Abstract
本发明公开了一种纳米金属放射增敏生物等效剂量的计算方法,属于肿瘤的放射治疗领域。通过对细胞形态、纳米金属胞内含量与分布的精确测定,建立摄入纳米金属的细胞几何模型;采用细胞几何模型与微观蒙特卡罗方法,计算细胞模型内的微观剂量分布,再与局部效应模型相结合,建立纳米金属增敏后细胞存活曲线的预测模型,并据此计算用于纳米金属增敏生物效应评价的增敏率,进而得到纳米金属放射增敏的相对生物效应(RBE),将RBE与处方剂量相乘得到纳米金属放射增敏生物等效剂量。本发明灵活性、适应性强,采用本发明方法进行一次细胞表征和纳米金属含量测定就可获得单种纳米粒子在多种治疗模式下、多种治疗剂量下的相对生物效应预测模型。

技术领域
本发明涉及肿瘤的放射治疗领域,具体涉及一种纳米金属放射增敏生物等效剂量计算方法。
背景技术
近年来,全球肿瘤发病和死亡人数逐年提高,已严重威胁到人民的生命与健康。放射治疗是癌症治疗的三大手段之一,约60-70%的恶性肿瘤患者需要接受放疗。纳米金属放射增敏(Metallic nanoparticle radiosensitisation,MNPR)是利用摄入肿瘤组织的纳米金属材料提高放疗对恶性肿瘤局部控制率的有效临床手段,已成为当今癌症治疗领域的重要研究与发展方向。生物实验与临床试验结果均表明纳米金属增敏能够有效提高肿瘤的局部控制率并保护正常组织,有重要的临床应用价值。最新研究认为,纳米金属放射增敏是物理、化学和生物学过程综合影响下的复杂效应。在物理层面,由于射线与高原子序数的金属的反应截面显著高于生物组织,摄入纳米金属的肿瘤部位的次级电子产量与微观剂量沉积得到提升,造成DNA直接损伤增强。在化学层面,次级电子所激发的自由基产量提升导致DNA间接损伤增强。在生物学效应层面,有氧化应激、细胞周期阻滞和DNA修复抑制效应发挥作用。
在常规放射治疗中,通常由医生给出肿瘤靶区的处方剂量和周围危及器官的剂量限值,再使用治疗计划系统进行治疗计划设计,计算三维剂量分布,进行肿瘤和危及器官的剂量评价,评价通过后再执行放射治疗。对摄入纳米金属的病人施行放射治疗时,放射增敏会增强纳米金属沉积部位的放射生物效应,因此需要获取肿瘤和危及器官的生物等效剂量,以正确设计处方剂量和评价治疗计划。
目前,仅有钆、铪等少数几种纳米金属用于肿瘤放射治疗增敏的临床试验。在临床使用中,目前没有建立放射增敏生物剂量的计算模型,不能预测治疗的生物等效剂量,只是通过细胞和动物实验来估算治疗处方。如需采用细胞照射实验或动物照射实验建立相对生物效应预测模型,需要针对每种治疗模式和治疗剂量设计一系列的生物辐照实验,操作复杂且容易受到人工因素影响。
发明内容
本发明的目的在于解决现有技术存在的问题,提供一种纳米金属放射增敏生物等效剂量的计算方法。
使用千伏级光子源的放射治疗(如使用铱-192、碘-125等放射源的近距离放射治疗、低能X射线术中放疗、X射线小动物放射研究平台等),物理效应在放射增敏效应中发挥主导作用,因此可以通过研究微观物理剂量分布和放射增敏生物效应之间的关系,建立放射增敏生物等效剂量的计算方法。本发明所提供的方法基于预先展开的细胞表征和细胞照射实验结果,使用蒙特卡罗(Monte Carlo,MC)方法和局部效应模型(Local effect model,LEM)方法建模计算纳米金属放射增敏的相对生物效应(Relative biologicaleffectiveness,RBE),进而计算纳米金属放射增敏生物有效剂量。
本发明的目的通过下述技术方案实现:
一种纳米金属放射增敏生物等效剂量的计算方法,包括以下步骤:
(1)将肿瘤细胞在含纳米金属颗粒的细胞培养基中培养,使肿瘤细胞充分摄入纳米金属颗粒。
所述的纳米金属颗粒包括钆、铪、金、铂、银、铁等的纳米颗粒,粒径优选为1~100nm。
(2)测量单个肿瘤细胞内的平均纳米金属含量;观测纳米金属在肿瘤细胞(细胞核和细胞质基质)内的分布情况,并获得细胞与细胞核的几何形态和尺寸。
所述的单个肿瘤细胞内的平均纳米金属含量优选通过电感耦合等离子体质谱仪(Inductively coupled plasma mass spectrometry,ICP-MS)或核磁成像仪测量;所述的纳米金属在肿瘤细胞内的分布情况通过使用透射电镜观测,所述的细胞与细胞核的几何形态和尺寸根据透射电镜观测结果设置。
(3)结合步骤(2)的观测数据,建立肿瘤细胞的几何模型,设置模型材料。按照步骤(2)观测到的纳米金属在细胞核和细胞质基质内的分布情况,将含金属结构(细胞核或细胞质基质)的模型材料设置为金属和水的均匀混合物,或按照结构(细胞核或细胞质基质)的成分设置为金属和有机物的均匀混合物;不含金属结构(细胞核或细胞质基质)的模型材料设置为水或者有机物。
优选的,含金属结构的模型材料设置为金属和水的均匀混合物,不含金属结构的模型材料设置为水。
(4)使用微观蒙特卡罗(Monte Carlo,MC)方法,模拟放射线照射步骤(2)所建立的含纳米金属的肿瘤细胞模型的过程,记录细胞核内的微观剂量分布DNP(x,y,z)。由于纳米金属的存在,细胞内的剂量分布不均匀。
使用相同的射线模拟照射其他参数相同、但不含纳米金属的肿瘤细胞模型,再次记录细胞核内的微观剂量分布Dcontrol(x,y,z)。含纳米金属的肿瘤细胞模型细胞核内的微观剂量分布DNP(x,y,z)除以不含纳米金属的肿瘤细胞模型细胞核内的微观剂量分布Dcontrol(x,y,z),得到细胞核内剂量增强比(Dose enhancement ratio,DER)的分布DER(x,y,z)。
由于细胞核内包含染色质,是放射损伤的关键靶,因此计算生物效应时仅考虑细胞核内的剂量增强。
所述的放射线由千伏级光子源产生,包括但不限于使用铱-192、碘-125等放射源的近距离放射治疗、低能X射线术中放疗、X射线小动物放射研究平台等。
(5)基于局部效应模型(Local effect model,LEM)建立计算方法,依据细胞核内的DER分布预测纳米金属放射增敏的增敏率(Sensitizer enhancement ratio,SER),进而获得纳米金属放射增敏的相对生物效应(Relative biological effectiveness,RBE)。具体计算方法如下:

其中,DP是处方剂量,DER(x,y,z)是细胞核内任一点的DER(x,y,z-直角坐标系中的坐标位置,表示某一点),D(x,y,z)是细胞核内任一点的增强剂量。α和β是肿瘤细胞的线性二次(Linear Quadratic,LQ)模型参数,每种肿瘤细胞的α和β值各不相同,可以在文献中查找或通过相应的细胞实验获得。分数线下的VN代表细胞核的体积,积分符号下的VN代表积分范围为整个细胞核,dV是积分中使用的体积微元。SFNP和SFcontrol分别代表处方剂量下肿瘤细胞内有无摄入纳米金属时的细胞生存率(Surviving fraction,SF)。SERDP是处方剂量下的增敏率。本发明在计算RBE时以细胞生存率作为生物学终点,即用SERDP指代RBE。
纳米金属放射增敏生物等效剂量为RBE与处方剂量之积。
