In recent years, the incidence of atrial fibrillation (atrial fibrillation) is increasing, and with the development of interventional medical devices and equipment, the application of cardiac interventional therapy is becoming more widespread, such as ablation surgery, wherein one important step in the surgical procedure is atrial septal puncture. Under the guidance of the imaging device, the surgical operator uses the interatrial septum puncture needle to penetrate the inner wall of the heart from the fossa ovalis, and a channel for the right atrium to enter the left atrium is established, so that the surgical operation is further prepared for the left atrium.
The existing atrial septal puncture needle is limited and fixed by the dilator in the puncture process when the stylet passes through the dilator, so that the length (or distance) of the stylet far end extending out of the dilator far end is a fixed value, an operator can hardly adjust the puncture needle in a limited space when using the puncture needle, and the needlepoint can scratch the vascular wall in the adjustment process, thereby causing unnecessary damage.
The inventor provides an interatrial puncture needle capable of adjusting the extending length of a needle point through repeated and intensive research, wherein an internal thread structure and a hollow structure are arranged in a control handle, a luer part similar to a luer male connector is matched with the internal thread structure of the control handle, the far end of the luer part is positioned in the hollow structure, an external thread matched with the internal thread structure of the control handle is arranged on the outer wall of the luer part, the luer part is movably connected with the control handle through the thread structure, so that the far end of the luer part can move back and forth in the hollow structure of the control handle, the far end of the luer part is also used for connecting the near end of a needle core, so that the needle core can be driven to move, when the luer part is rotated, the needle point at the far end of the needle core moves out of the far end of a dilator or moves back into the dilator, meanwhile, the thread structure provides the function of locking the luer part, so that the needle point stays at a position expected by an operator, the operation is convenient, and unnecessary human body injuries in the puncture process are reduced as much as possible.
The technical solutions provided by the embodiments of the present application are described below with reference to the accompanying drawings.
The interatrial septum puncture needle with the adjustable needle point extension length as shown in figure 1 comprises a puncture needle body 1 and a control handle 2, wherein the control handle 2 is connected with the near end of the puncture needle body 1. The puncture needle body 1 comprises a sheath tube (not shown in the figure), an expander 3, a needle tube 11 and a needle core 12 which are sleeved from outside to inside in sequence, namely the expander 3 is arranged in the sheath tube, the needle tube 11 is arranged in the expander 3, and the needle core 12 is arranged in the needle tube 11. The distal end of the dilator 3 is longer than the distal end of the sheath, thereby exposing the distal end of the sheath. The hub 12 is reciprocally movable within the tube 12.
As shown in fig. 3 and 4, the proximal end of the control handle 2 is threadedly coupled to the luer part 22, and the inside of the control handle 2 is provided with a hollow structure in which both the proximal end of the hub 12 and the distal end of the luer part 22 are located, and the proximal end of the hub 12 and the distal end of the luer part 22 are connected to each other. Specifically, a groove is formed in the handle body 21 of the control handle 2, the distal end of the groove is used for forming a hollow structure, the hollow structure can be provided with a smooth inner wall so as to facilitate the distal end of the luer part 22 to move inside the hollow structure, the proximal end of the groove is located at the proximal end vertex position of the handle body 21 and communicated with the outside, and an internal thread is formed in the inner wall of the proximal end of the groove. The luer part 22 is of a luer male-like structure, and the outer wall thereof is provided with an external thread 221 matching the internal thread, and the luer part 22 is movably connected to the handle body 21 by the thread structure. The portion of luer member 22 exposed from handle body 21 may be provided with a slip-resistant structure or set knob 222 (shown in fig. 4) to facilitate rotation of luer member 22 by a surgical operator.
When the luer part 22 is rotated in a first direction to move the luer part 22 in a radial direction by a first preset distance, the luer part 22 carries the needle tip 121 (shown in fig. 7) at the distal end of the stylet 12 to a first predetermined position at which the distal apex of the dilator 3 meets the first preset distance. Specifically, as shown in fig. 4 to 6, the external thread 221 is a right-handed thread, when the luer part 22 is rotated in a clockwise direction, the luer part 22 moves in a direction of the distal end of the handle body 21 in a radial direction thereof under the action of the internal thread, the stylet 12 fixed to the distal end of the luer part 22 moves, the distal end of the stylet 12 protrudes out of the needle tube 11 (as shown in fig. 2, the dilator 3 is not shown in the figure), so that the distal tip 121 of the stylet also moves, the tip 121 protrudes out of the distal end of the dilator 3 (as shown in fig. 8), and the movement distance of the tip 121 is the same as the movement distance of the luer part 22; when luer member 22 is rotated in a counterclockwise direction, luer member 22 moves in a direction radially toward the proximal end of handle body 21, thereby retracting needle tip 121 toward the proximal end of needle cannula 11, and further rotating luer member 22 is continued, so that needle tip 121 can be completely withdrawn into dilator 3 (as shown in fig. 7). It should be noted that, due to the threaded connection of the internal thread and the external thread 222, when the luer part 22 stops rotating, the needle tip 121 can stay at the position where the surgical operator wants to lift, that is, the needle tip 121 is exposed from the distal end of the dilator 3 to the length that the surgical operator wants, due to the locking effect between the internal thread and the external thread. Likewise, the external thread 221 may be a left-handed thread, in which case, when the luer part 22 is rotated in the counterclockwise direction, the needle tip 121 extends out of the distal end of the dilator 3, and when the luer part 22 is rotated in the clockwise direction, the needle tip 121 retracts into the inside of the dilator 3, and other structures and linkage manners may refer to the transseptal needle having the luer part 22 with a right-handed thread, which will not be described herein.
In the scheme, the luer part 22 is rotated to move the luer part 22, so that the needle core 12 drives the needle point 121 to move, the extending length of the needle point 121 is adjusted, and the atrial septal puncture operation in the operation is facilitated; and the needle tip 121 can stay at a preset position away from the far end of the dilator 3 through the locking of the thread structure; the needle point 121 can be retracted into the dilator by reversely rotating the luer part 22 when necessary, and the interatrial puncture needle with the structure can conveniently adjust the extending length of the needle point 121 and fix the needle point, thereby facilitating the operation, avoiding unnecessary human body injury and reducing the operation risk.
In some embodiments, as shown in fig. 4 and 5, the distal end of luer member 22 is also provided with a stop 223. As shown in fig. 5, when luer part 22 moves to the position where stopper 223 abuts against the proximal end of the hollow structure, needle tip 121 retracts to the second preset position inside dilator 3. Specifically, when the external thread 221 is a right-handed thread, the luer part 22 is rotated in the counterclockwise direction until the stopper 223 abuts against the proximal end of the hollow structure, and at this time, due to the action of the stopper 223 and the proximal end of the hollow structure, the luer part 22 cannot move in the proximal direction, and the needle tip 121 can be completely retracted into the dilator 3 by setting the length of the stylet 12.
Preferably, the apex of tip 121 is located at the distal apex of dilator 3, i.e., the apex of tip 121 is nearly or completely flush with the distal apex of dilator 3, as shown in fig. 7. By disposing the apex of the needle tip 121 at the position of the apex of the distal end of the dilator 3, this state can be set as the initial state of the interatrial septum puncture needle in which the needle tip protrusion length can be adjusted, and when the luer part 22 is rotated, the needle tip 121 can be immediately protruded out of the dilator 3, which is advantageous for improving the efficiency of the operation, and the needle tip 121 does not scrape the inner surface of the needle tube 11, and the generation of debris is avoided, thereby avoiding the introduction of foreign matter into the blood of the human body and the formation of thrombus.
In other embodiments, the apex of needle tip 121 may also be set to recede into the interior of dilator 3 a distance from the distal apex of dilator 3.
In some embodiments, as shown in fig. 4, the proximal end of luer assembly 22 is provided with a knob 222, when knob 222 abuts the distal end of control handle 2, at which time stop block 223 is located at a second preset distance from the proximal end of the hollow structure. In the above-described aspect, the maximum protruding length of the needle tip 121 may be controlled by setting the length of the second preset distance.
In some embodiments, the axis of luer member 22 coincides with the axis of hub 12. By arranging the axes of the two components to coincide, the needle core 12 obtains more stable pushing force or pulling force and can better move in the needle tube 11.
In some embodiments, the axis of the control handle 2 coincides with the axis of the luer part 22. By means of the arrangement, the Ruhr part 22 and the hollow structure can be located on the axis of the control handle 2, so that the rotation of the far end of the puncture needle body 1 can be controlled by rotating along the circumferential direction of the control handle 2, and the position of the far end of the puncture needle body can be adjusted conveniently.
In some embodiments, luer part 22 has a thread pitch of 1.0mm to 2.0mm. The distance of the rotating luer part 22 which advances or retreats along the axial direction when rotating for one circle can be conveniently set through arrangement, so that the extending and retreating distance of the needle point 121 can be conveniently and finely controlled by a surgical operator.
In some embodiments, luer member 22 is made from any one of Polycarbonate (PC), polyethylene (PE), 304 stainless steel, ABS plastic (Acrylonitrile-Butadiene-Styrene copolymer). Preferably, the control handle 2 is also manufactured using the same materials as the luer part 22.
By using the above materials, the materials themselves have appropriate coefficients of friction, which not only facilitates the adjustment of the rotating luer part 22 by the operator, but also keeps the luer part 22 staying in place
In some embodiments, dilator 3 is made using any one of Polyethylene (PE), polyether block amide (Pebax). By adopting the material, the inner wall of the expander 3 can be prevented from being scratched, and the scraps can be prevented from falling.
In some embodiments, the proximal end of the core needle 12 is bolted to the distal end of the luer member 22. By bolting the hub 12 to the luer member 22, luer members 22 of different lengths can be easily replaced, thereby allowing for easy adjustment of the maximum distance that the needle tip 121 extends beyond the dilator 3.
The embodiments in the present specification are described in a progressive manner, and the same and similar parts among the embodiments are referred to each other, and each embodiment focuses on differences from other embodiments. In particular, for the method embodiments described later, since they correspond to the system, the description is simple, and for the relevant points, reference may be made to the partial description of the system embodiments.
The above description is only for the specific embodiments of the present application, but the scope of the present application is not limited thereto, and any changes or substitutions that can be easily conceived by those skilled in the art within the technical scope of the present application should be covered within the scope of the present application. Therefore, the protection scope of the present application shall be subject to the protection scope of the claims.
