Abstract

CD45是尤其可用于识别白细胞(白血细胞)的抗原。
抗原是抗体和特定淋巴细胞受体可特异性结合的物质(其中特定淋巴细胞受体也可导致产生针对抗原的抗体)。抗原大多是蛋白,但也可以是碳水化合物、脂质或其它物质。它们可以被B细胞受体、T细胞受体或(由B细胞产生的)抗体识别或结合。
如果细胞来自上皮组织,则该细胞是源于上皮的。上皮组织是特殊的组织类型,其用作被覆组织和腺体组织的总称。上皮组织是覆盖多细胞有机体的所有内部和外部躯体表面的单层或多层细胞层。这里的一个例外是运动系统的关节囊和粘液囊。除了肌肉组织、神经组织和结缔组织之外,上皮组织是多细胞有机体中的四种基本组织类型之一。
核(DAPI检测)是一种检测,即这些细胞具有细胞核——因此不是无细胞核的细胞,例如红血细胞。原则上认为无细胞核的细胞不是CTC,即使如果它们满足所提到的其它标准。术语“裂解”是指细胞悬液中存在的细胞或细胞的细胞膜被破坏,以使得细胞悬液的细胞的各个成分在溶液中是游离可得的。这使得能够稍后分析。
CTC计数的目的是可以在医学研究中在总存活率(OS)和治疗后测量的CTC数量之间建立清楚的关系。也就是说,对CTC的纯计数已可以提供关于OS或治疗功效的重要信息。为了获得肿瘤状态的更广泛的图像,CTC研究目前也集中于识别来自CTC的遗传特征。因此,用于对CTC进行自动分类、计数和归类的系统非常重要。
对于CTC的遗传分析,绝对需要确保样品中实际上也存在细胞。否则,在不存在信号(=不存在遗传特征)的情况下,不清楚是样品中没有CTC还是一般而言没有细胞,例如因为样品被污染并且检测反应因此没有产生信号。
因此,希望获得可对样品的绝对细胞数进行计数的方法。
一种可能性是借助进行染色步骤和自动处理样品来对细胞进行计数。但是,这种方法仅可困难地在临床应用的全自动工作流程中实现——一方面因为通常需要图像处理,以便在可识别出特定染色特征时对样品进行光学分析——另一方面因为染色步骤通常持续较久。这也使得这种方法的临床应用变得困难。
发明公开
本文应描述借助识别遗传特征来检测分离出的CTC (或甚至对其进行计数)的方法。在某些情况下,该方法还能够省去染色步骤。
该方法用于检测样品中的特定特征细胞并包括以下步骤:
a) 提供细胞悬液的样品
b) 对样品进行第一裂解过程,以使得产生样品的第一裂解物,其中所述裂解如此进行,以使得细胞悬液的细胞的细胞核保持完好;
c) 提取在第一裂解物中游离可得的核糖核酸(RNA);
d) 将提取的核糖核酸归类,以确定在样品的裂解物中不同类型的核糖核酸的数量;
e) 对剩余的样品进行第二裂解过程,以使得产生样品的第二裂解物,其中如此进行裂解,以使得样品中的细胞的细胞核被破坏,
f) 提取在第二裂解物中游离可得的脱氧核糖核酸(DNA),
g) 根据脱氧核糖核酸的数量来计算样品中细胞的总细胞数
h) 根据在步骤d)中确定的不同类型的核糖核酸的数量和在步骤g)中计算的总细胞数来计算样品中具有各种细胞标志物的数量。
当步骤h)中的细胞标志物用于识别CTC (循环肿瘤细胞)时,该方法是特别优选的。
此外,当步骤h)中的细胞标志物包括以下标志物中的至少一种时,该方法是特别优选的:
- 上皮标志物;
- EpCAM;
- CD45;和
- 细胞角蛋白。
使用这种方法,分离出的CTC被定量,而不需要用于定量的染色步骤。在此,术语“定量”表示可以确定CTS的绝对数量。该方法还允许对样品进行标准化,即确定样品中总共有多少细胞,以使得该样品和计数的CTC分别与其它样品和在其它样品中计数的CTC可比较。
该方法适合于实现在基于芯片实验室的全自动工作流程中用于即时检验。
步骤a)例如可以包括在芯片实验室系统上提供细胞。步骤b)和e)是常规的裂解过程,其用于溶解细胞膜,并且通过其可以识别细胞的成分。提取步骤c)和f)分别用于为随后的分析步骤d)以及g)和h)作准备。
用于步骤a)的样品的有意义的尺寸例如包括10μl至10 ml的血液量。这样的样品包含5 x 107至5 x 1010个细胞。
本发明的核心点是通过遗传特征的多参数测量来定量检测分离出的CTC的方法。这在步骤h)中进行,其中计算各个细胞类型的数量。在此利用了这样的事实,即在现有技术中通过染色方法确定的特征(例如上皮标志物、EpCAM、CD45、细胞角蛋白)也可借助相应的分子来检测,即可检测细胞中的RNA (核糖核酸),尤其mRNA (信使RNA)。这引起细胞对特定染料起反应。因此,在使用染色方法时,间接地反正已依赖于这些分子的存在,以实现细胞的归类。通过所述方法,在一定程度上可以直接推断出这些分子。因此,在步骤d)中确定这些分子或将它们归类,以确定不同类型的RNA的数量。在步骤d)的归类中,优选使用遗传检测,以实现根据特定特征(上皮标志物、EpCAM、CD45、细胞角蛋白)对RNA进行归类。
原则上,这样的遗传检测的操作方式是,在检测RNA时首先制造来自RNA (cDNA)的反转录酶(RT),由此可以借助聚合酶链反应(PCR)扩增该cDNA,其中同时也进行定量。所有一起此时被称为RT-PCR。DNA简单地仅借助PCR来检测(如果定量,则被称为qPCR)。为了确定其在样品中的数量而在步骤d)中被归类的不同类型的核糖核酸尤其是如下核糖核酸,所述核糖核酸是特定癌细胞的标志物。优选地,在步骤d)中,一方面借助基于PCR的方法来确定上述(表面)标志物(例如上皮标志物、EpCAM、CD45、细胞角蛋白)的RNA量(特别是mRNA量)。在此,EpCAM被视为用于源于上皮的细胞的标志物并因此用于源自组织复合物(Gewebeverbund)的细胞,即CTC的标志物;CD45是用于白血细胞的标志物,即对于非特异性分离的细胞的标志物;细胞角蛋白是存在于源于上皮的细胞的细胞内部的蛋白,即又是用于CTC的标志物。PCR (特别是rtPCR = 定量实时PCR,或qPCR)是核酸的扩增方法。这种扩增方法基于常规聚合酶链反应(PCR)的原理。在PCR时借助在PCR循环过程中实时(英语realtime)获取的荧光测量来进行定量。
此外,分离出的细胞的总量通过DNA来确定。这借助步骤e)、f)和g)来进行。最后,对在mRNA层面上进行测定的量与细胞总量建立关系(步骤h)。
所述方法的各个步骤可以在其顺序方面变化。这尤其适用于不紧接着彼此布置且其处理顺序因此可调适的步骤。例如,步骤d)不必强制性地在e)之前进行。还可以将第一裂解物进行暂时存储,例如在反转录酶步骤之后,并然后与第二裂解的DNA检测同时进行分析。特别地,步骤c)和f)可以并行地进行。
该方法也可以被称为遗传计数方法,其中名称“计数方法”在此尤其是涉及步骤d)。
该方法相对于用于检测样品中的特征细胞的已知方法而言具有许多优点。
该方法的主要优点是,由于对遗传特征的测量,测量存在于分离出的样品中的肿瘤细胞的数量或数量范围。因此,不必在附加步骤中对样品量进行定量。样品的定量是必要的,以能够将对样品的测量与其它数据建立关系并且因此为样品赋予说服力。
该方法原则上还适合于基于CTC——通常借助基于PCR的方法——检测癌症特异性的突变、缺失或插入。该方法提供的优点是,这种检测和在此所介绍的方法从方法步骤来看原则上是相同的,并且因此特别好地适合于共同集成在芯片实验室上,因为存在的样品量的数量在此也是设计的重要基础。
染色方法具有如下基本缺点,即难以从电子评估的图像中得出明确结论。为此,需要非常多的智能手段来进行图像评估。也不存在用于确定CTC的统一标准。虽然由CANCER-ID联盟开发了所谓的ACCEPT5,其据说对于图像评估规定了一种标准。通过在此描述的方法的测量(确保)/提供更有说服力和明确的信号。特别地,可以在步骤h)中进行(经典的)数值评估。
本发明的方法在样品中存在仅少量细胞的情况下也确保可良好检测的信号,因为在转录的活性基因的情况下,在大多数情况下,在细胞中存在许多mRNA分子。因此,该方法基于依据所存在的量呈有利的标志物,其中该标志物相对于基于细胞DNA的标志物而言是特别有利的,原则上在每个细胞中可用的所述基于细胞DNA的标志物远远更少。
