Abstract

背景技术

在对不同方法的评价后(Altai 2016;Honarvar 2016),将互补肽核酸(PNA)探针的杂交选为基于亲和体的预靶向,因为它在肿瘤中提供了最佳的活性保留。PNA是一类能够进行沃森-克里克(Watson-Crick)碱基配对的合成DNA类似物(Egholm 1993;Nielsen1994)。PNA骨架是由重复的N-(2-氨基乙基)-甘氨酸单位通过酰胺键连接而构建,并且嘌呤和嘧啶核酸碱基经由羧甲基接头与该支架连接。PNA对核酸酶和蛋白酶的降解具有抗性,并在人血清中表现出极好的稳定性(Demidov 1994)。它们没有免疫原性并且一般毒性较低。第一代初级剂ZHER2:342-SR-HP1和二级探针HP2的分子设计是成功的,因为它提供了PNA杂交的高亲和力和特异性、初级探针在肿瘤中的特异性积累、以及放射性金属的有效特异性递送(Westerlund 2015,Honarvar2016)。用177Lu(Altai 2017NMB,Westerlund 2018)、111In(Westerlund 2015,Honarvar 2016)和68Ga(Vorobyeva 2018)对HP2进行标记,结果在肿瘤中的吸收明显高于在肾脏中,尽管肾的吸收在正常组织中是最高的。
使用ZHER2:342-SR-HP1/[177Lu]Lu-HP2预靶向系统的实验性疗法显著增加了携带表达HER2的异种移植物的小鼠的中位生存期(与仅用[177Lu]Lu-HP2的32天相比,治疗组为66天),而没有可观察到的骨髓和肾毒性(Westerlund 2018)。然而,要获得这种治疗的疗效,必须进一步提高肿瘤吸收剂量与正常组织(首先是肾脏)吸收剂量相比的比率。
可能的优化参数是二级探针的长度。缩短长度将减少探针的流体动力学半径,这可能会促进其在肿瘤间质中的外渗和扩散,改善在肿瘤中的定位和在肿瘤内分布的均匀性。然而,减少二级探针的尺寸也有明显的风险。首先,减少核酸碱基的数量可能会降低与初级探针的杂交强度。其次,碱基组成的修改可能会影响脱靶相互作用,导致在正常组织中的吸收升高。例如,与177Lu由111In或68Ga取代有关的微小结构变化导致在肾吸收上的显著差异(Vorobyeva 2018),或在血液、肝脏和骨中吸收的显著差异(Altai2017NMB)。生物分布不仅另外取决于核酸碱基的数量和性质,而且还取决于它们在PNA序列中的顺序。对在与编码MYC蛋白的mRNA结合的99mTc标记的反义PNA中的核酸碱基的扰乱,将导致在正常组织中的吸收减少超过两倍(Rao 2003,Mather 2004)。因此,需要进行体内实验研究,以评价第二代二级探针是否提供更好的生物分布和剂量测定特征。

本发明的公开
本文公开的是第二代杂交探针,即HP15初级探针和二级探针组:HP16(9-merPNA)、HP17(12-mer PNA)、HP18(15-mer PNA)和HP19(6-mer PNA)(表1)。如在实例中所示出,将携带DOTA螯合剂的探针进行设计、合成、体外表征,并且以177Lu标记。在表达HER2的SKOV3和BT474细胞系中研究了体外预靶向。在携带SKOV3异种移植物的BALB/C nu/nu小鼠中评价了这些新型探针的生物分布概况,并与之前研究的[177Lu]Lu-HP2进行了比较。
通过SPR和UV光谱法表征,证实了在HP15和二级探针HP16、HP17、HP18和HP19之间形成了高亲和力的双链体,其具有的亲和力与互补的PNA序列的长度相关。三种测试的基于PNA的探针(HP16、HP17、HP18)与表达HER2的细胞以高亲和力(11-12pM)在体外特异性地结合。体内研究表明,所有[177Lu]Lu标记的探针在表达HER2的异种移植物中都显示出HER2的特异性吸收。在肿瘤中累积的放射性与在肾脏中累积的放射性的比率取决于二级探针的尺寸,并随着核酸碱基数量的增加而减少。在体内测试的最短的PNA探针[177Lu]Lu-HP16,示出最高的肿瘤与肾脏比率,并且是亲和体介导的肿瘤预靶向的最有希望的二级探针。
在第一方面,本发明提供了用于将诊断剂或治疗剂靶向至靶位点的试剂盒,包括(a)第一缀合物,包括(i)能够选择性地与靶位点结合的靶向部分;和(ii)包括第一PNA低聚物的第一杂交探针部分;以及(b)第二缀合物,包括(i)包括与第一PNA低聚物互补的第二PNA低聚物的第二杂交探针部分;和(ii)诊断剂或治疗剂部分;其中在第二杂交探针部分中的第二PNA低聚物的长度不超过14个碱基。
在本上下文中,术语“试剂盒”应理解为意指或包括化学和/或生物化合物,诸如药物组合物的组合物。
根据本发明,当第一缀合物和第二缀合物通过预靶向方案向具有肿瘤的哺乳动物给药时,与不用预靶向方案即具有在同一实体中与治疗/诊断部分联接的靶向部分来给药诊断剂或治疗剂的其他可比情况相比,使用长度不超过14个碱基的第二PNA低聚物将导致诊断剂或治疗剂的肿瘤与非肿瘤组织比率的改善(增加)。
相关的非肿瘤组织将根据具体的应用范围而有所不同。例如,在治疗应用中,通常存在非肿瘤组织,治疗剂在其中引起的毒性限制了在没有不可接受的副作用的情况下可以向患者给药的剂量,称为剂量限制性组织。本发明得到的肿瘤与剂量限制组织的比率是相比于其他可比情况的至少增加2倍(优选至少2.5倍、更优选至少3倍、最优选至少3.5倍),该其他可比情况不用预靶向方案,即具有在同一实体中与治疗/诊断分子联接的靶向部分。特定的剂量限制的非肿瘤组织又将取决于因素诸如靶向部分和治疗剂部分的性质。非肿瘤组织例如可以是肾脏、骨、肝脏或胃,优选肾脏。在诊断应用中,期望将来自待调查的肿瘤周围组织,特别是血液中的诊断标志物的干扰背景信号最小化。因此,非肿瘤组织也可以是血液。
