Description
Technical Field
The invention relates to the technical field of hospital quality management, in particular to a method for establishing a field library of a hospital management statistical index source.
Background
The existing hospital management indexes comprise statistical reporting of national health and fitness committee and evaluation indexes of provinces and cities, part of the existing hospital management indexes are from various reports of hospitals, part of the existing hospital management indexes are from manual statistics, and part of the existing hospital management indexes are from acquisition and calculation of hospital informatization systems. Although the national health statistics survey system, the health statistics working specifications of each province and the instruction manual of each hospital management examination or rating file explain the concept and calculation of the evaluation index, compared with the actual site situation of the traditional Chinese medicine, the concepts are relatively rough, and no specific standard operation guide is given. Various factors influence the quality of the original data of the statistical indexes and the accuracy of calculation from generation to collection of the data of the statistical indexes calculated by each hospital and the calculation of the indexes, and further directly influence the reliability of data analysis conclusions and the realization of information value.
In the actual work of hospitals, or because the definition and the statistical caliber of part of statistical indexes are not clear enough, or because the statistical staff of each hospital understand the deviation and the informatization construction degree of each hospital are inconsistent, it is often seen that the statistical connotation of the same index in different hospitals is inconsistent, so that the difference occurs in the actual data acquisition process, and the data is inaccurate. Even in the same hospital, the same index is calculated by different departments, and the calculation results often have certain difference. The auditing after data acquisition is relatively limited, only inspection can be performed from a logic perspective, the self quality of the data is difficult to check, and the application of the data lacks homogeneity, so that the statistical index for evaluating the actual condition of the hospital cannot be accurately obtained.
How to scientifically, accurately and reliably acquire the raw data required by the hospital management statistical indexes from the complex hospital information system is obviously an important problem. The quality control of the original data acquisition is the guarantee of data accuracy, and the data is accurate and reliable, so that the effect of promoting medical quality management and control work by hospital management statistical indexes can be really exerted.

The operation field comprises a registration assembly line, and the condition field comprises registration time, registration departments, doctor and cost totalization and is used for determining the number of outpatients of the statistical index.
S5: according to the operation field, the condition field, the caliber acquired by the operation field in the hospital information system and the caliber acquired by the condition field in the hospital information system, screening data of the number of times of the statistical index outpatient in a hospital management statistical index source field library and acquiring;

wherein, for the relation between the clear operation field and the condition field, the calculation step of the operation field is established according to the processing strategy; and establishing a calculation step of managing statistical index entries according to the relation between the calculation variables and the operation fields.
This example provides a use case for this method, as follows:
hospital management statistical index entries: number of outpatients.
And (3) normalized definition: the number of patients in the clinic visit is counted during the period.
Connotation: including initial diagnosis, follow-up diagnosis, pregnancy examination in outpatient service, postpartum examination, one-way health examination, etc.; the workload of various examinations, treatments (such as skin tests, injections, debridement and suture), immunization and the number of times of health management service persons are not included according to the medical orders; the number of the patient who has returned and registered but has not yet visited the clinic is not counted.
Calculating the formula: the number of outpatients is the number of outpatients registered.
To increase workload, common confounders include: the times of partial physical examination persons, the times of massage physical therapy persons, zero-cost registration and the like, the number of times of no treatment after registration and the like; confounding factors due to the complex scene in the field are: the first multi-department visit, the second multi-department visit, the multi-department visit of different system diseases, the no-number visit, the self-fee medical insurance transfer, the number withdrawal and the like.
And establishing a processing strategy through system analysis.
And (4) counting a calculation formula, an operation field and an aperture of the hospital management type statistical index entry.
The calculation formula after the change is as follows: the number of outpatient times is the sum of the number of outpatient times of 'registration and visit'; the operation field is 'registration pipelining' with uniqueness, and the statistical caliber is an HIS registration table and an on-network reservation registration table.
Confounding factor processing strategy, condition field and caliber;
the processing strategy is as follows: the patients who see a doctor can control various subjective and objective mixed factors seen at present according to the registration time, the registration department, the doctor who receives the doctor and the record of the cost recorded by the hospital information system;
the condition field is: the 'registration time', 'registration department' and 'cost total', the caliber of the operation field collected in the hospital information system is the corresponding field table in the hospital information system;
and according to the total of the operation field registration running water and the condition field registration time, the registration department, the doctor and the cost, and standard interfaces of the operation field registration running water and the condition field registration time, the registration department, the doctor and the cost, which collect the caliber in a hospital system, the collection of the data related to the number of the outpatients is completed.
The current statistical methods are: the system registers tickets, and the statistical method has the following problems, for example: it is clearly specified that the workload of each examination, treatment and treatment according to the medical advice is not within the calculation, but in the implementation process, the workload of a hospital such as a plurality of times of treatment and physical examination in an outpatient department is counted by the hospital and does not meet the requirement.
Compared with the existing method, through the system analysis of data in the hospital information system and the quality control of the confounding factors, the calculated number of times of outpatients avoids the deviation of the value calculation statistical indexes of different personnel or different systems, and the direct scientific, accurate and reliable generation of the hospital management statistical indexes by the system is realized.
The method provided by the embodiment eliminates human intervention in the whole process, avoids errors in calculating the statistical indexes caused by subjective ambiguity and objective systematic complexity, and can obtain accurate and reliable statistical indexes of hospital management.
It should be understood that the above examples are only for clarity of illustration and are not intended to limit the embodiments. Other variations and modifications will be apparent to persons skilled in the art in light of the above description. And are neither required nor exhaustive of all embodiments. And obvious variations or modifications therefrom are within the scope of the invention.