这些mRNA标志物与DNA层面上的信息(作为测量样品的总细胞量的可靠来源)组合。这具有的优点是,通过参照DNA测量,可以将在mRNA层面上确定的相对比率转化/换算成绝对值。进行样品材料的明确标准化。
通过所述方法,还可以在样品分析中实现手动步骤的减少。根据分离系统,染色需要许多方法步骤。这些方法步骤通过所述方法变得非必需。在此提出的方法实现简单的自动化并且因此节省成本和时间。
在现有技术中使用的染色方法中,通过始终总是非特异性结合的抗体进行操作。因此,在这些方法中可能产生不希望的染色,其歪曲随后的染色细胞计数的结果。遗传检测在此提供了更高的特异性。此外,抗体在制备时可具有变异性,并且根据制备LOT而具有不同的灵敏度或特异性。
这里介绍的方法使得可以进行应当对于样品检查的靶的个性化汇总。这可以通过将步骤d)中的归类和步骤h)中的计算/评估相应地适配来进行。因此,该通用方法允许对于应用而言特异性的使用。术语“靶”在此是指特定的细胞标志物,应当对细胞检查其存在或应当对其确定其在样品的细胞中出现的频率如何。
所述方法不限于肿瘤学中的使用。一般而言,该方法提供了分析用于诊断的细胞表达模式的简化的过程方法。该方法尤其可有利地用于全自动的即时检验应用中。
为了使借助该方法进行的计算/评估更精确,优选至少在两个步骤g)和h)之一中考虑样品中细胞的细胞尺寸的变异性。细胞尺寸的这种变异性例如可以通过对样品的光学分析(图像分析)来确定,或从统计数据中获得。在该方法的实施变型中,该变异性作为用于在步骤g)和h)中进行计算评估的参数被固定地储存。
步骤g)和h)中的术语“计算”不应理解为是指,作为步骤g)中的结果必然得出确定地或实际地等于样品中细胞的确切数量的精确总细胞数。在此同样不是指,在步骤h)中精确地计算在样品中确定地或实际地包含何种数量的具有各种细胞标志物的细胞。由于在对于计算所述的输入参量(用于步骤g)的脱氧核糖核酸的数量和用于步骤h)的不同类型的核糖核酸的数量)中的不确定性或随机不精确性,这也可能根本不可行。相反,步骤术语“计算”在此是指,根据数学规则基于可用的输入参量来确定细胞的总数或确定具有各种细胞标志物的细胞的数量,并且对此优选不基于另外的估计值或假设。
特别优选地,通过细胞的管家基因来确定细胞尺寸的变异性。借助管家基因可以估计细胞尺寸,并且因此可以将细胞的尺寸差异进行标准化和使其可比较。管家基因通常是编码与细胞基础代谢,例如与葡萄糖代谢相关的结构分子和酶的基因。借助这样的基因可以推断出细胞尺寸。
此外,当在步骤h)之前进行以下步骤时,该方法是特别有利的:
g2) 区分活性核糖核酸和非活性核糖核酸。
由此产生方法步骤h)和g)的进一步改进的评估可能性。
特别地,通过将步骤d)中确定的量与数据矩阵进行比较来进行步骤g)中的估算,在该数据矩阵中为每种细胞标志物储存了关于存在的核糖核酸的数据。这种数据矩阵可以根据专家系统的类型来构造。
当在步骤c)之前进行以下步骤时,该方法是特别有利的:
b2) 提取细胞质。
该附加步骤简化了稍后的方法步骤。此外,通过该步骤使得能够避免可能由细胞质引起的步骤d)、g)和h)中的错误。
此外,当为了确定步骤d)中的量而将核糖核酸转化成脱氧核糖核酸时,该方法是特别有利的。这种转化为归类提供了有利的可能性。
此外有利的是,步骤b)中的裂解通过改变样品中的渗透平衡来进行,由此引起细胞的细胞膜的破裂。在这方面特别有利的是,在步骤b)中调节渗透平衡,以使得样品中细胞的细胞核保持完好。通过裂解过程的这种设计,可以确保RNA和DNA对于分析步骤d)、g)和h)而言不混合。
下面借助附图来解释该方法。附图示出了优选的实施例,然而该方法不能被缩小到该实施例。其中:

在图1中完全示意性地示出了所述方法所采用的方法步骤。在此,将(通常)预处理的细胞悬液1裂解。该细胞悬液在此可以是全血、血清、细胞提纯产物或在流式细胞仪上分类的样品。替代细胞悬液,也可以将粘附在表面上的细胞用于该方法。特别地,可以使用粘附到为该方法进行功能化的活检针上的细胞。
第一裂解过程2 (步骤b))在此由箭头表示。通过第一裂解过程2产生第一裂解物4,其可以通过步骤c)和d)分析。
在1 ml [毫升]典型量的起始样品的情况下,通过步骤b)优选产生2 ml至4 ml量的第一裂解物。
第二裂解过程3 (步骤e))在此同样由箭头表示。通过第二裂解过程3产生第二裂解物5,其可以通过步骤f)、g)和h)分析。
在2 ml [毫升]至4 ml典型量的起始样品的情况下,通过步骤e)优选产生4 ml至8ml量的第二裂解物。
在步骤b)中使用的裂解模式(第一裂解过程)可以任意选择,但必须与随后的RT-qPCR兼容。特别是省去裂解物的提纯,因为否则可能损失要定量的材料。合适的裂解方法可以通过将细胞材料固定在插管内来实现。然后可以将裂解缓冲液引入插管中,以从细胞材料中提取核酸,如DNA或RNA。在此首先引入(裂解缓冲液的)低渗溶液,其中缓冲液的盐含量高于细胞的盐含量。在此,细胞膜破裂,而细胞核不破裂。作为结果,因此可以提取细胞的RNA所处的细胞质。在RNA提取后,可以在第二步骤中提取DNA,其中吸取高渗的裂解缓冲液。在此,细胞核现在也破裂,并且可以提取DNA。也可以仅使用高渗缓冲液,并在一个步骤中提取DNA和RNA。
裂解缓冲液可以具有不同的组成。必须区分低渗(裂解)缓冲液和高渗(裂解)缓冲液。
低渗的裂解缓冲液的示例性组成包含20 mM [毫摩尔] Tris最终浓度。即在样品和Tri的混合物中出现20 mM NaCl的最终浓度。
高渗缓冲液的示例性组成包含3%的氯化钠(NaCl)。即在样品和NaCl的混合物中出现3% NaCl的最终浓度。
在成功裂解后,RNA借助反转录转化成DNA。在此产生仅由DNA组成的溶液。现在,为了检测反应,可以对该溶液借助定量实时聚合酶链反应(qPCR)进行检测。
所述方法的实际核心点在于PCR系统的构造,如图1 (步骤g)和h))中所示并且在下表中示出:
qPCR曲线的评估使得能够定量地确定和推断出细胞类型层面的样品组成。
该表描述了用于成功应用所述方法的PCR系统的设计策略。在此用作基础的是检测真核个体的一个细胞类型或细胞群α和另一个细胞类型或细胞群β的细胞悬液的组成。
具体实例在此是在血细胞(类型β)背景下检测循环肿瘤细胞(类型α)。在此的基本策略是,通过转录组(mRNA信息)确定细胞类型、细胞类型之间的相对细胞模式。通过测量基因组,然后可以确定样品中所有细胞的绝对量。如果知道该量以及比率,则可估算具有各种细胞标志物的细胞的绝对量或各种亚群(样品内不同类型的细胞)的数量。
细胞的所有mRNA的量被称作“转录组”。它是细胞类型特异性的参数。该转录组是可用于在步骤d)中归类的RNA总量。
优选地,逐步测试具有该RNA总量(转录组)的样品或样品的第一裂解物的靶(即应当通过所述方法识别的细胞标志物或细胞类型)。优选地,对各种靶以特定顺序进行操作。例如,要检验的靶以指数A、B、C等表示。首先,测试样品或第一裂解物的靶A,然后靶B,然后靶C等。这在步骤d)中进行。如果特定基因在细胞中是活性的,将该相应的DNA序列转录成RNA,其随后从细胞核迁移至胞质体并在那里转译成蛋白。
借助活性和非活性基因,可识别细胞类型。每种细胞类型具有特定的一组基因,其是特别活性的或必须是失活的。这些基因活性可以通过检测相应的mRNA或蛋白来证明。通过qPCR的RNA检测在此是特别灵敏的。
代替完全检验转录组和将转录组中的所有RNA进行归类,也可以在步骤d)中借助qPCR仅检测对于细胞类型而言典型的序列。然后将mRNA如前已经描述那样转录成DNA,以随后进行基因特异性的qPCR。转录测量使得能够得出关于(一种或多种)细胞的转录模式的比率的结论。为此选择至少一种靶A,其在所有细胞类型中被相同表达,即在两种类型中都是活性的。这是所谓的管家基因,其通常恒定地在所有细胞类型上都是活性的。其典型实例是GAPDH、肌动蛋白、SDHA、HSP、COX8或MYH9。作为第二靶B,使用对于细胞类型α而言被上调的靶,而在细胞类型β中该靶相应地被下调。第三靶C具有相对于靶B而言的交叉转录状况,即在细胞类型α中被下调并且在细胞类型β中被上调。
