一氧化氮是哺乳动物中必要的信号分子并且已知其发挥多种作用,范围从调节血液流动、神经传递和免疫反应不等。重要的是,被称为一氧化氮合成酶的特殊的酶家族已进化出在需要一氧化氮时和需要一氧化氮的地方使用氨基酸精氨酸作为前体物质、制造控制量的一氧化氮的专属工作。
然而,一氧化氮是非常疏水的化合物,并且所以它在水中的溶解度是有限的。在正常条件下,可实现的水中最大溶解度是大致约1.7mM,该溶解度与氧气的溶解度近似。
一氧化氮还与氧气迅速反应以形成二氧化氮。二氧化氮不具有在维持或控制内稳态中已知的作用、或对生物系统中重要刺激响应的能力。实际上,二氧化氮被认为是毒素或刺激物。
此外,哺乳动物(和其他脊椎动物)的生命机理由于组织中、尤其是皮肤内丰富的硫醇基而能够安全地管理一氧化氮的物流。一氧化氮与(例如蛋白质上的)硫醇基自发反应以形成S-亚硝基硫醇官能团,以这种形式可以安全且有效地储存或运输一氧化氮。S-亚硝基硫醇是能够释放一氧化氮的化合物。因此,也可以根据需要通过S-亚硝基硫醇的降解容易地释放一氧化氮。
因此,哺乳动物生命机理处理一氧化氮的这些问题,当被锁定为S-亚硝基硫醇时,它不能被氧化为二氧化氮,并且其在水中的不溶性不是问题。
然而,将外源性未经处理的一氧化氮递送至皮肤部位可能对二氧化氮的产生造成无法接受的损失。此外,任何进入皮肤的一氧化氮都可能在错误的位置被锁定入角蛋白S-亚硝基硫醇,或者一氧化氮无法溶解在目标系统的可用水中。
因此,在可以考虑将有价值的一氧化氮有效递送至皮肤部位之前,需要采用各种策略。
US 6,103,275披露了用于通过将亚硝酸盐、还原剂和特定酸聚集在一起来产生一氧化氮的生物相容性系统。典型地将该亚硝酸盐和酸保持分开,直到使用时。
如所述,S-亚硝基硫醇可通过自发分解来释放游离一氧化氮,并且因此是反应性且不溶的一氧化氮的非常方便的递送方式。然而,亚硝基硫醇自发分解,并且因此作为一氧化氮的递送载体的使用寿命有限。
WO 2006/095193披露了处于非活性状态的皮肤敷料,但可以使其变为活性并将S-亚硝基硫醇递送至皮肤部位。该敷料通过含有亚硝基硫醇的反应物、使其仅在使用前立即形成亚硝基硫醇来保持非活性,或通过将预先形成的亚硝基硫醇保持在干燥状态、以在需要时通过加水活化来保持非活性。
分解的速率在相当大的程度上取决于该硫醇的侧链而变化。例如,虽然亚硝基半胱氨酸在正常条件下在几分钟内可以全部分解,但实现亚硝基谷胱甘肽的100%分解花费几小时/几天。该分解通常在存在Cu2+和Hg2+的情况下加速。
发明内容
本发明通过直接合成S-亚硝基硫醇来模拟自然生物过程,S-亚硝基硫醇可以在不形成二氧化氮的情况下安全地且以水溶性形式将一氧化氮护送到目标位置。凭借其化学性质,S-亚硝基硫醇可以容易地将其一氧化氮与身体的硫醇交换,从而有效地将递送的一氧化氮置于身体内,与身体的一氧化氮物流相协调。
因此,本发明提供一种皮肤应用组合物,其包括包含处于干燥非活性状态的S-亚硝基硫醇的第一组分。
由于S-亚硝基硫醇处于干燥状态,所以该处理组合物处于非活性状态。然而,可以通过以下来活化该处理组合物:在使用前立即使水性组合物与干燥的S-亚硝基硫醇接触,以允许将S-亚硝基硫醇活性递送至待处理的皮肤部位。
干燥状态意指在第一组分中没有游离水,这样使得不会在正常的温度、压力和湿度环境条件下通过蒸发发生显著或可测量的水损失。干燥状态包括干枯的(desiccated)状态,其是极其充分的干燥状态。干枯的状态意指通过储存在由不透水分屏障封闭的环境中而保持的状态,其中通过添加的干燥剂将材料保持严格无水。

