Description
Technical Field
The invention relates to the field of medical appliances, in particular to an anastomotic mechanism of an endoscopic cutting anastomotic purse-string device.
Background
The anastomosis mechanism of the endoscopic cutting anastomotic purse-string device is a technology disclosed in publication No. CN213156149, and is an anastomotic instrument with one side for realizing anastomotic nail anastomosis and one side for realizing purse-string anastomosis.
After the product has been formed, it has been found that the product has some drawbacks, in particular:
regarding the anastomotic part of the anastomotic nail, the prior rectangular nail pit has high defective rate of forming effect, the titanium nail can not be well guided and formed in the nailing process, and defective nailing such as dovetail nails is often generated after nailing.
And the purse-string anastomotic part is easy to return to the purse-string nail bin after the purse-string nails are extruded by tissues, so that the purse-string nails lose support and rebound, and the purse-string orifice anastomosis is not tight.
In addition, there are some undesirable areas, such as staple pit distance, etc., with respect to some details.

The invention aims at: the anastomotic mechanism of the endoscopic cutting anastomotic purse-string device is provided, the anastomotic mechanism of the traditional endoscopic cutting anastomotic purse-string device is locally optimized, and the anastomotic effect is improved.
The invention is realized by the following technical scheme:
an anastomotic mechanism of an endoscopic incision anastomotic purse-string device comprises an upper forceps part 1 and a lower forceps part 2; the proximal ends of the upper forceps part 1 and the lower forceps part 2 are hinged with each other; a cutting knife guide groove extending along the far and near direction is arranged on the anastomotic surface of the upper forceps part 1 or/and the lower forceps part 2; wherein, the upper forceps part 1 and the lower forceps part 2 are divided into a left part and a right part by a cutter guide groove, wherein, one part has a structure of an anastomotic component for realizing anastomotic nail anastomosis, and the other part has a structure of a purse string suture component for realizing purse string anastomosis;
the nail pit of the nail propping seat in the anastomotic component is the middle part is provided with an infinite nail pit with a certain width.
Compared with the prior art, the invention has the beneficial effects that:
1. the specific parameters of the anastomotic nail pits are optimized, the anastomotic nail is ensured to be shaped like a B after being molded, and the anastomotic effect is improved.
2. The elastic buckle structure is added in the purse string suturing member, so that the phenomenon that the purse string nails are not tightly anastomosed due to rebound caused by loss of support of the purse string nails due to extrusion of the formed purse string nails by tissues back into the nail bin can be avoided.
3. The distance between the purse-string nail hole and the anastomotic nail hole is optimized, and the optimal anastomotic effect is ensured.

Detailed Description
The invention is described in detail below with reference to the accompanying drawings:
as shown in fig. 1-9: an anastomotic mechanism of an endoscopic incision anastomotic purse-string device comprises an upper forceps part 1 and a lower forceps part 2; the proximal ends of the upper forceps part 1 and the lower forceps part 2 are hinged with each other; a cutting knife guide groove extending along the far and near direction is arranged on the anastomotic surface of the upper forceps part 1 or/and the lower forceps part 2; wherein, the upper forceps part 1 and the lower forceps part 2 are divided into a left part and a right part by a cutter guide groove, wherein, one part has a structure of an anastomotic component 3 for realizing anastomotic nail anastomosis, and the other part has a structure of a purse string suture component 4 for realizing purse string anastomosis;
the specific structure and the using method of the anastomosis mechanism of the endoscope cutting anastomosis purse-string device are all in the prior art, and are disclosed in publication number 213156149, and the invention is not repeated.
The nail pit of the anastomotic nail in the anastomotic component is the middle part is provided with an infinite nail pit with a certain width.
The effect of stapling is ensured by optimizing the parameters of the +.infinity type staple pit. The parameters specifically adjusted are as follows:
the specific parameters of the ++type nail pit are as follows:
the width at the widest point is 0.59 unit length;
the width at the narrowest point is 0.22 unit length;
the overall length is 2.59 units;
taking the transverse central axis of the infinity type spike pit as a section line, wherein the bottom surface profile of the infinity type spike pit meets the following conditions:
establishing a cross coordinate system:
forming a first circle by taking 0.0.48 as a circle center and taking 0.71 as a diameter;
forming two second circles with 0.81, -0.35 and-0.81, -0.35 as circle centers and 1.6 as diameters;
forming two third circles with 0.845-1 and-0.845-1 as circle centers and 2.9 as diameters;
an intersection of the X-axis and the third circle, and an a-line segment 51 on the third circle taken between the intersection of the second circle and the third circle;
a second circular segment 52 taken between the intersection of the second and third circles and the intersection of the second and first circles;
a C line segment 53 on the first circle taken between the intersection of the second circle and the first circle and between the Y-axis and the intersection of the first circle;
the A line segment, the B line segment and the C line segment are connected to form the bottom surface contour shape of the ++type nail pit.
Through the design, the titanium nails can be better shaped and guided in the nailing process due to the characteristics that the middle section is in the shape of the arc and the shrinkage is small, and finally the nails are shaped like a B.
An elastic buckling structure is arranged on the purse-string pin pushing piece in the purse-string stitching member, and comprises a base 61, a limit column 62 and an elastic column 63; the limiting columns and the elastic columns are arranged on the base, two limiting columns are arranged and are respectively arranged at two sides of the limiting columns;
a limiting hole 64 is formed in the outer side of the limiting column; and a limiting lug 65 matched with the limiting groove is arranged at the outlet of the purse-string rivet hole corresponding to the purse-string rivet pushing piece.
In the process of discharging the purse string nails, the elastic buckle structure moves forward synchronously, and the elastic column bends towards the direction of the limiting column because the outlets of the purse string nail holes are smaller. After the limit hole 64 and the limit bump 65 are matched, the elastic buckle structure cannot retract into the purse-string nail hole, so that the purse-string nail cannot be extruded into the purse-string nail hole by tissues.
The distance between any two adjacent purse-string nail holes is 4.2-4.4mm along the extending direction of the nail propping seat; the distance between any two adjacent anastomotic nail holes is 3.9-4.1mm. The total length of the anastomosis nail is set as L1, the total length of the pouch nail is set as L2, as shown in a graph D, wherein a is the distance between two anastomosis nail holes, b is the distance between two pouch nail holes, various anastomosis mechanisms with different L1/L2 specifications, such as 45mm anastomosis mechanisms, 60mm anastomosis mechanisms, 75mm anastomosis mechanisms, 90mm anastomosis mechanisms and the like, are combined and matched by different n1 and n2 according to the changed values of n1 and n2, and the collected data of pressure resistance is verified through multiple tests to show that the combined anastomosis mechanism has the best pressure resistance/anastomosis/pouch effect when a=4mm and b=4.3mm.
It should be noted that the foregoing description is only a preferred embodiment of the present invention, and although the present invention has been described in detail with reference to the foregoing embodiments, it should be understood that modifications, equivalents, improvements and modifications to the technical solution described in the foregoing embodiments may occur to those skilled in the art, and all modifications, equivalents, and improvements are intended to be included within the spirit and principle of the present invention.
