Description

Neonate does not have nasal obstruction of wound breathing machine
Technical Field
The invention relates to the technical field of medical instruments for newborns, in particular to a nasal plug of a noninvasive ventilator for newborns.
Background
In the process of rescuing the critically ill infant in a Neonatal Intensive Care Unit (NICU), the ventilator is often used as an important rescue treatment device to assist the critically ill infant in breathing, so as to achieve the effects of improving ventilation and poor ventilation, reducing lung injury, reducing hemodynamic changes, reducing respiratory muscle work, treating respiratory failure, and avoiding complications such as brain injury. The noninvasive ventilator provides a certain positive pressure for the sick children who breathe autonomously to increase the functional residual capacity, ensure that the respiratory tract and the alveoli of the sick children are in an expansion state under the abnormal condition, prevent the alveoli from collapsing, and is mainly suitable for the sick children who have dyspnea such as apnea, respiratory distress syndrome, infectious pneumonia and the like. With the increase of the success rate of the therapy of the extremely low/ultra-low birth weight infants, the infants often need continuous positive pressure ventilation for 30-90 days.
The nasal plug is an important accessory for connecting the noninvasive ventilator with the infant, and the nasal plug type or nasal mask type auxiliary ventilation is adopted clinically for the infant, but at present, certain defects exist in the two modes in the using process:
1. rhinobyon type
(1) Because the nasal plug is connected with the air supply pipeline and is worn by the sick child, the air supply pipeline can generate backward traction force on the nasal plug, and the nasal plug can generate extrusion force on the nasal septum and the nasal tip, the nasal cavity of the sick child is easy to deform backwards after the nasal plug is worn for a long time, and the phenomenon of 'pig nose' is serious and even appears.
(2) The nasal plug air supply needs to be connected into the nasal cavity of the infant, in order to keep the nasal plug airtight and guarantee the ventilation effect, the diameter of the nasal plug is usually larger than or equal to that of the nasal cavity of the infant, the nasal plug presses the nasal mucosa for positive airway pressure for a long time, and the nasal mucosa, the nasal septum and the middle part of a person are very easily damaged and even bleeds after the nasal cavity is continuously pressed due to the fact that the respiratory mucosa of the infant is particularly delicate and fragile and has incomplete barrier function.
(3) The noninvasive assisted ventilation patient end comprises an air inlet end threaded pipe, a generator and a nasal plug, and is formed by combining three parts clinically used, after the noninvasive assisted ventilation patient end is connected with an air supply pipeline and is worn by a child patient, the phenomenon that the nasal plug and the generator fall off is frequently caused by factors such as twisting and fidgetiness of the child patient in the air supply process, so that the positive pressure ventilation is changed into normal pressure oxygen supply, and the recovery of the child disease is influenced.
2. Nose mask
For extremely low birth weight/ultra-low birth weight infants clinically, aiming at the phenomena of nasal cavity deformation, nasal mucosa crush injury/bleeding, nasal obstruction falling and the like, the nasal cavity is often treated by ways of pasting artificial skin/hydrocolloid dressing, loosening the nasal obstruction every 2 hours, coating cod liver oil in the nasal mucosa every 2 hours or replacing the nasal mask and the like, so that the workload of clinical nurses is increased, and the ventilation effect is not satisfactory. Therefore, the nose of the patient is very small and the nose bridge is short, and the nasal mask is usually dislocated in the nasal mask type auxiliary ventilation process and sometimes even dislocated to the lips or eyes of the patient; compared with nasal obstruction type ventilation, the nasal mask type auxiliary ventilation effect is not good.