在本上下文中,术语“预靶向方案”意指在给药第二缀合物之前向哺乳动物给药第一缀合物,使得第一缀合物和第二缀合物之间在体内发生缔合。
关于第一和第二PNA低聚物,术语“互补的”PNA低聚物是指可以通过匹配碱基对形成双链结构的PNA低聚物。优选地,在两条PNA链之间存在完全互补,即每个碱基与其相对的碱基相对。然而,只要两个探针能够杂交形成结构化的双链体,互补程度就可以是不完全的(100%)。
两条核酸链之间的互补程度可以变化,从完全互补(每个核苷酸都与其相对的碱基相对)到无互补(每个核苷酸与其相对的碱基不相对)。
优选地,所述靶位点存在于在包括人在内的哺乳动物的蛋白上,该蛋白在肿瘤细胞的表面上表达上。优选地,靶蛋白在肿瘤细胞表面过度表达,而在正常健康组织上没有或很少表达。例如,该蛋白可以选自由以下组成的组:表皮生长因子受体(EGFR)、人表皮生长因子受体2(HER2)、人表皮生长因子受体3(HER3)、胰岛素样生长因子1受体(IGF1R)、碳酸酐酶IX(CAIX)、血小板源性生长因子受体β(PDGFR-β)、nectin-4、分化群38(CD38)、分化群33(CD33)、分化群30(CD30)、分化群22(CD22)和分化群79b(CD79b)(见表7)。优选地,靶蛋白是人蛋白。优选地,所述靶位点存在于在人表皮生长因子受体2(HER2)上。
所述靶向部分优选地选自由以下组成的组:
·抗体,诸如单克隆抗体;
·抗体片段,诸如单结构域抗体(sdAb;纳米抗体)、单链可变区片段(scFv)、抗原结合片段(Fab)、双抗体(参照Holliger et al.1993)或小抗体(参照Hu et al.1996);
·肽,诸如选自组合文库的肿瘤靶向肽(参照Liu et al.,2017),或基于与G-蛋白联接受体(GPCR)结合的内源性肽的肽类似物(参照Hauser,2017;和Moody,2018)。
术语“分子”是指小型的工程支架蛋白,它可以被选择与广谱的生物分子以高亲和力结合。亲和体分子是源自葡萄球菌A蛋白的IgG结合结构域(Z结构域)的小型(58氨基酸残基)蛋白结构域。亲和体分子具有可以作为构建组合文库的支架的三螺旋束结构,从中可以选择靶向期望分子的亲和体分子变体。有关综述,参见例如Tolmachev 2020。
优选地,第一杂交探针部分与靶向部分共价连接。优选地,第一杂交探针部分经由定位酶A(sortase A)介导的连接来与靶向部分缀合,如在例如,Westerlund,2015中所描述的。金黄色葡萄球菌(Staphylococcus aureus)定位酶A是将表面活性型蛋白附接在细胞壁上的转肽酶;它在LPXTG基序的Gly和Thr之间进行切割,并且催化在苏氨酸的羧基基团与细胞壁肽聚糖的氨基基团之间的酰胺键的形成。基序(LPXTG)的识别被添加到目的蛋白的C末端,而单甘氨酸或具有自由N末端的寡聚甘氨酸肽被添加到待连接的第二分子。在向分子中加入定位酶后,两个分子通过天然肽键共价连接。

可以理解的是,修饰的PNA低聚物具有与其他杂交探针部分中的PNA低聚物的序列互补,并因此与之杂交的序列。如上所述,互补程度可能不完全(100%)。因此,在这样的双链体中可以有1或2个错配,而根据本发明,杂交的缀合物仍然是有用的。
当第二缀合物包括治疗剂部分时,治疗剂可以是放射性核素,或适用于抗体药物偶联物(ADC)或包括缀合物的其他药物的细胞毒性药物。(关于综述,参见例如Shim,2020。)例如,所述的细胞毒性药物可以选自卡奇霉素(calicheamicin)、澳瑞他汀类(auristatins)诸如单甲基澳瑞他汀E/F(MMAE/F),以及美登素类(maytansinoids)诸如美登素衍生物DM0-DM4。优选地,治疗剂是放射性核素。

实验方法
使用高质量Milli-水制备所有缓冲溶液,并使用/>100树脂(Bio-RadLaboratories,USA)从金属污染中纯化。无载体177LuCl3购买自PerkinElmer(美国,马萨诸塞州,沃尔瑟姆)。使用带有NaI(TI)检测器的自动γ-光谱仪(1480Wizard,Wallac,Finland)测量放射性。
使用表达HER2的卵巢癌症SKOV3和乳腺癌症BT474细胞进行体外细胞研究,这两种细胞均来自美国标准培养物收藏所(American Type Culture Collection(ATCC))。细胞在以下中培养:RPMI培养基(Flow Irvine,UK),补充有10%胎牛血清、2mM L-谷氨酰胺和包括100IU/mL青霉素和100mg/mL链霉素的PEST。

PNA预靶向探针的合成和纯化
肽核酸单体;Fmoc-PNA-A(Bhoc)-OH、Fmoc-PNA-G(Bhoc)-OH、Fmoc-PNA-C(Bhoc)-OH和Fmoc-PNA-T-OH,购自PolyOrg,Inc.Leominster,USA或PNA Bio,Inc.Thousand Oaks,USA。Rink Amide树脂(ChemMatrix,0.50mmol/g)购自Biotage(瑞典,乌普萨拉)。1,4,7,10-四氮杂环十二烷-1,4,7,10-四乙酸(DOTA)购自法国,第戎,CheMatech。Fmoc-NH-(PEG)2-CH2COOH(AEEA)购自德国,达姆施塔特,Merck KGaA。用于固相合成的溶剂和试剂从商业供应商处获得,并且无需进一步提纯即可使用。
