Abstract
The invention discloses a femoral medial condyle retrograde locking intramedullary needle, which comprises a needle tip and a needle rod, wherein the needle rod comprises a straight rod part and an arc-shaped part, the straight rod part is arranged at the top of the arc-shaped part, the needle tip is arranged at the top of the straight rod part, the top of the needle tip is provided with a 30-degree inclined part, the straight rod part is provided with a first screw and a second screw at the positions close to the needle tip, and the bottom of the arc-shaped part is provided with a third screw, a fourth screw and a fifth screw which are arranged at; the first screws and the second screws are arranged at intervals, and the included angle is 30-40 degrees; the third screw is arranged at the position close to the end part of the arc-shaped part, the third screw is positioned on the center line of the coronal plane, the included angles of the fourth screw, the fifth screw and the third screw are 20-30 degrees, and screw holes respectively matched with the first screw to the fifth screw are arranged on the needle rod. The invention can be used for treating femoral hypomere fracture, femoral unicondylar fracture or femoral intercondylar fracture, has simple operation during treatment, does not damage joints, has good fixing effect and good postoperative knee joint recovery, and can be widely applied to the field of medical appliances.
Description
Technical Field
The invention relates to the field of medical instruments, in particular to a femoral medial condyle retrograde locking intramedullary needle.
Background
The intramedullary pin fixation is an operation of inserting an intramedullary pin with proper size and length into the marrow cavities at both sides of the fracture end to fix the fracture. Intramedullary pins come in different types, such as "V" shaped, quincunx, triangular, etc.
The traditional retrograde intramedullary pin aiming at femoral fracture is mostly drilled from an intercondylar opening of a femur, and a patellar tendon and a joint capsule need to be longitudinally cut, and the retrograde intramedullary pin is drilled from an opening above a cruciate ligament insertion point before the intercondylar opening of the femur. The disadvantages are that: 1. the joint needs to be opened, so that the patellar tendon, the joint capsule adhesion and the like after the operation are easily caused, and the recovery of the knee joint function after the operation is influenced; 2. when joint tissues are damaged by the operation or the tail part of the intramedullary needle is too long and is exposed out of the articular cartilage, postoperative knee joint pain can be caused; 3. bone and cartilage fragments possibly generated in the operation are left in the knee joint cavity to form an episome, and cruciate ligaments can be damaged; 4. the second phase is difficult to remove the intramedullary nail.
Therefore, an intramedullary pin that can restore good joint function after operation without damaging joints is required for the intramedullary pin fixation.
Disclosure of Invention
The present invention is directed to overcoming the above problems and providing a femoral medial condyle retrograde locking intramedullary needle. In order to achieve the purpose, the invention adopts the following technical scheme:
the intramedullary nail comprises a needle point and a needle rod, wherein the needle rod comprises a straight rod part and an arc-shaped part, the straight rod part is arranged at the top of the arc-shaped part, the needle point is arranged at the top of the straight rod part, an inclined part of 30 degrees is arranged at the top of the needle point, a first screw and a second screw are arranged at the position, close to the needle point, of the straight rod part, and a third screw, a fourth screw and a fifth screw are arranged at the bottom of the arc-shaped part at intervals; the first screws and the second screws are arranged at intervals, and the included angle is 30-40 degrees; the third screw is arranged at the position close to the end part of the arc-shaped part, the third screw is positioned on the center line of the coronal plane, the included angle between the fourth screw, the fifth screw and the third screw is 20-30 degrees, and screw holes respectively matched with the first screw to the fifth screw are arranged on the needle rod.

1. the invention drills in from the external position of the femoral medial condyle joint, has simple operation, does not damage the joint and has good function recovery of the knee joint after operation.
2. A plurality of screws can be drilled into the needle rod position of the intramedullary nail, the distal 2 screws adopt a nail-in-nail locking design, and the most distal screw and a nut form a press-fit locking design, so that the mechanical stability is improved, the intramedullary nail can be used for fixing the diaphysis fracture of the far end of the femoral shaft and the distal unicondylar fracture of the femur, and the combined tension screw can be used for the intercondylar fracture of the femoral.

Detailed Description
The present invention will be described in detail and specifically with reference to the following examples so as to facilitate the understanding of the present invention, but the following examples do not limit the scope of the present invention.