通过靶A的标准化是必要的,因为细胞群在其测量参数方面是非常异源性的。特别地,细胞尺寸通常具有高的变异性。小细胞通常具有较少的mRNA绝对量,其比率是上和下调的,但是保守的。因此,使用管家基因,以将细胞的尺寸差异进行标准化和使其可比较。在血液中的CTC的实例中,因此靶B可以是上皮标志物,例如EpCAM、CDK、连环蛋白。作为用于血细胞(主要是白细胞,如T细胞、中性粒细胞、树突细胞、巨噬细胞)的靶C,可以使用造血标志物,如CD45。
现在通过基因组测量关于使用了多少样品材料(即多少细胞量)的信息。在此使用靶D,该靶D应在要测量的个体的所有细胞类型上是相同的。特别地,具有高SNP比例或高突变率的基因(例如致癌基因)不适于此,因为这样的基因正好在样品的不同细胞中以不同量存在,或靶细胞(细胞标志物)的量应当正好借助这样的基因通过所述方法来确定。理想地,在此使用高度保守、多重冗余的基因,例如hLINE1或APEX1。这些基因是多倍存在的,并且比传统基因如EGFR(其在基因组中正好出现两次)更容易定量。特别地当在样品中很少细胞可用时,如在常规CTC检查的情况下,这是有利的。
在此描述的方法的概念是可任意缩放的。可以在转录组中补充靶E,其用于更精细地选择细胞类型、用于统计学地确定质量或用于引入附加的细胞类型。
在肿瘤治疗监测范畴内的液体活检方面,所述方法可以如下进行。对于所用的靶BEpCAM,可以例如附加地还测量CDK19或波形蛋白。即附加的肿瘤细胞标志物。如果两种表达方式的差异太大,则可以推断出反应问题。因此,这两种表达模式是否相同的测试可用作样品材料的有效性和质量的控制元件。还可以将附加的靶E用于引入第三细胞类型γ。在CTC监测的范畴内,附加的靶E适合作为间叶肿瘤细胞的标志物,例如Twist、N-Notch、波形蛋白或Snail1。因此,可以将CTC群再次分为两种亚型。因此,该方法也可以用于上皮细胞-间叶转型(EMT)领域的检查中。这特别令人感兴趣,因为由此避免了EpCAM的偏差,因为在血液中也存在相当大部分的间叶细胞。
图2示出了上表中所述方法的分析链。在此,由各个组分A、B、C、D的qPCR曲线来确定各自的阈值循环CT值。对于转录元件,然后通过标准 ΔΔCT 方法来确定相对表达模式。由此可以确定各个细胞类型的表达比率。由遗传物质确定反应混合物中的细胞绝对量。
图3示出了所述方法的过程实施的替代性示意图。代替测量反应混合物中的靶,可以在第一步骤中将转录组与DNA分开。在此使用选择性的裂解方法,其中首先仅将mRNA所处的胞液借助高渗缓冲液(裂解缓冲液)进行裂解,然后将含DNA的细胞核经由低渗裂解缓冲液进行裂解。图3的图示出了,涉及RNA的分离和评估的步骤b)、c)和d)原则上被认为与涉及DNA的分离和评估的步骤e)、f)和g)分开。方法步骤d)和方法步骤g)分别提供信息,该信息是方法步骤h)的实施基础并且因此被提供给方法步骤h)。
Claims (11)
1.用于检测样品中特定特征细胞的方法,其包括以下步骤:
a) 提供细胞悬液1的样品
b) 对样品(1)进行第一裂解过程(2),以使得产生样品(1)的第一裂解物(4),其中所述裂解如此进行,以使得细胞悬液1的细胞的细胞核保持完好;
c) 提取在第一裂解物(4)中游离可得的核糖核酸(RNA);
d) 将提取的核糖核酸归类,以确定在样品的裂解物中不同类型的核糖核酸的数量;
e) 对剩余的样品(1)进行第二裂解过程(3),以使得产生样品(1)的第二裂解物(5),其中如此进行裂解,以使得样品(1)中的细胞的细胞核被破坏,
f) 提取在第二裂解物(5)中游离可得的脱氧核糖核酸(DNA),
g) 根据脱氧核糖核酸的数量来计算样品中细胞的总细胞数
h) 根据在步骤d)中确定的不同类型的核糖核酸的数量和在步骤g)中计算的总细胞数来计算样品中具有各种细胞标志物的细胞的数量。