HP15是在Initiator+Alstra微波肽合成器上,使用Rink Amide树脂(ChemMatrix,0.50mmol/g),在10ml反应器小瓶中以50μmol的规模合成。通过用哌啶-DMF(1:4)处理树脂3min,随后用哌啶-DMF(1:4)处理10min,在RT下分两个阶段进行Fmoc脱保护。使用在DMF中的5eq的PNA或氨基酸单体、5eq的Oxyma和5eq的DIC进行联接。在整个序列中,在75℃下使用10min的联接时间,随后使用NMP-二甲基吡啶-乙酸酐(89:6:5)进行2min的封端步骤。
引入正交保护的Lys(Mtt),以便有可能在特定的位点引入DOTA。在四个联接步骤(Fmoc-E-K(Mtt)-]AEEA]-E-树脂)之后,将自动合成中断,以添加5-10次新鲜TFA:TIS:DCM(1:2:97))进行Lys(Mtt)的选择性侧链脱保护,随后涡旋1min。使用在DMF中的5eq.的DOTA、5eq的Oxyma和5eq的DIC在RT下进行DOTA联接1h。在恢复自动合成并完成所有循环后,用DMF、DCM和最后用MeOH洗涤树脂,并且然后干燥过夜。使用TFA:H2O:TIS(95:2.5:2.5)的混合物在RT下将PNA-肽杂交体从固体载体上裂解4h。最后在二乙醚和水之间提取PNA产物并从水相中冻干。
最短的互补的PNA探针HP19是在与HP15相同的微波多肽合成器上合成的。与HP15的合成类似,进行了Fmoc脱保护、联接和封端。使用如DOTA与HP15联接的相同方案,在合成的最后,手动将DOTA螯合剂与PNA-探针联接。最终产物从树脂中裂解,并以类似于HP15进行提取。通过卡瑟(Kaiser)检测对合成进行持续监测。使用MALDI-TOF分析验证了HP19的最终产物的分子量。
其他互补的PNA探针(HP16、HP17、HP18)是使用与合成HP15所使用的相同的单体、树脂和溶剂来手工合成的。每次联接使用5eq PNA单体进行。在添加到树脂之前,在NMP和DMF中存在10eq DIEA的情况下,将PNA单体用5eq六氟磷酸苯并三唑-1-基-氧基三吡咯烷基磷(PyBOP;Sigma Aldrich)预活化1min。每次联接都在RT与轻柔振荡下进行30min至1h。未反应的PNA的封端的进行与HP15类似。在RT与与轻柔振荡下,使用在NMP中的20%的哌啶进行Fmoc脱保护20min。通过卡瑟检测监测合成,并通过MALDI-TOF分析后对几颗树脂珠粒进行微裂解。合成完成后,将树脂用NMP彻底洗涤,随后用DCM洗涤,并过夜干燥。随后对互补的PNA探针进行裂解和乙醚提取,与HP15类似。
RP-HPLC纯化使用半制备型Zorbax 300SB-C18柱(9.4x 250mm,5μm粒径;美国,圣克拉拉,Agilent进行,具有的线性梯度为5–50% B,其中A=0.1% TFA-H2O和B=0.1%TFA-CH3CN,超过25min,使用3ml/min的流速,70℃的柱温和在220和260nm处的UV检测。使用α-氰基-4-羟基肉桂酸基质通过MALDI-TOF(4800MALDI-TOF/TOF,AB SCIEX)来分析收集的级分。将被确定为包含正确产物的级分汇集并冻干。
HP16、HP17和HP18的纯度使用分析性RP-HPLC在Zorbax 300SB-C18柱(4.6x150mm,3.5μm粒径;Agilent)上进行确认,随后进行MALDI-TOF分析。使用MALDI-TOF分析确认了HP19的同一性和纯度。基于PNA组成和每个PNA单体的消光系数(A:13 700M-1cm-1、C:6600M-1cm-1、G:11 700M-1cm-1和T:8 600M-1cm-1),估计每个PNA探针在260nm(ε260)处的消光系数。每个探针在所有实验中使用的消光系数如下;HP16:98 000M-1cm-1、HP17:126 900M-1cm-1、HP18:155 800M-1cm-1、HP19:64 000M-1cm-1和HP15:150 700M-1cm-1。
亲和体-PNA缀合物的生产
将pAY430-ZHER2:342-SR-H6质粒(Westerlund 2015)转化到BL21(DE3)化学感受态大肠杆菌(E.coli)细胞(Life Technologies)中,并将细胞培养在补充有卡那霉素的复合培养基(具有酵母提取物的胰蛋白酶大豆肉汤)中。通过加入1mM IPTG(最终浓度)诱导蛋白的表达,并将培养物在RT下保持150rpm过夜。通过离心(4000rcf,10min,4℃)收获细胞,重悬于IMAC结合缓冲液(20mM Tris-HCl,300mM NaCl,10mM咪唑,pH 7.5)中,并且随后通过超声处理裂解。离心后,澄清的上清液通过穿过IMAC基质(HisPurTMCobalt Resin,ThermoScientific)捕获ZHER2:342-SR-H6。用洗涤缓冲液(20mM Tris-HCl,300mM NaCl,30mM咪唑,pH7.5)洗涤树脂,并且用洗脱缓冲液(20mM Tris-HCl,300mM NaCl,300mM咪唑,pH 7.5)洗脱蛋白。在PD-10脱盐柱(GE Healthcare)上将洗脱的ZHER2:342-H6缓冲液交换为定位酶A缀合缓冲液(50mM Tris-HCl、150mM NaCl、10mM CaCl2 pH 7.5)。使用SDS-PAGE和MALDI-TOF确认ZHER2:342-SR-H6的纯度和分子量。
使用定位酶A介导的连接(SML),将ZHER2:342-SR-H6与HP15进行位点特异性联接。下文描述的SML方法是基于之前公开的亲和体-PNA缀合的方案(Altai 2017)。具有P94S/D160N/K196T突变(Chen et al.2011)的定位酶A变体,表示为Srt A3*,被用于缀合。将甘氨酸修饰的HP15探针溶于10%的DMSO中,并且在80℃下加热5min,然后基于在260nm处的吸光度来估计浓度。将500nmol的HP15、1.25μmol的ZHER2:342-SR-H6和1.25μmol的NiCl2在定位酶A缀合缓冲液中混合,最终体积为5ml。将Srt A3*加入到反应,最终浓度为5μM,并且反应进行30min,并且随后使用预平衡的IMAC基质进行反向IMAC步骤。在对给定的基质进行30min的温育步骤后,可以在流出物(flow-through)中收集缀合产物、水解蛋白副产物和未缀合的HP15。然后将收集的流出物在PD-10柱上缓冲液交换至10mM NaOAc pH 3.6,并且随后进行冻干。使用与纯化PNA探针相同的柱和溶剂,将ZHER2:342-SR-HP15缀合物在RP-HPLC上纯化,但梯度在25分钟内从5%至50% B,并在220、260和280nm处监测吸光度。收集未缀合的HP15和ZHER2:342-SR-HP15缀合物级分,冻干并通过MALDI-TOF进行分析。为了确认纯化的缀合物的纯度,通过分析HPLC(Zorbax 300SB-C18,3.5μm粒度,4.6x150 mm,Agilent)和电喷雾电离质谱(ESI-MS,Impact II,Bruker)进行分析。未缀合的HP15可以与新一批ZHER2:342-SR-H6一起用于新的缀合反应。
PNA探针和ZHER2:342-SR-HP15的最终浓度通过测量在260nm处的吸光度来确定。HP15和ZHER2:342-SR-HP15两者使用相同的消光系数,因为蛋白部分对在260nm处的总吸光度的贡献可以忽略不计。
ZHER2:342-SR-HP15与HER2受体的结合通过表面等离激元共振(SPR)得到确认。
PNA预靶向探针的表征
在BiacoreTM8K仪器(GE Healthcare)上使用表面等离激元共振(SPR)分析了PNA探针杂交的动力学参数。使用标准胺联接程序,在四个表面上用ZHER2:342-SR-HP15将Dextranchips Series S Sensor CM5(GE Healthcare)功能化为385、194、185和353共振单元(RU)。将每个芯片上的参考表面进行激活,随后失活。将互补的PNA探针HP16、HP17、HP18和HP19采用单次循环注射以5种浓度;22.6、45.3、90.6、181.25和362.5nM注射。允许每种浓度的缔合进行300s,随后是下一次注射和缔合阶段。注射最终浓度后,允许解离10000s(2h 47min),之后用10mM Hcl再生30s随后用15mM NaOH再生30s。在25℃下使用50μl/min的流速,在PBST(0.05%吐温20)pH 7.4中进行所有运行。在Biacore Insight Evaluation软件中使用1:1结合模型计算动力学参数。
HP15:HP16、HP15:HP17和HP15:HP18的熔融温度是在20至95℃范围的温度下,使用1℃/min的温度变化,通过监测在260nm处的UV吸光度确定的(ChirascanTM,AppliedPhotophysics)。将PNA复合物加热到80℃持续5min,并且然后使其在室温下杂交5min,然后在室温下进行UV监测5min。在确定热变性之前和之后都收集了CD光谱。
HP15:HP19复合物的熔融温度是用配备有单细胞Peltier恒温器控制的吸收池支架的Varian Cary 50Bio UV可见光分光光度计测定的。小室的温度以0.5℃/min的速度在20至80℃之间调节,并且在每个温度点平衡60s后在260nm处进行测量。
放射性标记和体外稳定性
使用先前描述的方法(Westerlund 2018)用177Lu对初级和二级PNA探针进行放射性标记。简单地说,将30μg的肽溶解在100μL的抗坏血酸(1M,pH 5.5)中,在95℃下加热10min,随后超声5min以确保完全溶解。加入3μL(60-120MBq)的177LuCl3,随后涡旋。将混合物在95℃下温育60min。将反应混合物通过放射性ITLC分析,用0.2M柠檬酸,pH2.0洗脱。
为了去除松散结合的177Lu,用过量乙二胺四乙酸四钠盐(EDTA.Na4)进行了处理。将新制备的EDTA.Na4溶液(10mg/mL,在Milli-Q水中)以1000倍摩尔过量添加到反应混合物中,并在95℃下温育10min。由于放射化学产率和纯度高,新的二级探针无需进一步纯化。对于[177Lu]Lu-HP2和[177Lu]Lu-ZHER2:342-SR-HP15,使用一次性NAP-5柱,预平衡并用1%BSA/PBS洗脱,通过尺寸排阻色谱法,在EDTA处理后进行纯化。
为了评价稳定性,将部分新鲜的放射性标记的缀合物(0.4μg)与500倍摩尔过量的EDTA在37℃下温育60min。在PBS中作为对照也进行了温育。检测是以三份法进行的。
为了验证在由2L-2130泵、UV检测器(L-2400)组成的LaChrom系统(德国,达姆施塔特,Hitachi,VWR)上进行的放射性ITLC、反相HPLC的结果,使用了串联联接的辐射流检测器(美国,华盛顿特区,Bioscan)。[177Lu]Lu标记化合物的纯度分析使用分析柱(德国,阿沙芬堡Phenemenex;/>5μm C18,/>150×4.6mm柱)进行。HPLC条件如下:A=10mM TFA/H2O;B=10mM TFA/乙腈;在220nm处的UV检测;梯度洗脱:在5-至70%B时0-15min、70至95%B时15-18min、5%B时19-20min;并且流速为1.0mL/min。
