学实验
免疫组织化学(IHC)按常规操作进行。用HDGF或Ki-67抗体孵育组织切片,Jackson(West Grove,PA)的过氧化物酶标记的山羊抗兔二抗,与来自Pierce(Rockford,IL)的过氧化物酶底物二氨基联苯胺(DAB)进行特异性结合。
1.13统计分析
数据以平均值±标准差表示。采用GraphPad Prism 6.0软件拟合数据,确定IC50值。不同组间基因表达的比较采用Mann-Whitney U检验,其余比较采用独立样本t检验,P值<0.05为差异有统计学意义。所有统计分析均采用SPSS 18.0进行。
实施例1:干扰HDGF的基因表达,或拮抗HDGF的作用,可有效抑制NSCLC的体外增殖,迁移和侵袭。
通过2D凝胶和LC-MS/MS分析,HDGF被确定为吉非替尼或MTE单独或联合处理的H1975细胞中差异表达的蛋白之一。Western blot结果显示,在H1975细胞中,吉非替尼和MTE联合处理后,与单独处理和对照组相比,HDGF显著下调。
本发明检测了HDGF在7个对吉非替尼有不同反应的NSCLC细胞系中的表达。吉非替尼敏感的PC-9和H292细胞HDGF表达水平相对较低,而耐药的H1975细胞的HDGF表达水平非常高。利用CRISPR/Cas9系统在H1975细胞中敲低HDGF,在PC-9和H292细胞中转染HDGFplenti6-TR质粒建立稳定的HDGF过表达细胞系。通过Western blot和qRT-PCR验证HDGF过表达或敲低效率。
结果参见图1A至图1J。通过Western blot和qRT-PCR验证了HDGF过表达或敲低(图1A、1B)。HDGF sgRNA1和sgRNA2显著抑制H1975细胞的增殖(图1C)、克隆形成(图1E、图1F)、迁移和侵袭(图1G、图1H)能力(P<0.01,P<0.001vs.control),表明HDGF敲低抑制了NSCLC细胞的恶性表型。相反,HDGF过表达(图1C)或rhHDGF刺激(图1D)促进H292和PC-9细胞增殖。同时,HDGF过表达增强了H292和PC-9细胞的克隆形成能力(图1E、图1F)和迁移和侵袭能力(图1I、图1J)(P<0.01,P<0.001vs.control)。因此,上述数据表明HDGF促进了NSCLC细胞的恶性表型。
实施例2:HDGF的表达水平影响NSCLC肿瘤生长的体内研究
本发明研究了HDGF与异种移植小鼠肿瘤生长之间的关系。如图2A至图2P所示,与对照组相比,两种HDGF sgRNA均显著抑制了肿瘤生长(图2A-图2C),且sgRNA2组的抑制作用更明显,表明HDGF敲低限制了H1975的肿瘤发展。同时,Western blot证实H1975肿瘤组织中HDGF的表达被这两种sgRNA抑制(图2D、图2E)。相反,与对照组相比,HDGF过表达显著增加PC-9肿瘤的体积和重量(图2F-图2H)。Western blot检测也证实PC-9肿瘤组织中HDGF水平较高(图2I、图2J)。以上结果表明,HDGF促进肿瘤生长,但HDGF敲低可抑制这一过程。
本发明的研究表明,EGFR下游分子p-Akt和p-ERK在NSCLC对吉非替尼耐药时异常激活。在本发明的实验中,这两个分子的改变在NSCLC细胞和小鼠肿瘤组织中被确定。与对照组相比,敲低HDGF可显著降低H1975细胞中p-Akt和p-ERK的表达,sgRNA2的抑制作用更强。相反,HDGF过表达增强了H292和PC-9细胞中的p-Akt和p-ERK水平(图2K、图2M)。在小鼠肿瘤组织中,p-Akt和p-ERK的变化与细胞中的变化一致(图2L、图2N)。在Akt抑制剂MK2206或ERK抑制剂U0126存在的情况下,HDGF过表达或rhHDGF诱导H292和PC-9细胞生长的作用被消除或逆转(图2O、图2P)。
实施例3:HDGF的表达水平与吉非替尼药效之间的关系
