Improving the treatment effect and life quality of patients.
In one embodiment of the invention, determining the goal of pressure sore management includes ranking the goal, including at least a first stage, a second stage, and a third stage. Different patients are classified, different grades are provided, different evaluations are performed, more conditions are considered, and the evaluated result is more accurate.
Specifically, the first level comprises patients in a high risk group, the patients in the group do not have pressure sores, but are in the easily-occurring group, and the patients are subjected to pressure sore risk assessment; the second stage comprises patients with pressure sores, the patients need to carry out specific analysis on the conditions of the pressure sores, and the patients are subjected to pressure sore state assessment; the third stage comprises the patients with pressure sores in the rehabilitation stage, and rehabilitation evaluation is carried out on the patients.
In an embodiment of the present invention, the key result indicators include at least a plurality of first indicators, second indicators, third indicators, fourth indicators and fifth indicators, and in this embodiment, the first indicators, second indicators, third indicators, fourth indicators and fifth indicators may correspond to the age, the disease diagnosis, the action status, the health degree and the environment, respectively.
Obtaining a plurality of index scores corresponding to a first index, a second index, a third index, a fourth index and a fifth index, specifically, each index can obtain a plurality of scores, and then calculating the average index score and standard deviation of each obtained index; obtaining a standardized value according to the average index score and the standard deviation; adding the standardized values of each index to obtain a sum score; the status of the patient is determined based on the sum score.
The standard deviation calculation formula comprises;
E = sqrt(Σ(xi - x)^2 / n)
where xi is the score of each patient on the index, x is the average of the index, n is the number of samples, and E is the standard deviation;
the normalized value calculation formula includes;
zi = (xi - x) / σ
where xi is the score of each patient on the index, x is the mean of the index, σ is the standard deviation of the index, and zi is the normalized value.
The specific evaluation method comprises the following steps of if pressure sore state evaluation is carried out on a patient, judging that the state of the patient is good, and judging that the state of the patient is poor, wherein the sum score is positive;
if the risk assessment is carried out on the patient, the total score and the standard coincide or exceed the standard, the patient is judged to be in a healthy state, the total score is lower than the standard, and the patient is judged to be easy to suffer from pressure sores;
if rehabilitation evaluation is carried out on the patient, the total score and the standard coincide or exceed the standard, and the condition of the pressure sore of the patient is judged to be improved; and the total score is lower than the standard, and the patient is judged to have poor recovery of the pressure sore.
In one embodiment of the invention, the execution plan includes;
according to the actual pressure sore condition of a hospital and the pressure sore condition expected to be reached, a plan is made and is recorded into a OKR system, wherein the plan comprises a annual plan, a quarternary plan and a monthly plan;
the annual plan includes setting a start month and an end month of plan execution, defining a range of investigation, setting an explanation of the annual plan, setting an annual plan assessment index, and one or more assessment indexes such as a pressure sore occurrence rate, a pressure sore recovery rate improvement rate, and the like may be set.
And an assessment team or personnel can be set. A planned reminder time may also be set to alert the care department or other relevant team as to how many days to date money.
The quarter schedule includes setting a start month and an end month of schedule execution in the system, and prescribing a range of investigation. In this way, the execution condition of the index in the annual plan in the current quarter can be checked in the related statistical report, whether the annual plan is modified or not is determined according to the condition, the problems of meeting of the pressure sore group are summoned, and the like, and the next month/quarter task and the investigation point can be formulated or modified.
The month plan includes setting backlog to be completed for each month, including date and time, backlog content, backlog related person (selected hospital person), and backlog investigation point (a plurality of backlogs can be set).
In this embodiment, the execution plan may be completed by a nursing department of the hospital, and correspondingly, the execution plan further includes a pressure sore auditing team, and in the OKR system, the year/season/month plan contents formulated by the nursing department can be checked. And making specific implementation contents according to the planned contents. Uploading and reading pressure sore related literature data in the system.
Pressure sore group personnel set pressure sore nursing measures of patients according to relevant data, including but not limited to patient age groups, disease course diagnosis, consciousness states, action states, family members/careers accompanying conditions and the like, and corresponding nursing measures and frequency are required to be set for each evaluation type.
The pressure sore group personnel observe the real-time condition needing to be recorded after the nursing measures are set according to the related data, including but not limited to the area of the pressure sore, the condition of the wound surface, whether the pressure sore is improved or not, and the like.
The pressure sore group personnel can use the multiple disc function to finish browsing the full cycle process of the pressure sore nursing condition of a certain patient, verify whether nursing measures are effective, and screen problematic measure conditions if the pressure sore condition of the patient is worsened.
Pressure sore group personnel make pressure sore satisfaction survey content in the system, and after the pressure sore of a patient is healed, nurses or nurses find the patient for filling.
Pressure sore teams specify in the system the limits of pressure sore deterioration, such as how much the area increases from the original, how much the depth increases, etc.
The invention also provides a OKR-based pressure sore management system, which comprises;
a targeting module configured to determine a target for pressure sore management,
the index preparation module is used for preparing key result indexes configured for constant pressure sore management;
the plan making module is configured to make an execution plan according to the targets and the key result indexes;
and the execution module is configured to implement corresponding preventive and therapeutic measures according to the execution plan.
In an embodiment of the present invention, the pressure sore evaluation module is further included, and the pressure sore evaluation module is configured to; obtaining a plurality of index scores corresponding to the first index, the second index, the third index, the fourth index and the fifth index; calculating the average index score and standard deviation of each index; obtaining a standardized value according to the average index score and the standard deviation; adding the standardized values of each index to obtain a sum score; the status of the patient is determined based on the sum score.
In addition, each functional unit in the embodiments of the present invention may be integrated in one processing unit, or each unit may exist alone physically, or two or more units may be integrated in one unit. The integrated units may be implemented in hardware or in software functional units.
The integrated units, if implemented in the form of software functional units and sold or used as stand-alone products, may be stored in a computer readable storage medium. The computer software product is stored in a storage medium and includes instructions for causing a computer device (which may be a personal computer, a server, or a network device, etc.) to perform all or part of the steps of the methods of the various embodiments of the invention. And the aforementioned storage medium includes: a U-disk, a removable hard disk, a Read-only memory (ROM), a random access memory (RAM, random Access Memory), a magnetic disk, or an optical disk, or other various media capable of storing program codes.
The above is only a preferred embodiment of the present invention, and is not intended to limit the present invention, but various modifications and variations can be made to the present invention by those skilled in the art. Any modification, equivalent replacement, improvement, etc. made within the spirit and principle of the present invention should be included in the protection scope of the present invention.
