Description

Outward-turning type annular anastomat with fixing device
Technical Field
The invention relates to the field of medical instruments, in particular to an outward-turning type annular anastomat with a fixing device, and further relates to the field of digestive tract anastomats.
Background
The anastomat is a device widely used in medicine to replace manual suture, the main working principle is to cut and anastomose tissues by using titanium nails, and the anastomat is particularly suitable for operations of hollow organs such as esophagus, stomach, small intestine, large intestine, rectum and the like, and can be used for the operations of the hollow organs.
At present, a linear cutting anastomat and a circular anastomat are mainly used in an operation, a long pipe is arranged on a body of each of the linear cutting anastomat and the circular anastomat for conveniently extending into a human body, the circular anastomat is generally used for aiming at circular suture openings of cavity tissues such as esophagus, small intestine and the like, when the circular anastomat is used for performing lumen tissue anastomosis, one end of the lumen tissue is firstly sewed with a purse, then a nail anvil cap is embedded, a small opening is formed in the lumen tissue at the other end, and the circular anastomat is inserted for anastomosis.
However, the problems of the prior art solutions are as follows: the side wall incision needs to be closed after the end-to-end anastomosis is finished, so that the chances of intestinal tract operation trauma and operation pollution are increased, bleeding and foreign body residue are easily caused, the side wall incision is inverted during anastomosis, a gap exists between a mucous membrane layer and the mucous membrane layer, and hidden troubles are buried when the fistula occurs in the postoperative anastomotic stoma.
A circular anastomat is a mechanical digestive tract reconstruction operation tool developed in nearly two thirty years, mainly uses titanium or tantalum metal nails to fix intestinal tissues, compared with the traditional manual suture of needle threads, the anastomat can simplify the operation process, shorten the operation time, enable operations with difficult exposure and small operation space to be simpler and more convenient, is in an inverted anastomosis mode, needs to completely place a nail bin assembly in the intestinal tract during working, applies pressure to enable intestinal serosa at two ends to be attached, then triggers the metal nails to fix soft tissues, cuts off redundant residual ends to form digestive tract reconstruction, is the same as the needle thread suture, needs suture materials in the anastomosis process, and inevitably brings inflammatory reaction to the human body.
The ideal tissue repair is that the tissue defect is completely repaired by cells with the original property, and the original tissue structure and function are restored, namely the tissue defect is completely repaired, so that the modern suture technology emphasizes suturing according to anatomical layers, correctly closing and suturing tissues of the same type, improving the healing quality of wounds, and restoring the original anatomical structure and physiological function of the wounds. The intestinal section tissue is mainly divided into three layers of serous membrane, muscle layer and mucosa from outside to inside, and the layer-by-layer involution suture is the most ideal tissue repair mode, but in the clinical use process, the difficulty degree of implementation and the balance of risk and income need to be considered.
In the existing everting anastomat, for example, in the prior art with the patent number of CN201510282427.X, a nail bin and a nail box are respectively an integer, after suturing, a nail anvil, the nail bin and a cutting knife need to be cut off by scissors, the instrument can be opened, the instrument and an intestinal canal tissue can be separated, and the nail anvil needs to bear the larger striking force generated by impacting and bending a suturing nail and cutting off the intestinal canal tissue by the cutting knife, so that the requirement on the material strength is met, and the method for cutting off the nail anvil and the annular knife of the nail bin is difficult to realize.
In addition, the conventional eversion stapler, for example, the prior art eversion stapler of patent No. CN201410461465.7, has complicated structure of the instrument, complicated operation, inability of one-handed operation, and high requirement for the user, and thus, it is difficult to perform the eversion stapling operation, and it does not have a cutting function, and it is necessary to perform a secondary cutting operation after removing the instrument.
Therefore, an outward-turning annular anastomat is needed to be provided to solve the technical problem that after the end-end anastomosis of the intestinal tract is completed by arranging the working part in the intestinal tract side wall incision, the side wall incision needs to be closed, so that the injury of the intestinal tract operation and the operation pollution risk are increased. The existing inversion anastomat and the existing eversion anastomat need to fix excess tissues of an intestinal canal of a sutured part by adopting a manual purse-string method, have requirements on the proficiency of an operator and consume time. Therefore, a method for fixing the everted intestinal tube without a purse string is yet to be proposed.

When the opening limit is needed, the second screw 38 located at the lower part of the locking block 362 is pulled to drive the locking pin 35 to move downwards, so that the guide rail 32 moves forwards, and the anvil portion 1 can be disengaged, as shown in fig. 5.
The function of the firing part 4 is shown in figure 1: providing the firing force required to fire the staples.
The above-mentioned fixing portion 5 includes a fixing clip 51, a fixing puncture needle 52 and a locking screw 53 as shown in fig. 5. The locking screw 53 can lock the fixing clip 51 to the outer wall of the anvil 1. When in use, the fixing clip 51 and the fixing puncture needle 52 compress the everted tissue to achieve the function of fixing the tissue without a purse. The cut-off effect of the tissue after the eversion cannot be influenced by the forced contraction during the cut-off.
The above-mentioned fixed part 5 functions: the everted intestinal canal is respectively fixed on the outer surfaces of the nail anvil and the nail bin, and the support is provided for the semi-ring cutter to cut off the intestinal canal.
The fixing portion 5 can be simplified as an integral part as shown in fig. 7, and the fixing can be completed by inserting the integral fixing puncture needle 61 of the integral fixing clip 6 into the sixth holes 111 of the left anvil 11 and the right anvil 12 of the anvil portion 1 by utilizing the ductility of the plastic material.
The above-described realisation of the anchoring device of the anchoring portion 5 on other eversion staplers, as illustrated in figures 14-1, 14-2 and 14-3, is directly applicable in the corresponding position.
While the spirit and principles of the invention have been described with reference to several particular embodiments, it is to be understood that the invention is not limited to the disclosed embodiments, nor is the division of aspects, which is for convenience only as the features in such aspects may not be combined to benefit. The invention is intended to cover various modifications and equivalent arrangements included within the spirit and scope of the appended claims.