本发明进一步研究了HDGF表达与吉非替尼疗效的关系。如图3A所示,在H1975细胞中,sgRNA敲低HDGF对吉非替尼反应良好,IC50值分别为2.21μM和1.08μM,而sgRNA对照组为7.30μM。相反,与吉非替尼敏感的亲本H292和PC-9细胞相比,HDGF使吉非替尼的IC50值增加了17和20倍(图3B、图3C),提示发生了耐药。吉非替尼显著抑制HDGF敲低的H1975细胞的克隆形成、迁移侵袭能力,但对对照组的作用非常微弱(图3D、图3G、图3J、图3M)。相反,吉非替尼处理降低了H292和PC-9细胞的克隆形成(图3E、图3F、图3H、图3I)、迁移和侵袭能力(图3K、图3L、图3N、图3O),但这些抑制作用在HDGF过表达后减弱或消失。
实施例4:HDGF的敲除或者过表达在体内对吉非替尼药效的影响实验
为了评估HDGF与吉非替尼体内疗效的相关性,本发明在小鼠皮下种植了HDGF敲低或过表达的NSCLC细胞。如图4A-图4C所示,在H1975荷瘤小鼠中,50mg/kg吉非替尼没有出现明显的抗肿瘤作用。然而,在HDGF sgRNA2组中,肿瘤明显缩小,表明HDGF敲低提高了吉非替尼的敏感性。相反,在吉非替尼敏感的PC-9荷瘤小鼠中,与对照组相比,稳定表达HDGF的肿瘤生长迅速,但给药10mg/kg吉非替尼后,HDGF的促肿瘤作用减弱(图4D-图4F)。然而,HDGF过表达组和对照组均出现肿瘤消退,这可能与本实验中使用的吉非替尼剂量相对较高有关,因为PC-9细胞对吉非替尼非常敏感。尽管如此,在吉非替尼治疗后,HDGF过表达组的肿瘤生长仍然比对照组更快(P<0.05),提示HDGF过表达在一定程度上削弱了吉非替尼的疗效。如图4G所示,在H1975和PC-9异种移植的小鼠肿瘤组织中Ki-67和HDGF的表达与肿瘤体积和重量趋势一致。
HDGF是一种分泌性蛋白,本发明检测了NSCLC荷瘤小鼠血浆中的浓度,以探讨血浆HDGF水平是否与吉非替尼疗效相关。如图4H和图4I所示,小鼠血浆HDGF浓度与吉非替尼疗效的趋势一致。初步分析了8例NSCLC患者接受吉非替尼单药治疗前后血浆HDGF浓度。如图4J和图4K所示,与服药前相比,发生耐药时,每个病例及其平均血浆的HDGF水平均明显升高。
此外,本发明测定了5例NSCLC患者在接受第三代TKI AZD9291之前和之后的血浆HDGF浓度,与在吉非替尼获得性耐药的NSCLC患者中观察到的结果相似(图4L、图4M、图4N)。在TKI治疗前后配对活检标本中均检测到HDGF的表达,在TKI耐药后,两例标本的HDGF均呈高表达,提示较高的HDGF水平可能预示着TKI疗效不佳。
实施例5:HDGF驱动吉非替尼耐药的机制研究
本发明测定了NSCLC细胞(图5A)和小鼠肿瘤组织(图5B)中吉非替尼耐药相关蛋白p-Akt和p-ERK。在H1975细胞中,敲低HDGF后p-Akt和p-ERK的表达明显降低,吉非替尼使对照组p-Akt和p-ERK的表达升高,但仍然显著降低HDGF敲低组p-Akt和p-ERK的表达。吉非替尼显著抑制H292和PC-9细胞中p-Akt和p-ERK的表达,但不能逆转HDGF过表达导致的Akt和ERK磷酸化。上述数据进一步支持HDGF水平与吉非替尼耐药相关信号通路相关。
先前的研究结果表明,HDGF诱导的细胞生长被Akt或ERK通路的抑制剂所抑制或逆转。在图5C中,加入了MK2206(Akt抑制剂)或U0126(ERK抑制剂)来验证HDGF是否通过这两条信号通路促进吉非替尼耐药。结果表明,HDGF确实降低了PC-9细胞对吉非替尼的敏感性,而HDGF对吉非替尼耐药的促进作用则被MK2206或U0126明显减弱,提示HDGF可能通过Akt和ERK信号通路诱导吉非替尼耐药。
EGFR T790M突变是吉非替尼耐药的主要机制。将pcDNA3.1-EGFR T790M突变质粒和HDGF过表达载体单独或联合导入PC-9细胞,探讨HDGF是否参与了T790M介导的吉非替尼耐药。在图5D中,本发明的结果显示,T790M突变或HDGF过表达单独导致吉非替尼耐药,吉非替尼的IC50值分别为0.107μM和0.181μM。同时存在T790M突变和HDGF过表达的细胞对吉非替尼的耐药(IC50=0.253μM)比单独存在T790M突变和HDGF过表达的细胞对吉非替尼的耐药程度更高。这些结果提示T790M突变或HDGF过表达引起的吉非替尼耐药可能不具有相同的机制。因此,HDGF可能不参与由T790M突变引起的吉非替尼耐药。
