Description
Technical Field
The invention belongs to the technical field of medical care auxiliary instruments, and particularly relates to a pressure sore early warning method and system for severe patients.
Background
Pressure sores are also known clinically as pressure lesions, and refer to necrosis and ulcers formed by ischemia and hypoxia of the skin and tissues caused by varying degrees of pressure or shear force. Pressure sores can not only increase pain of patients and aggravate illness state and prolong recovery time of diseases, but also endanger life due to complications such as septicemia caused by secondary infection.
The incidence rate of pressure sores is positively related to skin tissue pressure and duration, and for severe patients, such as postoperative hypoproteinemia, elderly patients, human clinical high-risk patients suffering from coma, dementia, depression, quadriplegia, sensory disturbance and other diseases, long-term bedridden patients are required for disease or treatment, and even long-term braking by sedation treatment is required during hospitalization, so long-term bedridden nursing is the most main factor causing pressure sores to occur in the current intensive care unit.
The prevention and treatment of pressure sores are a core problem in nursing work of severe medical departments, and are also a key index for evaluating nursing quality, so that not only can medical resource loss be increased, but also economic pressure of patients can be increased. If the patient is subjected to pressure sore risk real-time monitoring and evaluation, comprehensive nursing can be realized according to risk early warning, so that the pressure sore incidence rate can be reduced, and the good prognosis of the patient is ensured.
According to the current research results, specific related risk factors for clinically forming pressure sores are as follows:
1. pressure: pressure sores are caused not only by vertical pressure but also by friction and shear forces. Usually associated with body position, is caused by the combined action of 2 or more forces, and when the sustained external pressure exceeds the normal capillary pressure, ischemia, hypoxia and cell metabolic disorder occur in the tissues to cause necrosis of the tissues and pressure sores.
(1) Vertical pressure: sustained pressure on the local tissue is the most important factor causing pressure sores. The skin and subcutaneous tissue can endure a certain pressure in a short time, if the pressure is more than or equal to 8.0kPa continuously, microcirculation is blocked continuously, so that the tissue is anoxic, blood vessel collapse and thrombosis are caused, and pressure sores are caused.
(2) Friction force: when a patient is in bed, the skin can be rubbed by the retrograde resistance on the surface of the bed sheet at any time, so that the contact surface of the bed sheet and the skin relatively move, and the damage to the stratum corneum is easily caused by acting on the skin. The skin is wet and polluted after injury, and pressure sores occur.
(3) Shear force: the bones and deep tissues slide downwards due to the action of gravity, the skin and superficial tissues stay in place due to friction force, relative displacement is generated between the two tissues, so that shearing force exists between the two tissues, and the influences of the stress of the tissues and blood vessels are elongated, twisted and even torn, so that deep tissue necrosis occurs. The shearing force is formed by adding vertical pressure and friction force, and is closely related to the body position. When a patient is lying on the bed to lift the head of the bed, the body slides downwards to cause friction between the skin and the bed, and the gravity in the vertical direction of the body is added to cause shearing force.
2. Movement: loss or diminution of physical movement is also the most common risk factor for pressure sores, such as stroke, spinal cord injury, long-term deep sedation, etc.
3. Skin is stimulated by moisture or faeces: the patient is braked for a long time, the skin is stimulated by sweat, excrement and the like, and the pH value is changed to reduce the protective capability of the epidermis layer, so that pressure sores are formed.
4. Nutritional status: patients with insufficient nutrition intake or postoperative hypoproteinemia are prone to negative nitrogen balance, muscle atrophy and subcutaneous fat reduction, and at the moment, if the stress points of the prone position are continuously stressed too strongly, blood circulation disturbance is easy to occur, so that pressure sores are caused. The force of the body weight of the obese patient on the skin is larger, and the force is positively related to the occurrence of pressure sores.