优选地,该皮肤处理组合物是皮肤敷料。术语“皮肤敷料”涵盖如贴片、膏药、绷带、吸收性泡沫和纱布等敷料。该术语还包括呈无定形或液体形式的材料。该术语总体上涵盖用于施用至体表的敷料,包括内部和外部的组织,特别是皮肤(包括头皮)。

第一组分可以作为固体材料提供,干燥的S-亚硝基硫醇优选以合理均匀的方式分散。该固体材料优选包含聚合物材料。
优选的聚合物包括水溶性聚合物,如聚乙烯醇(PVA)、聚乙烯吡咯烷酮、纤维素或改性纤维素(如羧甲基纤维素)。一种优选的聚合物材料包含PVA。PVA具有对皮肤处理用途而言方便且可接受的特性,例如是无毒的。PVA还易于处理和使用,PVA在水中的溶液干燥时容易形成膜,所得的膜易于处理。PVA还是容易获得且廉价的。不需要交联来形成固体材料,例如呈膜的形式,但是可以任选使用交联。基于影响材料物理特性的分子量和水解度的多种品级的PVA是可获得的。可以容易地选择适当品级的PVA,以生产具有特定预期用途的希望特性的聚合物产品。例如,对于用于皮肤敷料,通过使用分子量在100,000至200,000范围内、基本上完全水解(98%-99%水解)的、例如呈来自奥德里奇公司(Aldrich)的代码36,316-2的形式、从西格玛公司(Sigma)(363138)获得的呈非交联形式的并且Mw31,000-50,000、98%-99%水解的的PVA已获得良好结果。
另一种合适的聚合物材料包含聚乙烯吡咯烷酮(PVP)。PVP的特性与PVA的那些非常相似,并且对于皮肤处理用途而言,PVP也是可接受的。不同分子量范围内的PVP是容易获得的。可以容易地选择适当品级的PVP。例如,使用(例如呈来自西格玛公司的代码PVP360的形式)呈非交联形式的分子量平均值为360,000的PVP获得了良好的结果。
可以使用聚合物材料的混合物。
固体材料方便地呈薄片、层或膜的形式,典型地具有在0.01至1.0mm范围内、优选在0.05至0.5mm范围内的厚度。
固体材料可以任选地包括支撑物以在潮湿时提供刚度。
本发明的固体材料通过将聚合物(例如,PVA和/或PVP的水溶液)和试剂的溶液混合、并且干燥混合物以产生固体材料(例如通过流延程序成膜)来方便地制备。合适的技术是本领域技术人员熟知的,并且可以与以上所述的方法结合。
该一种或多种聚合物材料以导致成膜的适当的量合适地使用,浓度的上限典型地由(通常在水中的)溶解度的极限值决定并且浓度的下限是不形成膜的点。对于来自奥德里奇公司的PVA编码36,316-2,在水中溶解度的极限值是约6%w/w,其结果是在干燥前在膜中约5%的PVA浓度。
此类固体聚合物材料典型地处于干燥状态,并且因此可以在可以提供水源的渗出性伤口上使用。然而,优选地此类固体聚合物材料将具有如以下所讨论的包含水源的第二组分,并且因此还可以用于干燥伤口上。

此类网片或泡沫可以通过施用S-亚硝基硫醇的水溶液(例如通过浸渍、喷洒等)来制备,这允许其被吸收到网片或泡沫的多孔结构中。然后,这之后是干燥步骤,留下粘附到网片或泡沫的结构上的处于干燥状态的S-亚硝基硫醇。
如果施用到渗出性伤口上,网片或泡沫可以开始吸收伤口渗出物以产生从网片或泡沫至伤口部位的液体流动路径。然后,S-亚硝基硫醇可以溶解于伤口渗出物并沿着所产生的浓度梯度朝伤口部位扩散。
可替代地,该第一组分可以包含非水性液体基质,该非水性液体基质含有分散在其中的S-亚硝基硫醇。S-亚硝基硫醇可以以细小微粒材料的形式分散在基质中,或者可以溶解于基质本身中。合适的非水性液体包括丙二醇、聚乙二醇(例如PEG300、PEG400、PEG3350),并且可以包括非水性霜剂、软膏剂或洗剂。
此类非水性液体可施用于渗出性伤口。以与网片和泡沫类似的方式,可以由伤口渗出物提供能够使S-亚硝基硫醇释放的水。
