背景技术

眼内望远镜试图通过赋予眼内放大倍率的光学优势并消除对手眼协调的需求来利用手持放大镜的优势。与标准IOL相比,此类装置通常相对较大且复杂,并且更难于安全植入。取决于放大倍数,眼内望远镜的主要缺点是导致周边视野减小。对于一些装置,周边视野可能会受到限制,使得只能在一只眼睛中进行植入。眼内望远镜还将有限量的光分散在视网膜的较宽区域上,因此不可避免地会降低所得图像的对比度。因此,眼内望远镜和棱镜装置都可能损害患者应对中心视野缺损的自然机制,因此会影响视觉功能,因为对比敏感度和注视稳定性都与阅读能力相关。类似地,如果仅将装置植入一只眼睛中,则很可能会破坏阅读功能,因为这会损害双眼总和并且影响在阅读期间发生的黄斑上的图像扫描。

发明内容

实施方式的IOL在黄斑疾病的治疗中提供了优于现有的眼内望远镜的明显优势。相比之下,诸如植入式微型望远镜(IMT)和视力障碍者人工晶状体(IOL-VipTM)之类的眼内望远镜是昂贵且复杂的装置,相对而言,因需要眼睛中的大切口、视野缩小、有角膜代偿失调的风险以及需要术后视觉康复11,12,其风险收益特性变得不那么吸引人。

具体实施方式
标准单焦点IOL为中央凹提供聚焦图像。在干性AMD患者中,中央GA通常会导致中央凹处的功能性视力丧失。然而,如果患者能够偏心地注视,则外周黄斑中仍存在足够的受体密度/视觉功能,以使患者能够维持功能性视力。

可以使用光线追踪技术来优化先前描述的眼模型内不同IOL的光学性能。可以采用光学设计软件(Zemax OpticStudio,Zemax LLC,USA)。一旦选择了材料,就可以开发价值函数(merit function)来设计实施方式的IOL。通常,可以在与晶状体的预期性能相关的那些参数中选择参数。在优化过程期间,可以将不同的值系统地赋予自变量。随后,可以采用那些值来计算所选的价值函数分量。目标是找到变量的使价值函数最小化的一组值。在理想情况下,该过程以找到全局或绝对最小值而不是局部最小值结束。生成的价值函数包含了针对IOL的几何参数的约束,以将其保持在生理上兼容的范围内。在此示例中,用于优化的可变参数是晶状体的厚度、晶状体在囊内的位置、曲率半径和不同表面的非球面度。在价值函数中同时包括三种配置,对应于水平方向上的轴线上、偏心度为5度和偏心度为10度的入射光束。

