Description
Technical Field
The invention relates to the field of medical instruments, in particular to a fixing method and a fixing system for an angle-adjustable self-locking type lumbosacral anterior approach vertebral arch fixing nail plate.
Background
The diseases of the lumbar and sacral sections of the spine are common diseases and frequently encountered diseases in orthopedics department, which seriously troubles the work and life of people. The operation is an important treatment method for treating diseases of the lumbosacral segment, and the main operation modes comprise: the simple focus-removing bone grafting and the anterior focus-removing bone grafting combined posterior pedicle screw internal fixation are performed through an abdominal cavity or an extraperitoneal approach. The pure anterior focus is removed and the bone grafting is fixed due to the lack of a firm and reliable anterior internal fixing device, so that the stability of the lumbosacral vertebrae is difficult to reconstruct and the physiological curvature of the spine is recovered. Therefore, auxiliary external fixation is needed after operation, and patients are often difficult to tolerate. Although a good mechanical environment can be provided for the fusion of the spine in the anterior-posterior combined operation, the posture of the patient needs to be changed in the operation due to the anterior-posterior two operation approaches, the operation wound is large, the operation time is long, and the operation risk is increased undoubtedly. In addition, the combined approach also has the problems that the anterior bone graft is difficult to effectively open, the stability of posterior fixation to the anterior central column of the vertebral body is poor, and complications such as the absorption of the bone graft, the formation of a pseudoarthrosis, the relapse of deformity, the loosening and the fracture of an internal fixture and the like still occur in the long term.
At present, the clinical anterior internal fixation technology of the lumbosacral segment of the spine is developed, HaKY and the like give manual pre-bent anatomical plates to anterior internal fixation after focus clearing surgery of 5 sacral 1 segments of the waist by people in mid-buildings and the like, and good effects are obtained. In addition, a plurality of anterior internal fixators are arranged abroad to increase the stability of the anterior interbody fusion operation, and can complete the intervertebral fusion and the internal fixation operation through the anterior way at one time, thereby avoiding the complication of the posterior operation. These internal fixator devices have: FRA with front cross screws; an interbody cage with a forward steel plate; or a separate anterior steel plate internal fixation device. These internal fixation systems have the following problems: 1. higher risk of injury to common iliac arteries and veins; 2. the direction of the screw is relatively fixed, and the use is not flexible; 3. the biomechanical stability needs further verification; 4. the steel plate is composed of two small steel plates, and is not designed in the radian of sagittal plane and coronal plane, so the use in the operation is very inconvenient.