可替代地,第一组分可以以微粒材料如粉末的形式或以粒状形式提供。在这种布置中,第一组分将通常包括载体或填充剂如糖或多元醇。
第一组分还可以包含能够螯合二价金属离子如Cu2+、Zn2+和/或Fe2+的螯合剂。已知这些金属离子催化S-亚硝基硫醇分解成一氧化氮,并且因此螯合剂确保S-亚硝基硫醇保持非活性状态直到希望使用。合适的螯合剂包括EDTA、EGTA、组氨酸和/或柠檬酸盐。
第一组分还可以包含缓冲剂,以确保当与水接触时任何所得溶液的pH是在从4至7、优选从5至7的范围内。
通过使第一组分与(例如来自伤口的)水源接触、导致一种或多种S-亚硝基硫醇从处理组合物中释放来活化该组合物。
因此可以简单地通过将该层施用到潮湿的表面(例如创面)上来从该组合物中释放S-亚硝基硫醇。一旦释放,S-亚硝基硫醇经历缓慢分解以产生一氧化氮。S-亚硝基硫醇从该层的释放可以是相对快速的。这可以通过将该层施用于潮湿的皮肤上来证明,由于真皮血管舒张,其导致皮肤快速(在大致1min内)变红。变红完全可逆并且在去除组合物后在若干分钟内消失。
可替代地,处理组合物可以包括包含水源(水性组分)的第二组分,其与处于干燥状态的S-亚硝基硫醇分开。
水性组分可以是水合水凝胶。水合水凝胶意指一种或多种处于水合形式的基于水的或水性的凝胶。因此,水合水凝胶包括水源,用于该处理组合物的活化。水合水凝胶还可发挥作用来吸收水以及从伤口部位中渗出的其他材料,使得该处理组合物能够通过将此类材料从伤口部位去除来发挥有价值的并且有用的功能。该水合水凝胶还提供了水分源,其可在使用中发挥作用以保持伤口部位的潮湿,从而辅助愈合。
在WO 03/090800中披露了合适的水合水凝胶。该水合水凝胶合宜地包含亲水聚合物材料。合适的亲水聚合物材料包括聚丙烯酸酯和甲基丙烯酸酯,例如由第一沃特有限公司(First Water Ltd)以专有的水凝胶的形式提供,包括聚2-丙烯酰胺基-2-甲基丙烷磺酸(聚-AMPS)和/或其盐类(例如,在WO 01/96422中所描述);多糖,例如多糖胶,特别是黄原胶(例如,以商标Keltrol可获得的);各种糖类;多元羧酸(例如,从ISP Europe以商标GantrezAN-169BF可获得的);聚(甲基乙烯醚共聚-马来酸酐)(例如,以商标Gantrez AN 139可获得的,其分子量在20,000至40,000的范围内);聚乙烯吡咯烷酮(例如,以可商购品级的形式被称为PVP K-30和PVP K-90);聚氧化乙烯(例如,以商标Polyox WSR-301可获得的);聚乙烯醇(例如,以商标Elvanol可获得的);交联的聚丙烯酸类聚合物(例如,以商标Carbopol EZ-1可获得的);纤维素和改性纤维素,包括羟丙基纤维素(例如,以商标Klucel EEF可获得的)、羧甲基纤维素钠(例如,以商标Cellulose Gum 7LF可获得的)和羟乙基纤维素(例如,以商标Natrosol 250LR可获得的)。
可以在凝胶中使用亲水聚合物材料的混合物。
在亲水聚合物材料的水合水凝胶中,所希望的是该亲水聚合物材料以基于该凝胶总重量按重量计至少1%、优选至少2%、更优选至少5%、又更优选至少10%、或至少20%、所希望的是至少25%并且甚至更希望的是至少30%的浓度存在。可以使用基于该凝胶的总重量的甚至更高的量,按重量计最高达约40%。
以该凝胶总重量的按重量计约30%的量使用聚-AMPS和/或其盐的水合水凝胶已经获得了良好的结果。
