Description
Technical Field
The invention relates to an artificial prosthesis, in particular to a distal femur revision type tumor prosthesis, and belongs to the technical field of medical instruments.
Background
Distal femur is the most common site for the development of osteosarcoma and other malignant bone tumors. Artificial joint prosthesis replacement is a common reconstruction method after resection of malignant bone tumors around the knee joint, and conventionally, customized knee joint prostheses are mainly used, and in recent years, combined joint prostheses are mostly adopted. Although the artificial joint prosthesis reconstruction has good functional effects, various complications such as aseptic loosening, mechanical failure such as prosthesis handle fracture and deep infection exist.
Most of the failed knee joint tumor prostheses need to be rebuilt, and the rebuilt prosthesis surgery faces many challenges, which may face problems including extensive bone defects, poor quality of remaining bone, difficult removal of the prosthesis stem, etc. It is very difficult to remove the stem of the joint prosthesis from the cement-fixed medullary cavity, especially to remove the bone cement from the bone-poor medullary cavity.
If the poor bone and bone cement are removed altogether, the remaining bone segments are insufficient to secure the prosthesis stem. Of course, several prosthetic revision methods have been developed in recent years, including short stem compression fixation prostheses (Zimmer Biomet composites), prosthesis stem locking prostheses, telescopic techniques (televiewing methods), and the like. All of the above methods require taking out the broken stem of the prosthesis from a firm cement fixation or, in the case of loose stem of the prosthesis, bone cement bonded to bone in the medullary cavity, often requiring bone grooving or the like, seriously damaging the bone quality of the proximal end of the femur.
Disclosure of Invention
In view of the above problems, it is an object of the present invention to provide a distal femoral revision type tumor prosthesis, which does not require removal of a prosthesis stem or removal of bone cement from a bone-poor intramedullary cavity when performing a revision surgery.
In order to achieve the purpose, the invention adopts the following technical scheme: a distal femur revision type tumor prosthesis comprises a matched distal femur component and a femur stump fixing component; wherein the lower end of the matched femoral distal component is used for forming a hinge joint with the tibia, and the upper end of the matched femoral distal component forms a tapered end; the femoral stump fixation assembly includes: a matched stack extension segment having a lower end that is tapered to an upper end of the matched stack femoral distal component, the upper end of the matched stack extension segment forming a central platform; a femoral stump fixing sleeve integrally formed on the central platform of the assembled extension segment, the femoral stump fixing sleeve configured to fit over and be securely connected to the exterior of the femoral stump.
The distal end of femur renovation type tumour prosthesis, preferably, the incomplete fixed sleeve of femur is three fens of fork sleeves, promptly the incomplete fixed sleeve of femur includes along three fixed arms of axial extension, and three the fixed arm is arranged along circumference equidistance on the central platform of matched stack formula extension section, three the fixed arm encloses jointly and becomes the fixed chamber that is used for holding the incomplete end of femur.
The distal femur revision type tumor prosthesis preferably has a diameter of the central platform slightly larger than a diameter of the femoral stump so as to accommodate the femoral stump between the three fixing arms.
Preferably, each fixing arm is provided with a plurality of fixing screw holes at intervals along the axial direction.
The distal femur revision type tumor prosthesis preferably adapts to patients with different femur defect degrees by adjusting the length of the assembled extension section according to different bone defect lengths.
The distal femur revision type tumor prosthesis preferably has a porous metal bone trabecula structure with a pore size of 400-600 μm and complete communication prepared by a 3D printing technology on the upper surface of the central platform and the inner contact surface of each fixing arm contacting with the outer surface of the femur stump, wherein the porosity is more than 80%, and the pore layer thickness is 2 mm.
Due to the adoption of the technical scheme, the invention has the following advantages:
1. the invention can perfectly deal with the extreme conditions that the medullary cavity handle can not be taken out, bone cement can not be removed, bone is damaged and the like because the whole tumor prosthesis does not have components related to the medullary cavity. Even when the medullary cavity rod can be taken out, except the common condition of clean bone cement, the extramedullary fixing mode does not need the bone cement, thereby reducing the difficulty of subsequent revision.
2. According to the invention, the porous metal bone trabecular structure is prepared on the central platform and the inner contact surfaces of the fixing arms, which are contacted with the outer surface of the proximal end of the femur, by using a 3D printing technology, so that an ingrowth space is provided for the new bone tissue, the new bone tissue can crawl into the pore structure, and finally the prosthesis handle is integrated on the residual end of the femur, thereby greatly increasing the long-term stability and prolonging the service life of the tumor prosthesis.
3. The distal femur revision type tumor prosthesis provided by the invention is particularly suitable for the following situations: (1) the distal femoral prosthesis stem is broken, and the prosthesis stem is difficult to remove; (2) the remaining proximal femur bone is not long enough to implant the prosthetic stem; (3) poor quality of the remaining proximal femur; (4) the bone cement in the proximal medullary cavity of the remaining femur is difficult to remove or causes further damage to the remaining bone mass when removed.