体外研究
将细胞接种在细胞培养平皿中,密度为106个细胞/平皿。每个数据点使用了一组三个平皿。
[177Lu]Lu-ZHER2:342-SR-HP15与表达HER2的细胞的结合的特异性通过将细胞与1nM的标记缀合物在37℃下温育1h来测试。为了使受体饱和,在添加放射性标记探针前5min将未标记的ZHER2:342(1000nM)添加到对照组中。
如较前所述,使用四组细胞平皿对新型[177Lu]Lu-HP16、[177Lu]Lu-HP17和[177Lu]Lu-HP18进行了预靶向特异性测定(Honarvar 2016)。为了演示预靶向,将一组平皿与ZHER2:342-SR-HP15(1nM)在37℃下温育1h并洗涤。添加177Lu标记的二级探针(10nM)并且将细胞在37℃下温育1h。为了示出预靶向是HER2介导的,第二组平皿在加入ZHER2:342-SR-HP15之前与过量的亲和体分子ZHER2:342(1000nM)一起温育5min。添加177Lu标记的二级探针(10nM)并将细胞在37℃下温育1h。为了证明预靶向是PNA介导的,将第三组平皿与ZHER2:342-SR-HP15温育,随后与过量的未标记的二级探针(300nM)温育30min,并且然后加入177Lu标记的二级探针,随后温育1h。在第四组中,细胞仅与177Lu标记的二级探针温育以评估非特异性结合。温育结束后,通过胰蛋白酶洗涤和分离细胞,测量细胞中的放射性,以计算细胞结合放射性的百分比。

体内研究
动物实验是按照国家对实验动物工作的立法进行的。由乌普萨拉的动物研究伦理委员会(Ethical Committee for Animal Research in Uppsala)给予了批准。对于肿瘤植入,将107个SKOV3细胞皮下注射到雌性BALB/c nu/nu小鼠的右后腿。生物分布实验是在细胞植入后两周进行的。动物的平均体重为18±1g。平均肿瘤重量为0.23±0.11g。为了测量生物分布,在预定的时间点通过过量麻醉将小鼠安乐死,随后进行心脏穿刺。收集目的器官和肿瘤,并称重,测量它们的放射性。计算每克样品的总注射剂量的百分比(%ID/g)。
Westerlund等人使用[177Lu]Lu-HP2针对基于亲和体的PNA介导疗法优化了本研究中使用的预靶向方案。(Westerlund 2018)。扩大实验示出,当初级和[177Lu]Lu-HP2的注射质量加倍时,[177Lu]Lu-HP2的生物分布没有显著变化。基于这项研究,每只小鼠注射50μg的初级剂和等量的二级剂(HP16、HP17、HP18和HP2分别为0.69、0.89、1和1μg)。
在生物分布研究中,将小鼠随机分为五组。向30只小鼠静脉内注射ZHER2:342-SR-HP15(每只小鼠4nmol在100μL PBS中)。十六小时后,所有小鼠都注射了[177Lu]-HP16、[177Lu]-HP17或[177Lu]-HP18(194pmol在100μL的含2% BSA的PBS中,120kBq)。在注射二级探针后的4和144h时测量生物分布。为了比较,使用ZHER2:342-SR-HP:1作为初级剂以相同的方式测量第一代[177Lu]Lu-HP2的生物分布。
为了评价体内特异性,在没有预注射初级剂的注射后4h时测量[177Lu]Lu二级探针的生物分布。
在γ计数仪测量完成后,将肿瘤嵌入低温介质(Neg-50TM,Thermo Scientific,USA)中并在-80℃下冷冻。用低温切片机(CryoStarTMNX70,Thermo Scientific,USA)将冷冻的肿瘤进行连续切片(30μm厚),并且解冻安封固在玻璃载玻片上。对于数字放射自显影,将带切片的载玻片置于盒中,并在荧光屏上过夜曝光。荧光屏在Storage PhosphorSystem上以600dpi的分辨率扫描,并使用OptiQuant软件(PerkinElmer,USA)进行分析。
为了估计在肿瘤和肾脏中的吸收剂量的比率,评估了在肾脏和肿瘤中的累积活性。该估计是基于临床验证的两时间点法(Freedman 2020)。生物分布数据经过非衰变校正,并且使用GraphPad Prism软件计算时间活性图下的面积。假设的是,主要的吸收剂量是由β粒子引起的,因为交叉剂量可以忽略不计,并且吸收的部分等于1。
SPECT/CT成像
携带SKOV3异种移植物的小鼠在注射二级177Lu标记探针(680pmol,9-13MBq)之前16h静脉内注射7nmol的初级剂。在即将成像之前(注射后4h),通过CO2窒息处死动物。SPECT成像使用nanoScan SC(Mediso Medical Imaging Systems,Hungary)进行。CT采集是使用50KeV的X射线能量进行的。使用50-62、103-124和188-230keV的能量窗获得20min的SPECT螺旋扫描。使用Tera-TomoTM3D SPECT软件对数据进行了重建。
本发明的实施例
实施例1:亲和体-PNA缀合物和互补的PNA探针的生产和表征
如在上文实验方法中的描述制备PNA预靶向探针HP15、HP16、HP17、HP18和HP19(表1)。这些探针的设计避免了自互补序列以及嘌呤(A和G)的延伸片段,这些都已知促进聚集,并且因此可以使基于PNA的探针难以合成和纯化(Zhao 2020)。
