Description

Wire arranging tool and method for surgical suture
Technical Field
The invention belongs to the technical field of medical instruments, and relates to a wire arranging tool and a wire arranging method for flat wires/round flat wires for operations.
Background
Surgical sutures are mainly of the round, flat or round-flat type. The round wire is a cylinder of the whole suture line, the suture line is not easy to be knotted and wound, the round wire can enter human tissues for suturing more conveniently, the suture line is in line contact with the human tissues, and a certain cutting effect is achieved; the round flat wire is a suture line with round wires at two ends and a flat wire in the middle; the flat wire is a rectangular section of the whole wire segment, and the flat wire or the round flat wire is in surface contact with human tissues in the surgical suture process, so that the cutting effect of the suture on a suture area can be greatly reduced; compared with a round wire, the flat wire or the round flat wire can provide a larger footprint contact surface (namely, the footprint surface, and a larger contact area with an area needing to be sutured), especially the cutting effect of the sutured part in operations such as meniscus suture and shoulder joint is reduced, and meanwhile, the larger footprint surface is also beneficial to wound healing. However, due to the influence of the shape of the flat wire, the flat wire section is easy to be knotted and wound, the difficulty of wire arrangement and suture is increased, the operation efficiency is greatly influenced, and the advantage of large footprint surface cannot be exerted. At present, for round flat wires/flat wires, doctors can manually finish the round flat wires/flat wires by using a feeler (as shown in fig. 1), mainly use the feeler to hook the flat wires and then finish the round flat wires by slowly straightening and the like, and the mode causes long operation time and low finishing efficiency. And a special tool for arranging the round flat wire/the flat wire is not available.
Therefore, the development of a special tool and a method for arranging the flat round wires/the flat wires, which can be used quickly, efficiently and conveniently, is of great significance.
Disclosure of Invention
The invention aims to solve the problems in the prior art and provides a wire arranging tool and a wire arranging method for flat wires/round flat wires for operation.
In order to achieve the purpose, the invention adopts the following scheme:
a surgical suture line wire arranging tool is a flat wire or a round flat wire and mainly comprises a wire pressing plate and a handle connected with the wire pressing plate;
the pressing plate is of a strip-shaped structure, one side surface of the strip-shaped structure is a surface which can be attached to one footprint surface of a suture line and is marked as an attaching surface, and an inlet groove for the surgical suture line to pass through is formed in the attaching surface;
the handle is provided with a flat groove for the surgical suture to pass through, the flat groove is a strip-shaped groove, and the slotting direction of the flat groove is consistent with the length direction of the wire pressing plate;
the side walls of the inlet groove and the flat opening groove are attached to two footprint surfaces of the surgical suture, and the distance between the two sides of the inlet groove and the flat opening groove, which are attached to the surgical suture, is matched with the thickness of a thread belt of the surgical suture to keep the surgical suture to slide in the groove and not to curl.
The correction of the flat line segment by the inlet slot realizes that the flat line segment is not curled or knotted after entering the flat slot. The thickness of the unfinished flat line section is increased compared with the finished state, and only the finished state can be entered according to the matching of the height of the flat notch groove and the thickness of the flat line section. In the flat groove, the shape of the flat wire is fixed (the shape cannot be deformed due to thickness limitation), and the head of the flat wire is hardened, so that the flat wire can easily enter the flat groove and can easily enter the flat groove without deformation.
The head of the flat round wire is round, and the tool can be used for finishing. Only the round wire section needs to be passed through the handle. In the actual operation process, the round head is only used for threading, and the round wire segment is not used for suturing (otherwise, the advantage of no flat wire is exerted), so that the round head can be directly cut off by using the wire arranging tool, or the round wire segment is directly skipped over, and the flat wire segment is clamped into the wire arranging tool from the side opening.
As a preferred technical scheme:
according to the wire arranging tool for the surgical suture, the inlet groove and the flat groove are both side-opening type grooves, so that the surgical suture can be inserted into or taken out from the side (if the wire arranging effect is poor due to uncontrollable factors, the surgical suture can be directly taken out from the side for replacing or rearranging the wire); the side surface is a surface perpendicular to the thickness direction of the surgical suture. In order to further facilitate threading, the notch is designed to be open at one side, so that the operation suture can be plugged in or taken out from the opening. If the bundling is unsuccessful in the wire arranging process, the wire can be directly taken out from the side opening; meanwhile, the round flat wire can be directly plugged from the side opening.
According to the wire arranging tool for the surgical suture, the flat slot is positioned in the handle and is integrally formed with the handle; the junction of line ball board and handle is the entry in flat mouthful groove, the other end of handle (promptly with the one end that the junction is relative) is the export in flat mouthful groove, and a side of flat mouthful groove and the laminating of operation stylolite and line ball board binding face coplane, another side and the entry groove of flat mouthful groove and the laminating of operation stylolite are located same one side of binding face.
In the thread arranging tool for the surgical suture thread, the inlet groove is a U-shaped structure groove, the cross section of the U-shaped structure groove is in a U shape as a whole, two opposite sides of the U shape are parallel to each other and are in a straight line or a curve, and one of two opposite side surfaces on the U-shaped structure groove is coplanar with the binding surface (the position on the binding surface where the inlet groove is not arranged can be a plane or a curved surface, so long as the inlet groove can be bound with one footprint surface of the suture thread);
the number of the inlet grooves is n, and n is more than or equal to 2; the inlet slot closest to the flat slot is marked as the nth inlet slot, and the inlet slot farthest from the flat slot is marked as the first inlet slot; the operation suture can be threaded in sequence in a straight line and in a non-overturning state according to the serial number of the inlet slot. The cross-section of the inlet slot may be, but is not limited to, rectangular or curvilinear (e.g., a combination of rounded and arcuate shapes).
According to the wire arranging tool for the surgical suture, the inlets of the inlet grooves and the inlets of the flat-mouth grooves are designed into oblique angle structures; the oblique angle structure is formed by inclining the entrance of each entrance groove and each flat groove along the entering direction of the surgical suture (namely, along the thickness direction of the surgical suture, one side attached to the surgical suture faces away from the surgical suture) so as to increase the distance between the two sides attached to the surgical suture on the entrance grooves and the flat grooves; the inclination angle is reduced along with the increase of the serial number of the inlet groove until the inclination angle of the flat mouth groove is 0-5 degrees. Different notch incoming line angle designs (entrance oblique angle angles) are provided for different incoming line positions, so that surgical suture lines can be conveniently preassembled to enter the line arranging tool, and a line passing groove is formed in the handle end of the line arranging tool. For example, if the first inlet slot is provided with a 75-degree bevel, then the flat wire can be easily inserted into the arranging tool, the second bevel is provided with 60 degrees, the third bevel is provided with 45 degrees, the third bevel is provided with 30 degrees … degrees, and finally the bevel reaching the notch of the wire passing slot is provided within 0 to 5 degrees, so that the wire entering the wire passing slot is ensured to be arranged well.
According to the wire arranging tool for the surgical suture line, after the wire arranging tool passes through the bevel structure of the flat opening groove, the ratio of the distance between the two sides of the flat opening groove attached to the surgical suture line and the thickness of a wire belt of the surgical suture line is 1: 1-1.3. So that the flat wire section can conveniently enter the notch without deforming in the notch.
According to the wire arranging tool for the surgical suture, the handle is internally provided with the wire passing groove which has the same structure with the flat mouth groove and is positioned on the same horizontal plane, the joint of the wire pressing plate and the handle is an inlet of the wire passing groove, and a single roller or double rollers for pressing the surgical suture are arranged between the wire passing groove and the flat mouth groove; the single roller or the double rollers are fixed on the handle through the roller pin (namely, the roller is clamped on the handle through a pin). The cross section of the wire passing groove after the entrance is rectangular or curved (such as a combination shape of arc arch and the like). The single roller is matched with the flat groove to realize compression, and the double rollers are meshed with the upper roller and the lower roller to realize compression. Compression refers to placing the surgical suture in tension. The tough material of the roller presses the operation suture line tightly, and in the operation process, the doctor can tighten the other end to tighten the whole operation suture line. The tightening effect can make the line segment have better beam type effect (the taut operation suture line is difficult to take place to warp), and single gyro wheel or two gyro wheels control length when crossing the line simultaneously improve and cross line efficiency. After the roller compresses the wire, the roller is pushed to realize the wire inlet and outlet functions.
According to the wire arranging tool for the surgical suture, a layer of tough material (such as rubber, and the tough material is convenient for pressing the wire) is wrapped on the outer side of the single roller or the double rollers; the surface of the tough material, which is contacted with the operation suture, is provided with a bulge (the efficiency of the thread passing can be improved, and the thread passing skid can be avoided).
According to the wire arranging tool for the surgical suture, the length of the wire passing groove is 100 mm-200 mm, and is specifically selected according to the length required by a surgery.
The invention also provides a method for arranging the surgical suture by using the thread arranging tool for the surgical suture, which is characterized in that one footprint surface of the surgical suture is attached to the attaching surface of the thread pressing plate, and the surgical suture is sequentially inserted into the inlet groove, then is inserted into the flat-mouth groove along the attaching surface of the thread pressing plate and is pulled out of the flat-mouth groove, and thus the arrangement of the complete surgical suture is realized.
The mechanism of the invention is as follows:
because the flat wire which is not in a tight state is easy to deform, the side surface of the flat wire can be deformed when the flat wire passes through soft tissues in actual use, and the flat wire is not easy to recover after being deformed. The flat wire can be threaded into the tool by the wire arranging tool for arranging at the moment, the doctor pulls the suture line at the other end, and then the roller is pushed to cover the part needing to be sutured with the arranged wire in the flat opening groove.
According to the invention, the round flat wire/flat wire harness is designed and attached to a harness plane of a wire arranging tool (playing a role in limiting and keeping a suture line in a tight state to a certain extent) through the inlet groove, the wire passing groove and the flat groove, the inlet groove at the front end of the handle of the wire arranging tool continuously corrects the state of the flat wire, the wire passing groove is arranged at the head end of the handle, the height of the groove is matched with the thickness of the flat wire, and the flat wire is ensured not to curl or knot after entering the flat groove; the roller design can ensure the smooth wire passing of the flat wire section and ensure the flat wire to keep a well-organized state in the wire passing process; in the actual operation process, the doctor can pull the flat round wire/flat wire on the opposite side while operating the roller to lead out the wire, so that the well-organized flat wire segment can cover the area needing to be sutured.
