Description

Central venous puncture device
Technical Field
The invention belongs to the technical field of medical instruments, and particularly relates to a central venous puncture outfit.
Background
The central vein puncture catheterization technology is a clinical commonly used diagnosis and treatment means, and is widely applied to operative anesthesia, circulation function and treatment, interventional examination and treatment of critical patients with hemodynamics instability, hemodynamics monitoring of patients with low-temperature and controlled blood pressure reduction, chemotherapy infusion passages of tumor patients with difficulty in peripheral blood vessel puncture, venous hyperalimentation treatment, rescue treatment of critical patients and the like. At present, the main apparatus used for central venipuncture catheterization is a central venipuncture suite, and generally comprises a central venipuncture needle, a guide steel wire, an expander and a central venous catheter. The operation steps are as follows:
firstly, after the puncture needle penetrates into the central vein, the guide steel wire is placed firstly, and then the puncture needle is withdrawn; the skin is dilated by a skin dilator and then placed into the central venous catheter along the guide wire.
The disadvantages of this method are: if the guide steel wire is placed along the puncture needle for too long time, the guide steel wire can touch the heart, and arrhythmia is possibly caused; when the puncture personnel have poor skills, the guide steel wire is bent, the catheter is difficult to insert, and the puncture fails.
In addition, after the central vein of the existing puncture needle is punctured successfully, a guide steel wire needs to be placed into the central vein along the puncture needle cavity through the side hole of the central vein puncture needle. Although effective in placing the guidewire into the central vein, the guidewire must pass through a blood-filled needle lumen, which is about 10cm in length, during placement. The operation difficulty is higher, and the requirement on the technical skill of an operator is high. Particularly, when the mutual matching between patients or operators is unstable, the central vein or the puncture needle of the patients is easy to move, so that the puncture needle tip is deviated and departed from the central vein, thereby leading the failure of the guide steel wire to be placed into the central vein and bringing pain and discomfort to the patients.

In addition, the preset conduit 3 and the tube body 10 of the injector 1 are provided with scales. The detention position of the preset catheter can be conveniently controlled by means of the structure. When the preset catheter enters a preset scale mark on the preset catheter (12 cm-15 cm for adult).
In an embodiment of the present invention, as shown in fig. 1 and 2, a sealed cavity surrounded by the preset conduit 3, the housing 4 and the partition board can be filled with heparin water for soaking the preset conduit 3; an opening matched with the preset conduits 6 and 3 is formed in the end part of the shell 4, and a plugging component sleeved outside the preset conduit 3 is arranged at the opening; as shown in fig. 7, the plugging component is a split structure, and includes a plug 18 and an external clamp 17 thereof, the plug 18 and the clamp 17 are both sleeved outside the preset catheter 3, the lateral surfaces of the plug 118 and the clamp 17 are both provided with longitudinal slits, and the slits of the plug 118 and the slit of the clamp 17 are staggered; the split of clamp links to each other through the brace, the both sides of brace are equipped with the indentation, but brace and clamp one shot forming during the preparation. When the device is used, the tilting part at one end of the light pulling strip can be easily torn off, the hoop and the blockage can be conveniently stripped from the preset guide pipe, and then the preset guide pipe can be pulled out from the shell along the opening.
In one embodiment of the present invention, as shown in fig. 4 and 5, two openings 8 for penetrating the internal channel are correspondingly formed at the tip of the puncture part 23 of the puncture needle 2. By adopting the structure, the preset catheter can be conveniently and smoothly sleeved on the puncture needle, and the side wall of the preset catheter is prevented from being scratched. In particular, the shape of the opening can be circular, oval or other shapes. The specific shape of the opening is designed according to actual needs, and the specific shape is not limited herein.
The application process of the invention is as follows:
the hand-held puncture outfit inserts the end puncture part 23 needle of the puncture needle 2 into the preset vein to withdraw blood, a dermatotome is used for cutting the outer skin, the left hand holds the shell 4 of the puncture outfit to stabilize the direction, the right hand rotates the rear part to rotate the knob 6, the knob drives the screw rod 5 to rotate, further drives the screw nut 12 to move along the screw rod, the rotating wheel 7 which is meshed with the screw nut 12 drives the preset catheter 3 to move forwards in a rotating mode, and thus the preset catheter 3 rotates to enter the skin by taking the straight needle tube section 21 needle bar of the puncture needle as a guide and is placed into the vein.
Wherein: after the puncture needle 2 is inserted into the preset vein, the blood needs to be drawn back for confirmation. In the operation, blood is pumped through the syringe 1, the shell 4 is held by the left hand, the groove 15 on the sliding block 13 is pressed by the thumb, the sliding block 13 is easily pushed and pulled, the piston rod 11 of the syringe 1 is driven to translate, and the blood pumping operation can be completed by one hand in a stable state.
When the preset catheter enters a preset scale mark (12-15 cm for an adult or is kept to a proper depth required by operation), the left hand holds the skin external preset catheter, and the right hand withdraws the puncture outfit backwards to complete catheterization. The catheter is preset and then the conventional operation is carried out: an infusion connecting set is screwed at a joint at the tail part of the preset catheter (the infusion connecting set can be selected from the existing disposable invasive pressure sensor or a disposable sterile cock, and is provided with two or more pipe joints of a main pipe and an auxiliary pipe, so that subsequent operations such as infusion and the like are facilitated), an exhaust flushing pipe, a seam cover fixing and the like are arranged. The infusion connection kit can be a disposable invasive pressure monitoring sensor or a common infusion kit, and the product and the conventional operation belong to the prior art and are not described in detail herein.
The special-shaped needle tube section of the invention adopts an S-shaped connecting injector, can withdraw blood at any time to determine whether the preset catheter is in the blood vessel, and completes the tube placement by combining the alternate operation of rotating the tube, so that doctors and nurses can grasp and control the operation process of tube placement more easily. After the guide wire is placed in the original puncture operation process, whether the entering position is changed or not cannot be detected, and the shaking in the puncture tube placing process can possibly cause the needle head to deviate from the blood vessel.
The puncture outfit is particularly important for the use of emergency treatment and ICU departments, precious treatment time is won for patients, and the importance of the puncture outfit can be reflected in the unconventional environment of 120-visit (for example, a rescue site cannot be operated aseptically, and a large amount of fluid infusion treatment is needed).
In the description above, numerous specific details are set forth in order to provide a thorough understanding of the present invention, but the present invention may be practiced in other ways than those specifically described and will be readily apparent to those of ordinary skill in the art without departing from the spirit of the present invention, and thus the present invention is not limited to the specific embodiments disclosed above.
