Abstract
一种基于全自动三维成像的听神经瘤手术路径自动规划方法,包括以下步骤:1)获取患者CT、T1、T2、T1增强、MRA、diffusion MRI序列图像等,经过去噪校正并配准到同一坐标系内;2)利用深度学习及神经纤维图谱实现肿瘤提取、区域分割及纤维跟踪和自动识别;3)体素的风险值估计;4)候选路径生成及路径风险值估计;5)路径优化及风险图谱的生成。本发明有助于减少术前准备时间、降低手术风险、缓解医生手术压力。
Description
技术领域
本发明涉及计算机技术领域,具体涉及一种基于全自动三维成像的听神经瘤手术路径自动规划方法。
背景技术
如今,基于术前CT/MRI扫描的脑内探针,针头,导管或电极精确靶向肿瘤,病变和解剖结构已成为立体定向神经外科的标准手术任务。常见的立体定向神经外科手术可能包括立体定向脑活检,脑积水的治疗,深部脑血肿的抽吸和排空,Ommay导管插入,深部脑刺激,用于癫痫的深度电极插入,内窥镜脑室内手术。在所有手术中,错误地放置手术工具都可能导致不良结果,例如治疗无效,颅内出血并发症,尽管其发生率较低,但还是可能会带来严重危害神经系统后遗症以及暂时或永久性神经系统疾病的高风险损坏。不正确的手术工具放置也可能需要额外的手术程序才能达到预期的治疗效果和治疗随后的并发症。此类修正手术有时是开放式程序,可能会伴随着大量额外的风险,时间和发病率。
规划安全的手术轨迹,以最大程度地减少不正确的手术工具放置的预期风险及其可能的不良后果,具有重大的现实意义。当前,图像引导立体定向神经外科手术的术前轨迹规划通常涉及使用神经导航系统或立体定向框架来选择术前CT/MRI头部扫描的2D轴向,矢状或冠状横截面上的目标结构。这项手动的术前计划任务可能很复杂,耗时且取决于用户和患者。这要求外科医生在心理上识别和重建复杂的3D结构,包括血管,雄辩的脑部区域和心室系统,并得出这些结构与手术轨迹之间的空间关系。在某些情况下,根据经验,在手术本身的过程中,要在无菌条件下和压力下凭经验对手术计划进行手动校正和优化,而无法彻底,定量地分析最佳手术轨迹。实际上,即使对于有经验的神经外科医师而言,预期治疗风险的评估也因此是一项复杂,具有挑战性且耗时的任务。

为了克服现有技术的不足,本发明提供一种基于全自动三维成像的听神经瘤手术路径自动规划方法
为了解决上述技术问题本发明使用的技术方案如下:
一种基于全自动三维成像的听神经瘤手术路径自动规划方法,包括以下步骤:
步骤1、图像获取及预处理:获取患者CT、T1、T2、T1增强、MRA、diffusion MRI序列图像,经过去噪校正;
步骤2、图像配准:以CT图像为基准,将其他图像配准到CT图像中,得到各个图像的转换矩阵;
步骤3、肿瘤提取及区域分割:利用步骤1获得的数据,采用自动化的方法对听神经瘤进行提取,以及分割大脑内部的功能区、颅底血管及颅骨区域;
步骤4、纤维跟踪及自动识别:利用神经纤维图谱,对纤维进行跟踪及自动标识;
步骤5、数据融合:根据重建前的转换矩阵,将各个组织的三维模型转换到CT图像的坐标下;
步骤6、体素风险值估计:体素风险值的意义是量化表示当前位置体素所对应的解剖位置受到破坏时,对人体造成损伤的严重程度,风险值大小不仅由硬约束决定,而且受软约束影响;
步骤7、候选路径生成及风险估计:候选路径可以表示为由目标点和待进入点组成的一条具有直径的线段,整条路径的风险值由其所包围的体素风险值决定;
步骤8、路径优化及风险图谱生成:通过快速排序算法对所有待进入点代表的路径进行空间搜索,找到风险值最小的三条路径作为本方案规划的手术路径,同时将路径风险值归一化到0~255的范围,进行颜色编码,生成路径的风险图谱供医生参考。
进一步,所述步骤3中,利用深度学习,采用T1、T2和T1增强的数据实现听神经肿瘤的精确分割,并将肿瘤的质心位置作为手术路径规划的目标位置,即路径的目标点,大脑内部功能区的划分采用Freesurfer实现,颅底血管及颅骨利用深度学习的方法进行识别分割。
所述步骤4中,纤维跟踪及表示通过基于深度网络的神经纤维图谱重建实现,包括基于深度高阶张量成像模型的体素纤维方向估计、基于流线微分方程的全局纤维重建算法和数据驱动神经纤维图谱重建算法。
所述步骤6中,体素风险值估计包括软约束部分和硬约束部分,所述硬约束包括手术高危组织区域的约束,对高危区域如脑室、脑部血管距离小于安全距离的体素,通过赋予极大的风险值将其设为路径禁区;软约束是除硬约束以外的约束,由离功能区、神经纤维次重要的组织的距离影响,同时还由组织的重要程度影响,由以下公式计算:
