Abstract

技术领域
本申请涉及生物医学技术领域,具体涉及肝内胆管细胞癌患者预后检测生物标志物及检测试剂盒。
背景技术
肝内胆管细胞癌(Intrahepatic Cholangiocarcinoma, ICC)是发病率排名第二位的原发性肝癌,其发病率仅次于肝细胞癌,且恶性程度高,预后极差。由于ICC发病隐匿、恶性程度高,许多ICC病人在初诊时就已经不适合手术治疗。尽管如此,手术切除仍是最有效的根治有段。然而,手术治疗后的患者其五年生存率通常不佳,仅为15%-45%左右。即使进行了根治性切除,仍有57.9%~73.4%的ICC患者出现复发,并且41.3%~42.5%的ICC患者死于复发。
对于可以进行手术切除治疗的ICC患者,准确地预测其预后生存期具有重要的临床、科研及社会价值。准确的预后生存期预测可以指导医生针对高风险ICC患者制定个性化的检查及治疗方案,指引ICC患者依从治疗计划以避免过度医疗,有效降低复发的风险。由此,国内外涌现大量关于ICC预后相关危险因素的研究。但是,目前报道的预后评估模型通常包括肿瘤大小、血管侵犯、肿瘤数目、转移情况以及形态学特征等多项因素,患者需要做大量的临床医学检测,且评估计算方法复杂,评估结果一致性有限。并且,目前尚未有一种生物标志物或危险因素可以作为ICC预后的单一预测因子。
发明内容
本发明提供了一种肝内胆管细胞癌患者预后检测生物标志物及检测试剂盒,利用患者离体样本中特定基因的启动子区域甲基化分析,实现肝内胆管细胞癌患者预后生存期的准确评估。
本发明提供如下技术方案:
第一方面,提供一种用于肝内胆管细胞癌患者预后的生物标志物,包括:SEQ IDNo.1至SEQ ID No.24所示的多核苷酸序列的集合,或者与SEQ ID No.1至SEQ ID No.24互补的多核苷酸序列的集合。
根据本发明所述的生物标志物,所述多核苷酸序列包含甲基化的CpG二核苷酸(胞嘧啶-磷酸-鸟嘌呤二核苷酸)。
第二方面,提供一种用于肝内胆管细胞癌患者预后的试剂,包括:能特异性检测SEQ ID No.1至SEQ ID No.24所示的多核苷酸序列的DNA甲基化水平的试剂,或者能特异性检测与SEQ ID No.1至SEQ ID No.24互补的多核苷酸序列的DNA甲基化水平的试剂。
根据本发明所述的试剂,包括:
(1)能差异修饰甲基化DNA和非甲基化DNA的反应试剂,以及
(2)检测试剂,在用所述反应试剂处理DNA样本之后,所述检测试剂能确定SEQ IDNo.1至SEQ ID No.24所示的多核苷酸序列或者与SEQ ID No.1至SEQ ID No.24互补的多核苷酸序列中的每一个CpG是甲基化的还是非甲基化的。
根据本发明所述的试剂,所述试剂是选自以下方法的一个或多个所用的试剂:基于重亚硫酸盐转化的PCR(例如甲基化特异性PCR)、DNA测序(如亚硫酸氢盐测序、全基因组甲基化测序、简化甲基化测序)、甲基化敏感的限制性内切酶分析法、荧光定量法、甲基化敏感性高分辨率熔解曲线法、基于芯片的甲基化图谱分析、质谱(例如飞行质谱)。优选地,所述试剂用于亚硫酸氢盐测序。
根据本发明所述的试剂,包括:重亚硫酸盐或其衍生物和PCR试剂。

第三方面,提供一种用于检测肝内胆管细胞癌患者预后的试剂盒,包括:第二方面所述的试剂。

本发明还提供生物标志物在制备用于肝胆管细胞癌患者预后评估的试剂或试剂盒中的应用,其中,所述生物标志物包括SEQ ID No.1至SEQ ID No.24所示的多核苷酸序列的集合,或者与SEQ ID No.1至SEQ ID No.24互补的多核苷酸序列的集合。优选地,所述多核苷酸序列包括甲基化的CpG。

