Description

Doctor scheduling and distribution method and system based on medical conjunct consultation
Technical Field
The application relates to the field of information processing, in particular to a doctor scheduling and distributing method and system based on medical conjunct consultation.
Background
The medical conjunct consultation system is different from the traditional doctor-seeking mode of patient registration-doctor diagnosis, needs to use the patient as a tie, establishes connection and communication among doctors in different hospitals, and determines the applying doctor and the consultation doctor.
There are two main schemes for determining consultation doctors in the current medical conjunct consultation system: one is that the applicant doctor selects consultation doctor; the other is that the doctor of the same consultation at all the superior hospitals can see, and the doctor of the superior hospitals determines whether to receive the consultation to become the consultation doctor.
Unlike traditional in-hospital consultation, the medical conjunct consultation system requires communication between several different hospitals.
At present, most of the modes of determining the medical conjunctive consultation doctor are that the applicant doctor contacts the consultation doctor by means of a communication tool of a third party, and the consultation is carried out after two persons negotiate time. The mode increases the workload of the applicant doctor and the consultation doctor, and brings inconvenience to the consultation.
In the medical conjunct consultation system, because the consultation application hospitals are docked with a plurality of superior hospitals, when the consultation application doctors select the consultation receiving doctors, the doctor professional skills of different hospitals need to be considered, whether the doctors are willing to make the consultation, and when the doctors have time to make the consultation, so that a lot of workload irrelevant to the diagnosis can be brought to the application doctors.

s130, determining target consultation doctors according to the target consultation doctor list and the reserved name remaining quantity of each doctor in the target consultation doctor list;
and S140, distributing the consultation application to the target consultation doctor.
In the embodiment of the application, a consultation application sent by a consultation initiating hospital is acquired, and consultation items and consultation time are determined according to the consultation application; the consultation application comprises a special consultation application and a non-special consultation application, wherein the special consultation application comprises designated consultation doctor information or designated consultation hospital information; when the consultation application is a non-special consultation application, determining a target consultation doctor list according to the consultation project, the consultation time and a doctor total list; determining target consultation doctors according to the target consultation doctor list and the reserved name remaining quantity of each doctor in the target consultation doctor list; the consultation application is assigned to the target consultation doctor. The hospitals with insufficient resources, clinics and full experience are connected, the hospitals with high and new technologies are connected in a medical conjunct mode, the lower-level hospitals are taken as main bodies, a plurality of hospitals with large levels are arranged to receive various types of examination, the upper-level hospitals are arranged to carry out scheduling treatment on the examination from the lower-level hospitals according to the expert doctor field, the utilization rate of social resources is greatly improved, and the time cost and the economic cost of general patient seeing a doctor are reduced; according to the preset of the doctor's adept field, workload and consultation time, the consultation efficiency is improved; the consultation doctor is not required to be manually selected when the application doctor initiates the consultation application, the system can be automatically and accurately matched with the consultation doctor, and the user can use the system more conveniently.
Next, a doctor scheduling and assigning method based on the doctor's conjunctive consultation in the present exemplary embodiment will be further described.
As described in the step S110, a consultation application sent by the consultation initiating hospital is obtained, and a consultation item and a consultation time are determined according to the consultation application; the consultation application comprises a special consultation application and a non-special consultation application, wherein the special consultation application comprises designated consultation doctor information or designated consultation hospital information.
The technical scheme of the invention is characterized in that the medical conjunct and the scheduling are arranged, wherein the medical conjunct is a consultation relationship complex established with a superior hospital after the medical conjunct is agreed with the following hospital as a main body. The type of examination is unified in the complex, and site criteria are examined. The lower level hospitals set up the consultation relation complex formed by the upper level hospitals which correspondingly receive the consultation application aiming at different examination types.
As an example of this, in one embodiment,
as described in step S120, when the consultation application is a non-specific consultation application, a target consultation doctor list is determined according to the consultation item, the consultation time and the doctor total list.
It should be noted that the general list of doctors is a list of all doctors qualified to receive consultation applications; the doctor in the doctor total list can be in a state comprising the current reserved name, full pre-reserved name and pause of the consultation; when the current appointment amount is full or the consultation is paused, the corresponding doctor is not assigned a new non-special consultation application.
