具体实施方式
以下通过具体实施方式结合附图对本发明的技术方案进行进一步的说明和描述。
本发明从三名弱精子症患者、两个近亲家族的兄弟姐妹携带纯合突变和一名携带复合杂合突变的散发患者中鉴定了ARMC12中的双等位基因突变。这些患者的精子在线粒体鞘中存在多种缺陷。此外,Armc12基因敲除小鼠在线粒体鞘和弱精子症中表现出平行缺陷。这些结果证明ARMC12缺陷是人和小鼠线粒体鞘缺陷和弱精子症的新发病机制。
本发明的研究对象的获得具体为:125名患有弱精子症的原发性不育患者参加了为本发明所进行遗传分析,120名具有生育能力的男性作为对照受试者。参与者被排除在以下可能的致病因素之外:年龄因素、恶性肿瘤、睾丸损伤、男性副腺感染、隐睾、精索静脉曲张、内分泌功能不全、输精管和射精管异常、精浆异常、核型异常、染色体易位,或Y染色体微缺失等。常规分析严格按照世界卫生组织人类精液检查和处理实验室手册(第5版)的指导方针进行。本发明获取5mL外周血用于生殖激素和遗传分析。从每位参与者那里均获得了书面知情同意书。本发明是根据1964年赫尔辛基宣言及其后来的修正案或类似的道德标准进行的。本发明得到了青岛大学附属烟台毓璜顶医院和厦门大学附属妇女儿童医院的伦理委员会的批准。
实施例1
全外显子组测序和Sanger测序:使用DNeasy Blood and Tissue Kit(Qiagen,Düsseldorf,North Rhine-Westphalia,Germany)提取外周血中的基因组DNA,并通过TruSeqExome Enrichment kit(Illumina,San Diego,California,USA)富集,接着于药明康德(中国上海)根据制造商的协议进行二代测序。通过Burrows-Wheeler Aligner(BWA)软件将所获得的数据与人类参考序列(GRCh37-hg19)进行比对。用Picard软件去除PCR重复并评估变异的质量。然后,使用ANNOVAR软件进行功能注释。满足以下标准的变异被保留用于后续分析:(1)不存在或罕见的变异;(2)无义、移码、剪接位点或错义变异。符合以上筛选标准的睾丸特异性基因,尤其是那些在精子发生过程中动态表达的基因,被优先考虑。最后进行Sanger测序以验证患者及其家属的变异。
本实施例确定了三名携带ARMC12双等位基因突变的患者。具体来说,如图1A所示,本实施例在两个有血缘关系的患者家庭(R0036/II-1(P1)和R0036/II-3(P2))中筛选出ARMC12的纯合突变c.635T>C(p.Leu212Pro),并在非血缘关系的患者家庭W0031/II-1(P3)中筛选出ARMC12的复合杂合突变c.632G>A(p.Arg211Gln)及c.686G>A(p.Cys229Tyr)。对这些患者及其父母进行Sanger测序以确认这些变异。如图1B所示,在受试者P1和P2中验证了纯合突变c.635T>C(p.Leu212Pro)。他们的父亲和母亲携带杂合子.635T>C(p.Leu212Pro)变异;而复合杂合突变c.632G>A(p.Arg211Gln)和c.686G>A(p.Cys229Tyr)在P3中得到进一步证实,其父亲携带杂合c.686G>A(p.Cys229Tyr)变异,而他的母亲携带c.632G>A(p.Arg211Gln)变异。这些数据表明双等位基因ARMC12突变以孟德尔方式在表型和家族中分离,并且ARMC12缺陷引起的弱精子症符合常染色体隐性遗传。
Polyphen-2HDIV、VEST3、CADD和DANN预测的生物信息学分析结果表明这些变异是高度有害的。此外,gnomAD中的数据显示这些变异c.632G>A、c.635T>C和c.686G>A在人群中出现的频率非常低(表1)。

