Description

Prepuce ligation ring
Technical Field
The invention relates to a medical instrument, in particular to a foreskin ligation ring, and belongs to the field of medical mechanical articles.
Background
The foreskin is a component of male external genitalia and has important physiological functions. The medical community has been studying the foreskin function, and with the research, there is a renewed understanding and understanding of the foreskin function. At present, the medical field has recognized the following functions of foreskin: firstly, the prepuce covers the glans penis, and the glans penis is protected.
The portion of the penis skin extending forward is called the foreskin. If the glans penis is covered by the foreskin, the glans penis can be exposed by turning back the glans penis by hand, and the foreskin is called as 'overlong foreskin'; if the glans penis is completely covered by the foreskin and can not be turned outwards to expose the glans penis, the glans penis is called phimosis. No matter the foreskin is too long or the phimosis, because the foreskin covers the glans, the glans and the coronary sulcus are usually in a dark and humid environment, so that firstly, bacteria are easy to breed to cause balanitis; moreover, it is easy to accumulate secretions and deposits to form encrustations. Chronic inflammation and the stimulation of the periderm scale are easy to form terrible lesions of the glans skin.
There are various ways of circumcision, such as general circumcision, laser sleeve resection, ring cerclage, etc., and currently, the more popular is "american type circumcision" which combines korean plastic surgery with the american advanced plastic surgery. In any type of surgery, the inner and outer plates of the redundant foreskin are cut under anesthesia. At present, all the medical fields related to the arrangement of the circumcision ligation ring utilize the compression principle of an inner ring and an outer ring to lead the redundant foreskin tissues to be ischemic and necrotic. The inner ring is a circle in the shape of an integral circular tube. The outer ring consists of two C-shaped semicircular rings, and the inner sides of the C-shaped semicircular rings are provided with blades or knives (the blades or the knives are complete and can completely press the foreskin and completely block the blood supply of the foreskin to ensure that the foreskin is ischemic and necrotic), so that the two C-shaped semicircular rings are connected by a connecting device and are connected, adjusted and locked by an adjusting device; or the outer ring is of a C structure and is not provided with a connecting device; or compressing the inner ring and elastic rope to press the foreskin to make the redundant foreskin ischemic necrosis. Since the wound is separated from the normal foreskin tissue and the necrotic foreskin tissue in a purulent way from compression to ischemia necrosis and gradually separated from the necrotic foreskin tissue, the wound grows slowly under the stimulation of chronic inflammation, the whole foreskin is cut to a healing process which is about 4-5 weeks generally, and even more than 2 months, so that the pain is caused to a patient, the work and the study are influenced, and the pain is hard to say. Solves the problems of the foreskin cutting stitching instrument, the foreskin circumcision anastomat and other instruments used in the foreskin operation at present. The redundant foreskin tissue is cut and sutured by a suturing method at one time, wound bleeding, penis hematoma and cut foreskin length in foreskin operation cannot be effectively controlled, pressure hemostasis is performed to wrap wound pain, and fracture and nail removal pain are caused after foreskin operation.

When the outer ring and the wound protection cover are in a combined state, the outer ring positioning convex groove corresponds to the wound protection cover positioning groove.
In the prior art, the circumcision ligation ring is generally combined with an inner ring through a ring edge arranged on the inner side of the outer ring or densely arranged sharp puncture edges, and is locked and adjusted through a connecting and adjusting device, so that the cutting edge can completely compress redundant foreskin and block blood supply of the foreskin, and the ischemic necrosis of the foreskin is caused to fall off (generally, the redundant foreskin is cut off); however, as the foreskin is gradually separated from compression to ischemic necrosis and then the wound is suppurative separated between the necrotic foreskin tissue and the foreskin tissue, the foreskin slowly grows under the stimulation of chronic inflammation, the whole process from cutting to healing of the foreskin is long, and in the process, the patient suffers from pain for a long time and study and work are influenced. Generally, the prepuce wound after the existing circumcision ligation is directly exposed to the outside, the wound is easy to contact and rub with clothes, pain of a patient is increased, meanwhile, wound infection is easy to occur, the healing period is prolonged, and even operation failure can be caused in severe cases.