本发明发现,沉默或过表达HDGF影响EGFR下游Akt和ERK的激活,这与TKI耐药相关。本发明进一步研究了HDGF和EGFR之间是否存在串扰。首先,在GeneCards数据库(http://www.genecards.org)中搜索Pathcards,发现HDGF通路与EGFR有包括Akt和ERK信号通路在内的大量重叠(图6A)。接下来,本发明发现在NSCLC细胞和肿瘤组织中,p-EGFR水平与HDGF敲低或过表达呈负相关(图6B、图6C)。除H157细胞外,吉非替尼剂量依赖性地抑制p-EGFR而增强HDGF,而H157细胞的p-EGFR和HDGF表达水平均极低(图6D、图6E),表明吉非替尼诱导的HDGF升高仅发生在EGFR依赖的NSCLC细胞中。与此同时,吉非替尼处理后,除H157细胞外的NSCLC细胞上清液中分泌的HDGF浓度也增加(图6F)。
本发明进一步探索HDGF和EGFR是否在HDGF敲低和过表达的NSCLC细胞对吉非替尼的应答中起互补作用。如图6G和图6I所示,HDGF敲低引起HDGF表达下降,吉非替尼处理H1975细胞后,p-EGFR的上调被逆转。在H292和PC-9细胞中,HDGF过表达导致HDGF升高和p-EGFR抑制,吉非替尼进一步抑制p-EGFR,但对HDGF表达无明显影响。如图6H和图6J所示,在HDGF过表达的PC-9异种移植瘤中,吉非替尼增加了HDGF的表达(P<0.05),肿瘤组织中HDGF和p-EGFR的变化与细胞实验一致。因此,上述数据表明在NSCLC中HDGF与EGFR之间存在交互作用,HDGF可能作为EGFR的旁路信号分子激活下游分子Akt和ERK。此外,HDGF和EGFR在维持吉非替尼作用下的肿瘤细胞存活方面具有互补作用。HDGF在吉非替尼耐药中的作用如图7所示。
以上所述实施例并不能理解为对本发明专利范围的限制。应当指出的是,对于本领域的普通技术人员来说,在不脱离本发明构思的前提下,还可以做出若干变形和改进,这些都属于本发明的保护范围。
Claims (10)
1.肝癌衍生生长因子HDGF作为靶点在筛选和/或制备具有改善患者对酪氨酸激酶抑制剂耐药作用的药物中的应用。
2.检测肝癌衍生生长因子HDGF的表达水平的试剂在制备评估患者对酪氨酸激酶抑制剂耐药性的检测系统中的应用。
3.针对肝癌衍生生长因子HDGF的拮抗剂在制备用于治疗肿瘤和/或降低患者酪氨酸激酶抑制剂耐药性的药物中的应用,其中所述针对肝癌衍生生长因子HDGF的拮抗剂为降低HDGF的表达水平和/或拮抗HDGF的作用的试剂。
4.根据权利要求3所述的应用,其中,降低酪氨酸激酶抑制剂耐药性包括降低酪氨酸激酶抑制剂的半数有效浓度(IC50)。
5.根据权利要求3所述的应用,其中,降低酪氨酸激酶抑制剂耐药性包括减弱肿瘤细胞克隆形成能力,和/或抑制肿瘤细胞的增殖、迁移扩散和/或侵袭能力。
6.根据权利要求3所述的应用,其中,降低酪氨酸激酶抑制剂耐药性包括促进肿瘤细胞凋亡,和/或抑制体内肿瘤生长。
7.根据权利要求3所述的应用,其中,降低酪氨酸激酶抑制剂耐药性包括抑制耐药相关蛋白p-Akt和p-ERK的表达。
8.根据权利要求3所述的应用,其中,所述针对HDGF的拮抗剂包括sgRNA、小分子抑制剂、天然产物、抗体或其组合;
优选地,所述酪氨酸激酶抑制剂包括吉非替尼、埃克替尼、阿法替尼、AZD9291中的一种或多种。
9.根据权利要求1-8任一项所述的应用,其中,所述患者为肿瘤患者;
优选地,所述肿瘤为肺癌,例如非小细胞肺癌。
10.使酪氨酸激酶抑制剂敏感细胞过表达HDGF的试剂在制备具有以下至少一种功效的研究制剂中的应用:
提高酪氨酸激酶抑制剂的IC50;
促进细胞克隆形成能力和/或划痕愈合能力;
增强细胞的增殖、克隆形成以及迁移和侵袭能力;和/或
诱导耐药相关蛋白p-Akt和p-ERK的表达。