本发明方法概述:通过对细胞形态、纳米金属胞内含量与分布的精确测定,建立摄入纳米金属的细胞几何模型。微观蒙特卡罗方法在纳米尺度上模拟射线径迹结构及其与介质的相互作用过程。采用细胞几何模型与微观蒙特卡罗方法,计算细胞模型内的微观剂量分布。局部效应模型(LEM)多用于质子与碳离子治疗的相对生物效应计算,将不均匀的局部剂量造成的致死事件对整个细胞积分以预测细胞存活率。本发明将纳米金属引起的不均匀的局部剂量分布(使用微观蒙特卡罗方法计算)与LEM模型相结合,建立纳米金属增敏后细胞存活曲线的预测模型,并据此计算用于纳米金属增敏生物效应评价的增敏率(Sensitizer enhancement ratio,SER),进而得到纳米金属放射增敏的相对生物效应(Relative biological effectiveness,RBE),将RBE与处方剂量相乘得到纳米金属放射增敏生物等效剂量。
本发明具有的优点和有益效果:
(1)灵活性、适应性强
现有技术采用细胞照射实验或动物照射实验建立相对生物效应预测模型时,需要针对不同治疗模式和治疗剂量设计一系列的生物辐照实验,操作复杂且容易受到人工因素影响。采用本发明方法进行一次细胞表征和纳米金属含量测定就可以获得单种纳米粒子在多种治疗模式(即多种射线谱)下、多种治疗剂量下的相对生物效应预测模型。
(2)在研究新型金属纳米粒子的放射增敏效应时,也可以采用本发明方法,在开展一系列生物实验前,建立该增敏剂在不同的肿瘤细胞、治疗模式和照射剂量下的相对生物效应预测模型,为后续实验和研究提供指导和参考。
具体实施方式
以下实施例用于进一步说明本发明,但不应理解为对本发明的限制。若未特别指明,实施例中所用的技术手段为本领域技术人员所熟知的常规手段。
实施例1
以钆纳米粒子在宫颈癌细胞的铱-192源近距离放射治疗中的应用为例,描述放射增敏后的相对生物效应的建模计算方法。
(1)人宫颈癌细胞HeLa在含钆纳米粒子0.5mg/mL的DMEM培养基中于37℃、5%CO2的条件下培养24小时,收集细胞,然后清洗以去除细胞外多余的钆纳米粒子。所采用的钆纳米粒子的平均粒径为5nm。
(2)使用ICP-MS或核磁成像仪测量单个肿瘤细胞内的钆纳米粒子含量,重复3次并取平均值,测量值为1.17pg。使用透射电镜观测钆纳米粒子在肿瘤细胞内的分布情况,同时获得细胞与细胞核的几何形态和尺寸。
(3)结合第(2)步的观测数据,建立肿瘤细胞的几何模型,并设置模型材料。在本例中,观察到钆纳米粒子在细胞核内不存在,在细胞质基质中均匀分布;细胞核包含在细胞内,两者均大致为球形,直径分别为10μm和20μm。于是,细胞模型可以设置为两个相互嵌套的同心球,中间的直径为10μm的小球代表细胞核,模型材料设为水;外部的直径为20μm的球壳代表细胞质基质,模型材料设为钆和水的均匀混合物,钆的含量按照第(2)步的测量值设置为319g/m3。
(4)使用微观蒙特卡罗方法(如Geant4、TOPAS等工具软件),使用铱-192的射线谱模拟照射上述细胞模型,记录细胞核内的微观剂量分布DNP(x,y,z)。使用相同的设置,照射材料中不含钆纳米粒子的细胞模型,再次记录细胞核内的微观剂量分布Dcontrol(x,y,z)。将两次获取的微观剂量分布相除DNP(x,y,z)/Dcontrol(x,y,z),得到细胞核内DER的三维分布DER(x,y,z)。细胞核内的平均DER为1.07,最高DER为25。
(5)根据DER(x,y,z)、处方剂量DP、HeLa细胞的LQ模型参数α和β,按照式(1)-(4)计算RBE。在典型的处方剂量2Gy下,首先用式(1),DER(x,y,z)乘以DP得到D(x,y,z);然后用式(2),将含D(x,y,z)、α、β和VN的分式在细胞核内积分得到SFNP为0.70;然后用式(3),用DP、α和β计算得到SFcontrol为0.81;然后用式(4),用SFcontrol除以SFNP,得到RBE为1.16。最后将RBE与处方剂量2Gy相乘得到纳米金属放射增敏生物等效剂量2.32Gy。
对第(1)步中培养的细胞进行辐照实验,方法为:使用铱-192源辐照两组细胞,第一组细胞在普通培养基中培养,第二组细胞在含钆纳米粒子0.5mg/L的培养基中培养和清洗,照射剂量为处方剂量2Gy。照射后继续培养细胞4小时,然后清洗处理并进行细胞计数。根据两组细胞的计数结果分别获得有无摄入纳米金属时的细胞生存率SFNP(0.72)和SFcontrol(0.81),然后将SFcontrol除以SFNP得到RBE为1.13,将RBE与处方剂量2Gy相乘得到纳米金属放射增敏生物等效剂量为2.26Gy。
可见本发明获得的RBE与生物实验结果的差别在3%以内,符合较好。本发明在获得准确的RBE预测值的同时,其灵活性、适应性更强。
实施例2
以在X射线小动物放射研究平台下研究金纳米粒子对大鼠脑胶质瘤细胞C6的放射增敏效应为例,描述放射增敏后的相对生物效应的建模计算方法。
(1)C6细胞在含金纳米粒子0.5mg/mL的DMEM培养基中于37℃、5%CO2的条件下培养24小时,收集细胞,然后清洗以去除细胞外多余的金纳米粒子。所采用的金纳米粒子的平均粒径为15nm。
(2)使用ICP-MS测量单个肿瘤细胞内的金纳米粒子含量,重复3次并取平均值,测量值为3.64pg。使用透射电镜观测金纳米粒子在肿瘤细胞内的分布情况,同时获得细胞与细胞核的几何形态和尺寸。
(3)结合第(2)步的观测数据,建立肿瘤细胞的几何模型,并设置模型材料。在本例中,观察到金纳米粒子在细胞核内不存在,在细胞质基质中集中分布在细胞核膜上;细胞核包含在细胞内,两者均大致为球形,直径分别为15μm和30μm。于是,细胞模型可以设置为两个同心球,中间的直径为15μm的小球代表细胞核,模型材料设为水;外部的直径为30μm的球壳代表细胞质基质,模型材料大部分设为水,在细胞核膜外层设置厚度15nm的、模型材料为金与水均匀混合物的球壳层,球壳层中金的含量按照第(2)步的测量值设置为29.4g/m3。
(4)采用微观蒙特卡罗方法(如Geant4、TOPAS等工具软件),使用X射线小动物放射研究平台的220kVp X射线谱模拟照射上述细胞模型,记录细胞核内的微观剂量分布DNP(x,y,z)。使用相同的设置,照射材料中不含金纳米粒子的细胞模型,再次记录细胞核内的微观剂量分布Dcontrol(x,y,z)。将两次获取的微观剂量分布相除DNP(x,y,z)/Dcontrol(x,y,z),得到细胞核内DER的三维分布DER(x,y,z)。细胞核内的平均DER为2.24,最高DER为13。
(5)根据DER(x,y,z)、处方剂量DP、C6细胞的LQ模型参数α和β,按照式(1)-(4)计算RBE。在典型的处方剂量2Gy下,首先用式(1),DER(x,y,z)乘以DP得到D(x,y,z);然后用式(2),将含D(x,y,z)、α、β和VN的分式在细胞核内积分得到SFNP为0.56;然后用式(3),用DP、α和β计算得到SFcontrol为0.77;然后用式(4),用SFcontrol除以SFNP,得到RBE为1.38。最后将RBE与处方剂量2Gy相乘得到纳米金属放射增敏生物等效剂量2.76Gy。