5. Age: pressure sores often occur in elderly patients who are bedridden for a long period of time. With age, the body's ability to effectively distribute pressure is impaired, with changes in collagen synthesis, reduced mechanical strength of the tissue, and increased stiffness, resulting in reduced tolerance to fluid flow in the tissue. The older the age, the less elastic the soft tissue, the lower the sensory function, the slower the blood circulation, and the easy formation of thrombus at the pressed part, which causes necrosis, resulting in pressure sores.
Pressure sore good hair position: pressure sores are prone to occur in processes that lack tissue protection, no muscle wrapping, or thinner muscle layers. The body has different points of force applied under different positions, and the positions where pressure sores occur are different.
Supine position: pressure sores are well developed in the occipital tuberosity, shoulder blade, elbow, spinal tuberosity, sacral tail, lateral malleolus, heel, etc.
Lateral position: pressure sores are well developed in auricles, acromion, elbows, hips, medial and lateral knee joints, medial malleolus, lateral malleolus, etc.
Prone position: pressure sores are well developed in the forehead, cheeks, shoulders, pinna, breasts of females, genitals of males, iliac crest, knees, dorsum of feet, toes, etc.
Sitting position: pressure sores are well developed in the ischial tuberosities, sacral cauda.
The aim of evaluating the risk of pressure sores is to determine the dangerous population to be subjected to preventive measures and the special factors in danger, so that manpower and material resources are concentrated on the special population. Risk assessment scales include braking, fasting, nutritional status, disturbance of consciousness, etc., which can increase the incidence and severity of pressure sores. The currently common methods of assessing the risk of pressure sores in patients are the Braden scale and the Norton scale.
The Braden scoring is shown in the following table:
scoring content | 1 minute | 2 minutes | 3 minutes | 4 minutes |
Feel that: uncomfortable feeling ability to compression part | Complete loss of | Severe loss of | Mild loss of | Is not damaged |
Humidity: degree of skin exposure to moisture | Continuous wetting | Very humid | Occasionally wet | Little moisture |
Activity: degree of physical activity | Bedridden patient | Is limited to seats | Doll walking | Frequent walking |
Mobility capability: ability to change and control body position | Can not at all | Severe limitations | Mild restriction | Is not limited by |
Nutrition: ingestion situation | Severe and bad conditions | The defects are | Appropriately, is | Good quality |
Friction and shear forces | Has the following components | Potential for | Without any means for | Without any means for |
* In the table above, except that the scores of friction force and shearing force are 1-3 points, all the other points are 1-4 points, and the smaller the scores are, the worse the organ functions of the patient are, and the higher the risk of pressure sores is. The score is less than or equal to 16 points, and is considered to have certain danger; score is less than or equal to 12 points, belongs to high-risk patients, and corresponding measures should be taken to prevent pressure sores.
The Norton scoring is shown in the following table:
score value | General conditions | Mental condition | Ability to move | Exercise capacity | Fecal incontinence and urinary incontinence |
1 minute | Good (good) | Alertness | Free movement | Is not limited by | Without any means for |
2 minutes | In general | Desert of cold | Activities under help | Mild limitation | Occasionally |
3 minutes | Difference of difference | Confusion of | Dependent wheelchair | Is very limited | Urine collection |
4 minutes | Very poor | Coma of coma | Bed for lying in bed | Cannot move | Feces and urine |
* In the above table, the smaller the score, the higher the risk of developing pressure sores. The total score is 18-20, and the occurrence rate of pressure sores is only 5%; score is less than or equal to 14 points, which indicates that pressure sores are easy to occur.
The prior clinical pressure sore prevention measures mainly depend on timing evaluation of nursing groups and the use of pressure reduction patches, such as evaluation according to the two evaluation methods, which need to subjectively evaluate the occurrence of pressure sore according to the running force and skin condition of patients, and implement measures when the tendency of pressure sore appears, and early warning and early intervention cannot be performed to prevent the formation of pressure sore. In addition, the current conventional guiding method is to turn over a critical patient every 2 hours, but the conventional turning over every 2 hours still stays in an experience stage, and cannot scientifically guide clinic to prevent pressure sores, so that a more scientific and effective technology is needed to guide and prompt medical staff.