所有这些变异都位于ARMC12(NM_001286574.2)的第五个外显子上(图1C)。c.632G>A(p.Arg211Gln)和c.635T>C(p.Leu212Pro)分别导致ARM_2结构域的第211位和第212位氨基酸被取代。c.686G>A(p.Cys229Tyr)则导致ARM_2结构域和ARM_3结构域之间第229位氨基酸的替换(图1D)。
实施例2
蛋白质结构预测:ARMC12蛋白(NP_001273503.1)的结构由SWISS-MODEL在线数据库(https://swissmodel.expasy.org)预测,ARMC12及其突变蛋白的结构通过UCSFChimera(版本:1.15)可视化生成。
本实施例将从人到哈里斯嗜肉菌的ARMC12的氨基酸序列进行比对,发现受c.632G>A、c.635T>C和c.686G>A变异影响的氨基酸在这些物种中高度保守(图6)。此外,本实施例使用SWISS-MODEL构建了突变的蛋白质结构,发现这些变异影响了ARMC12的三维结构(图2)。与原来的ARMC12氨基酸(图2A)相比,三个突变氨基酸的侧链发生了显着变化(图2B)。c.635T>C(p.Leu212Pro)将212位的氨基酸侧链从脂肪酸变为亚胺酸(图2C)。c.632G>A(p.Arg211Gln)将211位氨基酸侧链从带正电荷的碱性氨基酸变为带负电荷的酸性氨基酸(图2D)。c.686G>A(p.Cys229Tyr)将229位的氨基酸侧链从亲水性含硫氨基酸变为疏水性芳香族氨基酸(图2E)。这些变化将显著改变ARMC12的近位位阻和三维结构,进而影响其稳定性和功能。
实施例3
透射电子显微镜观察:透射电子显微镜(TEM)观察在厦门大学生物医学科学核心设施进行。将准备好的精子进行洗涤并收集,然后固定在2.5%戊二醛中。样品用0.1mol/L磷酸盐缓冲液洗涤3次后,浸入1%四氧化锇中,接着经梯度乙醇脱水,丙酮和SPI-Chem树脂浸润。之后将样品包埋在Epon 812中,用超薄切片机切片,并用醋酸双氧铀和柠檬酸铅染色。通过TEM(JEM-1400,Jeol,Japan)观察精子的超微结构。
临床数据表明,ARMC12突变的患者身体和生殖发育正常,血清激素在正常范围内。值得注意的是,精液分析显示,3名患者的精液量和精子浓度均正常,但精子的进行性运动能力(PR)和非进行性运动能力(NP)显着降低(表2)。巴氏涂片显示,与来自对照受试者的完整精子中段相比,患者精子中段存在多种异常,例如缺失、散在或分叉的线粒体鞘(图3A)。本实施例TEM的结果进一步证明对照的精子具有完整的线粒体鞘,而来自ARMC12突变患者的精子中存在缺失、分散或分叉的线粒体鞘(图3B)。精子中段的横截面进一步证实了线粒体鞘中的这些异常。此外,本实施例还观察到患者精子中段的CP缺失和ODF排列紊乱(图3C)。

免疫荧光染色:将精子样本涂在载玻片上并在室温下干燥。然后将精子用4%多聚甲醛固定并用0.2%Triton X-100进行透明。在与5%BSA孵育后,将样品与一抗在4℃下孵育过夜。接下来,洗涤样品并用二抗孵育。然后用DAPI(H-1200,Vector Laboratories,Burlingame,CA,USA)进行细胞核染色,并通过共聚焦显微镜LSM 780(Zeiss,Ostalbkreis,Baden-Württemberg,Germany)观察荧光图像。
对于动物组织,将新鲜睾丸和附睾于4℃用4%多聚甲醛固定,经分级乙醇脱水,二甲苯透明,石蜡包埋,制成5μm切片贴于预包被0.1%聚L-赖氨酸的载玻片上。接着将该载玻片脱蜡后再水化,用柠檬酸钠缓冲液修复,然后在5%BSA中封闭。接着将这些载玻片与一抗在4℃下孵育过夜,然后用含有0.1%Tween20(v/v)的PBS洗涤,然后在室温下与二抗孵育1h。用DAPI染色后,经LSM 780共聚焦显微镜(Zeiss,Ostalbkreis,Baden-Württemberg,Germany)获得图像。
