Description

Tendon suture system for joint replacement
Technical Field
The invention relates to the field of medical operations, in particular to a tendon suture system for joint replacement.
Background
Joint replacement is a surgical operation in which a joint prosthesis imitating an artificial joint is placed in a body to replace a diseased joint to achieve the purposes of relieving pain and recovering the function of the joint. The joint replacement requires a suture and repair treatment of the tendon. At present, the tendon repair technology in the common surgical operation is mainly to fix the sutured tendon on the bone beside the joint through screws or other instruments, so that in the joint replacement operation, conventional tendon suture products such as a tendon repair net, a tendon repair sleeve, an artificial suture tendon kit and the like cannot be fixed on an effective position, which directly leads to that the effective tendon suture operation is difficult to be carried out after the joint replacement is carried out.
Disclosure of Invention
The present invention is directed to a tendon suture system for joint replacement, which solves one or more of the problems of the prior art and provides at least one of the advantages.
The technical scheme adopted for solving the technical problems is as follows:
a tendon suture system for joint replacement surgery, comprising:
the joint prosthesis is provided with an outer convex surface, and a mounting groove is arranged on the outer convex surface;
the stitching instrument comprises a first end and a second end, wherein the first end is oppositely positioned at the upper part of the stitching instrument, the second end is oppositely positioned at the lower part of the stitching instrument, the first end is provided with a stitching hole for tendon bundling and fixing, and the shape of the second end is matched with that of the mounting groove, so that the second end of the stitching instrument can be embedded into the mounting groove;
a locking structure for securing the stapler to the joint prosthesis against loosening;
the bone screw thread system comprises a plurality of bone screw holes and bone screws matched with the bone screw holes, the bone screw holes are formed in the joint prosthesis, and the bone screws are used for fixing the joint prosthesis to an external backbone after penetrating through the bone screw holes.
One of the methods of use of the present invention is as follows:
during surgery, according to the joint replacement process, firstly removing the necrotic joint and installing the joint prosthesis at a proper position; after the position of the joint prosthesis is determined, screwing in a bone screw, and fixedly connecting the joint prosthesis with the backbone firmly through a bone screw thread system; after the broken tendon and the artificial tendon are sutured well, putting the artificial tendon part into the position of the first end of the suturing device, and then suturing the suture holes of the artificial tendon and the suturing device together by using a suture line; the stitching instrument is inserted into the mounting groove on the joint prosthesis, and the stitching instrument is fixed on the joint prosthesis through the locking structure by aligning the second end of the stitching instrument with the mounting groove when the stitching instrument is placed.
Therefore, the invention can effectively combine the stitching instrument with the joint prosthesis, fix the combined assembly body on the joint backbone, and simultaneously successfully ensure the continuity of the tendon at the injured part, thereby being beneficial to the health of the tendon after the operation without displacement.
In some embodiments, the mounting groove is located at an apex of the outer convex surface geometry. For most joint prostheses used in joint replacement surgery, the tendon needs to have a terminal force point which is in accordance with traction of the human joint after repair, so that the arrangement of the mounting groove at the vertex position of the geometrical shape of the outer convex surface can enable the repaired joint to be closer to the original joint.
In some embodiments, the first end is generally ring-shaped, a central axis of the ring-shaped is called a first central axis, the first central axis is parallel to the concave direction of the mounting groove, and the suture holes are uniformly distributed on the peripheral wall of the first end around the first central axis. The suture holes uniformly distributed on the peripheral wall of the first end are more beneficial to the binding and fixing of the tendon in the matched operation, and the coverage angle is wider.
In some embodiments, the locking structure comprises:
the positioning ridge is arranged on the peripheral wall of the second end;
the positioning notch is matched with the positioning ridge and used for inserting the positioning ridge;
the locking screw hole comprises a first section of screw hole and a second section of screw hole which is coaxial with the first section of screw hole and is matched and continued with the first section of screw hole in a threaded manner, the first section of screw hole is arranged on the joint prosthesis, the second section of screw hole is arranged on the peripheral wall of the second end and avoids the positioning ridge, and the locking screw hole is obliquely directed to the first central axis;
and the locking screw is matched with the locking screw hole in shape, and the same locking screw hole is provided with one locking screw.
