Abstract

本发明提供了一组用于评估造血干细胞移植病人CMV感染后免疫状态的多肽组合物,所述组合物包括如SEQ ID NO:1~383所示的氨基酸序列。本发明还提供了一种检测试剂盒,包括以下试剂:试剂1、试剂2、试剂3、试剂4、试剂7和试剂8;所述试剂1包含了本发明的多肽组合物。本试剂盒覆盖了主要的CMV T细胞表位,使得在以CMV特异性CTL作为预测指标评估造血干细胞移植病人CMV感染后免疫状态的方法诊断更加灵敏,覆盖率高,不受HLA亚型限制。

一种用于评估造血干细胞移植病人CMV感染后免疫状态的多 肽组合物以及检测试剂盒
技术领域
本发明属于医学检测技术领域,具体涉及一种用于评估造血干细胞移植病人CMV感染后免疫状态的多肽组合物以及检测试剂盒。
背景技术
目前现有技术中的评估造血干细胞移植后病人巨细胞病毒 (Cytomegalovirus,CMV)感染免疫状态方法包括以下几种:1)基于高通量测序,同时检测T细胞免疫组库,通过计算TCR可变区的数量来对T细胞多样性进行定量,从而评价细胞免疫力。2)单纯分析人体外周血CD4+T细胞数量,由此对患者免疫重建做出评估。3)通过检测血清中抗原CMV-pp65、抗体 CMV-IgG和CMV-IgM或病毒核酸CMV-DNA的含量评估患者免疫状态。
但是,上述方法存在以下缺点:1)需测定T细胞免疫组库,费时费力费钱。且TCR可变区数量的高低并不等同于T细胞重建的好坏。CMV作为慢性病毒,其T细胞应答有一个重要的特征,就是病毒特异性记忆性CD8+T细胞持续扩增并长期保持较大数量,称之为“记忆膨胀”现象。这些特异性T细胞是趋向于优势表位克隆集中还是非优势表位也在后期扩张,还存在争论,故并不能单纯用T细胞多样性来判断患者的免疫力。2)用CD4数量来评价不够准确,异基因造血干细胞移植(allo-HSCT)后CD4+T细胞恢复缓慢,CD4+初始T细胞重建至正常水平甚至需要20年以上的时间,其原因在于预处理、移植物抗宿主病 (GVHD)的发生及其防治可通过抑制胸腺输出功能或改变胸腺基质进而影响T 细胞的发育。3)血清CMV-IgG和IgM检测基本针对的是既往感染,且在免疫抑制状态下容易出现假阴性,使患者错失最佳治疗时机;现阶段应用广泛的 CMV-DNA检测,阳性阈值标准判别不一(500copies/mL,1000copies/mL),且检测阴性不一定代表病人体内没有病毒复制,检测阳性也不一定代表是活病毒;CMV-pp65抗原检测适用性也非常有限,ELISA检测特异性高,但灵敏度低,检出率较DNA检测低。
目前临床上最常用的CMV诊断方法是检测CMV-DNA,但该方法存在种种限制性,例如,血浆PCR阴性不代表体内没有病毒;PCR阳性也不能说明体内有活病毒。且不同医疗机构的PCR检测限和诊断标准也不尽相同(500 copies/mL或1000copies/mL)。CMV从体内最终清除只能依靠机体自身的免疫功能,固有免疫和适应性免疫系统都对CMV病毒的复制起到监控和清除作用。研究表明,NK细胞,B细胞,CD4+T细胞和CD8+T细胞都参与了针对CMV 的免疫过程。T细胞(CD8+T和CD4+T细胞)介导的细胞免疫是机体针对CMV 感染最重要的免疫机制。如果利用CMV急性感染期间由CMV再激活引起的病毒特异性T细胞应答作为检测指标,寻找合适的阈值,既能当作移植预后的免疫重建指标,又可以作为移植后CMV感染的诊断指标。
虽然病毒特异性细胞毒性T淋巴细胞(cytotoxic lymphocyte,CTL)在CMV 疾病诊断和治疗方面表现出巨大优势,但该方法在技术上还存在一定瓶颈,主要表现在不同人类淋巴细胞抗原(human lymphocyte antigen,HLA)背景病人的CMV抗原特异性CD8+T细胞针对的抗原多种多样,因此,如何针对不同HLA 背景的CMV感染病人,准确评价其细胞免疫力(含针对CMV的细胞免疫状态),是本领域要解决的技术问题。
发明内容
为解决上述技术问题,本发明提供了一组评估造血干细胞移植病人CMV感染后免疫状态的多肽组合物以及检测试剂盒。本试剂盒覆盖了大部分CD4和 CD8表位,使得在以CMV特异性CTL作为预测指标评估造血干细胞移植病人 CMV感染后免疫状态的方法诊断更加灵敏,覆盖率高,不受HLA亚型限制。
本发明采用以下技术方案实现本发明的目的:
第一个方面本发明提供了一组用于评估造血干细胞移植病人CMV感染后免疫状态的多肽组合物,所述组合物包括如SEQ ID NO:1~383所示的氨基酸序列。该多肽组合物覆盖了大部分CD4和CD8表位,可用于刺激造血干细胞移植病人产生更丰富的CMV特异性CTL,避免由于不同HLA背景病人的CMV抗原特异性CD8+T细胞针对的不同抗原,而不能准确地评估病人的细胞免疫力。从而实现提高以CMV特异性CTL作为预测指标的评估病人的细胞免疫力的准确性。
第二个方面本发明提供了一种检测试剂盒,包括以下试剂:试剂1、试剂2、试剂3、试剂4、试剂7和试剂8;所述试剂1为本发明多肽组合物制备而成的多肽混合试剂,其浓度为0.26μM~1.04μM;试剂2包括死细胞染色试剂、 Anti-Human CD3-FITC、Anti-human CD4-APC-H7和Anti-human CD8-Percp-Cy5.5;试剂3为Anti-human IFN-γ-APC;试剂4为保存浓度为 1000U/mL的hIL-2;试剂7为蛋白质转运抑制剂;试剂8为细胞固定或破膜溶液试剂。
优选地,所述试剂1包含上述如SEQ ID NO:1~383所示的氨基酸序列;其浓度为0.26-1.04μM,使用时的终浓度为125-500nM。试剂1覆盖了CMV的表面抗原,可刺激PBMC获得更丰富的T细胞亚群,使得评估结果更加准确。
优选地,所述试剂2使用时死细胞染色试剂(LIVE/DEAD-V500)的稀释倍数为1:1000;Anti-human CD3-FITC的稀释倍数为1:50;Anti-human CD4-APC-H7 的稀释倍数为1:100;Anti-human CD8-Percp-Cy5.5的稀释倍数为1:100。试剂2 为抗体表面抗体组合物,不同的抗体带有不同的标签,便于识别产生的不同表面抗原的T细胞。
优选地,所述试剂3使用时Anti human-IFN-γ-APC的稀释倍数为1:100。试剂3为胞内抗体,用于检测T细胞释放的胞内因子。
优选地,所述试剂盒还包括C-RPMI溶液,所述C-RPMI溶液为含有质量浓度为10%的胎牛血清、1%的青霉素和1%的链霉素的RPMI 1640溶液。
第三个方面本发明提供了本发明多肽组合物在制备用于评估造血干细胞移植病人CMV感染后免疫状态的试剂盒中的应用。
第四个方面本发明提供了一种本发明所述的检测试剂盒在用于评估造血干细胞移植病人CMV感染后免疫状态中的应用。
本发明的有益效果为:本发明根据CMV的抗原蛋白构建了一系列交叠肽段,这些肽段覆盖了主要的CMV T细胞表位,能够刺激T细胞应答,解决了由于不同HLA背景的CMV感染病人不能准确评价其细胞免疫力的问题。本发明的试剂盒包含了本发明的抗原多肽组合,以及能够检测细胞免疫力的多种试剂,可准确的检测出PBMC经抗原多肽组合物刺激后产生的病毒特异性T细胞亚群及其释放的胞内因子,结合以CMV特异性CTL作为预测指标的计算公式以及阈值,可精准地评估造血干细胞移植病人CMV感染后免疫状态,进而指导病人的用药进程。

为了更加简洁明了的展示本发明的技术方案、目的和优点,下面结合具体实施例详细说明本发明的技术方案。如无特殊说明,本发明实施例中所涉及的试剂均为市售产品,均可以通过商业渠道购买获得。
实施例
1、材料和仪器
胎牛血清(Certified Fetal Bovine Serum,FBS);Ficoll-Paque PLUS淋巴细胞分离液;淋巴细胞分离管(LeucosepTM tube);RPMI 1640培养基、青霉素-链霉素溶液(Penicillin-Streptomycin)和DPBS缓冲液;二甲基亚砜(dimethyl sulfoxide,DMSO)、佛波醇酯(phorbol myristate acetate,PMA)和人白细胞介素 -2(hIL-2);Ionomycin Ca2+Salt;CMV病毒蛋白多肽;细胞计数板;96孔透明圆底微孔板(96-well Clear Round BottomTC-treated Microplate,Individually Wrapped,with Lid,Sterile)、96孔透明平底微孔板(96-well Clear Flat Bottom TC-treated Microplate,Individually Wrapped,withLid,Sterile)和5ml圆底聚苯乙烯试管(

2、前期准备
制备试剂1:构建CMV病毒蛋白多肽文库
选取AD169毒株的即刻早期蛋白IE-1、IE-2及磷酸化蛋白pp65,这些蛋白已被证明可覆盖大部分CMV T细胞免疫表位。按照蛋白序列,设计交叠肽文库 (每条多肽含有15个氨基酸,且多肽间有11个氨基酸的重叠)。此多肽文库共含有383条多肽,多肽用DMSO溶解,保存浓度为20mM,作为多肽母液,保存温度为-80℃。使用时,以含10%FBS、1%Penicillin-Streptomycin的RPMI 1640溶液(C-RPMI)将多肽DMSO母液稀释至浓度为0.26μM~1.04μM,此时将稀释好的383条多肽混合到一起,-20℃保存备用(试剂1)。所述多肽文库中的多肽序列如表1所示:

表面染色抗体试剂组合作为本发明的试剂2:包括活死细胞染色试剂、 Anti-Human CD3-FITC、Anti-human CD4 APC-H7、Anti-human CD8-Percp-Cy5.5。使用时,按括号内相应比例,稀释于含2%FBS的RPMI 1640溶液(每孔需50μl,现配现用)LIVE/DEAD-V500(1:1000)、Anti-human CD3 FITC(1:50)、Anti-human CD4 APC-H7(1:100)、Anti-humanCD8 Percp-Cy5.5(1:100)。本实施例中试剂2 中各试剂的信息如下表2:

胞内抗体试剂作为本发明的试剂3:包括Anti-human IFN-γ-APC。使用时,按括号内比例,稀释于1×Perm Wash溶液(每孔需50μl,现配现用),Anti-human IFN-γ-APC(1:100)。
试剂4:hIL-2(保存浓度1000U/mL,保存温度-20℃)现配现用;
试剂5:PMA(保存浓度100ng/mL,保存温度-20℃);
试剂6:Innomycin Ca2+(保存浓度100μg/mL,保存温度-20℃);
试剂7:蛋白质转运抑制剂(购自BD,商品名:GolgiPlugTM protein transportinhibitor containing brefeldin A);
试剂8:细胞固定/破膜溶液试剂(购自BD商品名:Cytofix/CytopermTM Fixation/Permeabilization Solution Kit)。
2、实验方法
(1)将新鲜分离或复苏的外周血单个核细胞(PBMCs)重悬于1mL C-RPMI 溶液,台盼蓝染色,活细胞计数。调整细胞数为5×105cells,用100μL C-RPMI 溶液重悬,将细胞均匀铺于96孔透明圆底微孔板中,并分别设置阴性对照孔、阳性对照孔、检测孔。
(2)阴性对照孔、阳性对照孔、检测孔中试剂的添加信息如下表3、4:

上述试剂1中的浓度为范围值表示,在该浓度范围内试剂1均能实现本发明的技术效果。
(3)按照步骤(2)的实验设计每孔加入相应的试剂后,在各组中加入试剂7(2μL/mL),然后轻轻晃动96孔透明圆底微孔板,随后将其转移至37℃, 5%CO2的细胞培养箱孵育16小时。
3、检测T细胞亚群及胞内因子释放
3.1检测T细胞亚群:PBMCs被刺激16小时后,将96孔圆底微孔板转移至离心机中,500×g,4℃离心,弃去上清液后重悬细胞于50μL表面抗体试剂 2中,4℃避光孵育30min。然后用含2%FBS的RPMI 1640溶液洗涤两次。
3.2检测T细胞胞内因子释放:对细胞进行表面染色固定及细胞胞膜破膜处理,每孔加入100μL BD固定/破膜液(试剂8),4℃避光孵育20min。用1×Perm Wash溶液洗涤两次。然后500×g,4℃离心5min,弃去上清液后重悬细胞于试剂3中,4℃避光孵育30min后。用1×Perm Wash溶液洗涤两次。以200μL 含2%FBS的RPMI 1640溶液充分重悬细胞,并将细胞悬液经70μm滤网过滤后转移至5mL带盖流式管中。
3.3计算结果:BD FACS ARIA III上机检测,通过Flowjo软件对流式数据进行分析,以及进行CMV特异性CTL计算。计算方法如下:
IFN-γ+CD8+T细胞数(即CMV特异性CTL)=106PBMC×淋巴细胞比例 (lymphocyte)×单细胞比例(single cell)×活细胞比例(LIVE/DEAD阴性) ×CD3+T细胞比例×CD8+T细胞比例(CD8阳性/CD4阴性)×IFN-g+CD8+T细胞比例。
结果判别:阳性孔有大量IFN-γ+CD8+T细胞(10%以上),阴性孔应没有或有极少量IFN-g+CD8+T细胞(小于0.05%),检测阈值为925个IFN-γ+CD8+T 细胞(每106PBMC)(检测孔减去阴性对照孔),高于此阈值,表示病人安全,此处没有检测孔获得的IFN-γ+CD8+T细胞数。
将获得的检测结果数据和数学模型结合,建立allo-HSCT后90天内(CMV 急性感染期)CMV病毒复制及IFN-γ+CD8+T细胞免疫应答规律模型(图1)。已通过ROC曲线分析,与目前主流CMV病毒检测方法(qPCR)进行对比,并显示出优越性(图2)。
诊断标准为:当CMV-CTL低于925个/106PBMC时,CMV病毒血症风险较高;当CMV-CTL高于925个/106PBMC时,CMV病毒血症风险较低。
应用实例
采用本发明的试剂盒并结合本发明的模型针对21个患者不同HLA型(患者信息如下表5)以及不同CMV-DNA检测结果(阳性和阴性)的病人检测其免疫水平,并指导其后期是否使用抗病毒药物,结果如下:
如图3所示,CMV-DNA检测呈阴性,IFN-γ+CD8+T细胞数也达到或超过阈值(即925个IFN-γ+CD8+T细胞/每106PBMC),表示其免疫力已达到病毒控制水平,针对这类病人可停止用药;如图4所示,CMV-DNA检测呈阳性,但IFN-γ+CD8+T细胞数达到或超过阈值,表示其免疫力已达到病毒控制水平,针对这类病人,即使停止抗病毒药物,病毒也会自行在2-9天内清除,因此,也可停止用药,同时应继续监测其免疫力;如图5所示,CMV-DNA检测阴性,但IFN-γ+CD8+T细胞数未达到阈值,表明其免疫力未达到病毒控制水平的病人,针对这部分病人应坚持用药,等待免疫力恢复,切不可因qPCR阴性结果而停止辅助性抗病毒药物,造成反复病毒血症,加大CMV疾病发生率。可见,本发明的试剂盒结合所述模型,依据阈值可以明确诊断,精确指导病人科学用药。
免疫抑制的allo-HSCT患者的CMV特异性细胞毒性T淋巴细胞(CTL)的反应水平与CMV病毒复制水平呈负相关。本发明利用CMV急性感染期间CMV 再激活引起的病毒特异性T细胞应答作为检测指标,寻找合适的阈值,既能当作移植预后的免疫重建指标,又可以作为移植后CMV感染的诊断指标。本试剂盒通过抗原对细胞的刺激,分析IFN-γ+CD8+T淋巴细胞经抗原刺激后细胞因子释放情况,来评估移植后CMV感染病人的免疫能力。本发明试剂盒结合计算公式,诊断灵敏,覆盖率高,不受HLA亚型限制。此外,也可对病毒特异性T细胞进行体外扩增,适合在造血干细胞移植后引起的机会性感染疾病中应用,同时可向危害免疫系统的疾病或者旨在重建免疫系统的治疗效果评价方面推广。

Claims (8)

1.一组用于评估造血干细胞移植病人CMV感染后免疫状态的多肽组合物,其特征在于,所述组合物包括如SEQ ID NO:1~383所示的氨基酸序列。
2.一种检测试剂盒,其特征在于,包括以下试剂:试剂1、试剂2、试剂3、试剂4、试剂7和试剂8;所述试剂1包含权利要求1所述的如SEQ ID NO:1~383所示的氨基酸序列,其浓度为0.26μM~1.04μM;试剂2包括死细胞染色试剂、Anti-Human CD3-FITC、Anti-human CD4-APC-H7和Anti-humanCD8-Percp-Cy5.5;试剂3为Anti-human IFN-γ-APC;试剂4为保存浓度为1000U/mL的hIL-2;试剂7为蛋白质转运抑制剂;试剂8为细胞固定和破膜溶液试剂。
3.如权利要求2所述的检测试剂盒,其特征在于,所述试剂1的浓度为0.26-1.04μM,使用时的终浓度为125-500nM。
4.如权利要求2所述的检测试剂盒,其特征在于,所述试剂2使用时死细胞染色试剂的稀释倍数为1:1000;Anti human CD3-FITC的稀释倍数为1:50;Anti human CD4-APC-H7的稀释倍数为1:100;Anti human CD8-Percp-Cy5.5的稀释倍数为1:100。
5.如权利要求2所述的检测试剂盒,其特征在于,所述试剂3使用时Anti-human IFN-γ-APC的稀释倍数为1:100。
6.如权利要求2所述的检测试剂盒,其特征在于,所述试剂盒还包括C-RPMI溶液,所述C-RPMI溶液为含有质量浓度为10%的胎牛血清、1%的青霉素和1%的链霉素的RPMI 1640溶液。
7.如权利要求1所述的多肽组合物在制备用于评估造血干细胞移植病人CMV感染后免疫状态的试剂盒中的应用。
8.如权利要求2所述的检测试剂盒在用于评估造血干细胞移植病人CMV感染后免疫状态中的应用。