Example 1
The embodiment discloses a femoral medial condyle retrograde locking intramedullary nail, which comprises a needle tip 1 and a needle rod 2, wherein the needle rod 2 comprises a straight rod part 21 and an arc-shaped part 22, the straight rod part 21 is arranged at the top of the arc-shaped part 22, the needle tip 1 is arranged at the top of the straight rod part 21, and the top of the needle tip 1 is provided with a 30-degree inclined part 11.
A first screw 3 and a second screw 4 are arranged at the position of the straight rod part 21 close to the needle point 1, and a third screw 5, a fourth screw 6 and a fifth screw 7 which are arranged at intervals are arranged at the bottom of the arc part 22; the first screw 3 and the second screw 4 are spaced by 20mm, and the included angle is 30 degrees; the third screw 5 is provided near the end of the arc-shaped portion 22. As shown in fig. 4, the third screw 5 is located on the middle line of the coronal plane, the included angles between the fourth screw 6, the fifth screw 7 and the third screw are respectively 25 degrees and 20 degrees, and the needle bar 2 is provided with screw holes 8 respectively matched with the first screw 3 to the fifth screw 7.
The fourth screw 6 and the fifth screw 7 are of a nail-in-nail structure and comprise an outer sleeve screw 61 and a core screw 62, the outer sleeve screw 61 comprises a first stud 611 and a first nut 612, the core screw 62 comprises a second stud 621 and a second nut 622, a cavity 613 is arranged in the middle of the first nut 612, a rotating groove 614 is arranged at the top of the first nut 612, a through hole 615 is formed in the side of the cavity 613, the core screw 62 penetrates through the through hole 615, female threads are arranged in the through hole 615, male threads are arranged on the outer side of the second nut 622, and the second nut 622 is in threaded connection with the outer sleeve screw 61 through the through hole 615 and enables the outer sleeve screw 61 and the core screw 62 to be locked. The angle between the outer sleeve screw 61 and the core screw 62 is 15 °.
The end part of the arc-shaped part 22 is provided with a second screw hole 221 convenient for installation, the intramedullary pin is also provided with a sixth screw 9 for sealing the second screw hole 221, and after the sixth screw 9 is screwed in, the third screw 5 is pressed and locked to form locking.
In the present embodiment, the first screw 3 to the fifth screw 7 are cortical screws. The needle bar 2 has a diameter of 9mm and a length of 180mm, and the radius of curvature of the arc-shaped portion 22 is 120 mm. The distances from the third screw, the fourth screw and the fifth screw to the tail end of the arc-shaped part are respectively 15mm, 25mm and 35 mm.
When the embodiment is used for femoral fracture treatment, a longitudinal incision with the length of about 2cm is made at the medial condyle of the femur, skin, subcutaneous tissue and deep fascia are incised, the lateral position is 1cm in front of the highest point of the medial condyle of the femur (the central point of the medial collateral ligament insertion point of the knee), and a kirschner pin is drilled along the lateral anatomical longitudinal axis direction of the femur; the orthodox kirschner wire is parallel to the cortical bone of the medial condyle of the femur, and has an included angle of 40 degrees with the orthodox anatomical longitudinal axis of the femur, and the insertion point is about 0.2cm higher than the lowest point of the intercondylar of the femur. After the C-arm X-ray machine confirms that the position is correct, the guider is inserted along the Kirschner wire to drill and expand the marrow, and then the intramedullary needle is inserted and fixed.
As shown in FIGS. 7-9, this embodiment may be applied to a lower femoral fracture (site A), a unicondylar femoral fracture (site B), or an intercondylar femoral fracture (site C). When the external and internal intercondylar fracture treatment device is applied to the intercondylar fracture of the femoral bone, the internal and external intercondylar fractures are connected into a whole by using a lag screw, and the femoral shaft fracture is treated by using the embodiment.
The invention drills in from the external joint part of the femoral medial condyle, has simple operation, does not damage the joint and has good function recovery of the knee joint after operation; a plurality of screws can be drilled into a needle rod of the intramedullary nail, the fourth screw 6 and the fifth screw 7 adopt a nail-in-nail locking structure, and the sixth screw 9 can be screwed in and then press-fitted to lock the third screw 5 to form locking, so that the mechanical stability is improved.
The embodiments of the present invention have been described in detail above, but they are merely exemplary, and the present invention is not equivalent to the above described embodiments. Any equivalent modifications and substitutions to those skilled in the art are also within the scope of the present invention. Accordingly, it is intended that all equivalent alterations and modifications be included within the scope of the invention, without departing from the spirit and scope of the invention.
