背景技术
新生儿缺血缺氧性脑病是指围生期窒息所引起的缺氧、脑血流减少或短期脑血流灌注中断所致的脑缺血缺氧性损伤,呈现一系列中枢神经异常的临床症状与体征,是严重威胁新生儿健康、导致新生儿死亡和儿童伤残的常见原因,据统计,我国每年出生婴儿1800万-2000万,窒息的发病率为13.9%,其中有15-25%患儿在新生儿期死亡,有高达25%的存活儿在以后的成长过程中出现智力障碍、脑瘫、癫痫、智力发育迟缓、视听障碍、学习障碍而存留永久性神经心理障碍,造成这种严重后果的原因除了疾病本身的发生发展因素外,还与是否进行早期干预和开始干预的早晚密切相关,但是目前,有很多对于新生儿缺氧缺血性脑病的早期诊断方法并不是很完善,还不能很明确的诊断出这种病情,并及时对其进行治疗,从而错过治疗的最佳时期。
发明内容
本发明所要解决的技术问题是提供一种新生儿缺氧缺血性脑病早期诊断方法,可以有效解决上述背景技术中提出的问题。
为解决上述问题,本发明所采取的技术方案是:一种新生儿缺氧缺血性脑病早期诊断方法,包括以下步骤:
S1、新生儿缺氧缺血性脑病前期观察判断:分别对新生儿出生前、出生时、出生后进行重点观察;观察出生前胎儿是否有缺氧及宫内的窘迫表现;观察出生时新生儿是否有窒息现象;观察出生后新生儿是否出现意识异常、肢体异常、原始反射异常、吮吸异常的现象;
S2、新生儿缺氧缺血性脑病后期精确诊断:对前期观察发现有异常现象的新生儿,对其进行颅脑超声检查、CT检查、脑电图检查;通过颅脑超声检查与CT检查对新生儿进行脑部诊断,确认新生儿是否存在缺氧缺血性脑病,且是否并存脑水肿、脑室内出血;通过对新生儿进行一段时间脑电图检查,确认新生儿脑部是否存在异常症状;在脑电图检查过程中,需清洁头皮、去除胎脂,使头皮电极电阻<10千欧,走纸速度为15mm/s;
S3、新生儿缺氧缺血性脑病临床分度;通过对新生儿出生后三天内的神经症状做细致的动态观察,并对其进行子分度;新生儿出生后24小时内症状表现最明显的为轻度;新生儿出生后24至72小时内症状表现最明显的为中度;新生儿出生至72小时内症状表现最明显的为重度。
作为本发明的进一步优选方案,胎儿宫内窘迫表现为胎动明显减少,胎心变慢<100/分钟,胎粪污染羊水呈Ⅲ度以上混浊。

作为本发明的进一步优选方案,所述新生儿缺氧缺血性脑病临床分度中的轻度症状表现为过度兴奋,肢体出现颤动,肢体肌张力正常或增高,拥抱反射和吸吮反射活跃,无惊厥,呼吸规则,瞳孔无改变,脑电图正常;所述新生儿缺氧缺血性脑病临床分度中的中度症状表现为嗜睡,反应迟钝,肢体肌张力降低,拥抱反射和吸吮反射减弱,出现惊厥,呼吸不规则,瞳孔缩小,脑电图呈低电压、惊厥活动症状;所述新生儿缺氧缺血性脑病临床分度中的重度症状表现为肢体肌张力松软,拥抱反射和吸吮反射消失,反复发生惊厥,呼吸不规则,瞳孔不对称,对光反应消失,且脑电图呈现暴发抑制或等电位波形。
作为本发明的一个具体实施例:
首先需要对新生儿进行缺氧缺血性脑病的初步临床诊断:查看是否有导致胎儿宫内缺氧的异常产科病史,以及严重的胎儿宫内窘迫表现,如胎动明显减少,胎心变慢<100/分钟,胎粪污染羊水呈Ⅲ度以上混浊;在新生儿出生时是否有窒息表现,尤其是重度窒息,比如Apgar评分1分钟≤3分钟,5分钟≤6分钟,经抢救10分钟后开始有自主呼吸,需要有气管内插管正压呼吸2分钟以上;在新生儿出生后12小时内:是否出现意识障碍的表现,如过度兴奋、嗜睡、昏睡甚至昏迷;肢体肌张力改变,如张力减弱、松软;原始反射异常,如拥抱反射过分活跃,减弱或消失;吸吮反射减弱或消失;而且病情较重时有时会出现惊厥或频繁发作惊厥,因脑水肿出现囟门张力增高;当出现重症病例有时会出现脑干症状,如呼吸节律不齐、呼吸减慢、呼吸暂停等中枢呼吸衰竭的情况,以及瞳孔缩小或扩大,对光反应迟钝,甚至消失,还有部分患儿会出现眼球震颤;应注意与产伤性颅内出血区别,并需除外宫内感染性脑炎和中枢神经系统先天畸形。
第二步使用相应的精确的检查设备仪器对新生儿缺氧缺血性脑病进行进一步的辅助检查:其中有颅脑超声检查、CT检查和脑电图检查;进行颅脑超声检查时:发现脑实质内广泛均匀分布的轻度回声增强,伴脑室、脑沟及半球裂腺的变窄或消失和脑动脉搏动减弱,表示存在脑水肿;当基底神经节和丘脑呈双侧对称性强回声反射,表示存在基底神经节和丘脑损伤;出现在脑动脉分布区见局限性强回声反射时,表示存在大脑大动脉及其分支的梗塞;进行CT检查时:在CT扫描时,要测定脑实质的CT值,正常足月儿脑白质的CT值大于20Hu,小于18Hu为低密度;然后需要排除与新生儿脑发育有关的正常低密度现象,即在早产儿的枕区和足月儿的额区呈现低密度为正常表现。双侧大脑半球呈弥漫性低密度影,脑室变窄,甚至消失,表示存在脑水肿;当双侧基底神经节和丘脑呈对称性密度增高,表示存在基底神经节和丘脑损伤,常与脑水肿并存;出现脑大动脉分布区见脑组织密度降低,表示存在大动脉及其分支的梗塞;在脑室周围,尤其是侧脑室前角外上方出现呈对称性低密度区,提示脑室周围白质软化,常伴有脑室内出血,早产儿多见;根据CT检查脑白质低密度分布范围可分为轻、中、重3度,Cr分度并不与临床分度完全一致,2至3 周后出现的严重低密度,CT值<8 Hu 至10Hu,则与预后有一定关系;进行脑电图检查时:脑电图表现以背景活动异常为主,以低电压,任何状态下电压都少于10 pV 至15pV,等电位或电静息现象与爆发抑制为最多见;在新生儿出生后1周内检查脑电图异常程度与临床分度基本一致,2至3周后脑电图仍无显著好转;在脑电图检查过程中,要注意清洁头皮、去除胎脂,使头皮电极电阻<10千欧,走纸速度为15mm/s;在CT检查中,轻度是指散在局灶低密度影分布2个脑叶内,中度是指低密度影超过2个脑叶,白质灰质对比模糊,重度是指弥漫性低密度影,灰质白质界限消失,但基底节、小脑尚有正常密度,中、重度常伴有蛛网膜下腔出血、脑室内出血或脑实质出血;在出现产伤性颅内出血有时会与新生儿缺氧缺血性脑病同时并存,使病情错综复杂,必须注意区别,除参考产科病史外,还应熟悉不同部位颅内出血的临床表现特点,并及时做CT检查进一步确诊。
第三步对新生儿缺氧缺血性脑病进行临床分度:当神经症状在新生儿出生后逐渐加剧时,有的病例会出现由兴奋转入抑制甚至昏迷的现象,在72小时的时候会达到最重程度,而72小时后逐渐好转、恢复;临床医生应对新生儿出生后3天内的神经症状做细致的动态观察,并给子分度;轻度症状表现为过度兴奋,肢体出现颤动,肢体肌张力正常或增高,拥抱反射和吸吮反射活跃,无惊厥,呼吸规则,瞳孔无改变,脑电图正常;中度症状表现为嗜睡,反应迟钝,肢体肌张力降低,拥抱反射和吸吮反射减弱,出现惊厥,呼吸不规则,瞳孔缩小,脑电图呈低电压、惊厥活动症状;重度症状表现为肢体肌张力松软,拥抱反射和吸吮反射消失,反复发生惊厥,呼吸不规则,瞳孔不对称,对光反应消失,且脑电图呈现暴发抑制或等电位波形。
以上实施例仅用以说明本发明的技术方案,而非对其限制;尽管参照前述实施例对本发明进行了详细的说明,本领域的普通技术人员应当理解:其依然可以对前述实施例所记载的技术方案进行修改,或者对其中部分技术特征进行等同替换;而这些修改或者替换,并不使相应技术方案的本质脱离本发明实施例技术方案的精神和范围。
Claims (7)
1.一种新生儿缺氧缺血性脑病早期诊断方法,包括以下步骤:
S1:新生儿缺氧缺血性脑病前期观察判断:分别对新生儿出生前、出生时、出生后进行重点观察;观察出生前胎儿是否有缺氧及宫内的窘迫表现;观察出生时新生儿是否有窒息现象;观察出生后新生儿是否出现意识异常、肢体异常、原始反射异常、吮吸异常的现象;
S2:新生儿缺氧缺血性脑病后期精确诊断:对前期观察发现有异常现象的新生儿,对其进行颅脑超声检查、CT检查、脑电图检查;通过颅脑超声检查与CT检查对新生儿进行脑部诊断,确认新生儿是否存在缺氧缺血性脑病,且是否并存脑水肿、脑室内出血;通过对新生儿进行一段时间脑电图检查,确认新生儿脑部是否存在异常症状;在脑电图检查过程中,需清洁头皮、去除胎脂,使头皮电极电阻<10千欧,走纸速度为15mm/s;
S3:新生儿缺氧缺血性脑病临床分度;通过对新生儿出生后三天内的神经症状做细致的动态观察,并对其进行子分度;新生儿出生后24小时内症状表现最明显的为轻度;新生儿出生后24至72小时内症状表现最明显的为中度;新生儿出生至72小时内症状表现最明显的为重度。
2.根据权利要求1所述的一种新生儿缺氧缺血性脑病早期诊断方法,其方法在于,胎儿宫内窘迫表现为胎动明显减少,胎心变慢<100/分钟,胎粪污染羊水呈Ⅲ度以上混浊。
3.根据权利要求1所述的一种新生儿缺氧缺血性脑病早期诊断方法,其方法在于,新生儿出生后出现意识异常的表现为过度兴奋、嗜睡、昏睡甚至昏迷;新生儿出生后出现肢体异常的表现为肢体张力减弱、松软;新生儿出生后出现原始反射异常的表现为拥抱反射过分活跃,减弱或消失;新生儿出生后出现吮吸反射异常的表现为吸吮反射减弱或消失。
4.根据权利要求1所述的一种新生儿缺氧缺血性脑病早期诊断方法,其方法在于,所述颅脑超声检查中提示存在脑水肿的表现为脑实质内广泛均匀分布的轻度回声增强,伴脑室、脑沟及半球裂腺的变窄或消失,及脑动脉搏动减弱;所述颅脑超声检查中提示存在基底神经节和丘脑损伤的表现为基底神经节和丘脑呈双侧对称性强回声反射;所述颅脑超声检查中提示存在大脑大动脉及其分支的梗塞的表现为脑动脉分布区见局限性强回声反射。
5.根据权利要求1所述的一种新生儿缺氧缺血性脑病早期诊断方法,其方法在于,CT检查时,CT扫描测定脑白质的CT值;足月儿的脑白质CT值大于20Hu为正常,足月儿的脑白质CT值小于20Hu为低密度;所述CT检查中早产儿的枕区和足月儿的额区呈现低密度为正常;所述CT检查中脑水肿的表现为双侧大脑半球呈弥漫性低密度影,脑室变窄,甚至消失;所述CT检查中基底神经节和丘脑损伤的表现为双侧基底神经节和丘脑呈对称性密度增高;所述CT检查中大动脉及其分支的梗塞的表现为脑大动脉分布区见脑组织密度降低;所述CT检查中脑室周围白质软化的表现为脑室周围与侧脑室前角外上方呈对称性低密度区。
6.根据权利要求5所述的一种新生儿缺氧缺血性脑病早期诊断方法,其方法在于,所述CT检查中脑白质低密度分布范围可分为轻度、中度、重度;其中轻度是指散在局灶低密度影分布2个脑叶;中度是指低密度影超过2个脑叶,且白质灰质对比模糊;重度是指弥漫性低密度影,灰质白质界限消失。
7.根据权利要求1所述的一种新生儿缺氧缺血性脑病早期诊断方法,其方法在于,所述新生儿缺氧缺血性脑病临床分度中的轻度症状表现为过度兴奋,肢体出现颤动,肢体肌张力正常或增高,拥抱反射和吸吮反射活跃,无惊厥,呼吸规则,瞳孔无改变,脑电图正常;所述新生儿缺氧缺血性脑病临床分度中的中度症状表现为嗜睡,反应迟钝,肢体肌张力降低,拥抱反射和吸吮反射减弱,出现惊厥,呼吸不规则,瞳孔缩小,脑电图呈低电压、惊厥活动症状;所述新生儿缺氧缺血性脑病临床分度中的重度症状表现为肢体肌张力松软,拥抱反射和吸吮反射消失,反复发生惊厥,呼吸不规则,瞳孔不对称,对光反应消失,且脑电图呈现暴发抑制或等电位波形。