有益效果
本申请提供了一种肝内胆管细胞癌患者预后检测生物标志物及检测试剂盒,利用患者离体样本中特定基因的启动子区域甲基化分析,实现肝内胆管细胞癌患者预后生存期的准确评估。该生物标志物可以作为肝内胆管细胞癌患者预后的单一预测因素,也可以基于包含基因启动子甲基化评分在内的多个变量来预测肝内胆管细胞癌患者预后的生存期。相较于现有的肝内胆管细胞癌患者预后生存期预测标志物,具有无创检测、准确率高、可靠性强、预测稳定的优点,可以应用于临床实践以指导肝内胆管细胞癌患者预后的治疗方案。

本文中,甲基化水平代表核苷酸序列中一个或多个位点处于甲基化状态的比例。一个区域的甲基化水平是该区域中所有位点的甲基水平的均值。因此,区域的甲基化水平上升或下降并不表示区域中所有甲基化位点的甲基化水平都上升或下降。本领域知晓将检测DNA甲基化的方法(例如简化甲基化测序)所得结果转化为甲基化水平的过程。示例性实施例利用MOABS软件获得CpG位点的甲基化水平参数;例如,利用MOABS软件获得公式(1)中M系数所代表的CpG位点的甲基化水平参数。
本发明涉及分析ICC患者预后的方法,更具体来说,本发明提供了可作为ICC患者预后评估的生物标志物,该生物标志物包括如SEQ ID No.1-SEQ ID No.24所示的多核苷酸序列的集合或其互补序列的集合。如SEQ ID No.1-SEQ ID No.24所示的多核苷酸序列的具体信息如表1所示,24个多核苷酸序列为特定基因的启动子区域,是以转录起始位点为基础的上游2000bps和下游2000bps组成的总长度为4000bps的多核苷酸序列。本发明的发明人发现,该24个启动子区域的甲基化评分可作为ICC患者预后评估的单一预测因子。