本实施例进一步通过免疫荧光染色研究了人类睾丸中ARMC12的表达,发现ARMC12主要表达在伸长精子细胞的中段,而这些伸长的精子细胞则出现在梗阻性无精子症患者的睾丸组织切片中(图7)。此外,Cox IV(一种线粒体标记物)的表达沿着对照受试者的精子上的线粒体鞘特异性定位,但它在来自P1的精子上呈现出不规则的斑点分布(图4A)。值得注意的是,AKAP4(纤维鞘的指标)的表达和位置在来自对照受试者和来自P1的精子之间没有显著差异(图4B)。
实施例4
使用CRISPR-Cas9建立小鼠模型:根据在线网站http://crispr.mit.edu/设计向导RNA(gRNA)的序列,选择评分最高的两个gRNA分别敲除Armc12的外显子2和外显子3。该两个gRNA序列如下:gRNA-1:5’-gagtttccaactaggtcgtgggg-3’(SEQ ID NO.01);gRNA-2:5′-aagccgagtttgtaacccggagg-3′(SEQ ID NO.02)。本实施例中的所有小鼠均在厦门大学实验动物中心标准条件下饲养,所有小鼠实验均经厦门大学研究机构动物福利委员会批准。
繁殖试验:预备五只10周大的野生型和五只10周大的Armc12ko/ko雄性小鼠用于繁殖试验。每只小鼠与10周龄的野生型雌性小鼠交配过夜,第二天早上检测阴道塞以鉴定雌性小鼠是否怀孕。将带有阴道塞的雌性小鼠转移到另一个笼子中,并在标准分娩时间之前测试这些雌性小鼠的生育能力。记录所有的出生日期和幼仔数量。实验中,每只雄性小鼠与一只野生型雌性小鼠至少交配六次,两次繁殖试验的间隔时间大于一周。
为了进一步验证人类ARMC12和线粒体鞘缺陷之间的关系,本实施例通过使用CRISPR-Cas9技术生成了缺乏外显子2和外显子3的Armc12基因敲除小鼠(图8A)。测序结果显示,在Armc12ko/ko小鼠中,包括外显子2和外显子3在内的1228bp碱基被删除(图8B)。正如预期的那样,Armc12ko/ko小鼠在连续繁殖实验中是无菌的。与对照小鼠相比,Armc12ko/ko小鼠在睾丸(图9A)或附睾(图9B)中没有显示出明显的异常。此外,本实施例在Armc12ko/ko小鼠的睾丸中观察到正常的精子发生(图9C)。
与实施例3中人类表型一致,巴氏涂片显示来自对照受试者的精子显示出完整且紧密堆积的线粒体鞘(图5A,如*所示),而来自Armc12ko/ko小鼠的精子显示存在多个异常的中段,例如松散排列的线粒体鞘(图5A,如&所示)、分叉式线粒体鞘(图5A,如#所示)或弯曲的线粒体鞘(图5A,如@所示)。Armc12ko/ko小鼠的精子的纵切面进一步证实了线粒体鞘的无序排列(图5B)。此外,TEM的横截面表明来自Armc12ko/ko小鼠的精子的线粒体非常松散,间隙太大,无法形成完整的线粒体鞘(图5C)。
实施例5
胞浆内单精子注射:在进行胞浆内单精子注射(ICSI)后计算以上受试对象的受精率,其中受精是通过两个极体和两个原核的出现来确定的。所得胚胎在G-SERIES培养基(Vitrolife,Goteborg,Sweden)中培养。将第3天胚胎移植到受试对象的配偶的子宫中,受试对象P1的配偶获得一个优质胚胎,受试对象P3的配偶获得两个优质胚胎。最后通过分娩健康婴儿来评估临床结果。
具体来说,对于P1夫妇,共采集MII期卵母细胞20个,显微注射P1.18卵母细胞的精子,成功受精,且最佳胚胎得到移植。对于P3夫妇,在MII阶段共采集了12个卵母细胞,并用来自P3的精子进行显微注射,其中11个卵母细胞受精并移植前2个胚胎。所植入胚胎的胚胎发生形态见图10。他们的妻子均怀上单胞胎并成功诞生一个健康的孩子。具体结果如下表3所示:
