Description

Root taking device for maxillary sinus
Technical Field
The invention relates to the technical field of medical appliances, in particular to a maxillary sinus root taking device.
Background
The misinsertion of the root into the maxillary sinus is a common and serious complication in the clinical midmaxillary posterior tooth extraction. If the tooth root is not taken out in time, great physiological and psychological wounds are caused to the patient. Dental root entry into the maxillary sinus is often caused by both patient anatomy and medical personnel mishandling. The existing clinically common root taking methods mainly comprise a maxillary sinus windowing root taking method, an endoscopic method, a flushing method, a combined suction method and the like:
1. the maxillary sinus windowing root extraction method comprises the following steps: an operation area is required to be opened outside the tooth extraction operation area, a window of about 1.5cm multiplied by 1.5cm is required to be drilled on the front wall of the maxillary sinus to reach the maxillary Dou Douqiang by using a chisel, sinus mucosa is cut, and broken roots are taken out under direct vision. The method can take out broken roots, but has the defects. Disadvantages: when the flap is turned over, bones are removed and the tooth roots are taken, the operation is needed in an oral vestibule, the bone of the front wall of the maxillary sinus is removed, special operation instruments are needed, the operation is relatively complicated, more bone is removed, the minimally invasive operation concept is not met, the time consumption is long, the operation field is unclear, the wound is large, the postoperative reaction is serious, and the height of an alveolar ridge and the damage to the mucosa of the maxillary sinus can be damaged;
2. endoscopic method: the root taking method is divided into a paranasal sinus endoscope root taking method and an arthroscope root taking method according to different used instruments, and the basic principles of the root taking method are consistent. When the nasal sinus endoscope is used for taking out roots, the endoscope enters the maxillary sinus from the lower nasal meatus, and meanwhile, the front wall of the maxillary sinus is windowed, and the roots are taken out from the windowed position. The sinus endoscope can also be placed in the anterior wall fenestration of the maxillary sinus, and the root of the tooth can be taken out. When the arthroscope takes out the root, the front wall of the maxillary sinus is directly windowed, the arthroscope is inserted, and a surgical instrument is inserted at the same windowed position, and the root is taken out. The broken root can be found under the direct view of the television screen through the endoscope, and the pertinence is strong. Disadvantages: the application of the endoscope to take out the tooth root requires a whole set of instruments of the endoscope, and also requires a professional doctor to operate, so that the requirements on equipment conditions and operation technology are high, and the full popularization is impossible.
3. The flushing method is combined with the suction method: according to the different positions of the flushing ports, the flushing method is divided into flushing through the tooth extraction pit, flushing through the front wall of the maxillary sinus and flushing through the lower nasal meatus. The method which is more commonly used at present is a method for flushing through tooth sockets. Firstly, a dental drill is used for enlarging the alveolus, or a triangular tappet is used for removing part or all of the alveolus intervals, so that the visual field and the operation range are enlarged. Then, the patient is enabled to be low in head, the patient is ordered to feel suffocated, the physiological saline is quickly injected into the sinus cavity by the injector, the maxillary sinus is instantly filled with the physiological saline, then the aspirator is placed in the alveolus nest, the physiological saline is quickly sucked out, and the tooth root moves along with the physiological saline by utilizing the vortex of the physiological saline in the sinus cavity and is sucked out along with the aspirator. The method can repeatedly wash and attract in different directions, does not need complex and precise instruments, accords with the concept of minimally invasive surgery, and is easy to operate. And the method has the advantages of small damage to the mucosa of the maxillary sinus, short operation time, small postoperative response and easy acceptance by patients. Disadvantages: at present, a special water injection device is not used in clinical work, the injection operation of the injector is inconvenient, the water injection speed is low, and the risk of damaging mucosa of the maxillary sinus is caused; without special aspirator, the diameter, length and suction of aspirator can not meet the operation requirements of patient anatomy and doctor. And need water injection and water absorption need two kinds at least apparatuses, can't satisfy the urgency that water injection absorbed water and link up, the operation is complicated, and the effectiveness is not good enough.
Based on the above, the invention provides a device which can integrate flushing and suction functions into a whole and is simpler to use, and the maxillary sinus root taking device has not been reported at present.