In the embodiment, the positioning edges and the positioning notches are mutually inserted and nested, so that the stitching instrument is quickly positioned, the second end of the stitching instrument is not easy to rotate after being embedded into the positioning groove, dislocation is prevented, and the stitching instrument can be stably installed on the joint prosthesis after the locking screw penetrates through the first section of screw hole and the second section of screw hole. Because the displacement dislocation between the joint prosthesis and the stitching instrument is mostly generated in the tangential direction, the locking screw hole is obliquely directed to the first central axis, so that the dislocation direction is avoided, the axial traction force of the locking screw is reduced, and the assembled structure of the whole assembly body is more stable.
In some embodiments, the mounting groove is generally a cylindrical slot and the second end is generally cylindrical with an outer diameter that is a clearance fit with the mounting groove; the positioning ridge protrudes outwards from the outer peripheral wall of the second end and is in a semi-cylindrical shape, and the extending direction of the positioning ridge is parallel to the first central axis; the locking screw holes, the positioning notches and the positioning edges are distributed around the first central axis at equal intervals. The cylindrical and semi-cylindrical shapes are matched, so that assembly actions among the components can be guided and are not easy to clamp, the due positioning effect of the positioning notch and the positioning edge is reserved, and the locking screw hole, the positioning notch and the positioning edge are distributed around the first central axis at equal intervals, so that convenience in component assembly is improved.
In other embodiments, the second end has an overall outer perimeter dimension that is less than the diameter of the bottom surface of the first end such that the bottom perimeter of the first end and the second end define an annular surface therebetween, the locking structure comprising:
the positioning column is arranged on the annular surface;
the positioning hole is arranged at the bottom of the positioning groove, is matched with the positioning column and is used for the insertion of the positioning column;
the connecting hole penetrates through the bottom of the mounting groove, and the second end penetrates through the connecting hole;
the elastic connecting cap is integrally made of elastic materials and is in an inverted umbrella shape with a large upper part and a small lower part, the upper end of the elastic connecting cap is connected with the lower part of the second end, the maximum diameter of the elastic connecting cap is slightly larger than the aperture of the connecting hole, and the distance between the upper end of the elastic connecting cap and the annular surface is slightly larger than the thickness of the bottom of the mounting groove, so that the elastic connecting cap can be extruded and deformed to pass through the connecting hole from top to bottom and then elastically recover and fasten the first end of the stitching instrument in the mounting groove.
Those skilled in the art should understand that the maximum diameter of the elastic coupling cap is greater than the diameter of the coupling hole and the distance between the upper end of the elastic coupling cap and the annular surface is greater than the thickness of the bottom of the mounting groove, because the elastic connecting cap is made of elastic material, the elastic connecting cap can be extruded and deformed, and after the elastic connecting cap passes through the connecting hole from top to bottom, the elastic connecting cap is elastically restored, the upper end of the elastic connecting cap is slightly larger than the aperture of the connecting hole, so that the second end can be prevented from falling out of the connecting hole, the upper end surface of the elastic connecting cap and the annular surface clamp the bottom of the mounting groove together, the suturing device cannot move up and down, and the matching relation of the positioning column and the positioning hole is combined, the stitching instrument can be firmly and firmly arranged in the mounting groove in the embodiment, is more convenient and quicker to use, and is a scheme suitable for small injured area.
In the above embodiment, further, the lower part of the elastic connection cap is provided with a radial through groove, and the radial through groove divides the lower part of the elastic connection cap into two opposite sides. This increases the amount of compression deformation of the elastic connection cap, facilitating its insertion into and through the connection hole.
In some embodiments, the invention further comprises a shifting fork, wherein the shifting fork comprises a buckle, a connecting handle and a fork head which are connected in sequence, the fork head comprises a left finger and a right finger which are separated from each other, and the shape structure of the right finger and the left finger are mirror symmetry; the ring buckle is annular, the side wall of the ring buckle is provided with at least one bulge, and a central shaft of the ring buckle is called as a second central shaft; the connecting handle is used as a relative transverse shaft, the second central axis is used as a relative vertical shaft and has an up-down direction, the extending locus of the left interdigital comprises a first locus and a second locus, and the joint of the first locus and the second locus is an inflection point; the path of the first track is as follows: firstly, transversely extending from the end part of the connecting handle to the direction far away from the buckle, gradually extending upwards, gradually deviating from the transverse shaft, and then reaching an inflection point; the path of the second track is: and starting from the inflection point, the transverse direction is continued to be far away from the direction of the buckle, and the transverse direction gradually extends downwards and is gradually close to the transverse shaft.