对于设计中的标准IOL参考,可以使用相同的眼模型(Liou-Brennan角膜,轴向长度23.5mm),但是代替实施方式的IOL,该IOL将光聚焦在视网膜上所需的最佳屈光力被建模。对于该眼模型和该IOL位置(从第二角膜表面到前IOL的轴向距离也是4.16mm),使用具有球面(前表面和后表面的曲率半径分别为27.51mm和-14.59mm)的、折射率为1.54并且厚度为0.70mm的21.5D IOL(浸入1.336的折射率中)。
为了评估实施方式的IOL的图像质量,使用NIMO仪器(LAMBDA-X,Nivelles,Belgium),该仪器包括光具座并且其软件版本为4.5.15。该仪器的工作原理是基于相移纹影技术71,72。通过将纹影成像原理与相移方法相结合,NIMO仪器允许测量光束偏差,该偏差可以用于在考虑36个Zernikes系数的情况下计算波前分析。该技术已被证明有效地测量人工晶状体的体外光学质量。该设备符合国际标准组织(ISO)11979-216。在将所有IOL浸入成分为每毫升NaCl 0.154毫当量盐溶液中的同时测量所有IOL(Laboratoires Sterop SA,Anderlecht,Belgium)。测量期间用于将IOL和盐溶液保持在适当位置的比色杯(cuvette)或湿槽(wet cell)已借助干涉仪进行了验证;并被证明屈光力<0.005D。对湿槽进行了此附加的交叉检查,以排除对测量的潜在干扰。此外,只有在对设置进行了彻底校准的情况下,才能进行准确的屈光力测量,这就是为什么每次测量都要对仪器进行校准的原因。
在下面详细说明的示例1中,眼模型的轴向长度被设置为23.5mm。角膜参数(曲率、非球面度、厚度和折射率)取自Liou-Brennan眼模型(参考)。视网膜被模拟成-12mm的球面,并且前IOL表面被轴向地设置在距角膜的第二表面4.16mm处。
使用1.585的折射率(聚碳酸酯)和5mm的中央厚度、设置在距角膜顶端12mm(顶点距离)处来对外部眼镜镜片进行建模。前表面和后表面的曲率半径分别为32mm和36mm(+3屈光度)。此外,还模拟了+6D眼镜镜片(将对象设置在距角膜33cm处),该眼镜镜片具有相同的前曲率,但后曲率半径为44.7mm。所有计算均在550nm的波长和3mm的瞳孔直径下进行。
对实施方式的IOL晶状体进行了优化,使其折射率为1.54(针对550nm)、阿贝数为40并且厚度为0.70mm。
针对具有规定的轴向长度值的不同模型重复该优化过程。这提供了优化的不同屈光力的晶状体。图2至图6示出了不同屈光力的优化实施方式的IOL的实际形状。对应的正视目标位于各个晶状体的底部。对于这些非限制性示例,使用这些方法设置和对准:随着IOL的屈光力增加,晶状体的后表面变得更加弯曲,而前表面改变了曲率的符号,从正曲率变到较平坦的曲率,然后到负曲率。这是各个模型的形状因子优化过程的结果。
示例1
研究
设计并进行了研究,以评估在晚期、双侧年龄相关性黄斑变性患者中植入这种新式人工晶状体之后的安全性和初步结果。患有≤1+白内障(no LOCSIII分级参数>2)、双侧晚期地图状萎缩/干性年龄相关性黄斑变性(AMD)且术前矫正远视力≥0.60(CDVA;LogMAR)的7个受试者的八只眼睛接受了晶状体摘除和具有远视术后屈光目标的IOL植入。在与患者仔细讨论使用这种方法利用眼镜提供放大倍率的潜在益处同时平衡了日常生活活动对眼镜依赖性增加的缺点之后,决定了目标术后远视的量。所有患者的手术眼具有中度至重度视力丧失,因此取决于个体的情况(包括同意在必要时对第二只眼睛进行手术)选择1.5D至4.5D的远视结果。在基线、1周、1个月和2个月时进行了初始随访和评估。
在基线和术后的1周、1个月和2个月执行了以下检查:完全主观屈光、矫正近视力(N点,30cm处,具有LogMAR转换)、矫正远视力(LogMAR)、眼内压(Goldmann压平眼压计)、镜面显微镜检查(Nidek CEM-530,Nidek Co.Ltd.;从中央角膜获得的3个可接受图像)、临床检查、眼前节OCT(Visante,Carl Zeiss Meditec AG)和黄斑OCT(Stratus OCTTM CarlZeiss Meditec,Germany)。晶状体混浊根据LOCSIII体系分级。通过全阈值80点测试来评估视野。在术后1个月对手术眼进行屈光不正矫正后,使用MNREAD图表来评估阅读视力(reading acuity)、临界字体大小(critical print size)和阅读速度(reading speed)。使用黄斑完整性评估仪(MAIA,Ellex Medical Lasers Ltd.)在基线和术后1个月至2个月进行微视野检查(Microperimetry);间隔1个月至3个月执行附加微视野评估,以确认观察到的任何变化。在无散瞳的中间视觉(mesopic)条件下使用“专家”算法进行微视野检查,以评估黄斑阈值敏感度和注视稳定性(在以优选视网膜位点为中心的10度区域中测试37个点;4-2策略;刺激大小为Goldmann III,持续时间200ms)。排除标准包括:招募后6个月内治疗的活动性脉络膜新生血管(CNV);轴向长度>24.5mm或<20.5mm;招募后6个月内不受控的青光眼和眼内手术。
患者的平均年龄为77±16岁(范围为43至91),男女比例为5:3。手术由单个外科医生(MAQ)使用标准技术执行。局部散瞳剂用于瞳孔扩张并且麻醉由筋膜囊下(sub-Tenon)的传递引起。使用飞秒激光手术平台(Fort Worth,Texas,USA)进行5mm的囊切开术和晶状体粉碎,并使用WHITESTAR超声乳化系统(Abbot Medical Optics,Abbot Laboratories Inc.,Illinois,USA)以位于100°处的标准2.6mm角膜切口完成晶状体摘除。向囊袋填充粘性眼科粘弹性装置(OVD),然后将晶状体装入注射器筒中,随后经由主伤口将其注射到囊袋中、对中晶状体以及OVD/平衡盐溶液交换。所有受试者实现了目标屈光(均值+2.9±1.3D)的1D内的术后球镜当量。
结果
镜面显微镜检查显示术后减少的内皮细胞计数的均值为13%±14%(范围为0%-37%)。2只眼减少了37%和31%——该受试者在2周后失去了随访,并且缺乏滴剂依从性(drop compliance)可能是这些变化的原因。否则结果与标准超声乳化白内障手术后的预期减少(4%-13%)8相符。术前和术后的80点视野测试结果相似(术前所看到的平均点数为50±31,而术后为53±27),并且眼前节和黄斑OCT成像显示良好居中的IOL和术后稳定黄斑。术后所有受试者的眼内压均保持稳定——平均术前眼内压和2个月时的术后眼内压分别为16±2.8mmHg和14±2mmHg。
一个受试者的术后MN阅读数据无法获得。在其余受试者中,观察到平均阅读视力从1.07±0.31LogMAR适度提高到0.9±0.37LogMAR,并且临界字体大小从1.04±0.25适度提高至0.95±0.27。观察到平均阅读速度从每分钟28±19个单词增加到每分钟44±31个单词,提高了57%。
在除了一个受试者的眼睛之外的所有受试者眼睛中,在手术后大约1个月和/或2个月获得微视野数据。观察到微视野阈值敏感度从8.2±4.6dB平均提高到12.0±5.6dB。4度圆圈内注视点的平均百分比从77%±17%增加到91%±11%。尽管无法获得一个受试者的术后微视野数据,但该个体的植入后期中的视力显著改善。术后在三只眼睛中观察到的微视野测试变化最小,在其余眼睛中在1个月和2个月时有逐渐改善的迹象。
对手术眼中的三只眼睛进行了进一步的微视野测试,并且这些数据表明在两个月的时间点之后,视觉功能逐渐改善。观察到这些眼睛中的优选视网膜位点逐步从地图状萎缩区域转移。在受试者的眼睛1中,平均阈值敏感度在5个月时从0dB增加到16.6dB,并且4度圆圈内注视点的平均百分比从64%相关联地增加到94%。对于该受试者的第二只眼睛,术后4个月进行的测试表明,平均阈值敏感度从4.2dB略降低至3dB,但4度圆圈内注视点的平均百分比从57%增加到93%,这些点集中在视盘与大面积的地图状萎缩区域之间的狭窄通道中。在4个月时对第三个受试者进行的测试显示,阈值敏感度从12.9dB增加到27dB,并且4度圆圈内注视点的平均百分比从99%略微降低到83%。
没有报告不等像视(aniseikonia)的症状,但是所有受试者随后继续将该装置植入其另一只眼睛。
中度至重度年龄相关性黄斑变性的研究对象的视力预后与实验室模拟的结果一致,相当于远视力和近视力平均提高了18个ETDRS字母,并且观察到平均阅读速度增加了57%。这些结果与公开的AMD患者(包括接受CNV治疗的患者)的白内障手术效果相比非常有利——最近的meta分析表明,经受白内障手术并植入标准单焦点IOL的AMD受试者可以预期在6个月至12个月的随访后,视力提高6.5个至7.5个ETDRS字母9,10。
