背景技术
最近开发了靶向癌细胞内p53-HDM2蛋白相互作用的癌症治疗方法。然而,一些类型的癌细胞中不含有p53基因,而有些类型的癌细胞以突变形式和/或无活性的形式表达p53。由于这些类型的癌症不会导致癌细胞死亡,使得靶向p53的癌症治疗方法受到限制,因此,靶向p53的癌症治疗方法对于多种类型的癌症无效。
发明内容

治疗方法
在一种实施方式中,本文公开的组合物引起选择性癌细胞的坏死、膜溶解或穿孔,而不引起其他癌细胞的坏死或膜溶解。在一种实施方式中,本文公开的组合物引起下列癌症的选择性细胞坏死:急性髓细胞性白血病(AML)、慢性淋巴细胞性白血病(CLL)、慢性髓细胞性白血病(CML)、多发性骨髓瘤、胆管肿瘤、胆道肿瘤、骨髓异常增生综合征、真性红细胞增多症、儿童白血病、神经内分泌肿瘤、胶质母细胞瘤、星形细胞瘤、视网膜母细胞瘤、神经母细胞瘤、肉瘤、子宫癌、生殖细胞瘤/癌、睾丸癌、肾母细胞瘤、肾癌、间皮瘤、脂肪肉瘤、纤维肉瘤、纤维组织细胞瘤、尤文肉瘤、伯基特B细胞淋巴瘤、T细胞急性淋巴细胞白血病(ALL)、非霍奇金淋巴瘤、套细胞淋巴瘤、甲状腺肿瘤、膀胱肿瘤、头颈癌、食管癌、肝癌、腹膜癌、胸膜癌、肾上腺肿瘤、胃肠间质瘤(GIST)、表皮样瘤、浆细胞瘤和皮肤T细胞淋巴瘤。
术语“有效治疗量”和“有效量”是指足以杀死一种或多种癌细胞的量。治疗反应可以是使用者(例如临床医生将认识的)表现出对治疗产生疗效的任何反应,包括前述症状和替代临床标志物。因此,治疗反应通常是疾病或病症(如癌症)的一种或多种症状的改善或缓解。
本领域普通技术人员无需过量实验即可容易地确定给予受试者本发明的一种组合物时的适宜剂量。通常,医生将确定最适于单个患者的实际剂量,而且它将取决于多种因素,包括所使用的具体化合物的活性、该化合物的代谢稳定性和作用时间、年龄、体重、一般健康状况、性别、饮食、给药方式和时间、排泄速率、联合用药、具体病症的严重程度以及接受治疗的个体。本文公开的剂量是一般情况的示例。当然,可以有更高或更低剂量范围的个别情况,均在本发明的范围内。
根据需要,药剂可以以0.01~30毫克/千克体重的剂量给予,例如0.1~10毫克/千克体重,更优选0.1~1毫克/千克体重。在一个示例性实施方式中,将对患者施用10~150毫克/天范围内的一个或多个剂量。

适于注射的药物形式包括无菌水溶液或分散体系以及用于临时配制无菌注射溶液或分散体系的无菌粉末。在所有情况下,最终的溶液形式必须是无菌的和液态的。典型的载体包括溶剂或分散介质,其包含例如水溶性缓冲液,即生物相容性缓冲液、乙醇、多元醇如甘油、丙二醇、聚乙二醇、适宜的上述载体的混合物、表面活性剂或植物油。可以利用任何本领域公认的技术进行灭菌,包括但不限于过滤或添加抗细菌剂或抗真菌剂。
给药可以包括接触。术语“接触”是指直接或间接将细胞与本文公开的组合物在物理上接近在一起。接触可以在体外或体内进行。例如,可以通过已知技术将本文公开的组合物递送到细胞中,使细胞与组合物接触,如直接显微注射到肿瘤中、将本文公开的组合物注射到受试者的血液中、在包含本文公开的组合物的培养基中孵育细胞。
本发明的组合物以实现其目的的有效量施用于人。本领域技术人员可以容易地确定待施用的组合物的有效量,例如,在临床前试验和临床试验期间通过医生和临床医生熟知的方法。通常每日剂量包括约1mg~1000mg。

体内方法通常限于将本文公开的组合物(例如上文所述)施用于哺乳动物,优选为人。本文公开的可用于本发明方法的组合物以有效使癌细胞坏死的量施用于哺乳动物,以治疗哺乳动物的癌症。有效量在临床前试验和临床试验期间通过医生和临床医生熟知的方法确定。
可以通过多种公知的药物组合物的施用方法中的任何一种,将有效量的本文公开的可用于本发明方法的组合物,优选为药物组合物,施用于有此需要的哺乳动物。本文公开的组合物可以全身或局部给药。
用于本发明方法的组合物也可以如本领域公知的缓释方式给予哺乳动物。缓释给药是一种在特定时间内达到一定药物水平的药物递送方式。所述水平通常通过血清或血浆浓度来测量。
药学领域中已知的任何制剂均适于本发明方法的组合物的施用。对于口服给药,可以使用液体制剂或固体制剂。制剂的一些实例包括片剂、胶囊剂(如明胶胶囊)、丸剂、含片、酏剂、混悬剂、糖浆、膜剂(wafers)、口嚼剂等。如本领域技术人员所理解的,本文公开的组合物可以与合适的药物载体(载体)或赋形剂混合。载体和赋形剂的实例包括淀粉、牛奶、糖、某些类型的粘土、明胶、乳酸、硬脂酸或其盐(包括硬脂酸镁或硬脂酸钙)、滑石粉、植物脂肪或油、树胶和乙二醇。

本发明的组合物和方法可以与药学上可接受的载体混合或以其他方式组合。如本文所用,“药学上可接受的载体”包括任何和所有溶剂、分散介质、包衣、抗细菌剂和抗真菌剂、等渗剂等。这些介质和试剂的使用在本领域是公知的。

本发明的一种或多种方法可以进一步包括确定本文公开的组合物能否降低癌细胞活性的步骤,还包括测量细胞培养基中乳酸脱氢酶(LDH)的水平,观察细胞穿孔的形成或膜溶解,或观察细胞在一段时间内的分解。可以通过本领域已知的任何方法来测量细胞坏死水平,包括PCR分析、RT-PCR、Northern印迹、蛋白质印迹、免疫组织化学、ELISA测定和荧光素酶报告基因检测。

在另一种实施方式中,本发明的方法可以进一步包括通过在施用本文公开的组合物之前和之后测量诊断性癌症生物标志物的水平来确定本发明的组合物是否引起癌细胞坏死、膜溶解或穿孔的步骤。
在另一种实施方式中,本发明的方法可以进一步包括通过测量血清生物标志物的水平来确定本发明的组合物是否引起癌细胞坏死、膜溶解或穿孔的步骤。在一种实施方式中,血清生物标志物是乳酸脱氢酶(LDH)、细胞角蛋白或HDM-2。血清生物标志物的水平在施用本文公开的组合物之前和之后测量。治疗前后血清生物标志物的增加表明癌细胞坏死、膜溶解或穿孔。在一种实施方式中,增加大于2倍、大于3倍或大于5倍。
本发明的一种或多种方法可以在一个对象上重复或多次重复。例如,如果受试者患有难治性或复发性癌症,可能是希望重复的。此外,如果在一个治疗周期内或根据联合疗法重复施用较低剂量,则可能需要重复施用。
本发明的组合物和方法可以被设计成具有一个或多个期望的特征。当将该组合物施用于至少一种癌细胞或施用于需要癌症治疗的生物体体内,特别是哺乳动物时,所希望的特征可以导致有效性的提高。
理想地,本发明的肽和组合物可以具有α-螺旋-环-α-螺旋的三维结构或构象。这是已确定的PNC-27和PNC-28的肽基组合物的三维形状。α-螺旋-环-螺旋结构允许组合物利于与癌细胞膜相互作用。

另一个期望的特征是用于本发明的方法的组合物及作为本发明的组合物具有相对小的尺寸。大分子肽、非肽和大分子肽/非肽组合物的缺点是比小分子更有可能引起免疫反应,而小分子肽在体内可能不会被识别出。因此,与大分子组合物(即大于35个氨基酸(35AA)的蛋白质)相比,小分子(即小于35个氨基酸(35AA)的肽)不太可能引发针对被治疗的生物体免疫系统的免疫反应。优选地,本发明的合成肽组分约为五十(50)个氨基酸或更少。

PNC-27和PNC-28对癌细胞的细胞毒性

实施例4
PNC-27和PNC-28处理的癌细胞中坏死和凋亡的标志物
细胞死亡可以通过坏死或凋亡来发生。靶向p53的治疗通常通过凋亡导致细胞死亡。坏死不是基因控制的,而凋亡是基因控制的。细胞凋亡是细胞对特定环境和发育刺激或程序性细胞死亡的蓄意反应。经历凋亡的细胞表现出细胞收缩、膜泡化、染色质凝集和断裂。坏死涉及细胞质细胞器的破坏和质膜完整性的丧失。尽管细胞凋亡不具有癌细胞因坏死而死亡时导致的炎症,但靶向p53的治疗不能治疗那些未表达p53的癌症,或者因突变表达出对靶向p53治疗无反应的非活性p53形式的癌症。在半胱天冬酶途径的DNA损伤后,发生了一系列涉及钙激活和钙蛋白酶的事件,这些事件进一步导致其他细胞变化和细胞质酶的调节。在p53依赖性的细胞凋亡过程中,膜联蛋白V结合膜磷酸丝氨酸、Bax wafp21和半胱天冬酶依次表达;这些蛋白质作为p53依赖性凋亡的标志物。
体内细胞坏死和细胞凋亡的主要区别在于,在细胞凋亡中,凋亡细胞在免疫反应出现之前被完全清除。坏死通常会引起免疫反应。尽管凋亡可以被认为是一个干净整洁的过程,靶向p53的治疗并不能在所有类型的癌症病例中引发凋亡。尽管细胞坏死通常可引起靶向HDM-2的治疗部位的免疫反应,但靶向HDM-2的治疗对各种形式的癌症更为有效,包括癌细胞中不存在p53的癌症,或者p53处于突变或失活形式的癌症。
为了确定PNC-27或PNC-28是通过坏死还是通过凋亡诱导细胞死亡,如上所述,通过PNC-27或PNC-28处理的癌细胞中乳酸脱氢酶和半胱氨酸蛋白酶的表达来确定。乳酸脱氢酶的早期释放(乳酸脱氢酶水平的升高)表明细胞坏死。半胱天冬酶水平升高表明细胞凋亡。
对应用PNC-27或PNC-28处理的癌细胞或非癌细胞以及未处理的细胞进行电子显微照相,以确定细胞是否表现出质膜溶解或是否具有完整的质膜。质膜溶解是肿瘤细胞坏死的特征。
实施例5
应用编码PNC-27或PNC-28中p53序列的质粒转染癌细胞。
应用空载体或编码PNC-27或PNC-28中p53序列的绿色荧光蛋白载体转染癌细胞和非癌细胞。转染完成2小时后,使用光学显微镜在载玻片上进行细胞计数,然后对显示绿色荧光蛋白的绿色荧光的细胞数量进行计数。并进行倒置光学显微镜可视化的转染细胞的形态学检查,以鉴别坏死或凋亡的细胞。
用空载体转染的表达绿色荧光蛋白的癌细胞(例如MiApaA-2细胞),将表达PNC-27或PNC-28中p53序列的载体裂解,并针对p53、wafp21(一种由p53依赖性途径诱导的蛋白质)和p5317-26肽本身进行印迹。在这些实验中,使用了识别含有p53残基17-26的决定簇的DO-1抗p53抗体。此外,测量这些细胞中半胱氨酸蛋白酶的活性。为了进行比较,在用0.001~750μM的PNC-27或PNC-28处理的癌细胞上进行相同的实验。测定半胱氨酸蛋白酶活性和p53的waf21水平。肌动蛋白印迹作为对照。
在细胞凋亡的早期,通常在完整细胞双层膜的内小叶中存在磷脂酰丝氨酸(PS),发现于经历凋亡的细胞的外质膜上。膜联蛋白V结合PS,可以通过携带红色荧光TRITC探针的探针来定位。将用PNC-27或PNC-28或对照载体的p53序列转染的细胞进行处理,依次用膜联蛋白V-生物素、链霉亲和素-TRITC染色,并通过共聚焦显微镜检查。还进行了BMRPA1对照细胞的对照实验,所述对照细胞被表达PNC-27或PNC-28中p53序列的载体或对照载体转染,并分析这些细胞的膜联蛋白V的表达。
实施例6
应用仅编码PNC-27或PNC-28中p53序列的pTracer-SV40质粒转染细胞
为了明确界定MRP的作用,确定PNC-27或PNC-28中p53肽本身对肿瘤细胞生长的影响,即,即使没有MRP,其是否能诱导肿瘤细胞坏死。使用组成型表达该肽的pTracer-SV40质粒,PNC-27或PNC-28中p53肽通过转染被引入癌细胞。测量转染细胞中凋亡和坏死标志物的表达,并与用PNC-27或PNC-28处理的癌细胞的重复样品中这些标志物的水平进行比较。BMRPA1对照细胞中进行相同实验。
实施例7
PNC-27或PNC-28中p53肽处理的癌细胞的诱导凋亡
在表达PNC-27或PNC-28中p53肽的癌细胞中进行实验。为了确定是否发生凋亡,测定Wafp21的表达。Wafp21水平升高表明细胞凋亡。同时也测定半胱氨酸蛋白酶水平。半胱氨酸蛋白酶水平增加表明细胞凋亡。膜中膜联蛋白V结合磷脂酰丝氨酸的表达水平是细胞凋亡中已知的早期现象,其在PNC-27或PNC-28中p53肽转染的癌细胞以及用空载体转染的细胞中测定。同时测定LDH的释放水平。如果肽诱导肿瘤细胞坏死,则LDH的释放将会增加。
实施例8
PNC-27或PNC-28活性的体内分析
在重20~22g的Nu/Nu小鼠(Harlan Laboratories,Indianapolis,Ind.,n=10)皮下异种移植活性癌细胞。允许肿瘤扩增生长。对下列活性癌细胞进行检测:急性髓细胞性白血病(AML)、慢性淋巴细胞性白血病(CLL)、慢性髓细胞性白血病(CML)、多发性骨髓瘤、胆管肿瘤、胆道肿瘤、骨髓异常增生综合征、真性红细胞增多症、儿童白血病、神经内分泌肿瘤、胶质母细胞瘤、星形细胞瘤、视网膜母细胞瘤、神经母细胞瘤、肉瘤、子宫癌、生殖细胞瘤/癌、睾丸癌、肾母细胞瘤、肾癌、间皮瘤、脂肪肉瘤、纤维肉瘤、纤维组织细胞瘤、尤文肉瘤、伯基特B细胞淋巴瘤、T细胞急性淋巴细胞白血病(ALL)、非霍奇金淋巴瘤、套细胞淋巴瘤、甲状腺肿瘤、膀胱肿瘤、头颈癌、食管癌、肝癌、腹膜癌、胸膜癌、肾上腺肿瘤、胃肠间质瘤(GIST)、表皮样瘤、浆细胞瘤和皮肤T细胞淋巴瘤。
肿瘤形成后,将小鼠分为三组。一组小鼠施用PNC-27或PNC-28;第二组小鼠施用PNC-29(一种大小相似的对照肽,具有以下氨基酸序列:MPFSTGKRIMLGE);第三组小鼠不施用肽。将肽注射到小鼠体内,剂量方案如下所述:在14天的过程中,每只小鼠的总剂量为1、10、20、30和50mg。
由于应用的动物为Nu/Nu小鼠,其免疫受损,因而当暴露于病原体时,小鼠高度敏感。所以,手术及所有术前和术后的治疗均在无菌罩中进行的。
可选地,应用与上述相同的方法,将上述活癌细胞(1×106细胞/小鼠)移植到一组小鼠的腹膜腔中,并在肿瘤细胞移植的同时将本文公开的组合物注射到右肩区域。
实施例9
本发明小分子对癌细胞的细胞毒性
将本发明的小分子与6×106个癌细胞(例如Mia-PaCa-2细胞)以0.001~750μM的浓度共同孵育5天。在治疗后的不同时间点,评估细胞的坏死情况,例如膜起泡和破裂,以及形成聚集成细胞碎片聚集体的细胞团块。细胞死亡百分比通过测量台盼蓝染料吸收来确定。癌细胞分别应用阴性对照肽PNC-29和仅具有HDM-2靶向组分的小分子(由于小分子缺乏MRC而无法穿过细胞膜)处理,施用与测试肽相同的浓度。测定这些对照肽对细胞生长、细胞形态和生存力的影响。
作为额外的对照,将本文公开的小分子与未转化的BMRPA1腺泡细胞和未转化的乳腺上皮细胞系MCF-10-2A以0.001~750μM的浓度混合,以确定该组合物对正常细胞生长是否具有致死性。

实施例10
抗HDM-2单克隆抗体的制备、鉴定及人源化
制备表达HDM-2或其片段的细胞,用含有25mM EDTA的磷酸盐缓冲液(PBS)收集,并用于免疫BALB/c小鼠。在第0、2、5和7周,小鼠腹膜内注射约溶解约100(~100)个细胞的0.5mL PBS。在第9周和第13周,给具有免疫沉淀32P标记的HDM-2或其片段的抗血清小鼠腹腔注射小麦胚芽凝集素-琼脂糖凝胶(WGA)纯化的HDM-2膜提取物。然后静脉注射0.1mLHDM-2制剂,将脾细胞与小鼠骨髓瘤细胞系X63-Ag8.653融合。通过ELISA和放射免疫沉淀筛选杂交瘤上清液中的HDM-2结合物。
