the review plan making module is used for making a review plan of the patient based on the nearest classifier according to the medical advice and the evaluation result of the chronic disease evaluation module; the rechecking plan is a chronic disease condition evaluation result uploaded by each expert and a rechecking suggestion corresponding to the chronic disease condition evaluation result;
and the abnormal condition management module is used for realizing the management of the patient reinspection condition data, and starting the short message early warning module to carry out early warning when a patient who does not complete the reinspection plan within the preset time is found.