对第(1)步中培养的细胞进行辐照实验,方法为:使用X射线小动物放射研究平台的220kVp X射线源辐照两组细胞,第一组细胞在普通培养基中培养,第二组细胞在含金纳米粒子0.5mg/mL的培养基中培养和清洗,照射剂量为处方剂量2Gy。照射后继续培养细胞4小时,然后清洗处理并进行细胞计数。根据两组细胞的计数结果分别获得有无摄入纳米金属时的细胞生存率SFNP(0.53)和SFcontrol(0.78),然后将SFcontrol除以SFNP得到RBE为1.47,将RBE与处方剂量2Gy相乘得到纳米金属放射增敏生物等效剂量为2.94Gy。
可见本发明获得的RBE与生物实验结果的差别在7%以内,符合较好。本发明在获得准确的RBE预测值的同时,其灵活性、适应性更强。
上述实施例为本发明较佳的实施方式,但本发明的实施方式并不受上述实施例的限制,其他的任何未背离本发明的精神实质与原理下所作的改变、修饰、替代、组合、简化,均应为等效的置换方式,都包含在本发明的保护范围之内。
Claims (6)
1.一种纳米金属放射增敏生物等效剂量的计算方法,其特征在于,包括以下步骤:
(1)将肿瘤细胞在含纳米金属颗粒的细胞培养基中培养,使肿瘤细胞充分摄入纳米金属颗粒;
(2)测量单个肿瘤细胞内的平均纳米金属含量;观测纳米金属在肿瘤细胞内的分布情况,并获得细胞与细胞核的几何形态和尺寸;
(3)结合步骤(2)的观测数据,建立肿瘤细胞的几何模型,设置模型材料;
(4)使用微观蒙特卡罗方法,使用放射线模拟照射步骤(3)所建立的含纳米金属的肿瘤细胞模型,记录细胞核内的微观剂量分布DNP(x,y,z);
使用相同的放射线模拟照射其他参数相同、但不含纳米金属的肿瘤细胞模型,再次记录细胞核内的微观剂量分布Dcontrol(x,y,z);
DNP(x,y,z)除以Dcontrol(x,y,z),得到细胞核内剂量增强比的分布DER(x,y,z);
(5)基于局部效应模型建立计算方法,依据细胞核内的DER分布预测纳米金属放射增敏的增敏率SER,进而获得纳米金属放射增敏的相对生物效应RBE;计算方法如下:

其中,DP是处方剂量,DER(x,y,z)是细胞核内任一点的DER,D(x,y,z)是细胞核内任一点的增强剂量;α和β是肿瘤细胞的线性二次模型参数;分数线下的VN代表细胞核的体积,积分符号下的VN代表积分范围为整个细胞核,dV是积分中使用的体积微元;SFNP和SFcontrol分别代表处方剂量下肿瘤细胞内有无摄入纳米金属时的细胞生存率;SERDP是处方剂量下的增敏率;在计算RBE时以细胞生存率作为生物学终点,用SERDP指代RBE;
纳米金属放射增敏生物等效剂量为RBE与处方剂量之积。
2.根据权利要求1所述的纳米金属放射增敏生物等效剂量的计算方法,其特征在于:步骤(1)中,所述的纳米金属颗粒的粒径为1~100nm。
3.根据权利要求1所述的纳米金属放射增敏生物等效剂量的计算方法,其特征在于:步骤(2)中,所述的单个肿瘤细胞内的平均纳米金属含量通过电感耦合等离子体质谱仪或核磁成像仪测量;所述的纳米金属在肿瘤细胞内的分布情况通过使用透射电镜观测,所述的细胞与细胞核的几何形态和尺寸根据透射电镜观测结果设置。
4.根据权利要求1所述的纳米金属放射增敏生物等效剂量的计算方法,其特征在于:步骤(3)中,按照步骤(2)观测到的纳米金属在细胞核和细胞质基质内的分布情况,将含金属结构的模型材料设置为金属和水的均匀混合物,或按照结构的成分设置为金属和有机物的均匀混合物;不含金属结构的模型材料设置为水或者有机物。
5.根据权利要求4所述的纳米金属放射增敏生物等效剂量的计算方法,其特征在于:含金属结构的模型材料设置为金属和水的均匀混合物,不含金属结构的模型材料设置为水。
6.根据权利要求1所述的纳米金属放射增敏生物等效剂量的计算方法,其特征在于:步骤(4)中,所述的放射线由千伏级光子源产生。