In the present invention, the outer ring is a circular ring structure formed by one or more C-shaped half ring assemblies, and generally, the number of the C-shaped half ring assemblies is preferably two (when only one C-shaped ring is formed, two ends of the C-shaped ring are connected to form a circular ring), and any one of the C-shaped half ring assemblies is formed by an outer ring body and an outer ring layer. The outer ring layer is annularly and parallelly arranged on the inner wall of the outer ring body and extends to the ring center direction from the outer ring body. The inner ring comprises an inner ring body and an inner ring wall protruding structure, and a fixing protrusion in the inner ring wall protruding structure is an inner ring layer fixing protrusion: comprises an upper ring layer fixed bulge and a lower ring layer fixed bulge or an upper fixed bulge and a lower fixed bulge. Has the functions of positioning and extruding foreskin, and blocking partial foreskin blood supply and supplying nutrition). An inner ring nutrition groove (an upper ring nutrition groove, a lower ring nutrition groove, a V-shaped nutrition fork groove and the like) is formed between the adjacent fixing bulges and is used for accommodating the foreskin, so that a channel is provided for the establishment of collateral circulation, and the function of providing nutrition supply for the wound is achieved.
Further, in order to achieve better operation effect, the upper ring layer fixing bulges and the upper ring layer nutrition groove are in serial spaced design. The lower ring layer fixing bulges and the lower ring layer nutrition grooves are also in serial spaced design (assuming that the bulges are marked as '1', the nutrition grooves are marked as '2', the serial spaced design of the upper ring layer fixing bulges and the upper ring layer nutrition grooves is '1-2-1-2-1.', and correspondingly, the serial spaced design of the lower ring layer fixing bulges and the lower ring layer nutrition grooves is '2-1-2-1-2.').
Furthermore, in the direction perpendicular to the ring surface of the inner ring, the fixing bulges on the upper ring layer correspond to the nutrition grooves on the lower ring layer. The upper annular layer nutrient tank corresponds to the lower annular layer fixing bulge. Namely, in the axial direction from the upper ring layer of the inner ring to the lower ring layer of the inner ring, the lower ring layer nutrition groove of the lower ring layer of the inner ring is correspondingly arranged opposite to the fixing bulge of the upper ring layer of the inner ring. The lower ring layer fixing bulges of the inner ring lower ring layer correspond to the upper ring layer nutrition groove of the inner ring upper ring layer. Meanwhile, the width of the edge of the fixing bulge of the upper ring layer far away from the direction of the ring center of the inner ring is equal to or less than the width of the notch of the nutrition groove of the lower ring layer. When the foreskin ligation ring is used, the inner ring with the special inner ring layer fixing bulge and the inner ring nutrition groove is matched with the outer ring, and redundant foreskin tissues to be cut are fixed and pressed on the inner wall of the outer ring body (double-layer solid extrusion is adopted, so that the effect is better). Wherein, the foreskin is fixed by the fixing bulge of the lower ring layer of the inner ring, and the whole extrusion blocks part of blood from flowing to the foreskin tissue ring cutting end for supplying nutrition; the foreskin is also accommodated and fixed by the inner ring lower ring layer nutrition groove, blood conveying and large amount of nutrition circulation are not completely blocked, the foreskin is not completely pressed due to the groove, a certain amount of blood circulation exists, and the blood and nutrition at the moment flow into foreskin tissues of the total-nutrition inner ring buffer groove between the inner ring upper ring layer and the inner ring lower ring layer through the inner ring lower ring layer nutrition groove to be accumulated and supplied with nutrition. Then the foreskin is fixed by the fixing bulges on the upper ring layer of the inner ring, and the blood flow direction of the foreskin tissues extruded by the nutrition groove on the lower ring layer of the inner ring is stopped. Therefore, the blood flowing from the inner ring lower ring layer nutrition groove to the foreskin tissue in the full-nutrition inner ring buffer groove can be effectively prevented from further flowing to the wound end foreskin tissue, so that wound bleeding is fixed, and the blood is completely extruded and prevented from flowing to the redundant foreskin tissue at the wound end for nutrition supply, so that wound bleeding is caused; the inner ring upper ring layer nutrition groove is also used for slightly fixing and containing the foreskin, a channel is provided for collateral circulation and providing nutrition for the wound, and blood flows into the edge of the inner ring upper ring layer nutrition groove close to the wound end through the inner ring lower ring layer nutrition groove to provide collateral nutrition for the foreskin tissue. The foreskin tissue is interactively fixed (hemostasis) and accommodated (blood supply and nutrition supply) through the fixing protrusion of the lower inner ring layer, the nutrition groove of the upper inner ring layer and the fixing protrusion of the upper inner ring layer, after two steps of blood flow buffering (once for the lower inner ring layer and once for the upper inner ring layer), the bridge-shaped collateral circulation blood supply and nutrition supply are started for the foreskin tissue at the wound end, and the purpose and the requirement of healing the foreskin wound tissue for one time are met on the premise that the foreskin wound is not bleeding.
Furthermore, in order to achieve a better operation effect, the upper fixing bulges and the lower fixing bulges are designed in an up-and-down staggered manner, and V-shaped nutrient fork grooves are formed between the upper fixing bulges and the lower fixing bulges (assuming that the upper fixing bulges are marked as ' 1 ', the end faces of the bulges point to the inclined upward direction, the lower fixing bulges are marked as ' 2 ', the end faces of the bulges point to the inclined downward direction, and the V-shaped nutrient fork grooves are marked as ' 3 ', and the up-and-down staggered manner between the upper fixing bulges and the lower fixing bulges is marked as ' 1-3-2-3-1-3-2-3-1.
Furthermore, the foreskin is fixed by the inner ring layer fixing protrusions (including the upper ring layer fixing protrusions, the lower ring layer fixing protrusions, the ascending fixing protrusions, the descending fixing protrusions and the like) (the foreskin is extruded and fixed between the ring layer fixing protrusions and the outer wall of the inner ring), and most of blood is extruded and blocked to flow to the foreskin tissue ring cutting end for supplying nutrition. The inner ring nutrition groove is also used for slightly fixing and containing the foreskin, and meanwhile, the inner ring nutrition groove (comprising an upper ring layer nutrition groove, a lower ring layer nutrition groove, an inner ring buffer groove, a V-shaped nutrition fork groove and the like) which is formed by interval type or staggered type design and is fixedly connected in series with the inner ring layer fixing protrusion has the function of providing a channel for collateral circulation and can provide limited nutrition supply for the wound. Blood flows into the edge of the inner ring nutrition groove close to the wound end to establish a collateral nutrition supply mechanism of foreskin tissues. The foreskin tissue is fixed in an interval or staggered mode (blood supply stopping) and contained (blood supply and nutrition supply) through the inner ring layer fixing protrusions and the inner ring nutrition grooves, after the foreskin tissue is buffered by blood flow, a bridge-shaped collateral circulation blood supply and nutrition supply mechanism is started for the foreskin tissue at the wound end, and the foreskin wound tissue can achieve the purpose and the requirement of healing for one period on the premise of ensuring that the foreskin wound does not bleed.
In the invention, the convex surfaces of the fixing bulges of the inner ring layer are designed in a concave arc shape, and the concave arc shape can be more matched with the physiological structure of foreskin tissues, so that the buffering blood supply effect is better. And the convex concave arc-shaped convex surface of the inner ring layer can form convex points at two ends, which is very beneficial to the fixation of foreskin tissues and can effectively prevent the foreskin from retracting to cause extrusion to stop blood supply failure and cause wound bleeding.
In the invention, the inner ring further comprises an upper limiting bulge, a lower limiting bulge and a foreskin cutting groove. The upper limiting protrusion and the lower limiting protrusion have the functions of preventing the inner ring from sliding out of the outer ring (the height of the outer ring is less than that of the inner ring), and the problem that the operation fails and the pain of a patient is increased due to the fact that the outer ring body of the outer ring is located between the upper limiting protrusion and the lower limiting protrusion of the inner ring in a combined assembly state is solved. Meanwhile, a foreskin cutting groove is formed in the side wall of the inner ring body (the foreskin cutting groove is formed in the outer wall of the inner ring and located on the upper side of the upper limiting protrusion), so that the wound can be cut conveniently, the wound can be tidy, the wound can be healed beautifully, medicines can be smeared, and the postoperation psychological pressure of a patient can be relieved. In the present invention, the upper stopper projection may be formed of a plurality of arc-shaped projections, or may be an annular projection provided on the upper edge portion of the inner ring as a whole. The lower limit protrusion may be formed of a plurality of arc-shaped protrusions, or may be an annular protrusion provided on the lower edge portion of the inner ring as a whole.