The shifting fork is a tool for assisting in installing the stitching instrument, and the stitching instrument is connected with the artificial tendon in a bundling mode when the stitching instrument is plugged into the installation groove, so that the risk that the stitching instrument and the artificial tendon are pulled by hands to loose or tear in a bundling mode due to the fact that the stitching instrument is pushed and pressed remotely by using the fork head of the shifting fork and the stitching instrument is temporarily positioned is reduced. The two interdigital stretching tracks of the shifting fork are curves, the first track and the second track are designed for matching the trend of the outer convex surface of the joint prosthesis and the shape of the side periphery of the stitching instrument, the stitching instrument can be effectively held by claws, the ring buckle is used as a ring and a hanging piece, and the bulge on the side wall of the ring buckle is convenient to hold and apply force.

The invention includes a variety of embodiments, only two of which are listed below, along with some of their respective sub-embodiments.
The first embodiment is as follows:
referring to fig. 1 to 6, the invention is a kit comprising a joint prosthesis 1, a stapler 2, a locking structure, a bone screw system, a fork 5. Wherein the locking structure employs threads as the primary means of attachment. The joint prosthesis 1 is provided with an outer convex surface, a mounting groove 11 is arranged at the position of the outer convex surface close to the top point of the geometric shape or the position of the top point, the stitching instrument 2 is embedded into the mounting groove 11 and then fixed, the upper end of the stitching instrument 2 is used for being connected with an artificial tendon in a bundling manner, and the joint prosthesis 1 is connected and fixed with an external diaphysis through a bone screw thread system.
The joint prosthesis 1 can be subjected to bionic printing by adopting a 3D printing technology, so that the shape and the precision of a product can be ensured, all thread hole sites can be synchronously printed, and the manufacture and the modification are very flexible.
The suture instrument 2 includes an upper end and a lower end, and the upper end is referred to as a first end, which is integrally formed in a loop shape and provided with a plurality of suture holes 21. The suture device 2 is a key component for connecting the tendon and the joint prosthesis by traction, the shape of the suture device is generally designed symmetrically, and the suture hole 21 is arranged on the peripheral wall of the first end and can be a round hole or a hole with other shapes, so long as the suture device is convenient to bind and combine. The lower part of the stitching instrument 2 narrows to form a step, and then a cylinder grows downwards and serves as a second end of the stitching instrument 2, and the peripheral wall of the second end is uniformly provided with semi-cylindrical positioning edges 22. Correspondingly, the mounting groove 11 is also in a circular hole shape, a semi-cylindrical positioning gap 12 is arranged on the peripheral wall of the hole, and the size, position and number of the positioning gap 12 are all matched with the positioning ribs 22. Thus, when the positioning rib 22 is aligned with the positioning notch 12, the stapler 2 can be positioned in the mounting recess 11 without being rotated. The semi-cylinder refers to a characteristic that the outer peripheral surface belongs to a partial outer peripheral surface of a cylinder, and the arc length of the characteristic can be more than, equal to or less than a semi-arc. Of course, the positioning edge 22 and the positioning notch 12 may have other shapes such as a triangular prism, a rectangular prism, a plum blossom prism, etc., as long as they match each other.
The locking structure adopts threaded connection as a main locking mode, specifically, the locking structure comprises a locking screw hole besides the positioning ridge 22 and the positioning notch 12, the locking screw hole comprises two sections, a first section of screw hole 41 is arranged on the joint prosthesis 1, a second section of screw hole 42 is arranged at the second end of the stitching instrument 2, and after the first section of screw hole 41 and the second section of screw hole 42 are aligned, the locking screw 25 is simultaneously connected with the first section of screw hole 41 and the second section of screw hole 42, so that the stitching instrument 2 is completely and fixedly connected with the joint prosthesis 1. More specifically, the plurality of locking screw holes and the plurality of locking screws 25 are distributed and uniformly distributed around the central axis of the suturing device 2 and obliquely penetrate into the second end of the suturing device 2 from outside to inside, so that the intentional oblique distribution and staggering is favorable for avoiding the influence of axial force on the locking screws 25 and preventing loosening. Of course, since the first threaded hole 41 is obliquely driven into the outer convex surface of the joint prosthesis 1, the first threaded hole 41 has a partially incomplete part, but this does not affect the structural connection. The locking screw 25 is a slender common screw with a small pitch, and when the locking is completed, the top end of the screw will not protrude out of the joint prosthesis 1, and meanwhile, in order to facilitate the screwing of the screw into the printing plate of the joint prosthesis, the head part of the screw is provided with a quincunx countersunk hole.
