the doctor end 2 logs in/registers the system, acquires the entrusting information of the patient and selects whether to accept the entrusting; after accepting the entrustment of the patient, setting a threshold value of a key physical sign index authorized by the patient;
the health information evaluation module 23 is in data connection with the threshold setting module 22, checks the information authorized by the patient end 1 in real time, and provides health information evaluation guidance for the patient; issuing health consultation information to the patient end 1 for a terminal operator of the patient end 1 to check; and providing a platform for communication between the doctor and the patient;
the management end 3 is used for managing the data of the system, is in data connection with an external doctor identity data platform and verifies the doctor identity information input by the doctor end 2; classifying according to personal information of doctors, classifying according to various information of the doctors and setting priority parameter levels for the doctor end 2/the patient end 1 to carry out priority screening; and storing the disease knowledge base updated in real time for the patient end 1 to query.

the intelligent recommendation engine module (14) is in data connection with the management terminal (3) and the patient personal information storage module (12) and is used for analyzing the personal information input by the patient and intelligently matching doctors;

a delegation information receiving module (21) for acquiring delegation information of a patient and selecting whether to accept the delegation;
the threshold setting module (22) is in data connection with the delegation information receiving module (21) and is used for setting the threshold of the key sign index authorized by the patient after accepting the delegation of the patient;
the health information evaluation module (23) is in data connection with the threshold setting module (22) and is used for checking in real time according to information authorized by the patient end (1) and providing health information evaluation guidance for the patient;
the health consultation information publishing module (24) is used for publishing the health consultation information to the patient end (1) for a terminal operator of the patient end (1) to check, can set and authorize the level, and can be published or published aiming at authorized patients; and the number of the first and second groups,

A disease management platform based on personal authorization according to claim 1, wherein: the information acquisition module preset by the patient end (1) comprises patient basic information, patient physical sign information and medical information; the patient basic information comprises a real name, gender, month of birth and geographic location; the patient sign information comprises height, weight, blood type, blood pressure and pulse rate; the medical information comprises allergy history, operation history, blood transfusion history, smoking history and drinking history.

A disease management platform based on personal authorization according to claim 2, wherein: the information acquisition module preset by the patient end (1) also comprises physical condition evaluation information, a patient can acquire personal information through internet of things equipment such as an intelligent bracelet and home function equipment and upload the personal information to the system in real time, an authorized doctor can monitor key indexes in real time, and if the indexes are abnormal, real-time early warning can be realized to inform the patient in time; meanwhile, the background can also automatically analyze the data of the key indexes, and by combining with the threshold set by the doctor, when the parameters of the key indexes exceed the range of the threshold, or the threshold is in the rising or falling trend and gradually approaches the threshold boundary value, warning information is sent to the patient end (1) and the doctor end (2) in advance.
