Description

Disposable mucosa incision knife with injection needle and use method
Technical Field
The invention relates to the technical field of medical instruments, in particular to a disposable mucosa incision knife with an injection needle and a using method thereof.
Background
Endoscopic mucosal dissection (ESD) is a new treatment means in recent years, and refers to an endoscopic minimally invasive technique for performing submucosal dissection on lesions larger than 2cm by using various electrotomes, and can treat precancerous lesions such as giant polyp of gastrointestinal tract, lateral developmental tumor, severe atypical hyperplasia and high-level intraepithelial neoplasia, early cancer of esophagus and gastrointestinal tract, esophagus and submucosal tumor of gastrointestinal tract. The technique can realize the whole excision of larger lesions and provide accurate pathological diagnosis staging. With the continuous development of endoscopic instruments, ESD becomes the first choice treatment method for early cancer and precancerous lesion of digestive tract, and has the advantages of definite curative effect, small wound, quick recovery, complete and accurate pathological diagnosis and the like.
Compared with surgical operation, the utility model can maintain normal physiological structure and has little influence on the quality of life of patients after operation. In the operation process, liquid such as physiological saline and the like is injected under the mucosa to form a water cushion at the submucosa, and the use of different cutting knives to cut off the focus is an important link for ensuring the success of the operation.
An important surgical instrument for endoscopic mucosal dissection is a mucosal incision knife. Common cutting knives for stripping early cancers on the market are needle knives, IT knives (with ceramic heads), hook knives, triangular knives, Flush knives, woodpecker knives and the like.
As an important means in the treatment of digestive tract lesions, in order to prevent perforation caused in the operation process, a doctor often needs to inject physiological saline under the mucosa to a position with a high focus first in the process of performing mucosa dissection operation under an endoscope, a mucosa incision knife needs to be replaced after the injection with an injection needle, the physiological saline injected in the operation process can run off along with the operation time, a large amount of time can be wasted in the process of injecting the physiological saline again, and the risk of perforation in the operation is increased because the loss of the physiological saline in the operation can not be supplemented in time.
The invention patent with the patent application number of CN201410281645.7 discloses a combined electric knife, wherein the knife head part comprises at least two or more knife heads which can move or rotate relatively, and doctors can select proper electric knife types for the tissues of the focus part according to the condition analysis and judgment of the tissues of the focus part and different states of the proper electric knife, so that the operation is safe and quick.
The invention patent with the patent application number of 201510937882.9 discloses a multifunctional knife combining an IT knife, a needle knife and water spraying, which has the functions of the IT knife and the needle knife, and also has the function of water spraying, and the sprayed liquid can wash out blood spots, improve the visual field of an endoscope, and also wash burnt tissues adhered to an electrode.
However, the aforesaid patent still can not cover up ESD technique complicated operation, and it is long-lived, the technical requirement to the operation doctor is rigorous, postoperative bleeding, perforation scheduling complication incidence such as high. The main reason for this is that the following problems exist with the current devices used for ESD treatment:
problem 1: at present, the mucosa incision knife and the injection needle which are generally used clinically are two independent instruments, which are expensive, and the cost expenditure of hospitals and the economic burden of patients are increased.
Problem 2: the mucosa incision knife and the injection needle are two independent instruments, so that the operation of an operator is not easy to master, and the incision depth of the mucosa incision knife is not easy to control.
Problem 3: before the lesion is cut off, a needle needs to be used for lifting the lesion to a lesion injection water cushion, then the lesion is peeled off by a mucosa incision knife, the mucosa incision knife and the needle are two independent instruments, an operator needs to replace the needle back and forth, and the operation is tedious and consumes a large amount of time.
Problem 4: the apparatus all need be in when business turn over scope chamber way and close the recovery state, but in the reality at the change apparatus in-process repeatedly, often has the phenomenon that personnel use the apparatus improper (forget to close tool bit and syringe needle and retrieve in the protective sheath) to cause accurate and expensive endoscope damage, unable normal use has increased the fault rate and the maintenance cost of endoscope, has also increased patient's treatment time and effect simultaneously.
Problem 5: although some mucosa incision knives on the market have an injection function, the knife head of the mucosa incision knife is integrated with the needle head of an injection needle, the injection wound surface is large, and the mucosa incision knife is provided with an electric pump, so that the cost is increased, the price is high, the structure is complex, the force is too large during injection, the water quantity is not easy to control, the excessive depth of injection is easy, and the injection liquid is wasted; the length of the knife head of the mucosa incision knife is too long and is not easy to control, so that the pathological change is easily stripped to a too deep depth, and the risks of bleeding and perforation are increased.