In an embodiment of the present invention, the specific process of determining the target consultation doctor list according to the consultation item, the consultation time and the doctor total list in step S120 when the consultation application is a non-specific consultation application may be further described in conjunction with the following description.
As described in the following steps, a current list of doctors currently having a reservation title in the total list of doctors is determined.
Specifically, a doctor list with the remaining number of the current reservation names greater than zero is screened from the doctor total list, and the part of doctor list is set as the current doctor list. The remaining number of reserved names is used as a primary screening condition, so that corresponding operation resources can be saved in the subsequent screening steps. The maximum number of the appointment names can be the daily average visit number of each doctor in a certain time period (1-3 months), the maximum number of the appointment names can be customized by the doctor, and when the maximum number of the appointment names is customized by the doctor, the selectable range of the customized numerical value is 50% of the daily average visit number of the doctor to 120% of the daily average visit number of the doctor. To prevent excessive accumulation of scheduled consultations from leading to reduced treatment efficiency under the name of the same doctor.
And determining the target consultation doctor list according to the consultation project, the consultation time and the current doctor list as described in the following steps.
In one embodiment of the present invention, the specific process of determining the target consultation doctor list according to the consultation item, the consultation time and the current doctor list may be further described in conjunction with the following description.
And determining a project doctor list according to the consultation project and the current doctor list as described in the following steps.
The method includes the steps that screening of consultation project good for each doctor is conducted from a current doctor list obtained by removing doctors with full current appointment names, so that a project doctor list good for the consultation project is obtained; wherein, each doctor's adept consultation item is selected by the doctor and entered into the database, the doctor's selected adept item may include examination type and examination site, such as: CT chest, CT head; it is also possible to include only the examination type, such as: CT, DR, MR (when the doctor selects only the examination type, the doctor defaults to be good at all examination sites to reduce repetitive operations at the time of information entry to improve the efficiency of information entry). It should be noted that, the consultation items included in the consultation application initiated by the consultation application hospital include an examination type and an examination site. When doctor list screening based on consultation projects is performed, the screening sequence is examination type- & gt examination part.
And determining the target consultation doctor list according to the consultation time and the project doctor list as described in the following steps.
It should be noted that, a doctor list containing the consultation time is selected from the project doctor list, and the target consultation doctor list is generated, where the consultation time corresponding to each doctor is a time period (within a working time period range) customized by the doctor.
As described in step S130, the target consultation doctor is determined according to the target consultation doctor list and the remaining number of reserved names of the doctors in the target consultation doctor list.
In an embodiment of the present invention, the specific process of "determining the target consultation doctor according to the target consultation doctor list and the reserved name remaining number of each doctor in the target consultation doctor list" in step S130 may be further described in conjunction with the following description.
The remaining number of appointment names for each doctor in the list of target consultants is determined as follows.
It should be noted that, the termination time node for calculating the remaining number of the reserved names may be consultation time; the current latest appointment time point can also be; the latest appointment time point corresponding to the week of the consultation time can also be provided.
And setting the doctor with the largest reserved name remaining in the target consultation doctor list as the target consultation doctor as follows.
It should be noted that, according to the remaining number of reserved names of each doctor in the target consultation doctor list, the target consultation doctor list is ranked (the more the remaining number of reserved names is, the higher the ranking is), and the doctor corresponding to the highest ranking is set as the target consultation doctor. When doctors with the same reserved name amount remain, the method can sequentially sort the reserved names according to the pinyin initials corresponding to the last name, the first 1 st word, the first 2 nd word and the first n th word in the names of the doctors; the calculation of the reserve name remaining amount may also be performed for the part of doctors using another expiration time node for the reserve name remaining amount calculation as mentioned before; the two modes can be integrated in a progressive manner.
As an example, when the reserved names of two doctors are the same in number, the initial letters corresponding to the surnames of the two doctors are obtained, and if the initial letters are different, the two doctors are ordered according to the letter ordering sequence corresponding to the initial letters; if the first letters are the same, the first letters corresponding to the 1 st characters of the two doctors are obtained, and the above processes are repeated until the ordering is obtained; or, all words in the name are judged. If the result can not be obtained after judging all the words of the name, calculating the reserved name residual quantity in different modes (for example, when the consultation time is currently used as a termination time node, the latest reserved time point corresponding to the week of the consultation time is replaced as the termination time node).
The consultation application is distributed to the target consultation doctor as described above in step S140.
It should be noted that, the consultation application distributed to the target consultation doctor includes patient information, consultation time and consultation project.
In an embodiment of the present invention, further includes:
when the consultation application is a special consultation application with designated consultation hospital information, determining a designated consultation hospital doctor list according to the designated consultation hospital information and the doctor total list;
the doctor list of the designated consultation hospital is a list generated by the part of doctors corresponding to the designated consultation hospital in the doctor total list; and screening the doctor total list through the designated consultation hospital information during generation, screening a doctor list containing the designated consultation hospital information, and generating the designated consultation hospital doctor list according to the part of doctor list.
Determining the target consultation doctor list according to the consultation project, the consultation time and the designated consultation hospital doctor list as follows;
firstly, a doctor list with reserved names is screened out from the doctor list of the designated consultation hospital; secondly, screening the screened doctor list based on consultation items, and screening a doctor list with consultation qualification of the consultation items; and screening the consultation time of the doctor list subjected to the secondary screening, screening the doctor list with the reserved name in the consultation time, and finally generating the target consultation doctor list from the doctor list subjected to the tertiary screening.
In one embodiment of the present invention, the specific process of determining the target consultation doctor list according to the consultation item, the consultation time and the designated consultation hospital doctor list may be further described in conjunction with the following description.
As described in the following steps, a list of project doctors is determined based on the consultation project and the list of designated consultation hospital doctors.
And determining the target consultation doctor list according to the consultation time and the project doctor list as described in the following steps.
Determining the target consultation doctor according to the target consultation doctor list and the reserved name remaining quantity of each doctor in the target consultation doctor list as follows;
specifically, determining the reserved name remaining quantity of each doctor in the target consultation doctor list; and setting the doctor with the largest reserved name remaining in the target consultation doctor list as the target consultation doctor.
It should be noted that, the termination time node for calculating the remaining number of the reserved names may be consultation time; the current latest appointment time point can also be; the latest appointment time point corresponding to the week of the consultation time can also be provided.
The consultation application is distributed to the target consultation doctor as follows.
It should be noted that, the consultation application distributed to the target consultation doctor includes patient information, consultation time and consultation project.
In an embodiment of the present invention, further includes:
when the consultation application is a special consultation application with designated consultation doctor information, determining the target consultation doctor according to the designated consultation doctor information and the doctor total list;
when the consultation application is a special consultation application containing the information of the designated consultation doctor, the consultation application is directly distributed to the name of the doctor as long as the doctor total list contains the information of the designated consultation doctor, and whether the doctor currently has the remaining reserved name or not. However, when the designated doctor is pre-filled during the assignment, the doctor is reminded to conduct pre-consultation negotiation with the applicant during the assignment of the consultation application.
The consultation application is distributed to the target consultation doctor as follows.
It should be noted that, the consultation application distributed to the target consultation doctor includes patient information, consultation time and consultation project.
Referring to fig. 5, in a specific implementation, the system implementation can be divided into two modules, i.e., consultation service and storage service, where the storage service module processes the inspection images and reports, and provides the reading support. The subordinate hospital sends images in standard DICOM (Digital imaging and Communications in Medicine, digital imaging and communication in medicine) format to WADO (Web Access to DICOM Persistent Object) service, and WADO service receives the request and then pushes the images to a Kafka message queue, and the consumption service monitors the message queue, consumes the images, invokes a storage service for archiving and storing. By using the method, the concurrent pressure is greatly relieved, the concurrent capability of the system for processing the image uploading is improved, and the fault tolerance of the image uploading is realized by means of the fault tolerance of Kafka. The images are stored, and meanwhile, the images are cached, and the caching is preferentially queried when the images are queried during the film reading, so that the performance is improved. The consultation module provides a unique access entrance through the reverse proxy, thereby realizing unified control and load balancing of the request, enhancing control through the micro-service gateway, and further improving the safety of the system by utilizing gateway safety authentication, current limiting and load balancing logic called by the micro-service cluster, and realizing high availability and high performance of the system.
