Description

Method and system for sorting emergency values of radiology department emergency examination reports
Technical Field
The invention belongs to the technical field of radiology department emergency examination report auditing systems, and particularly relates to a method and a system for sorting emergency values of radiology department emergency examination reports.
Background
The radiology emergency examination report of large-scale third-class hospitals is mostly a double-examination system, which is written by a low-age-capital report doctor firstly, and then issued to patients and clinicians after being examined and modified by a high-age capital hospitalization general doctor on duty on the day. When a reporting doctor finds that emergency examination patients are critically ill, such as cerebral hemorrhage, aortic dissection, pulmonary embolism, esophageal foreign bodies, digestive tract perforation and the like, the general hospitalization doctor needs to be immediately informed to perform report priority examination.
Currently, the communication between the report writer and the resident to the patient depends mainly on the internal communication system in the PACS system. If the doctor reports that the state of illness of a certain patient is critical, the doctor can manually input the personal information and the prior examination reason of the patient in the communication system; after the resident general doctor sees the system information, the patient is inquired and report auditing is carried out in the personal workstation. However, when the system messages are too many, the existing communication method may cause the resident physicians to fail to find effective information in the dialog box in time, which results in the report of the critical patient losing the opportunity of the prior review, and is not favorable for the timely diagnosis and treatment of the critical patient. In addition, relevant national industrial guidelines stipulate that the time for radiology emergency report should be accurate to minutes, and the difference in message delivery time in manual communication can result in prolonged review time for emergency report of critically ill patients.
In view of the above problems, it is desirable to develop a system for sorting the critical values of the emergency examination reports of the radiology department to be examined, so that the emergency examination reports with higher critical values can be automatically and preferentially sent to the general hospitalization physician for preferential examination. Based on the purpose, the Chinese patent application CN201810404112.1 intelligent critical value diagnosis and treatment system provides a method for identifying diagnosis images and reports by using a deep learning method, and further sorting the critical values of the reports. However, the above system still has a certain problem, because it adopts deep learning method, it needs to adopt a large amount of labeled reports for model training in the early stage, and to obtain a model with high accuracy, it needs patient data of various symptoms, and many hospitals do not have such conditions. Therefore, the intelligent critical value diagnosis and treatment system based on the deep learning model has certain difficulties and limitations in practical application. Thus, there remains a need for a more easily implemented method and system for critical value ranking of radiology emergency examination reports.

Drawings
FIG. 1 is a maintenance interface of the contents of the critical value of the inspection item in the system software of embodiment 1;
FIG. 2 is a perspective of an "emergency audit queue" in the system software of example 1;
fig. 3 is a report audit interface in the system software of example 1.
Detailed Description
It should be noted that, in the embodiment, the algorithm of the steps of data acquisition, transmission, storage, processing, etc. which are not specifically described, as well as the hardware structure, circuit connection, etc. which are not specifically described, can be implemented by the contents disclosed in the prior art.
Example 1
The embodiment comprises a system consisting of a report identification module and a report sending module and system software thereof. The report identification module is used for sorting critical values of input radiology department emergency examination reports. And the report sending module is used for sequentially sending the radiology emergency examination reports to a terminal for auditing the radiology emergency examination reports according to the emergency value sequence. Through the system, the examining doctor can preferentially examine the radiology emergency examination report with higher critical value sequence.
The working process of the report identification module is as follows:
1. critical value information acquired on radiology department emergency examination report writing interface
For the imaging structured report of the emergency treatment, keywords such as the cause of the emergency treatment, priority, time threshold value and the like are automatically assigned through filling in options of the report, and the keywords are provided with RADLEX codes, so that the critical value condition can be accurately extracted without an NLP technology.
For traditional text reports, accurate reading is difficult due to the inaccuracy of NLP. Therefore, in the present embodiment, the appropriate part of the following table is directly placed in the report corresponding to the examination item, and the doctor is required to perform forced check.
Overall critical value: □ negative, □ positive
Figure BDA0003430345240000041
In a most preferred embodiment, where the "positive" and "negative" options on the head of the table are not selected, the criticality value is positive for the particular emergency cause selected from the table above. Otherwise, the critical value is default and is judged to be negative.
In the system software of the present embodiment, a maintenance interface for the content of the critical value of the inspection item is shown in fig. 1.
2. Radiology emergency examination report priority logic
And when the emergency examination report of the radiology department is filled and closed, judging whether the critical value condition is prioritized or not. And (4) the critical value is negative, and the treatment is carried out according to the conventional emergency treatment flow (generally conventional queuing treatment) without preferential examination. If the critical value is positive, the review is required prior to the negative radiology emergency examination report. Patients with a prior review flag are directed to a total "emergency review queue" view of the hospital. The system software of the present embodiment has a "emergency audit queue" view as shown in fig. 2.
3. Real-time sequencing logic for' emergency audit queue
The emergency audit queue is ordered in real time as follows.
1) The radiology emergency examination reports are ranked according to their priority from high to low.
2) The end of the queue of all the radiology emergency examination reports of the same priority is added for the newly added radiology emergency examination report according to the priority (the "end" refers to the position in the queue where the audit is sent last in chronological order).
3) Timing the time waiting for examination after submitting each radiology department emergency examination report with the non-highest priority; and when the time waiting for the examination is greater than the time threshold, modifying the priority of the radiology emergency examination report to be the highest level, and changing the sequence of the radiology emergency examination report in the queue in a manner of being placed at the tail of the queue of the radiology emergency examination report with the highest priority.
4. Report review interface optimization
The system software of this embodiment further optimizes the report audit interface, as shown in fig. 3, as follows:
1) the patient list (and its radiology emergency examination reports) is automatically refreshed according to the ranking logic described above, with the most urgent (highest priority and first submitting a radiology emergency examination report) patients always ranked highest.
2) And setting an examination starting button, directly clicking, and opening the examination of the most advanced patient without double clicking from the patient list.
3) After one report is finished, the report does not need to be closed, only the next patient needs to be clicked on the interface, the audit report of the patient is automatically closed under the condition of prompting the closing, the next patient report with the highest priority is automatically opened for auditing, and the observation sequence of the report list does not need to be returned.
As can be seen from the above embodiments, the method and system of the present invention can rank the radiology emergency examination reports according to the criticality value, so that the reports of the emergency patients are preferentially reviewed. The method and the system save the time for manually communicating the illness states of the patients, improve the emergency treatment work efficiency of the radiology department, and strive for time for the diagnosis and treatment of critically ill emergency treatment patients. Therefore, the invention has good application prospect.