进一步地,本发明还提供了可用于ICC患者预后评估的试剂,所述试剂是检测SEQID No.1-SEQ ID No.24所示的多核苷酸序列甲基化水平的试剂。
在一些实施方案中,所述ICC患者预后评估包括:(a)获取SEQ ID NO.1至SEQ IDNO.24所示的多核苷酸序列或其互补序列的甲基化水平参数;以及(b)将步骤(a)获取的甲基化水平参数用于ICC患者病程分析。
在本发明的步骤(a)中,对患者来源的离体样本进行处理,分离基因组DNA或cfDNA,获得DNA样本,然后将所述DNA样本用检测试剂进行处理,通过基因测序获得所述多核苷酸序列的甲基化水平参数。在一些实施方案中,所述试剂包括:
(1)能差异修饰甲基化DNA和非甲基化DNA的反应试剂,以及
(2)检测试剂,在用所述反应试剂处理DNA样本之后,所述检测试剂能确定SEQ IDNo.1至SEQ ID No.24所示的多核苷酸序列或者与SEQ ID No.1至SEQ ID No.24互补的多核苷酸序列中的每一个CpG是甲基化的还是非甲基化的。
在一些实施方案中,所述试剂是选自以下方法的一个或多个所用的试剂:基于重亚硫酸盐转化的PCR(例如甲基化特异性PCR)、DNA测序(如亚硫酸氢盐测序、全基因组甲基化测序、简化甲基化测序)、甲基化敏感的限制性内切酶分析法、荧光定量法、甲基化敏感性高分辨率熔解曲线法、基于芯片的甲基化图谱分析、质谱(例如飞行质谱)。优选地,所述试剂用于亚硫酸氢盐测序。所述测序过程可以采用本领域已知的DNA测序方法,例如WGBS高通量测序法。
在一些实施方案中,所述试剂包括:重亚硫酸盐或其衍生物、和PCR试剂。
在一些实施方案中,所述PCR试剂包括能特异性扩增SEQ ID No.1至SEQ ID No.24所示的多核苷酸序列或者与SEQ ID No.1至SEQ ID No.24互补的多核苷酸序列的引物对和/或特异性探针。其中,本领域技术人员可以根据本领域已知的PCR引物设计方法/工具和探针设计方法/工具获得所述引物对和特异性探针。所述引物可以是扩增SEQ ID No.1-SEQID No.24中任一序列中一个或多个甲基化位点的PCR引物。这些引物的扩增产物可以包含区域中某一个或多个或全部甲基化位点,在对扩增产物中包含的甲基化位点进行检测后,获取整个启动子区域的甲基化水平。
在本发明的步骤(b)中,将步骤(a)所获得的SEQ ID No.1至SEQ ID No.24所示的多核苷酸序列或其互补序列的甲基化水平参数通过公式(1)计算得到所述启动子区域的甲基化评分,进而基于该评分进行ICC患者的病程分析。
在一些实施方案中,所述ICC患者病程分析包括以下一项或多项中:对患者进行分类、提供预后、监测疾病进程、预测治疗干预的功效、挑选肿瘤治疗策略或评估治疗干预的功效。本发明的发明人尤其发现,通过公式(1)计算得到的启动子区域的甲基化评分在评估ICC患者的生存期中有良好的有效性和一致性。该甲基化评分模型可以单独使用、作为单一预测因子对ICC患者进行分类、提供预后等病程分析;或者,所述甲基化评分可与其他因素,例如腹水状态、肿瘤大小、大血管浸润状态、微血管浸润状态、淋巴结转移状态、肿瘤分化程度以及CA19-9糖抗原浓度中的一种或多种组合使用。
所述对患者进行分类或提供预后包括根据所述甲基化评分将患者分类为不良预后或良好预后;或者分类为高危、中危或者低危组;或者,将患者分类为具有短生存期或长生存期。所述预测治疗干预的功效包括在治疗干预之前、期间和/或之后监测患者病程情况,对比治疗干预之前、期间或之后患者的生存期,进而预测治疗干预手段的有效性。
在一些实施方案中,基于该评分进行ICC患者的病程分析包括基于一甲基化评分参考阈值,指示ICC患者具有短生存期或长生存期。低甲基化评分的患者相对于高甲基化评分的患者具有更长的生存期。在一些实施方案中,所述甲基化评分参考阈值为-11.20716。
本发明中,所述步骤(b)采用至少一种算法和/或计算机进行。本领域技术人员可以采用本领域内已知的任何方式,采用基因表达信息来分析患者(例如归类、提供预后、挑选治疗策略等)。一般来说,采用训练群体的表达值建立数学模型,该模型将基因表达值作为输入并输出预后结果。然后采用该数学模型来分类(例如将不良或良好预后分配给)新患者。
在一些实施方案中,本发明提供一种装置,所述装置包括存储器、处理器以及存储在所述存储器上并可在处理器上运行的计算机程序,所述处理器执行所述计算机程序时实现以下步骤:
(1)获取SEQ ID NO.1至SEQ ID NO.24所示的多核苷酸序列或其互补序列的甲基化水平参数;
(2)将所述甲基化水平参数通过计算得出SEQ ID NO.1至SEQ ID NO.24所示的多核苷酸序列的甲基化评分;
(3)根据评分预测肝内胆管细胞癌患者的预后参数;
其中,所述甲基化评分通过公式(1)计算获得。
在一些实施方案中,所述步骤(1)包括采用本发明的试剂或试剂盒对所述DNA样本进行处理,然后通过基因测序和计算获得所述甲基化水平参数。
在一些实施方式中,本发明提供的计算机可读存储介质,存储有计算机程序,当所述计算机程序被处理器执行时,实现下述步骤:
(1)获取SEQ ID NO.1至SEQ ID NO.24所示的多核苷酸序列或其互补序列的甲基化水平参数;
(2)将所述甲基化水平通过计算得出SEQ ID NO.1至SEQ ID NO.24所示的多核苷酸序列的甲基化评分;
(3)根据评分预测肝内胆管细胞癌患者的预后参数;
其中,所述甲基化评分通过公式(1)计算获得。
根据本发明的计算机可读存储介质,所述步骤(1)包括采用试剂或试剂盒对所述DNA样本进行处理对所述DNA样本进行处理,然后通过基因测序和计算获得所述甲基化水平参数。

除非另有说明,以下实施例中使用的原料和试剂均为市售商品,或者可以通过本领域已知方法制备。
本发明实施例基因组DNA分离由WGBS(全基因组亚硫酸盐测序)数据库的生成和测序由中国的Novogene(北京诺禾致源科技股份有限公司)完成。
本发明实施例中涉及的试剂盒Tiangen DP304购置于天根生化科技(北京)有限公司;DNA分析试剂盒Qubit®2.0 Flurometer购置于美国加利福尼亚州生命技术公司;Agilent Bioanalyzer 2100购置于安捷伦。
本发明实施例中所有操作均符合制造商的说明。
本发明实施例所用的样本是针对四川大学华西医院、复旦大学中山医院、天津医科大学肿瘤医院接受根治性肝切除的ICC患者的多中心回顾性研究。从2010年5月至2019年7月,四川大学华西医院(WCH队列)共有164例患者,复旦大学中山医院(ZSH队列)有117例患者,天津医科大学肿瘤医院(TMU队列)有53例患者被纳入我们的研究。所有患者均在组织学上首次被诊断为ICC,且不包括复发ICC患者。该研究方案已获得每个中心伦理委员会的批准,并且在手术前已获得每位患者的书面知情同意。患者的队列分组如表2所示,基线特征如表3所示。

如表3所示,两个队列之间一些基线特征不平衡。这种差异可以用地缘和经济因素解释来解释,因为西南大陆(训练队列)的社会经济状况落后于中国东部沿海地区(验证队列),但该基线特征不平衡的程度有限,不影响结果的可信度。
实施例1:启动子区域甲基化水平检测
首先,使用TiangenDP304试剂盒分离基因组DNA,并使用琼脂糖凝胶监测DNA的降解和污染。用分光光度计(IMPLEN,CA,USA)测量DNA纯度。使用Qubit®2.0 Flurometer中的DNA分析试剂盒确认DNA浓度,用Covaris S220超声处理至200-300bp,然后加末端修复和腺苷酸化,将掺有26 ng λDNA的总量为5.2μg的基因组DNA片段化。根据试剂盒制造商的说明,将胞嘧啶甲基化的条形码连接到超声处理的DNA 。使用EZ DNA Methylation-GoldTM试剂盒(Zymo Research)用亚硫酸氢盐处理连接得到的DNA片段两次。使用KAPA HiFi HotStartUracil + ReadyMix(2X)试剂盒对亚硫酸氢盐处理的单链DNA片段进行PCR扩增。文库浓度通过Qubit 2.0荧光计(Life Technologies,CA,USA)和定量PCR定量,插入片段大小在Agilent Bioanalyzer 2100系统上测定。
在Illumina NovaSeq平台上对所制备的基因文库进行测序,并产生150bp的配对末端读数。使用Illumina CASAVA流水线进行图像分析和碱基检出,最终产生150bp的配对末端读数。每个样品的测序深度至少达到30×。
采用FastQC生成原始WGBS测序读数的质量报告(Andrews,2010年)。 Fastp用于C++开发的质量控制和低质量读过滤,它对高通量测序读执行快速质量控制和数据过滤(Chen等,2018)。从原始reads中修剪适配器(AGATCGGAAGAGCACACGTCTGAACTCCAGTCA,AGATCGGAAGAGCGTCGTGTAGGGAAAGAGTGT)。质量小于3或具有N个基数的两端也被修剪(通过质量值过滤每一条reads:cut_front,cut_front_window_size 1,cut_front_mean_quality 3,cut_tail,cut_tail_window_size 1,cut_tail_mean_quality 3)。使用4个基准的滑动窗口检查平均质量,并修剪基数以使其平均质量低于15(cut_right,cut_right_window_size4,cut_right_mean_quality 15)。由于前10 bps经偏向性的胞嘧啶亚硫氢盐处理,因此将两个末端reads从前部修剪10 bps(trim_front1 10,trim_front2 10)。保留修整reads的最小长度为36 bps(length_required 36)。仅保留正确配对的reads用于下游分析,它们是WGBS文库的干净读段(clean reads)。
采用MOABS软件进行DNA甲基化分析。 MOABS的BSMAP模块用于将clean reads与参考基因组(hg38)进行比对。 MOABS的MCALL模块执行了单个样品分析,以报告整个基因组中CpG二核苷酸的甲基化水平,以及CpG深度覆盖和平均甲基化水平。为了检查启动子区域的甲基化水平,从UCSC表浏览器下载了Gencode转录注释(V33)。在转录起始位点上游2,000bps和下游2,000 bps定义了一个启动子区域,得到总长度4,000 bps启动子区域。将含有CpG位点的平均甲基化水平用来代表启动子区域(SEQ ID No.1-SEQ ID No.24)的甲基化水平。
实施例2 获得启动子区域甲基化评分
利用表1所示的24个启动子区域,计算PMS(promoter methylation score,启动子区域甲基化得分)。PMS是每个候选启动子区域甲基化水平与相应系数的乘积之和。所述肝内胆管细胞癌患者预后评估模型具有一个或多个最佳阈值,所述最佳阈值可将肝内胆管细胞癌患者预后评估模型计算出来的得分对应到高PMS和低PMS中的一组或者是高PMS、中PMS和低PMS中的一组。
PMS的详细公式如公式(1)所示。
训练队列中的164名患者,总生存期(Overall Survival, OS)的C-index为0.772(95%CI:0.731-0.813),PMS的最佳阈值为-11.20716。
验证队列中的170例患者,总生存期的C-index为0.735(95%CI:0.676-0.793)。验证队列中的PMS由与训练队列中的PMS相同的系数计算得出,并且由相同的阈值定义PMS高组和PMS低组。
如图1中A和B所示,结合表4的数据可知,在训练队列中,总生存期的1年,2年和3年AUC(Area Under Curve,曲线下面积)为0.844、0.830和0.883。低PMS和高PMS组分别有96例和68例患者。低PMS组的总生存率明显高于高PMS组(P <0.001,HR = 4.261,95%CI:2.743-6.619)。低PMS组的中位生存期为55.2±3.3个月,一年,三年和五年的总生存率分别为92.7%,71.3%和45.2%。同时,高PMS组的中位生存期为11.8±2.5个月,一年,三年和五年的总生存率分别为48.5%,13.9%和4.9%。
如图1中C和D所示,在验证队列中,总生存期的1年,2年和3年AUC为0.773、0.770和0.779。低PMS组和高PMS组分别有109例和61例。低PMS组的总生存率明显高于高PMS组(P <0.001,HR = 3.999,95%CI:2.283-7.004)。低PMS组的中位生存期为53.0±11.6个月,1年,3年和5年总生存率分别为92.9%,71.3%和42.1%。相比之下,高PMS组的中位生存期为16.9±2.0个月,而1年,3年和5年总生存率分别为66.1%,23.7%和23.7%。
由图1和表4的数据可以看出,在训练队列中PMS的总生存期C-index为0.772。低PMS组的中位生存期显著高于高PMS组;验证队列中PMS的总生存期C-index为0.735,低PMS组的中位生存期明显高于PMS高组。
由此可见,PMS可以精确预测患者的生存情况,并可以在训练队列中将进行根治性手术后的患者分为高PMS组和低PMS组,进而区分高死亡率和低死亡风险组,并且PMS的效用已在验证队列中得到验证。尽管训练队列和验证队列在临床病理和治疗策略存在差异,训练队列中的患者往往病情较差,疾病分期更深,接受辅助治疗的频率较低,但PMS的有效性仍在验证队列中得到证实,这进一步说明了本发明提供的生物标志物在ICC患者预后生存期预测中的有效性。

AJCC TNM:T 分期主要包括肿瘤数目、血管侵犯和肝外直接侵犯三个主要因素;N分期则基于是否有一个或多个区域淋巴结的转移;M分期基于有无远处转移(基于美国癌症联合委员会(American Joint Committee on Cancer, AJCC)颁布ICC 的第8 版TNM 分期系统)。
WCHSU-PMS:为PMS和WCHSU变量相结合的多变量分析。
将PMS预测系统、WCHSU预测系统、JHUSM预测系统、EHBSH预测系统以及AJCC TNM分期系统分别对实施例1中的334名ICC患者进行预测,334名ICC患者作为一个整体而不进行分类,变量信息详见下表5:

采用PMS预测模型、WCHSU预测模型、JHUSM预测模型、EHBSH预测模型、AJCC TNM预测模型、WCHSU-PMS预测模型分别对实施例2中的334名ICC患者构建列线图,所述334名ICC患者作为一个整体而不进行分类,预测结果如图2、图3和表6所示。

以上统计中P值总体上具有统计学意义。
图2中D、E、F、G、H分别显示了PMS、WCHSU、JHUSM、EHBSH以及AJCC TNM分期系统的Kaplan-meier曲线图,分别具有四条清晰分开的Kaplan-meier曲线(整体P <0.001),并且五个模型的Kaplan-meier曲线图均显示为相同的趋势。图3为WCHSU-PMS模型的时间依赖性ROC曲线图。
由表6和图2、图3可知,PMS模型、WCHSU-PMS模型的C指数显著高于WCHSU预测模型、JHUSM预测模型、EHBSH预测模型、AJCC TNM预测模型、WCHSU-PMS预测模型。此外,PMS模型的一年、两年和三年生存期曲线中的AUC也均高于WCHSU、JHUSM、EHBSH以及AJCC TNM分期系统,WCHSU-PMS模型也有较高的三年生存期曲线的曲线下面积(AUC)。
以上实施例和对比例充分说明相较于其他四个模型,PMS和WCHSU-PMS模型具有更高的准确性,即SEQ ID NO.1-SEQ ID NO.24所示多核苷酸序列的集合作为ICC患者预后的生物标志物是有效的,其甲基化水平可以作为肝内胆管细胞癌患者预后的单一预测生物标志物,也可以基于包含基因启动子甲基化评分在内的多个变量来预测肝内胆管细胞癌患者预后的生存期。

Claims (15)

1.一种用于肝内胆管细胞癌患者预后检测的试剂,其特征在于,包括:能特异性检测SEQ ID No.1至SEQ ID No.24所示的多核苷酸序列的DNA甲基化水平的试剂。
2.根据权利要求1所述的试剂,其特征在于,所述试剂包括:
(1) 能差异修饰甲基化DNA和非甲基化DNA的反应试剂,以及
(2) 检测试剂,在用所述反应试剂处理DNA样本之后,所述检测试剂能确定SEQ IDNo.1至SEQ ID No.24所示的多核苷酸序列中的每一个CpG是甲基化的还是非甲基化的。
3.根据权利要求1所述的试剂,其特征在于,所述试剂包括:重亚硫酸盐和PCR试剂。
4.根据权利要求3所述的试剂,其特征在于,所述PCR试剂包括能特异性扩增SEQ IDNo.1至SEQ ID No.24所示的多核苷酸序列的引物对和/或特异性探针。
5.根据权利要求2所述的试剂,其特征在于,所述DNA样本来自哺乳动物的离体生物样本,所述离体生物样本是来自所述哺乳动物的组织、细胞或体液中的至少一种。
6.一种用于检测肝内胆管细胞癌患者预后的试剂盒,其特征在于,包括:权利要求1-5任一项所述的试剂。
7.根据权利要求6所述的试剂盒,其特征在于,所述的试剂盒还包括内参基因、对照物和缓冲液。
8.如权利要求1-5任一项所述试剂在制备用于肝胆管细胞癌患者预后评估试剂盒中的应用。
9.根据权利要求8所述的应用,其特征在于,所述试剂盒用于检测SEQ ID NO.1至SEQID NO.24所示的多核苷酸序列的DNA甲基化水平。
10.根据权利要求8所述的应用,其特征在于,所述预后评估包括:(a)获取SEQ ID NO.1至SEQ ID NO.24所示的多核苷酸序列的甲基化水平参数;以及(b)将步骤(a)获取的甲基化水平参数用于ICC患者病程分析。
11.根据权利要求10所述的应用,其特征在于,所述ICC患者病程分析包括以下一项或多项中:对患者进行分级、分类、提供预后、监测疾病进程、预测治疗干预的贡献、挑选肿瘤治疗策略或评估治疗干预的功效。
12.根据权利要求10所述的应用,其特征在于,所述步骤(b)包括将所述甲基化水平参数通过计算得出SEQ ID NO.1至SEQ ID NO.24所示的多核苷酸序列的甲基化评分,根据评分对患者进行预后参数评估;其中,所述甲基化评分通过公式(1)计算获得:
