背景技术
精神运动迟滞(PMR)主要表现为显著的言语、思维及行动受限或迟缓,被认为是抑郁症最基本的临床特点之一。由于精神运动迟滞常见于忧郁型抑郁症患者,因此被视作忧郁型抑郁症亚型最具特征性的辨别标准,精神运动迟滞通常作为抑郁症对抗抑郁剂反应不佳的预测指标,精神运动迟滞潜在的神经生物学机制为揭示抑郁症的病理生理机制提供重要的证据,有助于更深层次的理解抑郁症的发病机制。功能磁共振成像为各种神经精神疾病的脑功能异常提供了大量证据。fMRI技术通过检测与脑血流量(CBF)相关的指标来衡量脑功能活动,可通过基于动脉自旋标记血流灌注信号的检测方法,用于定量检测大脑的脑血流量。现有抑郁症患者的精神运动迟滞的大脑脑血流量变化的关系尚未明确。
发明内容
本发明的目的在于提供一种基于脑血流量的抑郁症精神运动迟滞客观诊断标记物,检测抑郁症患者的脑血流量,并通过横断面与纵向两个角度的比较,寻找与精神运动迟滞相关的脑血流量异常脑区,为抑郁症精神运动迟滞的客观诊断提供一种基于脑血流量变化的影像学标记物。

下面将结合本发明实施例中的附图,对本发明实施例中的技术方案进行清楚、完整地描述,显然,所描述的实施例仅仅是本发明一部分实施例,而不是全部的实施例。基于本发明中的实施例,本领域普通技术人员在没有作出创造性劳动前提下所获得的所有其它实施例,都属于本发明保护的范围。
一种基于脑血流量的抑郁症精神运动迟滞客观诊断标记物,采用确诊的抑郁症患者107例,应用汉密尔顿抑郁量表评估抑郁症患者的抑郁严重程度,正常对照组来源于医院周围社区的健康志愿者及医院内的健康工作人员,共48例。
临床评估:应用24项汉密尔顿抑郁量表(HAMD)评估抑郁症患者的抑郁严重程度,特里埃迟滞评定量表(SRRS)评定抑郁症患者的精神运动迟滞症状,<20分为无精神运动迟滞症状,≥20分为有精神运动迟滞症状。107例抑郁症患者中有35例抑郁症患者SRRS≥20分,归为精神运动迟滞组,72例患者SRRS<20分,归为非精神运动迟滞组。
SRRS量表共包括15条,前三条评估运动,接下来三条评估口语表达,三条评估话题展开能力,然后5条分别评估疲劳感,兴趣、时间感知、记忆力及注意力,最后一条总体评估迟滞程度,每条得分0-4分,总得分0-60分。研究表明,SRRS评估结果与HAMD等评估均具有较好的相关性,临床上被广泛地应用于PMR严重程度的评估。此外,连线测试TMT在本研究中也用来辅助检测精神运动迟滞症状,TMT用于检测精神运动迟滞的注意及转移处理过程有着明显的效用。TMT测试经常用于神经认知功能检测,包括TMT-A与TMT-B两部分,TMT-A要求受试者将数字1-25按数字顺序连接起来,主要检测运动速度,TMT-B需要受试者注意数字外围的形状是圆形还是正方形,要求受试者在按顺序连接数字的同时将正方形与圆形交替相连,主要检测视觉空间转移速度,对精神运动迟滞具有较好的检测效能。
MRI数据采集:请受试者放松平躺在检查床上,双手置于身体两侧。正确安装12通道头线圈发射和接收MRI信号。采用MRI扫描仪配属的头部固定器置于受试者两侧颞部减少其扫描期间的头动。定位时两侧眼裂连线位于线圈横轴中心并对准“+”字定位灯的横向连线,头颅正中矢状位与眼圈纵轴保持一致并对准“+”字定位灯的纵向连线。每位受试者共扫描五个模态,即定位像、高分辨率T1脑三维结构像、T2加权像及脉冲式动脉自旋标记成像。具体模态与扫描顺序如下:
定位像操作如下:
采用快速三维定位成像序列扫描轴状位、冠状位和矢状位的中间层。扫描参数:重复时间TR=8.6ms,回波时间TE=4.0ms,翻转角=20°,层厚=7mm,矩阵=256×256,视野FOV=250mm×250mm,扫描时间=7秒。定位像扫描目的:检查受试者大脑纵裂是否垂直于床面,为后续模态扫描定位提供参考。
高分辨率T1脑三维结构像操作如下:
采用T1加权三维磁化预备快速采集梯度回波序列。矢状位无插值扫描,范围覆盖全脑。扫描参数:TR=1900ms,TE=2.48ms,TI=900ms,翻转角=9°,层数=176,层厚=1mm,层间距=0mm,矩阵=256×256,FOV=250mm×250mm,扫描时间=4分18秒。结构像扫描完成后即刻检查扫描图像是否存在因受试者头动所致的运动伪影,以及因扫描范围未覆盖全脑所致的卷褶伪影。
T2加权像操作如下:
采用快速自旋回波TSE序列。以AC-PC为参考平面轴状位扫描,范围覆盖全脑。扫描参数:TR=6000ms,TE=95ms,翻转角=150°,层数=20,层厚=5mm,层间距=1.5mm,矩阵=384×384,FOV=230mm×230mm,扫描时间=1分02秒。T2加权像扫描目的为辅助排除严重脑血管损害及其他病灶的受试者。
pASL成像操作如下:
采用PICORE-Q2TIPS序列,以水平面为参考平面轴状位扫描,范围尽可能覆盖全脑。扫描参数:TR=4000ms,TE=12ms,TI1=600ms,TI2=1600ms,层数=27,层厚=4mm,层间距=1mm,翻转角=90°,矩阵=64×64,FOV=220mm×220mm,扫描时间=7分14秒。pASL成像主要反映静息状态下脑血流灌注状况。
所有107例患者(精神运动迟滞72例,非精神运动迟滞35例)均接受了基线期MRI扫描,基线期扫描后,患者接受抗抑郁剂治疗,根据病情需要,不同患者分别使用了五羟色胺再摄取抑制剂,五羟色胺去甲肾上腺素再摄取抑制剂,及去甲肾上腺素与特异性五羟色胺能抗抑郁剂三种抗抑郁剂。治疗第8周末,39例(精神运动迟滞15例,非精神运动迟滞24例)症状缓解的患者(HAMD与SRRS减分率均≥50%)接受了随访期MRI扫描。
MRI数据预处理:首先,所有图像先由人工检查,排除定位偏差、脑组织残缺或有伪影的图像。pASL数据预处理应用基于SPM12开发的ASLtbx工具包,步骤包括:(1)图像复位,即将原点设置在前联合-后联合线上。(2)头动校正,ASL数据的头动校正,控制组和标签组数据需要分开来做,这样就需要将两个组额外配准一下;(3)功能像与结构像配准,ASL图像需要与结构图像配准之后,才能标准化到MNI空间(或者其他模板空间,标准空间)进行组分析。目标图像和源图像分别选择T1图像和meanASL图像。T2加权结构图像也可以使用。(4)平滑,在计算脑血流量(脑血流量)之前,需要对ASL图像进行平滑操作来减少噪声。计算完脑血流量之后再进行一次平滑操作,空间标准化之后如果脑内结构变化较大也可进行二次空间标准化,进一步将体素大小重采样至2×2×2mm3。预处理后,排除了fMRI图像质量控制不合格的患者(基线期精神运动迟滞组5例,非精神运动迟滞组2例;随访期精神运动迟滞组4例,非精神运动迟滞组1例),因此,基线期剩余100例(精神运动迟滞组30例,非精神运动迟滞组70例),随访期剩余34例(精神运动迟滞组11例,非精神运动迟滞组23例)。计算脑血流量:在Matlab命令窗口,敲:asl_perf_subtract,回车会弹出对话框窗口,要求选择ASL图像。图像需要是预处理过的,设置的原点以及体素大小一致。选择好源文件之后,点“Done”,之后会有第二个对话框窗口弹出来,需要输入ASL图像采集参数以及计算脑血流量需要的设置。如果需要用指定的M0b(M0 blood)来计算脑血流量,那么问题“Using a unique M0value for all voxels?”选1。然后会继续有个问题,问是否直接指定(M0wm or M0csf)值或者运行脚本文件从M0图像中得到M0i。如果你已经提取了M0i(M0wm or M0csf),并且想利用它来计算脑血流量的话,问题“Using a unique M0 value for all voxels?”选择1,问题“Select an M0 image or input an M0 value?”选择“M0 value”,然后就会弹出对话框,输入你的M0i值即可。选择使用指定的M0b来计算脑血流量,并且问题“Select an M0image or input an M0 value”选择“M0 image”时,会弹出对话框,问是否利用提取M0i(M0csf或者M0wm,取决于上一步的选项)的图像来提取ROI,否则选择ROI模型文件。如果选择“yes”,就会有个弹出窗口显示选择的M0图像的某一层,而Matlab的命令行可以选择M0图像的层数。层数范围为1到M0图像的最大层数。如果发现了很好提取ROI的图像层,可以先在Matlab命令行输入0终止选择层数进程,然后打开提取ROI的弹出窗口。双击完成ROI的提取。当所有参数都设置完成之后,脚本文件“asl_perf_subtract”会开始计算所有指定的输出,例如灌注差异信号、平均血流信号、脑血流量、pseudo BOLD、平均灌注差异、meanBOLD、平均脑血流量等。这些输出会保存在源图像文件夹中。除此之外,还会生成一个文本文件,里面存储着总体灌注差异和脑血流量信号。
人口学信息与临床资料统计分析及结果:为减少年龄、性别、教育程度及体重指数对统计结果的影响,在影像学统计分析中,我们从基线期对照组(NC)组与非精神运动迟滞组各随机抽取30例患者与30例精神运动迟滞组进行人口学资料(即年龄、性别、教育程度及体重指数)的匹配,随访期同样从非精神运动迟滞组随机抽取11例患者与11例精神运动迟滞组进行了人口学资料的匹配。被试的筛选流程如图1所示。如表1所示人口学信息与临床资料的统计结果均应用SPSS 18.0统计分析(SPSS,Inc.,Chicago,IL),P<0.05被认为有统计学差异,其中,分类变量应用卡方检验,三组连续变量应用单因素方差分析,两组连续变量应用独立样本T检验,精神运动迟滞组与非精神运动迟滞组的迟滞程度(SRRS与TMT评分)的比较应用一般线性模型,将HAMD评分作为协变量。结果显示,基线期NC、非精神运动迟滞及精神运动迟滞三组的人口学资料如性别、年龄、教育程度及体重指数均无显著差异。非精神运动迟滞及精神运动迟滞两组的临床资料如发病年龄、总病程、发病次数、家族史及自杀倾向均无显著差异,但本次发病病程存在显著差异,精神运动迟滞组的本次发病病程显著短于非精神运动迟滞组,我们进一步应用Pearson相关分析了本次发病病程与精神运动迟滞严重程度的相关性,发现二者呈显著负相关(r=-0.383,P=0.003)。精神运动迟滞组的HAMD得分显著高于非精神运动迟滞组,为减少抑郁对精神运动迟滞的影响,在比较非精神运动迟滞及精神运动迟滞两组的迟滞程度时,将HAMD得分作为协变量回归掉,精神运动迟滞组的迟滞程度(SRRS与TMT得分)仍显著高于非精神运动迟滞组。随访期非精神运动迟滞及精神运动迟滞两组的人口学资料及临床资料统计结果与基线期相似,但HAMD得分、SRRS与TMT得分无显著差异,为控制不同抗抑郁剂对随访期脑血流量的影响,我们还比较了非精神运动迟滞及精神运动迟滞两组应用的抗抑郁剂,结果显示两组应用的抗抑郁剂无显著差异(χ2=0.958,P=0.620)。
组间脑血流量及精神运动迟滞组内脑血流量基线期与随访期比较及结果:脑血流量比较采用REST工具包(REST,www.resting-fmri.sourceforge.net),基线期三组(对照组、非精神运动迟滞组与精神运动迟滞组)脑血流量比较与随访期两组(非精神运动迟滞组与精神运动迟滞组)脑血流量比较分别应用单因素方差分析及独立样本T检验,均将HAMD评分作为协变量;精神运动迟滞组基线期脑血流量与随访期脑血流量比较应用配对样本T检验。所有比较结果得到的脑血流量图均应用AlphaSim校正(基于Monte Carlo模拟算法)进行多重比较校正(单个体素P=0.01,FWHM=6mm,91×109×91mm3全脑mask,校正P<0.05,体素大小>1016mm3)。组间脑血流量及精神运动迟滞组内脑血流量基线期与随访期比较结果显示:与对照组及非精神运动迟滞组相比,精神运动迟滞组右侧运动皮层基线期的脑血流量显著降低,如图2所示,而随访期右侧运动皮层则显著增高如图3所示。精神运动迟滞组基线期右侧运动皮层的脑血流量显著低于随访期如图4所示。右侧运动皮层的脑血流量与SRRS评分呈显著负相关如图2所示。

4.根据权利要求3所述的标记物在抑郁症精神运动迟滞患者诊断中的应用,其特征在于,所述脑血流量降低为抑郁症患者精神运动迟滞的标记。
5.根据权利要求1所述的一种基于脑血流量的抑郁症精神运动迟滞客观诊断标记物,其特征在于,所述脑血流量由功能磁共振检测。