分子ZHER2:342-SR-H6和HP15使用Srt A3*缀合(参见实验方法)。基于在RP-HPLC中在260nm处的峰下的积分面积,将该反应的连接效率估计为40%。对于HP1与ZHER2:342-SR-H6的缀合,40%的连接效率低于之前报道的70%的连接效率(Altai 2017)。然而,与在HP1的N末端处有三个甘氨酸残基相比,HP15在N末端处只有单甘氨酸残基,这可能影响连接的效率。40%的连接效率与使用野生型定位酶A的HP1的缀合处于相同的范围内,后者被报道为45%(Westerlund,2015)。
通过SPR分析四种互补的PNA探针与固定化的ZHER2:342-SR-HP15的杂交。在图1中示出了使用单循环注射分析的相互作用的代表性传感图。HP16、HP17和HP18的结合速率常数ka估计分别为4.6x104、4.3x104和5.7x104 M-1s-1。因此,与HP15相互作用的这三种PNA探针的结合率(on-rate)都在同一范围内。HP19的ka估计为2.1x 107M-1s-1。HP16的解离速率常数kd估计为1.2x 10-5s-1,HP19的解离速率常数kd估计为7.2x 10-2s-1,而对于其他两个互补的PNA探针(HP17和HP18),kd太慢而无法使用Biacore确定。HP16的平衡解离常数经计算为大约280pM(表2),而HP17和HP18对ZHER2:342-SR-HP15的亲和力估计更高。
在HP15和HP16(9聚体)之间的相互作用确定的KD(280pM)对预期的预靶向应用似乎表明具有足够高的亲和力。在HP15和HP19(6聚体)之间的相互作用确定的KD(3.4nM)更高,但预计这种较低的亲和力也足以满足应用。较早期已经使用结合肿瘤相关抗原和放射性标记的半抗原的双特异性抗体构建体证明了成功的预靶向,其中,在双功能抗体和111In标记的苄基EDTA衍生物之间的相互作用的KD估计仅为10-9–10-10M(Stickney 1991)。
在HP15和二级探针H16、HP17或HP18之间的杂交产生了CD光谱,其最小值为大约215nm和260nm。诱导信号是携带C末端L-氨基酸的互补PNA探针之间杂交后形成PNA:PNA双螺旋的结果(Corradini 2012)。与HP15形成双链体后,在260nm处监测每个杂交探针的熔融温度,并且估计HP16、HP17和HP18分别为73℃、75℃和87℃(图2a)以及HP19为55℃(图2b)。光谱表征表明,HP16、HP17和HP18都与初级PNA探针HP15形成高热稳定性的双链体。HP16、HP17和HP19的熔融温度预期较低是由于它们的低聚物长度与HP18相比更短。虽然明显低于HP16、HP17和HP18的熔融温度,但HP19的熔融温度仍然远远高于人体温度。
实施例2:放射性标记和体外稳定性
表3示出了用177Lu对所有探针进行放射性标记的结果。在EDTA处理后,新的二级剂的放射化学产率超过98%。因此,在体外和体内研究中没有进行使用NAP-5的进一步纯化。使用NAP-5柱纯化[177Lu]Lu-ZHER2:342-SR-HP15和[177Lu]Lu-HP2,产生了两种标记的放射化学纯度均为100±0%。用177Lu标记的所有探针在PBS中和在EDTA存在下温育最长达1h时都是稳定的。
为了验证放射性ITLC的结果,进行了放射性HPLC鉴定,并且结果表明,在标记和纯化后没有发生碎片化。所有探针的放射性HPLC保留时间为约5.8min。未标记的探针的保留时间与标记的探针相同。
实施例3:体外研究
使用饱和实验测试初级剂[177Lu]Lu-ZHER2:342-SR-HP15的HER2结合特异性。当将细胞用抗HER2亲和体分子预饱和时(图3A),结合显著降低(P<0.0005),表明该结合是HER2介导的。[177Lu]Lu-ZHER2:342-SR-HP15的缓慢内化有利于预靶向应用,因为这使得初级杂交探针能够在靶细胞表面上长期存在。
评价了[177Lu]Lu-HP16、[177Lu]Lu-HP17和[177Lu]Lu-HP18在体外与ZHER2:342-SR-HP15预处理的表达HER2的细胞的结合的特异性(图3B、C和D))。当将HER2受体用抗HER2亲和体分子饱和或当ZHER2:342-SR-HP15处理的细胞与大量未标记的二级剂预温育时,所有二级剂的结合显著降低(p<0.0005)。与预靶向相比,所有二级剂在没有与初级剂预温育的情况下与细胞的结合都显著更低(P<0.0005)。[177Lu]Lu-HP2的特异性已在较早期得到证实(Altai2017NMB)。
用测量与用ZHER2:342-SR-HP15预处理的活SKOV3细胞的结合的动力学证明所有二级探针的结合都非常强。Interaction MapTM计算示出[177Lu]Lu-ZHER2:342-SR-HP15和预靶向[177Lu]Lu二级探针的快速缔合之后非常缓慢的解离,产生平衡时的皮摩尔解离常数(KD)。KD值在11和12pM之间。二级探针之间的表观解离常数没有差异。因此,将PNA的长度从15个氮碱减少到9个,这与它们在细胞试验中与初级探针的结合的任何明显的降低都不相关。