通过使用包含相对高的浓度(按重量计至少2%)的亲水聚合物材料的凝胶,该凝胶可以特别有效地发挥作用以便在该处理组合物的使用中吸收水,例如在与伤口接触时从血清渗出物中吸收水。因为该凝胶是水性体系,所以该处理组合物的使用不具有诱发不希望的伤口整体干燥的作用。这是因为在使用该处理组合物时在围绕该皮肤的封闭环境中保持水蒸气压力。因此,该凝胶作为用于去除水分(例如,伤口渗出物)的吸收剂实体而发挥作用,它还提供有帮助的背景水平的过量水分。
水合水凝胶(包括高浓度的凝胶)的吸水能力使该处理组合物能够通过去除大量的渗出物来辅助伤口愈合,而它在这样做时发生膨胀。通过使用仔细配制的、可即刻水合的凝胶,防止了伤口出现无益的干燥状态。即刻水合还确保在处理组合物与伤口之间快速形成水性液体界面,由此而防止粘附,否则当必须被替换该处理组合物时它会妨碍将该处理组合物容易地揭开。伤口与处理组合物之间良好的水性液体界面在允许凝胶中所携带的任何有益的产品穿过可供使用的所有表面进入该伤口方面也是重要的。
该水合水凝胶材料典型地处于固体层、薄片或膜材料的形式,这种材料典型地是交联的,并且它可以掺入机械加强结构。可对该层、薄片或膜的大小和形状进行选择以适应该处理组合物的预期用途。在0.05至5mm的范围内、优选0.5至3mm的厚度是特别合适的。此类凝胶的实例包括ActiFormCoolTM系列(L&R公司)和IntrasiteTM系列(施乐辉公司)。
可替代地,该水合水凝胶可以处于不具有固定形式或形状的无定形凝胶的形式,它可以在三维上变形和成形,包括通过喷嘴进行挤压。无定形凝胶典型地不是交联的或具有低水平的交联。可以使用剪切稀化的无定形凝胶。这种凝胶在经受剪切应力时(例如,当被倾泻或通过喷嘴挤压时)是液体,但在静止时凝固。因此,该凝胶可处于可倾泻或可挤压的组分的形式,该组分可以例如从可压缩的管或注射器样的分配器进行分配,该分配器包括活塞和气缸,典型地具有约3mm直径的喷嘴。这种凝胶可以以表面层的形式进行施用,或作为完全适形的凝胶被施用至伤口腔中,它填充了可供使用的空间并接触到伤口表面。此类凝胶的实例包括PurilonTM系列(康乐保公司)、Nu-GelTM系列(希丝塔杰尼斯公司)、GranugelTM系列(康维德公司)和IntrasiteTM系列(施乐辉公司(Smith and Nephew))。
无定形凝胶配方的典型实例是:15%w/w AMPS(钠盐)、0.19%的聚乙二醇二丙烯酸酯以及0.01%的羟基环己基苯基酮,用分析级的DI水将体积补足至100%。将这些试剂充分混合并溶解,然后使用输送大致100mW/cm2的UV-A灯聚合30秒与60秒之间,以形成所要求的水凝胶。这可容纳在塑料注射器中,然后可以将该无定形凝胶从该注射器分配到靶位点,作为表面层或以填充腔。
第二组分可以可替代地包含在合适的容器(如盒或可挤压袋或管)中携带的保湿霜剂。
第二组分还可以包含缓冲剂,以确保任何所得溶液的pH是在从4至7、优选从5至7的范围内。
第二组分还可以包含如Cu2+、Zn2+和/或Fe2+的二价金属离子源。已知这些金属离子催化S-亚硝基硫醇分解成一氧化氮,并且因此在第二组合物中提供它们可以在使第一和第二组合物放置在一起时辅助一氧化氮的递送。
当第一组合物包括螯合剂时,优选第二组分中二价金属离子的浓度超过螯合剂螯合二价金属离子的能力。这是因此,当聚集在一起时,第一组分中的任何螯合剂将能够仅螯合第二组分中的一部分二价金属离子。过量的二价金属离子可以然后发挥作用以催化S-亚硝基硫醇分解成一氧化氮。