The bone screw thread system comprises bone screw holes 31 and bone screws 32 matched with the bone screw holes 31, the bone screw holes 31 penetrate through the joint prosthesis 1, the positions of the bone screw holes 31 are generally dispersedly arranged on the periphery of the joint prosthesis 1, and the bone screws 32 can fixedly connect the joint prosthesis 1 with an external backbone after being screwed into the bone screw holes 31. The bone screw 32 used in this embodiment is different from the common screw used in the common joint replacement, and is composed of an upper tapered thread and a lower thread, and the upper tapered thread can provide longitudinal and transverse fixing component force, so as to improve the biting force and holding force of the bone screw 32 and the bone, and make the bone screw closely fit with the joint prosthesis 1. The lower thread part is a triangular thread, which improves the matching degree of the screw and the backbone, and can reduce the use number of the bone screws while ensuring sufficient stability.
The fork 5 comprises a ring 51, a shank 52 and a fork 53, the ring 51 being provided with a protrusion for facilitating gripping and application of force, the fork 53 being divided into a left and a right fork which are mirror images of each other and have a unique trajectory profile, as has been described in the preceding summary. More specifically, the surface of the fork 5 should be smoothly transited to prevent sharp edges and corners from interfering with the surgical operation. The left and right two interdigital are used for assisting in pulling the stitching instrument 2, when in use, the side edge of the first end of the stitching instrument 2 is placed in the clamping range of the interdigital, the lower end of the interdigital naturally hooks the periphery of the first end of the stitching instrument 2, and the vacancy between the interdigital can be convenient for observation and adjustment or even slightly push the stitching instrument 2, so that the stitching instrument 2 can be accurately positioned.
Example two:
referring to fig. 7 to 9, the second embodiment is basically the same as the first embodiment, but the locking structure adopts an elastic buckle as a main connecting means. Specifically, the upper portion of the second end of the stapler 2 is connected to the lower surface of the first end of the stapler 2 and the outer diameter of the second end is smaller than the outer diameter of the first end, thereby leaving an annular surface on which a positioning post 23 is disposed to extend downward, the bottom surface of the mounting groove 11 is provided with a positioning hole 13 matching with the positioning post 23, and the positioning hole 13 and the positioning post 23 function similarly to the positioning notch 12 and the positioning ridge 22 in the first embodiment. And the elastic connection cap 24 is arranged below the second end and is in an inverted umbrella shape with a large upper part and a small lower part, the elastic connection cap 24 is made of elastic materials, and the lowest end of the elastic connection cap is provided with a radial through groove to cut the top of the elastic connection cap into two halves, so that the elastic connection cap 24 is convenient to extrude and deform, and more deformation spaces are reserved. The center of the mounting groove 11 is provided with a connecting hole 14 penetrating through the bottom wall of the mounting groove 11, and when the elastic connecting cap 24 is pressed into the connecting hole 14 from top to bottom and penetrates through the connecting hole, the elastic connecting cap 24 restores to the original appearance, thereby clamping the suturing device 2 without falling out of the mounting groove 11. Further, the annular surface is provided as an inclined funnel surface, and accordingly, the upper surface of the bottom wall of the mounting recess 11 is also provided as a funnel surface having the same inclination angle, which further facilitates the fitting of the components and prevents relative rotation.
The other parts of the suturing device 2 may be integrally formed of the same material as the elastic connecting cap 24.
While the preferred embodiments of the present invention have been illustrated and described, it will be understood by those skilled in the art that the present invention is not limited to the details of the embodiments shown and described, but is capable of numerous equivalents and substitutions without departing from the spirit of the invention as set forth in the claims appended hereto.