Further, the inner ring also comprises a diagonal flow groove and/or a diagonal flow hole. The oblique chute and/or the oblique chute hole are/is arranged on the inner ring body and are positioned on two sides of the foreskin cutting groove. The chute is designed in a notch mode with an inclined bottom surface, and one end of the chute is communicated with the foreskin cutting groove. The oblique flow hole is designed to be an oblique long hole (an opening at the upper part of the oblique flow groove can be understood as a groove communicated with the foreskin cutting groove), one end of the oblique flow hole is communicated with the foreskin cutting groove, and the other end of the oblique flow hole penetrates through the outer wall of the inner ring body and then is communicated with the outside. The oblique flow groove and the oblique flow hole can be arranged simultaneously or independently. When the prepuce cutting groove is arranged, the prepuce cutting groove can be arranged on two sides of the prepuce cutting groove, and the prepuce cutting groove can be arranged on one side of the prepuce cutting groove. The foreskin wound treating device comprises a foreskin cutting groove, a foreskin wound, a skew chute, a skew flow hole, a skew chute and/or a skew flow hole, wherein the skew chute and the skew flow hole are communicated with the foreskin cutting groove, and the purpose is to discharge liquid generated in the foreskin wound in the foreskin cutting groove through the skew chute and/or the skew flow hole, so that the propagation and infection of bacteria and the deterioration of wound injury of the foreskin are avoided, the wound healing period is further prevented from being prolonged, and the pain of a patient is prevented from being increased.
In the invention, the foreskin ligation ring also comprises a wound protection cover. The wound protection cover comprises a bottom plate, an outer cover ring, an inner cover ring and a U-shaped groove. The bottom plate is of an annular plate-shaped structure. The outer cover ring is a cylindrical structure which is arranged on the bottom plate and surrounds the periphery of the outer edge end part of the bottom plate. The inner cover ring is a cylindrical structure which is arranged on the bottom plate and surrounds the end part of the inner edge of the bottom plate for one circle. The U-shaped groove is arranged among the bottom plate, the outer cover ring and the inner cover ring.
Furthermore, the wound protection cover can be further provided with a buckle, corresponding buckle holes are formed in the outer ring layer, and the buckle is arranged on the edge of the outer ring. After the prepuce is fixed and extruded by the outer ring and the inner ring, the redundant prepuce is cut off, the wound of the prepuce is positioned between the inner wall of the outer ring and the outer wall of the inner ring, and then the buckle of the wound protection cover is placed into the buckle hole on the ring layer of the outer ring to be locked (the wound protection cover is locked to prevent the wound protection cover from loosening). The inner cover ring of the wound protection cover is positioned in the inner wall of the inner ring (the inner cover ring of the wound protection cover can be just attached to the inner wall of the inner ring), and the outer cover ring of the wound protection cover is positioned on the inner wall of the outer ring, so that one part of the inner side wall of the outer ring, the foreskin tissue wound end and the inner ring are all positioned between the protection U-shaped grooves formed by the outer cover ring, the inner cover ring and the bottom plate of the wound protection cover, and the infection caused by the contact of the foreskin wound and the outside can be effectively prevented. Accelerate wound healing and reduce pain of patients. Meanwhile, when the wound protection cover is fixed (the buckle is screwed from one end with a larger opening of the buckle hole to one end with a smaller opening of the buckle hole for locking), the function of assisting in fixing one or more (generally two) C-shaped semi-ring components can also be achieved.
Furthermore, a bottom plate and a screw groove can be arranged on the wound protection cover. The bottom plate is of an annular plate-shaped structure.