测定了SKOV3和BT474细胞在中断温育后对所有放射性标签的细胞处理和保留。对于[177Lu]Lu-ZHER2:342-SR-HP15,内化很慢,这是HER2结合亲和体分子及其衍生物的典型特征。[177Lu]Lu-ZHER2:342-SR-HP15的内化部分在SKOV3细胞中略高于在BT474细胞中(在24h时间点,SKOV3为18±2%,而BT474为12±2%)。标记二级探针的内化模式类似于[177Lu]Lu-ZHER2:342-SR-HP15的模式。[177Lu]Lu-ZHER2:342-SR-HP15的放射性随时间的保留最高,而[177Lu]Lu二级探针示出结合放射性随时间释放更快。
实施例4:体内研究
在表4中提供了有关未预注射初级探针的[177Lu]Lu标记的二级探针的生物分布数据。[177Lu]-HP16和[177Lu]-HP17的生物分布非常相似,除了在骨吸收方面存在微小但显著的差异。与[177Lu]-HP18相比,更短的变体在血液、肝脏和肾脏中的吸收显著更低。[177Lu]-HP17在肺和骨中的吸收也显著更低。
在图4中显示了ZHER2:342-SR-HP15(4nmol)预注射和未预注射的情况下,[177Lu]Lu二级探针(注射后4h)的体内特异性结果。当小鼠预注射ZHER2:342-SR-HP15时,所有[177Lu]Lu二级探针的肿瘤吸收显著高于没有预注射的情况(p<0.00005)。有趣的是,在预先注射初级探针的情况下,正常组织的吸收也显著更高。
预先注射初级剂后,二级探针在肿瘤中的吸收急剧增加,这令人信服地证明了预靶向的特异性。有趣的是,在注射ZHER2:342-SR-HP15后,在血液、肾脏、脾和肌肉中也观察到二级探针的吸收增加。这可能是由于二级探针与初级剂的缔合,初级剂没有完全从血液循环中清除,或在与肿瘤中的受体解离后重新进入血流。这种效应对于[177Lu]-HP16不太明显。
[177Lu]Lu-HP16、[177Lu]Lu-HP17、[177Lu]Lu-HP18与[177Lu]Lu-HP2在携带SKOV3的小鼠中注射初级探针后的比较生物分布结果呈现于表5中。
生物分布测量示出,所有研究的基于PNA的探针都能从血液和正常器官和组织中迅速清除。观察到缀合物之间的生物分布存在一些差异。在注射后4h,[177Lu]Lu-HP18的血液浓度显著(p<0.0001)高于其他二级探针。在此时间点,[177Lu]Lu-HP16、[177Lu]Lu-HP17和[177Lu]Lu-HP2(0.1±0.0% ID/g)的肝脏吸收相等,但显著(p<0.05)低于[177Lu]Lu-HP18(0.2±0.1% ID/g)。具有突出吸收的唯一组织是肾脏和肿瘤。[177Lu]Lu-HP16(6±1%ID/g)在肾脏中的吸收显著(p<0.05)低于[177Lu]Lu-HP18(12±2% ID/g)和[177Lu]Lu-HP2(10±2%ID/g)。[177Lu]Lu-HP16示出最高的平均肿瘤吸收(24±6% ID/g),但与其他探针吸收的差异并不显著。在注射后4h,肾脏吸收较低和肿瘤中吸收高的组合导致[177Lu]Lu-HP16的肿瘤与肾脏比率较高(是肾吸收的4倍高)。[177Lu]Lu-HP16和[177Lu]Lu-HP17的肾吸收显著(p<0.005)低于[177Lu]Lu-HP18。在注射后144h,肿瘤吸收的顺序是[177Lu]Lu-HP17和[177Lu]Lu-HP18(两者均为5±1% ID/g)>[177Lu]Lu-HP2(4±1%ID/g)>[177Lu]Lu-HP16(两者均为3±1% Id/g)。[177Lu]Lu-HP17在肿瘤中示出更好的放射性保留和在肾脏中更快的清除(肿瘤吸收是肾吸收的7倍高),导致注射后144h肿瘤与肾脏的比率更高。
实施例5:SPECT/CT成像
SPECT/CT成像的结果(图5)证实了基于亲和体分子的PNA介导的所有变体的有效预靶向。肿瘤是吸收最高的部位。具有显著吸收的唯一组织是肾脏和肿瘤。每只动物在肿瘤中的放射性吸收都极大地高于在肾脏中。
在肿瘤中的放射性分布是用放射自显影评价的。注射后4h的活性分布是相当均匀的,并且反映了异种移植物的成群特征。在较晚的时间点,从肿瘤核心到边缘的放射性浓度梯度更加明显。
肾脏和肿瘤的估计剂量测定结果示出在表6和图6中。HP16、HP17、HP18和HP2的肿瘤和肾脏的时间活性图下的面积比率分别为3.8、2.8、2.0和2.0。这表明[177Lu]Lu-HP16将为治疗应用提供最有利的剂量测定。

Claims (28)
1.一种用于将诊断或治疗剂靶向至靶位点的试剂盒,包括:
(a)第一缀合物,包括
(i)能够选择性地与所述靶位点结合的靶向部分;和
(ii)包括第一PNA低聚物的第一杂交探针部分;和
(b)第二缀合物,包括
(i)包括与第一PNA低聚物互补的第二PNA低聚物的第二杂交探针部分;和
(ii)诊断剂或治疗剂部分;
其中,在所述第二杂交探针中的所述第二PNA低聚物的长度在5个碱基和12个碱基之间。
2.根据权利要求1所述的试剂盒,其中,所述靶位点存在于包括人的哺乳动物的蛋白上,所述蛋白在细胞,优选肿瘤细胞的表面上表达。
3.根据权利要求2所述的试剂盒,其中,所述靶位点存在于包括人的哺乳动物的蛋白上,所述蛋白选自由以下组成的组的:表皮生长因子受体(EGFR)、人表皮生长因子受体2(HER2)、人表皮生长因子受体3(HER3)、胰岛素样生长因子1受体(IGF1R)、碳酸酐酶IX(CAIX)、血小板源性生长因子受体β(PDGFR-β)、nectin-4、分化群38(CD38)、分化群33(CD33)、分化群30(CD30)、分化群22(CD22)和分化群79b(CD79b)。
4.根据权利要求3所述的试剂盒,其中,所述靶位点存在于人表皮生长因子受体2(HER2)上。
7.根据权利要求1至6中任一项所述的试剂盒,其中,所述杂交探针的一种或两种包括增溶部分,优选(a)基于PEG的接头,诸如包括2-[2-(2-氨基乙氧基)乙氧基]乙酸(AEEA)的接头;
或(b)包括带电荷的或极性的氨基酸的肽基接头。
8.根据权利要求1至7中任一项所述的试剂盒,其中,在所述第一杂交探针部分中的所述第一PNA低聚物的长度至少是在所述第二杂交探针部分中的所述第二PNA低聚物的长度,并且不超过15个碱基。
9.根据权利要求8所述的试剂盒,其中,在所述第一杂交探针部分中的所述PNA低聚物的长度为15个碱基。
10.根据权利要求1至9中任一项所述的试剂盒,其中,在所述第二杂交探针部分中的所述互补的PNA低聚物的长度为在9和12个碱基之间。
11.根据权利要求10所述的试剂盒,其中,所述互补的PNA低聚物的长度为9个碱基。
12.根据权利要求10所述的试剂盒,其中,所述互补的PNA低聚物的长度为12个碱基。
13.根据权利要求1至12中任一项所述的试剂盒,其中,所述治疗剂选自由以下组成的组:放射性核素、卡奇霉素、澳瑞他汀类诸如单甲基澳瑞他汀E/F(MMAE/F),以及美登素类诸如美登素衍生物DM0-DM4。
14.根据权利要求13所述的试剂盒,其中,所述治疗剂是选自由以下组成的组的放射性核素:镥-177(177Lu)、钇-90(90Y)、铋-212(212Bi)、铋-213(213Bi)、砹-211(211At)、锕-255(255Ac)、铜-67(67Cu)、镓-67(67Ga)和铼-186(186Re)。
15.根据权利要求1至14中任一项所述的试剂盒,其中,所述诊断剂生成通过选自由以下组成的组的方法能检测到的信号:磁共振成像(MRI)、正电子发射体层成像(PET)、单光子发射计算机体层成像(SPECT)、计算机体层成像(CT)、X射线成像、超声和光学成像。
16.根据权利要求15所述的试剂盒,其中,所述诊断剂是放射性核素。
17.根据权利要求13或16所述的试剂盒,其中,所述诊断剂或治疗剂是放射性核素,并且至少所述第二杂交探针部分包括螯合剂,优选1,4,7,10-四氮杂环十二烷-1,4,7,10-四乙酸(DOTA)。
18.一种用于将诊断或治疗剂递送至包括人的哺乳动物的靶位点的方法,所述方法包括:
(a)向所述哺乳动物给药第一缀合物,所述第一缀合物包括
(i)选择性地与所述靶位点结合的靶向部分;和
(ii)包括第一PNA低聚物的第一杂交探针部分;
(b)任选地,向所述哺乳动物给药清除剂,并且允许所述清除剂从循环中清除非定位的第一缀合物;和
(c)向所述哺乳动物给药第二缀合物,所述第二缀合物包括
(i)包括与所述第一PNA低聚物互补的第二PNA低聚物的第二杂交探针部分;和
(ii)诊断剂或治疗剂部分;
其中,在所述第二杂交探针部分中的所述第二PNA低聚物的长度为5和12个碱基之间,并且其中,所述第二PNA低聚物与在所述第一缀合物中的第一PNA低聚物结合,从而将所述诊断剂或治疗剂部分靶向至所述靶位点。
19.一种药物组合物,包括根据权利要求1至17中任一项所述的试剂盒的所述第一缀合物和第二缀合物。
20.根据权利要求19所述的药物组合物,用于诊断或治疗医学病症的用途,所述医学病症选自由癌症、传染性疾病、炎性疾病和自身免疫性疾病组成的组,优选癌症。
21.一种诊断或治疗缀合物,用于将诊断或治疗剂递送至包括人的哺乳动物的靶位点的方法的用途,所述方法包括:
(a)向所述哺乳动物给药靶向缀合物,所述靶向缀合物包括
(i)选择性地与所述靶位点结合的靶向部分;和
(ii)包括第一PNA低聚物的第一杂交探针部分;
(b)任选地,向所述哺乳动物给药清除剂,并且允许所述清除剂从循环中清除非定位的靶向缀合物;和
(c)向所述哺乳动物给药所述诊断性或治疗性缀合物;
其中,所述诊断性或治疗性缀合物包括:
(i)包括与所述第一PNA低聚物互补的第二PNA低聚物的第二杂交探针部分;和
(ii)诊断剂或治疗剂部分;
并且其中,在所述第二杂交探针部分中的所述第二PNA低聚物的长度为5和12个碱基之间,并且其中,所述第二PNA低聚物与在所述第一缀合物中的所述第一PNA低聚物结合,从而将所述诊断剂或治疗剂部分靶向至所述靶位点。
22.根据权利要求21所述用途的诊断或治疗缀合物,其中,所述方法是用于诊断、预后或治疗包括人的哺乳动物的医学病症的方法。
23.根据权利要求22所述用途的诊断或治疗缀合物,其中,所述医学病症选自由以下组成的组:癌症、传染性疾病、炎性疾病和自身免疫性疾病;优选癌症。
24.根据权利要求23所述用途的诊断或治疗缀合物,其中,所述医学病症是能够形成实体肿瘤的癌症,所述癌症选自由乳腺癌症、前列腺癌症、肺癌症、头颈癌症、胃癌症和结肠癌症组成的组。
25.根据权利要求24所述用途的诊断或治疗缀合物,其中,所述靶位点存在于选自由以下组成的组的人蛋白上:表皮生长因子受体(EGFR)、人表皮生长因子受体2(HER2)、人表皮生长因子受体3(HER3)、胰岛素样生长因子1受体(IGF1R)、碳酸酐酶IX(CAIX)、血小板源性生长因子受体β(PDGFR-β)和nectin-4。
26.根据权利要求23所述用途的诊断或治疗缀合物,其中,所述医学病症是选自由黑素瘤、白血病和骨髓瘤组成的组的血液学癌症。
27.根据权利要求26所述用途的诊断或治疗缀合物,其中,所述靶位点存在于在选自由以下组成的组的人蛋白上:分化群38(CD38)、分化群33(CD33)、分化群30(CD30)、分化群22(CD22)和分化群79b(CD79b)。
28.根据权利要求21至27中任一项所述用途的诊断或治疗缀合物,其中,在所述第二杂交探针部分中的所述第二PNA低聚物的长度为9和12个碱基之间。