因此,在一个优选实施例中,本发明包括包含含有S-亚硝基硫醇的干燥聚合物基质、优选干燥PVA的第一组分以及包含一层水合水凝胶的第二组分。该第二组分可以与皮肤相接触而进行使用,因为该水合水凝胶如以上所讨论具有对皮肤接触有益的特性,而该第一组分则置于该第二组分之上。前提是将这些组分在使用前保持分开,该处理组合物保持在未活化状态。然而,当将这两种组分进行接触时,这具有活化该处理组合物的作用。
在这种类型的一个优选实施例中,该处理组合物包含一层含有预产生的S-亚硝基硫醇的干燥PVA。该层由PVA、亚硝酸盐源(优选亚硝酸钾)和硫醇(优选L-谷胱甘肽)形成。在此实施例的典型实例中,在干燥前将这些添加剂中的两者添加至PVA溶液中。在干燥步骤期间,在该层内产生S-亚硝基硫醇(在优选情况下GSNO)。已知亚硝基硫醇在水溶液中相当不稳定。但是,发现GSNO在PVA的干燥层中非常稳定,尤其如果储存在无水分的氛围中。
在另一个优选的实施例中,该处理组合物包括无定形的组分。对于S-亚硝基硫醇分散在非水性液体基质中的优选实施例,这尤其如此。这些无定形组分可以处于例如凝胶、半固体、糊剂、霜剂、洗剂或液体的形式。这种无定形组分可以自行提供,并从伤口渗出物本身获得所需的水。可替代地,如以上所讨论的,水可以由水合水凝胶或其他无定形材料提供。
将这两种无定形的组分分开保存,直到希望将该处理组合物施用至体表。合宜的是将它们装入具有喷嘴的容器中,这些无定形的组分可通过该喷嘴被递送。优选地,将这两种组分装入两室分配器中,优选是可操作的以便同时递送两种组分。
另一个优选实施例是使用包含处于干燥状态的S-亚硝基硫醇的网片或泡沫。可吸水的此类泡沫可以用于渗出性伤口以从伤口获得水。可替代地,此类泡沫可以具有如以上所讨论的水源,或者在使用前立即润湿。
该处理组合物任选地包括覆盖层或外层,或与该覆盖层或外层一起使用,用于按已知的方式使敷料粘附在人类或动物的皮肤上。
根据本发明的处理组合物可被制造成各种不同大小和形状用于处理皮肤区域,例如不同大小和形状的伤口。针对具体敷料的合适的试剂量可以通过实验容易地确定。
在使用之前将处理组合物组分适当地储存在无菌的、密封的、不透水的包装中,例如叠层的铝箔包装。为了确保保持干燥状态,希望将干燥剂材料包括在包装中。
在使用中,将这种或这些处理组合物组分从它们的包装中移出,并以适当的顺序定位在人类或动物的皮肤上,例如,出于美容或治疗的目的置于伤口或其他有待处理的皮肤区域上。如以上所指出的,该处理组合物还可以用作用于经皮递送的辅助剂。
实例
为了证明S-亚硝基硫醇的释放速率,准备了许多模型系统。下面每个结果部分开始处的表格中详细介绍了这些系统。对于每个系统,在系统‘活化’(即所有组分放置在一起)后的三个时间点从水性组分中提取样品等份,以确认S-亚硝基硫醇释放的存在。第一个时间点始终为t=零(即,在用干燥/非水性组分‘活化’之前,测量水性组分中的S-亚硝基硫醇,以证明在开始时没有S-亚硝基硫醇,在所有系统中都是如此),然后t=活化后2小时,并且最后t=活化后6小时。

PVA储备溶液制造程序
测量出462.5ml的DI水,并将其在热板上加热至80℃-85℃之间的恒定温度(用数字温度计控制)。测量出37.5g PVA粉末,并将其分为5x7.5g等份。将单一等份的PVA粉末添加至正在搅拌(防止PVA凝固)的加热的水中。在整个添加过程中,将水/PVA温度保持在80℃-85℃。在保持水/PVA混合物的温度的同时重复添加,直到PVA溶解。在从热板上移除并冷却后,用DI水将最终体积补充至500ml。
PVA膜制造
通过将PVA储备溶液与活性组分混合并允许混合物在40℃下在陪替氏培养皿(Petri plate)中干燥来产生PVA膜。将20ml的每个包含活性组分的预制备的PVA溶液倾倒入10×10cm陪替氏培养皿中,并将其在40℃恒温箱中干燥过夜。在干燥前PVA混合物的组成在表1中示出。在形成PVA膜之前,允许谷胱甘肽和亚硝酸盐一起反应以形成S-亚硝基硫醇。
随后使用包含S-亚硝基硫醇的PVA膜来跟踪与水性体系接触后的S-亚硝基硫醇释放的速率。

粉末制造
通过将谷胱甘肽和亚硝酸钠在水性山梨糖醇背景中混合、允许谷胱甘肽和亚硝酸钠反应并形成S-亚硝基硫醇、随后干燥混合物来生产包含S-亚硝基硫醇的粉末。对于每种粉末,将20ml的每种预制备的溶液倾倒入10×10cm陪替氏培养皿中,并使其在40℃恒温箱中脱水24小时,之后充分干燥。一旦呈粉末形式,将配制品分散在纯净丙二醇(0.1g的粉末对1ml丙二醇)中。
在干燥前用于粉末制备的水性混合物的组成在表2中示出。
随后使用悬浮在非水性丙二醇中的包含S-亚硝基硫醇的粉末来跟踪与水性体系接触后的S-亚硝基硫醇释放的速率。

浸渍泡沫制造
通过允许泡沫吸收包含谷胱甘肽和亚硝酸钠的水溶液、允许谷胱甘肽和亚硝酸钠反应并形成S-亚硝基硫醇、随后在40℃下充分干燥来生产包含S-亚硝基硫醇的浸渍泡沫。
使用ActivHealTM(Advanced Medical Solutions公司)泡沫。
在干燥前用于浸渍泡沫制品的水性混合物的组成在表3中示出。
随后使用包含S-亚硝基硫醇的浸渍泡沫来跟踪与水性体系接触后的S-亚硝基硫醇释放的速率。

对于浸渍泡沫生产,将20ml的每种预制备的组分溶液倾倒入10x10cm陪替氏培养皿中。将泡沫敷料的4x4cm2片放置于溶液中,并且允许该溶液在4小时内渗透入敷料中。然后将泡沫敷料片移出并在40℃的恒温箱中干燥过夜。
水性组分
预制备的PVA膜/粉末/浸渍泡沫需要与水性组分接触以活化S-亚硝基硫醇产生。所使用的水性组分在表4中示出。

S-亚硝基硫醇测量
通过使用以下描述的格里斯(Griess)试剂法由在490nm处的吸光度读数来测量S-亚硝基硫醇的存在。S-亚硝基硫醇浓度可以由使用约10,000M-1cm-1的消光系数的吸光度测量计算。吸光度测量使用FisherbrandTM数字色度计模型45进行。
取决于水凝胶(AQ1至AQ4)或溶液(AQ5和AQ6)用作水性组分,需要两种不同方法来测量S-亚硝基硫醇浓度。
用于S-亚硝基硫醇测量的试剂
试剂1:磷酸钠缓冲剂(pH 7.4,0.1M)。
试剂2:格里斯试剂:20mg N-(1-萘基)乙二胺二盐酸盐(NADD)+溶解于2mL DMSO中的500mg磺胺。
试剂3:DMSO中的氯化汞(10mM)(5mL DMSO中13.58mg HgCl2)。
测量凝胶中S-亚硝基硫醇浓度的程序
1.将25mL的试剂1和825μL的试剂2分配至250ml的罐中
2.精确称量300mg的水凝胶并将其浸没在试剂混合物中。在温和振荡时,培育30min。
3.将2.6ml的试剂混合物从罐转移到塑料比色皿中
4.添加25μL的试剂3
5.在10min内读出在490nm处所得混合物的吸光度
测量溶液中S-亚硝基硫醇浓度的程序
1.将1.5mL的试剂1分配至塑料比色皿中
2.添加200μL的样品
3.添加1.17μL的DI水
4.添加100μL的试剂2
5.添加30μL的试剂3并充分混合
6.在10min内读出在490nm处所得混合物的吸光度
结果

7.根据权利要求1或权利要求2所述的皮肤应用组合物,其中,该第一组分是多孔的可吸水性材料。
8.根据权利要求1或权利要求2所述的皮肤应用组合物,其中,该第一组分包含含有分散在其中的该S-亚硝基硫醇的非水性液体基质。
9.根据前述权利要求中任一项所述的皮肤应用组合物,其中,该第一组分包含能够螯合二价金属离子的螯合剂。
